Pharmacological basis and clinical evidence of dabigatran therapy
Dabigatran is an emerging oral anticoagulant which is a direct inhibitor of thrombin activity. It has been approved in the European Union and the United States of America for the prevention of thrombosis after major orthopedic surgery. It has also been approved by the American Food and Drug Administration and the European Medicines Agency for the prevention of stroke in chronic atrial fibrillation. Dabigatran provides a stable anticoagulation effect without any need to perform periodical laboratory controls. Of note, there is a growing amount of clinical evidence which shows its safety and efficacy. For these reasons, dabigatran may suppose a revolution in oral anticoagulation. However, two important limitations remain. First, it is contraindicated in patients with end-stage renal disease. Second, there is no evidence of the prevention of thrombosis in mechanical heart valves
CONTACTO: A Social Networking Site for Supported-Employment Interventions for People With Intellectual and Developmental Disabilities
The main objective of this work is to determine whether social networks can be used to improve self-management skills in people with Intellectual or Developmental Disability (IDD) given the high rate of acceptance of such tools among them. The proposed methodology applies an employment-based intervention to evaluate whether social network services help improve self-management skills. A fit-for-purpose social networking service is built, and different evaluation mechanisms are applied to determine the degree to which users with IDD could use the tool without assistance. The working hypothesis is that in order to ensure the usability of the implemented tool, users (with and without IDD) should be involved throughout the whole software development cycle, following a co-design methodological approach. Results demonstrate that this methodological approach leads to high satisfaction and motivation rates. In terms of effectiveness and efficiency, results demonstrate that the tool is useful and usable for people with IDD. It can therefore be concluded that social network services built having users with IDD at its center will help improve their self-management skills. The main implication of this research is that when using social network services as a tool to support interventions, special attention should be paid to previously evaluating the usability and motivation rate as a measure of how autonomously the person with IDD can use the tool. Nevertheless, further research is needed to determine the impact that such tools can have on other types of intervention.El objetivo principal de este trabajo es determinar si las redes sociales pueden ser utilizadas para mejorar las habilidades de autogestión en personas con Discapacidad Intelectual o del Desarrollo (DDI) dado el alto Ãndice de aceptación de este tipo de herramientas entre ellas. La metodologÃa propuesta aplica una intervención basada en el empleo para evaluar si los servicios de redes sociales ayudan a mejorar las habilidades de autogestión. Se crea un servicio de red social adecuado para el propósito y se aplican diferentes mecanismos de evaluación para determinar el grado en que los usuarios con DDI podrÃan usar la herramienta sin ayuda. La hipótesis de trabajo es que para asegurar la usabilidad de la herramienta implementada, los usuarios (con y sin DDI) deben estar involucrados durante todo el ciclo de desarrollo del software, siguiendo un enfoque metodológico de codiseño. Los resultados demuestran que este enfoque metodológico conduce a altos Ãndices de satisfacción y motivación. En términos de efectividad y eficiencia, los resultados demuestran que la herramienta es útil y usable para personas con DID. Por lo tanto, se puede concluir que los servicios de redes sociales construidos con usuarios con DDI en su centro ayudarán a mejorar sus habilidades de autogestión. An Affective-computing Approach to Provide Enhanced Learning Analytics
Detecting emotions in a learning environment can make the student-learning process more efficient, avoiding
stressful situations that might eventually lead to failure, frustation and demotivation. The work presented here
describes a perceptive desktop devised to capture the sensations of any person facing learning activities. To
this end, we propose a perceptive environment enhanced with capabilities to perform an analysis of electroen cephalography, facial expression, eye tracking and particularly a very distinctive indicator of stress as it is
the galvanic response of the skin. This work focuses on the galvanic response of the skin, comparing the
performance of two devices in the context of the perceptive desktop. One of the devices was very attractive to
our environment as it was a mouse that fit very well to our computer-based desktop, equipped with low-cost
sensors to detect the galvanic response. The other device is more tedious to place and more expensive but we
use it as a reference to know if the mouse is accurate. Four people were exposed to an experiment with the
two devices connected, and observing the results it can be concluded that there is no correlation between the
captures of both devices. Therefore, we could not select the mouse for our environment even though at first it
looks like a very promising device.Detectar las emociones en un entorno de aprendizaje puede hacer que el proceso de aprendizaje de los alumnos sea más eficiente, evitando situaciones estresantes que puedan llevar al fracaso, la frustración y la desmotivación. El trabajo que aquà se presenta describe un escritorio perceptivo ideado para captar las sensaciones de cualquier persona que se enfrente a actividades de aprendizaje. Para ello, se propone un entorno perceptivo potenciado con capacidades para realizar un análisis de electroencefalografÃa, expresión facial, seguimiento ocular y particularmente un indicador muy distintivo de estrés como es la respuesta galvánica de la piel. Este trabajo se centra en la respuesta galvánica de la piel, comparando el rendimiento de dos dispositivos en el contexto del escritorio perceptivo. Uno de los dispositivos resultaba muy atractivo para nuestro entorno, ya que se trataba de un ratón que se adaptaba muy bien a nuestro escritorio informático, equipado con sensores de bajo coste para detectar la respuesta galvánica. El otro dispositivo es más tedioso de colocar y más caro, pero lo utilizamos como referencia para saber si el ratón es preciso. Se expuso a cuatro personas a un experimento con los dos dispositivos conectados, y observando los resultados se puede concluir que no hay correlación entre las capturas de ambos dispositivos. Por lo tanto, no pudimos seleccionar el ratón para nuestro entorno a pesar de que en principio parece un dispositivo muy prometedor
Phyx.io: Expert-Based Decision Making for the Selection of At-Home Rehabilitation Solutions for Active and Healthy Aging
While the importance of physical activity in older adults is beyond doubt, there are significant barriers limiting the access of older adults to physical exercise. Existing technologies to support physical activity in older adults show that, despite their positive impacts on health and well-being, there is in general a lack of engagement due to the existing reluctance to the use of technology. Usefulness and usability are two major factors for user acceptance along with others, such as cost, privacy, equipment and maintenance requirements, support, etc. Nevertheless, the extent to which each factor impacts user acceptance remains unclear. Furthermore, other stakeholders, besides the end users, should be considered in the decision-making process to develop such technologies, including caregivers, therapists and technology providers. In this paper, and in the context of physical rehabilitation and exercise at home, four different alternatives with incremental characteristics have been defined and considered: a software-based platform for physical rehabilitation and exercise (Alternative 1), the same software platform with a conventional RGB camera and no exercise supervision (Alternative 2), the same software platform with a convention RGB camera and exercise supervision (Alternative 3) and finally, the same software platform with a depth camera and exercise supervision (Alternative 4). A multiple attribute decision-making methodology, based on the ordinal priority approach (OPA) method, is then applied using a group of experts, including end users, therapists and developers to rank the best alternative. The attributes considered in this method have been usefulness, cost, ease of use, ease of technical development, ease of maintenance and privacy, concluding that Alternative 3 has been ranked as the most appropriate.Si bien la importancia de la actividad fÃsica en los adultos mayores está fuera de toda duda, existen importantes barreras que limitan el acceso de los adultos mayores al ejercicio fÃsico. Las tecnologÃas existentes para apoyar la actividad fÃsica en adultos mayores muestran que, a pesar de sus impactos positivos en la salud y el bienestar, en general existe una falta de compromiso debido a la renuencia existente al uso de la tecnologÃa. La utilidad y la facilidad de uso son dos factores principales para la aceptación del usuario junto con otros, como el costo, la privacidad, los requisitos de equipo y mantenimiento, soporte, etc. Sin embargo, la medida en que cada factor afecta la aceptación del usuario sigue sin estar clara. Además, otras partes interesadas, además de los usuarios finales, deben ser consideradas en el proceso de toma de decisiones para desarrollar dichas tecnologÃas, incluidos los cuidadores, terapeutas y proveedores de tecnologÃa. The novel KV7 channel activator URO-K10 exerts enhanced pulmonary vascular effects independent of the KCNE4 regulatory subunit.
KV7 channels exert a pivotal role regulating vascular tone in several vascular beds. In this context, KV7 channel agonists represent an attractive strategy for the treatment of pulmonary arterial hypertension (PAH). Therefore, in this study, we have explored the pulmonary vascular effects of the novel KV7 channel agonist URO-K10. Consequently, the vasodilator and electrophysiological effects of URO-K10 were tested in rat and human pulmonary arteries (PA) and PA smooth muscle cells (PASMC) using myography and patch-clamp techniques. Protein expression was also determined by Western blot. Morpholino-induced knockdown of KCNE4 was assessed in isolated PA. PASMC proliferation was measured by BrdU incorporation assay. In summary, our data show that URO-K10 is a more effective relaxant of PA than the classical KV7 activators retigabine and flupirtine. URO-K10 enhanced KV currents in PASMC and its electrophysiological and relaxant effects were inhibited by the KV7 channel blocker XE991. The effects of URO-K10 were confirmed in human PA. URO-K10 also exhibited antiproliferative effects in human PASMC. Unlike retigabine and flupirtine, URO-K10-induced pulmonary vasodilation was not affected by morpholino-induced knockdown of the KCNE4 regulatory subunit. Noteworthy, the pulmonary vasodilator efficacy of this compound was considerably increased under conditions mimicking the ionic remodelling (as an in vitro model of PAH) and in PA from monocrotaline-induced pulmonary hypertensive rats. Taking all together, URO-K10 behaves as a KCNE4-independent KV7 channel activator with much increased pulmonary vascular effects compared to classical KV7 channel activators. Associations between Screen Time and Physical Activity among Spanish Adolescents
Excessive time in front of a single or several screens could explain a displacement of physical activity. The present study aimed at determining whether screen-time is associated with a reduced level of moderate to vigorous physical activity (MVPA) in Spanish adolescents living in favorable environmental conditions. or more to total screen-time showed a 64% (OR = 0.61, 95% CI, 0.44–0.86) increased risk of failing to achieve the recommended adolescent MVPA level. Participation in organized physical activities and sports competitions were more strongly associated with MVPA than screen-related behaviors.No single screen-related behavior explained the reduction of MVPA in adolescents. However, the total time accumulated through several screen-related behaviors was negatively associated with MVPA level in boys. This association could be due to lower availability of time for exercise as the time devoted to sedentary screen-time activities increases. Participation in organized physical activities seems to counteract the negative impact of excessive time in front of screens on physical activity
Contribution of Individual and Environmental Factors to Physical Activity Level among Spanish Adults
BACKGROUND: Lack of physical activity (PA) is a major risk for chronic disease and obesity. The main aims of the present study were to identify individual and environmental factors independently associated with PA and examine the relative contribution of these factors to PA level in Spanish adults. METHODOLOGY/PRINCIPAL FINDINGS: A population-based cross-sectional sample of 3,000 adults (18-75 years old) from Gran Canaria (Spain) was selected using a multistage stratified random sampling method. The participants were interviewed at home using a validated questionnaire to assess PA as well as individual and environmental factors. The data were analyzed using bivariate and multivariate logistic regression. One demographic variable (education), two cognitive (self-efficacy and perceived barriers), and one social environmental (organized format) were independently associated with PA in both genders. Odds ratios ranged between 1.76-2.07 in men and 1.35-2.50 in women (both p<0.05). Individual and environmental factors explained about one-third of the variance in PA level. CONCLUSIONS/SIGNIFICANCE: Self-efficacy and perceived barriers were the most significant factors to meet an adequate level of PA. The risk of insufficient PA was twofold greater in men with primary or lesser studies and who are employed. In women, living in rural environments increased the risk of insufficient PA. The promotion of organized PA may be an efficient way to increase the level of PA in the general population. Improvement in the access to sport facilities and places for PA is a prerequisite that may be insufficient and should be combined with strategies to improve self-efficacy and overcome perceived barriers in adulthood
