The system of medical and psychological support for women at high risk of preterm birh
Threatened premature labor (TPL) is a severe obstetric complication which affects the mental and physical health of both the mother and fetus. Family resilience may have protective role against psychological distress in women experiencing these pregnancy complications. There may be resilience related risk factors in TPL women, and interplays may exist among psychological variables and within couples. This study aims to examine psychological outcomes influenced by different levels of resilience, and explore psychological interactions in TPL women, spouses, and between women and spouses
Рівень прив’язаності до плоду у жінок із загрозою передчасних пологів на ІІІ триместрі вагітності та психомоторний розвиток дитини першого року життя у світлі взаємодії в системі «мати – дитина»
The aim of the research is to study the level of attachment to the fetus of women with the threat of premature birth in the third trimester of pregnancy and psychomotor development of the baby in the first year of life.Materials and methods. The study included 150 pregnant women in the third trimester (23-37 weeks), and their men. Of these, the main group consisted of 120 women with the risk of preterm birth in the last trimester of pregnancy (MG). The comparison group consisted of 30 women with physiological pregnancy (CG).Results and discussion. Women with MG were found to have a maternal fetal attachment level of 71.0±1.0 (attachment disorder) and women with a CG of 51.0±2.0 (sufficient attachment level). Assessment of the psychomotor development of the child during the first year of life showed a tendency to lag its indicators in children of mothers from MG (after the second examination of MTG compared with the infants of women with CG and MGK during all periods of observation (p<0.05). Such a difference in indicators of psychomotor development was observed at the third survey: the level of development of children of mothers of CG was significantly higher than in women with MG. In the fourth survey, children of MTG mothers (29.0) showed better psychomotor development rates than infants of MCT women (27.5), but lower than those of CG (29.4). Subsequently, the level of psychomotor development of children with CG and MTG was almost equal to that of children of MCG (28.1), which were lower than in other groups. When reaching the age of one year, the children of MTG mothers showed the best level of psychomotor development (29.2), almost the same results were observed in children with CG (29.1), and indicators of MCG children were lower (28.5).Conclusions. Among the negative factors that provoke the formation of dyadic interaction disorders, psychological unpreparedness for motherhood and a low level of attachment to the fetus of women with the threat of premature birth during the third trimester of pregnancy are leadingЦель работы - изучение уровня привязанности к плоду женщин с угрозой преждевременных родов на III триместре беременности и психомоторного развития ребенка первого года жизни.Материалы и методы. В исследование было включено 150 беременных на III триместре беременности (23 - 37 недель), и их мужчин. Из них основную группу составили 120 женщин с с угрозой преждевременных родов на последнем триместре беременности (ОП). Группу сравнения составили 30 женщин с физиологическим течением беременности (ГП).Результаты и обсуждение. Установлено, что у женщин ОГ уровень привязанности матери к плоду составил 71 ± 1 (нарушение привязанности), а у женщин ГП 51 ± 2 (достаточный уровень привязанности). Оценка психомоторного развития ребенка в течение первого года жизни доказала существование тенденции отставания его показателей у детей матерей ОГ (после второго обследования ОГК) по сравнению с младенцами женщин ГП и ОГТ в течение всех периодов наблюдения (р <0,05). Такая разница показателей психомоторного развития наблюдалась уже на третьем обследовании: уровень развития детей матерей ГП был достоверно выше, чем у женщин ОГ. На четвертом обследовании дети матерей ОГТ (29.0±0.2) продемонстрировали лучшие показатели психомоторного развития, чем младенцы женщин ОГК (27.5±0.3), однако ниже, чем в ГП (29.4±0.1). В дальнейшем уровень психомоторного развития детей ГП и ОГТ практически сравнялся в отличие от показателей детей ОГК (28.1±0.4), которые были ниже, чем в других группах. При достижении детьми возраста одного года лучший уровень психомоторного развития продемонстрировали дети матерей ОГТ (29.2±0.3), практически одинаковые результаты были у детей ГП (29.1±0.3), а показатели детей ОГК были ниже (28.5±0.2).Выводы. У дослідження було включено 150 вагітних на III триместрі вагітності (23 – 37 тижнів), та їх чоловіків. Із них основну групу склали 120 жінок з із загрозою передчасних пологів на останньому триместрі вагітності (ОГ). Групу порівняння становили 30 жінок з фізіологічним перебігом вагітності (ГП).Результати та обговорення. Встановлено, що у жінок ОГ рівень прив’язаності матері до плоду становив 71±1 (порушення прив’язаності) , а у жінок ГП 51±2 (достатній рівень прив’язаності). Оцінка психомоторного розвитку дитини протягом першого року життя довела існування тенденції щодо відставання його показників у дітей матерів ОГ (після другого обстеження ОГК) порівняно з немовлятами жінок ГП та ОГТ протягом усіх періодів спостереження (р<0,05). Така різниця показників психомоторного розвитку спостерігалася вже на третьому обстеженні: рівень розвитку дітей матерів ГП був достовірно вищим, ніж у жінок ОГ. На четвертому обстеженні діти матерів ОГТ (29.0±0.2) продемонстрували кращі показники психомоторного розвитку, ніж немовлята жінок ОГК (27.5±0.3), проте нижчі, ніж у ГП (29.4±0.1). У подальшому рівень психомоторного розвитку дітей ГП та ОГТ практично зрівнявся на відміну від показників дітей ОГК (28.1±0.4), що були нижчими, ніж в інших групах. При досягненні малюками віку одного року найкращий рівень психомоторного розвитку продемонстрували діти матерів ОГТ (29.2±0.3), практично однакові результати були у дітей ГП (29.1±0.3), а показники дітей ОГК були нижчими (28.5±0.2).Висновки. Cеред негативних факторів, що провокують формування порушення діадичної взаємодії, провідними є психологічна неготовність до материнства та низький рівень прив’язаності до плоду жінок із загрозою передчасних пологів на III триместрі вагітності

Psychological and sociocultural adaptation of medical students from India in Ukraine
International students from culturally and linguistically diverse backgrounds experience personal and academic challenges when studying health sciences in Ukraine. Moving from one academic environment to another and relocating to a new country to study is becoming an increasingly common occurrence. As well as providing cultural and intellectual enrichment, this experience also presents challenges and difficulties. With the increasing number of international students in Ukraine, an understanding of the psychological factors that facilitate adaptation is becoming evermore important. The aim of the present research was to consider elements facilitating or impeding Indian international students’ adaptation to Ukraine. The variables of trait-anxiety, attachment intimacy and attachment anxiety, psychological distress, satisfaction with social support, and cultural identification were measured. Attachment intimacy and trait-anxiety moderated by psychological distress were found to be relevant to the process of adaptation. Co-national identification was also relevant to adaptation for the Indian students
Composition of Sedimentary Organic Matter across the Laptev Sea Shelf: Evidences from Rock-Eval Parameters and Molecular Indicators
Global warming in high latitudes causes destabilization of vulnerable permafrost deposits followed by massive thaw-release of organic carbon. Permafrost-derived carbon may be buried in the nearshore sediments, transported towards the deeper basins or degraded into the greenhouse gases, potentially initiating a positive feedback to climate change. In the present study, we aim to identify the sources, distribution and degradation state of organic matter (OM) stored in the surface sediments of the Laptev Sea (LS), which receives a large input of terrestrial carbon from both Lena River discharge and intense coastal erosion. We applied a suite of geochemical indicators including the Rock Eval parameters, traditionally used for the matured OM characterization, and terrestrial lipid biomarkers. In addition, we analyzed a comprehensive grain size data in order to assess hydrodynamic sedimentation regime across the LS shelf. Rock-Eval (RE) data characterize LS sedimentary OM with generally low hydrogen index (100–200 mg HC/g TOC) and oxygen index (200 and 300 CO2/g TOC) both increasing off to the continental slope. According to Tpeak values, there is a clear regional distinction between two groups (369–401 °C for the inner and mid shelf; 451–464 °C for the outer shelf). We suggest that permafrost-derived OM is traced across the shallow and mid depths with high Tpeak and slightly elevated HI values if compared to other Arctic continental margins. Molecular-based degradation indicators show a trend to more degraded terrestrial OC with increasing distance from the coast corroborating with RE results. However, we observed much less variation of the degradation markers down to the deeper sampling horizons, which supports the notion that the most active OM degradation in LS land-shelf system takes part during the cross-shelf transport, not while getting buried deeper
Psychometric properties of the Opening Minds Stigma Scale for Health Care Providers in 32 European countries – A bifactor ESEM representation
AimsTo measure the stigma of healthcare providers toward people suffering from mental illness, the Opening Minds Stigma Scale for Health Care Providers (OMS-HC) is a commonly applied instrument. However, this scale has not been thoroughly validated in many European countries, its psychometric properties are still unknown and data on practicing psychiatrists is lacking. Therefore, this multicenter study aimed to assess the psychometric characteristics of the 15-item OMS-HC in trainees and specialists in adult and child psychiatry in 32 countries across Europe.Materials and methodsThe OMS-HC was conducted as an anonymous online survey and sent via Email to European adult and child psychiatrists. Parallel analysis was used to estimate the number of OMS-HC dimensions. Separate for each country, the bifactor ESEM, a bifactor exploratory structural equation modeling approach, was applied to investigate the factor structure of the scale. Cross-cultural validation was done based on multigroup confirmatory factor analyses and reliability measures.ResultsA total of 4,245 practitioners were included, 2,826 (67%) female, 1,389 (33%) male. The majority (66%) of participants were specialists, with 78% working in adult psychiatry. When country data were analyzed separately, the bifactor model (higher-order factor solution with a general factor and three specific factors) showed the best model fit (for the total sample χ2/df = 9.760, RMSEA = 0.045 (0.042–0.049), CFI = 0.981; TLI = 0.960, WRMR = 1.200). The average proportion of variance explained by the general factor was high (ECV = 0.682). This suggests that the aspects of ‘attitude,’ ‘disclosure and help-seeking,’ and ‘social distance’ could be treated as a single dimension of stigma. Among the specific factors, the ‘disclosure and help-seeking’ factor explained a considerable unique proportion of variance in the observed scores.ConclusionThis international study has led to cross-cultural analysis of the OMS-HC on a large sample of practicing psychiatrists. The bifactor structure displayed the best overall model fit in each country. Rather than using the subscales, we recommend the total score to quantify the overall stigmatizing attitudes. Further studies are required to strengthen our findings in countries where the proposed model was found to be weak

Introduction. Pregnancy can both change a couple's relationships for the better and worsen them. The scenario of developing relationships after pregnancy depends mainly on the predominance of constructive and non-constructive motives of conception. Unfortunately, expectant parents are generally unable to grasp these patterns by resorting mainly to rationalization and displacement.
The purpose of the study. The purpose of the study was to evaluate the external and internal resources of support for women with the threat of preterm delivery during the third trimester of pregnancy and after childbirth.
Materials and methods. The study was conducted during 2011-2019 on the basis of KNP "Storozhinets Regional hospital". The study included 150 pregnant women in the third trimester (23-37 weeks), and their husbands. Of these, the main group consisted of 120 women with the risk of preterm birth in the last trimester of pregnancy (MG). The comparison group consisted of 30 women with physiological pregnancy (CG). The study conducted a clinical interview and psychodiagnostic examination of women using the modified Experiences in Close Relationships-Revised (ECR-R) Adult Attachment questionnaire, Fraley, RC, K. Brennan and R.K. Waller, NG, & Brennan, KA, 2000) and the Multidisciplinary Scale of Perceptions of Social Support (MSPSS) (D.Zimet, 1988).
Results and discussion. Thus, it was found that women from MG at all stages of the survey are dominated by disorientation (41-46), somewhat less dependent type (35-36), autonomous (23-29), and much less - avoidance of closeness (14-16), while women from CG are predominantly autonomous type (10-13), less often dependent type (8-10) and avoidance of proximity (4-9), and much less often - disorientation (3-4). The definition of the level of social support showed the following. For MG, the mean of the major scales was as follows: “family support” - µ = 91,667 at standard deviation σ = 2,099; 2) "support for friends" - µ = 89,167 at standard deviation σ = 2,4706; 3) “support for significant others” - µ = 68,125 at standard deviation σ = 3.8; 4) "support of colleagues at work" - µ = 30,833 at standard deviation σ = 3,6324; 5) “NGO support” - µ = 11,458 at standard deviation σ = 2,3464. For GPs, the mean of the baseline scales was higher: “family support” - µ = 94.167 at standard deviation σ = 2.8576; 2) "support for friends" - µ = 91,667 at standard deviation σ = 4,3769; 3) “support for significant others” - µ = 75 at standard deviation σ = 7, 2912; 4) “support of colleagues at work” - µ = 58,333 at standard deviation σ = 7,7063; 5) “NGO support” - µ = 10,833 at standard deviation σ = 4,2689. Significant differences in the distributions of relative frequencies are revealed. At each stage of the survey, there was a decrease in the level of social support in MG compared to CG. Based on the results of a psychodiagnostic study, statistically confirmed results were found regarding higher levels of social support in women with CG.
Conclusions. Pregnancy can both change a couple's relationships for the better and worsen them. The scenario of developing relationships after pregnancy depends mainly on the predominance of constructive and non-constructive motives of conception. The threat of preterm birth is a complex psycho-traumatic situation that, if constellations of the relevant personal characteristics and lack of proper family support can become a trigger for the formation of psychological maladaptation. The development and progression of this phenomenon lead to a worsening of the emotional state of the woman and the child during the first year of life. In this case, the family is not a powerful resource, but a traumatic factor. Studying the external and internal resources of support for women with the threat of premature birth during the third trimester of pregnancy and after childbirth will prevent the negative impact of the threat of premature birth, which will significantly improve the state of psycho-emotional, marital and maternal-child spheres of women.Вступ. Вагітність може як змінювати взаємини подружжя на краще, так і погіршувати їх. Сценарій розвитку взаємин після настання вагітності залежить, головним чином, від переважання конструктивних і неконструктивних мотивів самого зачаття. На жаль, майбутні батьки, як правило, не здатні усвідомити ці закономірності, вдаючись переважно до раціоналізації та витіснення.

Методи. Дослідження проводилось протягом 2011-2019 років на базі КНП "Сторожинецька ЦРЛ" Cторожинецької районної ради у Чернівецькій області. У дослідження було включено 150 вагітних на III триместрі вагітності (23 – 37 тижнів), та їх чоловіків. Із них основну групу склали 120 жінок з із загрозою передчасних пологів на останньому триместрі вагітності (ОГ). Групу порівняння становили 30 жінок з фізіологічним перебігом вагітності (ГП). Під час дослідження проводилось клінічне інтерв’ю та психодіагностичне обстеження жінок із використанням модифікованого опитувальника «Досвід близьких стосунків» К. Бреннан та Р. К. Фрейлі (Experiences in Close Relationships-Revised (ECR-R) Adult Attachment questionnaire, Fraley, R. C., Waller, N. G., & Brennan, K. A., 2000) [190] та методики «Багатопрофільна шкала сприйняття соціальної підтримки» («MSPSS») (D.Zimet, 1988).
Результати та обговорення. Отже, встановлено, що у жінок ОГ на всіх етапах обстеження переважає дезорієантація (41-46), дещо рідше залежний тип (35-36), автономний (23-29), і значно менше – уникнення близькості (14-16), у той час, як а у жінок ГП домінує автономний тип (10-13), дешо рідше – залежний тип (8-10) та уникнення близькості (4-9), і значно рідше - дезорієнтація (3-4). Визначення рівня соціальної підтримки показало наступне. Для ОГ середнє значення основних шкалбуло наступним: «підтримка сім'ї» - µ = 91,667 при стандартному відхиленні σ = 2,099; 2) «підтримка друзів» - µ = 89,167 при стандартному відхиленні σ = 2,4706; 3) «підтримка значущих інших» - µ = 68,125 при стандартному відхиленні σ = 3.8; 4) «підтримка колег по роботі» - µ = 30,833 при стандартному відхиленні σ = 3,6324; 5) «підтримка громадських організацій» - µ = 11,458 при стандартному відхиленні σ = 2,3464. Для ГП середнє значення середнє значення основних шкал було вищим: «підтримка сім'ї» - µ = 94,167 при стандартному відхиленні σ = 2,8576; 2) «підтримка друзів» - µ = 91,667 при стандартному відхиленні σ = 4,3769; 3) «підтримка значущих інших» - µ = 75 при стандартному відхиленні σ = 7, 2912; 4) «підтримка колег по роботі» - µ = 58,333 при стандартному відхиленні σ = 7,7063; 5) «підтримка громадських організацій» - µ = 10,833 при стандартному відхиленні σ = 4,2689. Виявлено значущі відмінності в розподілах відносних частот. На кожному етапі обстеження встановлено зниженні рівня соціальної підтримки у ОГ порівняно з ГП. На основі результатів психодіагностичного дослідження виявлено статистично підтверджені результати щодо вищого рівня соціальної підтримки у жінок ГП.
Висновки. Загроза передчасних пологів є складною психотравмуючою ситуацією, яка за умови констеляцій відповідних персонологічних особливостей та відсутності належної сімейної підтримки може стати тригером формування психологічної дезадаптації. Розвиток і прогресування даного явища призводить до погіршення психоемоційного стану жінки та дитини протягом першого року життя. Сім’я у такому випадку виступає не потужним джерелом ресурсу, а психотравмуючим чинником. Вивчення зовнішніх та внутрішніх ресурсів підтримки жінок із загрозою передчасних пологів на III триместрі вагітності та після пологів дозволить попередити негативний вплив загрози передчасних пологів, що значно покращить стан психоемоційної, подружньої та материнсько-дитячої сфер жінки
