Communication intentionnelle dans le domaine du leadership

    Get PDF

    Get PDF
As Catholic healthcare organizations form a substantive part of healthcare delivery in the USA and Australia, ethical standards for Catholic health care were developed to guide practice. This study examined junior staff’s understanding of Catholic ethics. Using a qualitative descriptive design, we recruited 22 medical and nursing staff to interviews/focus groups. Though Catholic ethics seldom informed ethical approaches, the principles were acknowledged as being useful to support development of confident and respectful care approaches. Findings provide early insights into challenges faced in considering implementation of ethical codes across both secular and religious healthcare organizations, suggesting that a more creative and pastoral approach to dialoguing and implementing Catholic ethics is required

Objectives: To perform molecular diagnosis of microbial agents (FHV-1, FCV, Mycoplasma felis, and Chlamydophila felis) in kittens with conjunctivitis and correlate the clinical signs with clinical severity. Material and Methods: A total of 108 conjunctival swab were collected from kittens without (G1; n = 40) and with (G2; n = 68) clinical signs of conjunctivitis. Animals from G2 group were scored from 1 (mild) to 4 (severe) according to the severity of conjunctivitis. All samples were submitted to PCR and RT-PCR. Results: FHV-1 was detected in 62/108 (57.4%) of samples, FCV in 40/108 (37.0%), M. felis in 11/108 (10.2%) and C. felis in 26/108 (24.1%). Mixed infections were detected in 39/108 (36.1%). In G1, 28/40 (70.0%) were positive for one or more agents, in G2, 58/68 (85.3%) were positive (P = 0.03). In 1, single infections by FHV-1were found in 21/40 (52.5%) samples, FCV in 2/40 (5.0%), C. felis in 1/40 (2.5%), and no pathogens were detected in 12/40 (30%) of samples, while mixed infections accounted for 29/40 (72.5%) of the cases. In G2, single FHV-1 infections were found in 31/68 (45.6%) samples, FCV in 10/68 (14.7 %), M. felis in 2/68 (3.0%) and C. felis also in 2/68 (3.0%), and no pathogens were detected in 10/68 (14.7%) samples, while mixed infections accounted for 36/68 (52.0%) of the cases. They were categorized as grade 1, 20/68 (29.4%), grade 2, 14/68 (20.6%), grade 3, 21/68 (30.9%) and grade 4, 13/68 (19.1%). The presence of FHV-1 and FCV is equally distributed among the four categories. More severe clinical signs, scores 3 and 4, are related to coinfections by C. felis and M. felis. Conclusions: FHV-1, FCV, C. felis and M. felis were identified in feline conjunctivitis. Co-infections are related to more severe cases of conjunctivitis. Molecular diagnosis is helpful to detect asymptomatic carriers and is a rapid and accurate method to determine the pathogen of feline conjunctivitis.O objetivo deste estudo foi realizar diagnóstico molecular de agentes microbiológicos (FHV-1, FCV, Mycoplasma felis e Chlamydophila felis) em gatos filhotes e associar a presença dos patógenos à gravidade dos sinais clínicos de conjuntivite. No G2, 85,3% apresentavam infecções (P = 0,03).

Patients’ and caregivers’ contested perspectives on spiritual care for those affected by advanced illnesses: A qualitative descriptive study

    Get PDF
Context; Spiritual care refers to practices and rituals addressing spiritual/religious concerns. It supports coping with loss and finding hope, meaning, and peace. Although integral to palliative care, its implementation is challenging. Objective: To understand an Australian cohort of patients’ and caregivers’ perspectives about experiencing and optimizing spiritual care in the context of advanced illness. Methods: Patients and caregivers of patients with #12 month prognosis were recruited from a broader spiritual study via criterion sampling and agreed to opt-in interviews. Participants from an Australian, metropolitan health service received a spiritual care definition and were interviewed. Transcripts were analyzed using qualitative description. Results: 30 patients (17 male; mean age 70 years) and 10 caregivers (six male; mean age 58.9 years) participated. 27 identified as Christian, and 10 had no religion. Participants described multifaceted and contested beliefs about spirituality. Many queried the tangibility of spirituality, but all valued respectful staff who affirmed personhood, that is, each individual’s worth, especially when care exceeded expectations. They also resonated with positive organizational and environmental tones that improved holistic well-being. Participants stressed the importance of the hospital’s welcoming context and skilled care, which comforted and reassured. Conclusion: Although many patients and caregivers did not resonate with the term ‘‘spiritual care,’’ all described how the hospital’s hospitality could affirm their values and strengthen coping. The phrase ‘‘spiritual care and hospitality’’ may optimally articulate and guide care in similar, pluralist inpatient palliative care contexts, recognizing that such care encompasses an interplay of generalist and specialist pastoral care staff and organizational and environmental qualities

Multiscale habitat characterization of herbaceous Atlantic Coastal Plain Flora on lakeshores in Nova Scotia

    Get PDF
Accepted VersionAtlantic Coastal Plain Flora (ACPF) are a group of plants mostly inhabiting lakeshores along the Atlantic coast of the United States, with disjunct populations in Nova Scotia and Ontario. To better define their ecological requirements, the main objective of this study was to determine the factors (biotic and abiotic habitat components) influencing ACPF communities (distribution, species abundance, and richness) at both the landscape and local scales. On 16 lakeshores in southwestern Nova Scotia, we characterized ACPF communities and habitat within 20 cm square contiguous quadrats distributed along 20 m transects (landscape scale) and in 5 m × 5 m grids (local scale). Performing redundancy analysis (n = 16 transects), we found that at the landscape scale, shoreline slope and shrub species distribution influenced the quantity of suitable habitat available for ACPF, with mineral shorelines supporting higher ACPF richness. Using spatial generalized linear mixed models (n = 3125 quadrats in five grids), we found that elevation, vegetation elements (shrubs, sundews, graminoids), and substrate type mostly influenced ACPF presence and abundance. ACPF also showed inter-specific differences in habitat preferences. Defining ACPF ecological requirements at both the landscape and local scales is important to guide conservation and management actions in Nova Scotia and throughout their North American range

Background: The combination of systemic arterial hypertension and diabetes mellitus (DM) induces greater cardiac remodeling than either condition alone. However, this association has been poorly addressed in senescent rats. Therefore, this study aimed to analyze the influence of streptozotocin-induced DM on ventricular remodeling and oxidative stress in aged spontaneously hypertensive rats (SHR).Methods: Fifty 18 month old male SHR were divided into two groups: control (SHR, n = 25) and diabetic (SHR-DM, n = 25). DM was induced by streptozotocin (40 mg/kg, i.p.). After nine weeks, the rats underwent echocardiography and myocardial functional study in left ventricular (LV) isolated papillary muscle preparations. LV samples were obtained to measure myocyte diameters, interstitial collagen fraction, and hydroxyproline concentration. Gene expression of atrial natriuretic peptide (ANP) and alpha- and beta-myosin heavy chain (MyHC) isoforms was evaluated by RT-PCR. Serum oxidative stress was assessed by measuring lipid hydroperoxide concentration and superoxide dismutase and glutathione peroxidase activities. Statistics: Student's t test or Mann-Whitney test, p < 0.05.Results: SHR-DM presented higher blood glucose (487 +/- 29 vs. 89.1 +/- 21.1 mg/dL) and lower body weight (277 +/- 26 vs. 339 +/- 38 g). Systolic blood pressure did not differ between groups. Echocardiography showed LV and left atrial dilation, LV diastolic and relative wall thickness decrease, and LV systolic and diastolic function impairment in SHR-DM. Papillary muscle study showed decreased myocardial contractility and contractile reserve in SHR-DM. Myocyte diameters and myocardial interstitial collagen fraction and hydroxyproline concentration did not differ between groups. Increased serum pro-oxidant activity and gene expression of ANP and beta/alpha-MyHC ratio were observed in DM.Conclusion: Diabetes mellitus induces cardiac dilation and functional impairment, increases oxidative stress and activates fetal gene program in aged spontaneously hypertensive rats.Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES)Conselho Nacional de Desenvolvimento Científico e Tecnológico (CNPq)Fundação de Amparo à Pesquisa do Estado de São Paulo (FAPESP

Mechanochemical feedback control of dynamin independent endocytosis modulates membrane tension in adherent cells.

    Get PDF
Plasma membrane tension regulates many key cellular processes. It is modulated by, and can modulate, membrane trafficking. However, the cellular pathway(s) involved in this interplay is poorly understood. Here we find that, among a number of endocytic processes operating simultaneously at the cell surface, a dynamin independent pathway, the CLIC/GEEC (CG) pathway, is rapidly and specifically upregulated upon a sudden reduction of tension. Moreover, inhibition (activation) of the CG pathway results in lower (higher) membrane tension. However, alteration in membrane tension does not directly modulate CG endocytosis. This requires vinculin, a mechano-transducer recruited to focal adhesion in adherent cells. Vinculin acts by controlling the levels of a key regulator of the CG pathway, GBF1, at the plasma membrane. Thus, the CG pathway directly regulates membrane tension and is in turn controlled via a mechano-chemical feedback inhibition, potentially leading to homeostatic regulation of membrane tension in adherent cells

Abstract: The transmission system operators (TSO) face increasing and more unpredictable challenges. The high penetration of renewable energy and the liberalization of the electricity market rises the volatility in electricity generation and bigger differences between production and consumption, in the course of the electrical system operation. Ancillary services are a complement to electricity markets and are characterized as a necessary service that allows a proper and reliable functioning of the system as it is the mechanism that allows the TSO to balance generation and consumption in real time operation and control the frequency of the system. The present work aims to make a thorough analysis of what are the ancillary services for frequency regulation, particularly on secondary regulation and tertiary regulation and to address the laws that allow to develop the frequency regulation in Portugal. It will also be studied the operation of the ancillary services market for frequency regulation in Portugal and the chronological connection with the daily market and the various intraday market sessions of Iberian Electricity Market (MIBEL). Itwill be studied also various ancillary services in other European countries compared to Portugal. To support this data analysis a Matlab® tool was developed allowing the user to view in the graphical form the offers of market players participating with their physical units (power plants), the marginal cost of these same power plants and the market clearing price, all for the same selected period of time. The results show a dependence of the bidding profile relative to the power plant technology. It will be also studied the correlations between different power plants and the price variation of the market due to the variation of the hydroelectric capability factor (HCF) variation. It is also relevant the high share of hydro power plants participating in the ancillary services market for secondary frequency control
