A low Lyman Continuum escape fraction of <10 per cent for extreme [O III] emitters in an overdensity at z ~ 3.5
Recent work has suggested that extreme [OIII] emitting star-forming galaxies are important to reionization. Relatedly, [O III]/[O II] has been put forward as an indirect estimator of the Lyman Continuum (LyC) escape fraction (fesc) at z ≳ 4.5 when the opaque IGM renders LyC photons unobservable. Using deep archival U-band (VLT/VIMOS) imaging of a recently confirmed overdensity at z~3.5, we calculate tight constraints on fesc for a sample (N=73) dominated by extreme [OIII] emitters.We find no LyC signal (f escrel 3, but we establish here that [O III]/[OII] > 3 does not guarantee significant LyC leakage for a population. Since even extreme star-forming galaxies are unable to produce the fesc ~ 10-15 per cent required bymost theoretical calculations for star-forming galaxies to drive reionization, there must either be a rapid evolution of fesc between z~ 3.5 and the epoch of reionization, or hitherto observationally unstudied sources [e.g. ultrafaint lowmass galaxies with log (M/M⊙) ~ 7-8.5] must make an outsized contribution to reionization
Generalized coupled common fixed point results in partially ordered A-metric spaces
Sedghi et al. (Mat. Vesn. 64(3):258-266, 2012) introduced the notion of a S-metric as a
generalized metric in 3-tuples S : X3→[0,∞), where X is a nonempty set. The aim of
this paper is to introduce the concept of an n-tuple metric A : Xn→[0,∞) and to
study its basic topological properties. We also prove some generalized coupled
common fixed point theorems for mixed weakly monotone maps in partially ordered
A-metric spaces. Some examples are presented to support the results proved herein.

Specialist training in Fiji: Why do graduates migrate, and why do they remain? A qualitative study
<p>Abstract</p> <p>Background</p> <p>Specialist training was established in the late 1990s at the Fiji School of Medicine. Losses of graduates to overseas migration and to the local private sector prompted us to explore the reasons for these losses from the Fiji public workforce.</p> <p>Methods</p> <p>Data were collected on the whereabouts and highest educational attainments of the 66 Fiji doctors who had undertaken specialist training to at least the diploma level between 1997 and 2004. Semistructured interviews focusing on career decisions were carried out with 36 of these doctors, who were purposively sampled to include overseas migrants, temporary overseas trainees, local private practitioners and public sector doctors.</p> <p>Results</p> <p>120 doctors undertook specialist training to at least the diploma level between 1997 and 2004; 66 of the graduates were Fiji citizens or permanent residents; 54 originated from other countries in the region. Among Fiji graduates, 42 completed a diploma and 24 had either completed (21) or were enrolled (3) in a master's programme. Thirty-two (48.5%) were working in the public sectors, four (6.0%) were temporarily training overseas, 30.3% had migrated overseas and the remainder were mostly in local private practice. Indo-Fijian ethnicity and non-completion of full specialist training were associated with lower retention in the public sectors, while gender had little impact. Decisions to leave the public sectors were complex, with concerns about political instability and family welfare predominating for overseas migrants, while working conditions not conducive to family life or frustrations with career progression predominated for local private practitioners. Doctors remaining in the public sectors reported many satisfying aspects to their work despite frustrations, though 40% had seriously considered resigning from the public service and 60% were unhappy with their career progression.</p> <p>Conclusion</p> <p>Overall, this study provides some support for the view that local or regional postgraduate training may increase retention of doctors. Attention to career pathways and other sources of frustration, in addition to encouragement to complete training, should increase the likelihood of such programmes' reaching their full potentials.</p
The JWST FRESCO Survey: Legacy NIRCam/Grism Spectroscopy and Imaging in the two GOODS Fields
We present the JWST Cycle 1 53.8hr medium program FRESCO, short for “First Reionization Epoch Spectroscopically Complete Observations”. FRESCO covers 62 arcmin2 in each of the two GOODS/CANDELS fields for a total area of 124 arcmin2 exploiting JWST’s powerful new grism spectroscopic capabilities at near-infrared wavelengths. By obtaining ∼2hr deep NIRCam/grism observations with the F444W filter, FRESCO yields unprecedented spectra at R ∼ 1600 covering 3.8 to 5.0 μm for most galaxies in the NIRCam field-of-view. This setup enables emission line measurements over most of cosmic history, from strong PAH lines at z ∼ 0.2 − 0.5, to Paα and Paβ at z ∼ 1 − 3, HeI and [SIII] at z ∼ 2.5 − 4.5, Hα and [NII] at z ∼ 5 − 6.5, up to [OIII] and Hβ for z∼7-9 galaxies. FRESCO’s grism observations provide total line fluxes for accurately estimating galaxy stellar masses and calibrating slit-loss corrections of NIRSpec/MSA spectra in the same field. Additionally, FRESCO results in a mosaic of F182M, F210M, and F444W imaging in the same fields to a depth of ∼28.2 mag (5 σ in 032 diameter apertures). Here, we describe the overall survey design and the key science goals that can be addressed with FRESCO. We also highlight several, early science results, including: spectroscopic redshifts of Lyman break galaxies that were identified almost 20 years ago, the discovery of broad-line active galactic nuclei at z &gt; 4, and resolved Paα maps of galaxies at z ∼ 1.4. These results demonstrate the enormous power for serendipitous discovery of NIRCam/grism observations
The Influence of Natural Barriers in Shaping the Genetic Structure of Maharashtra Populations
BACKGROUND: The geographical position of Maharashtra state makes it rather essential to study the dispersal of modern humans in South Asia. Several hypotheses have been proposed to explain the cultural, linguistic and geographical affinity of the populations living in Maharashtra state with other South Asian populations. The genetic origin of populations living in this state is poorly understood and hitherto been described at low molecular resolution level. METHODOLOGY/PRINCIPAL FINDINGS: To address this issue, we have analyzed the mitochondrial DNA (mtDNA) of 185 individuals and NRY (non-recombining region of Y chromosome) of 98 individuals belonging to two major tribal populations of Maharashtra, and compared their molecular variations with that of 54 South Asian contemporary populations of adjacent states. Inter and intra population comparisons reveal that the maternal gene pool of Maharashtra state populations is composed of mainly South Asian haplogroups with traces of east and west Eurasian haplogroups, while the paternal haplogroups comprise the South Asian as well as signature of near eastern specific haplogroup J2a. CONCLUSIONS/SIGNIFICANCE: Our analysis suggests that Indian populations, including Maharashtra state, are largely derived from Paleolithic ancient settlers; however, a more recent (∼10 Ky older) detectable paternal gene flow from west Asia is well reflected in the present study. These findings reveal movement of populations to Maharashtra through the western coast rather than mainland where Western Ghats-Vindhya Mountains and Narmada-Tapti rivers might have acted as a natural barrier. Comparing the Maharastrian populations with other South Asian populations reveals that they have a closer affinity with the South Indian than with the Central Indian populations
Unveiling the nature of infrared bright, optically dark galaxies with early JWST data
Over the last few years, both Atacama Large Millimeter/submillimeter Array (ALMA) and Spitzer observations have revealed a population of likely massive galaxies at z > 3 that was too faint to be detected in Hubble Space Telescope (HST) rest-frame ultraviolet imaging. However, due to the very limited photometry for individual galaxies, the true nature of these so-called HSTdark galaxies has remained elusive. Here, we present the first sample of such galaxies observed with very deep, high-resolution NIRCam imaging from the Early Release Science programme CEERS. 30 HST-dark sources are selected based on their red colours across 1.6–4.4 μm. Their physical properties are derived from 12-band multiwavelength photometry, including ancillary HST imaging. We find that these galaxies are generally heavily dust-obscured (AV ∼ 2 mag), massive (log (M/M☉) ∼ 10), star-forming sources at z ∼ 2-8 with an observed surface density of ∼0.8 arcmin-2. This suggests that an important fraction of massive galaxies may have been missing from our cosmic census at z > 3 all the way into the Epoch of Reionization. The HST-dark sources lie on the main sequence of galaxies and add an obscured star formation rate density of 3.2+1.8-1.3 × 10-3 M☉ yr-1 Mpc-3 at z ∼ 7, showing likely presence of dust in the Epoch of Reionization. Our analysis shows the unique power of JWST to reveal this previously missing galaxy population and to provide a more complete census of galaxies at z = 2-8 based on rest-frame optical imaging
Global patient outcomes after elective surgery: prospective cohort study in 27 low-, middle- and high-income countries.
BACKGROUND: As global initiatives increase patient access to surgical treatments, there remains a need to understand the adverse effects of surgery and define appropriate levels of perioperative care. METHODS: We designed a prospective international 7-day cohort study of outcomes following elective adult inpatient surgery in 27 countries. The primary outcome was in-hospital complications. Secondary outcomes were death following a complication (failure to rescue) and death in hospital. Process measures were admission to critical care immediately after surgery or to treat a complication and duration of hospital stay. A single definition of critical care was used for all countries. RESULTS: A total of 474 hospitals in 19 high-, 7 middle- and 1 low-income country were included in the primary analysis. Data included 44 814 patients with a median hospital stay of 4 (range 2-7) days. A total of 7508 patients (16.8%) developed one or more postoperative complication and 207 died (0.5%). The overall mortality among patients who developed complications was 2.8%. Mortality following complications ranged from 2.4% for pulmonary embolism to 43.9% for cardiac arrest. A total of 4360 (9.7%) patients were admitted to a critical care unit as routine immediately after surgery, of whom 2198 (50.4%) developed a complication, with 105 (2.4%) deaths. A total of 1233 patients (16.4%) were admitted to a critical care unit to treat complications, with 119 (9.7%) deaths. Despite lower baseline risk, outcomes were similar in low- and middle-income compared with high-income countries. CONCLUSIONS: Poor patient outcomes are common after inpatient surgery. Global initiatives to increase access to surgical treatments should also address the need for safe perioperative care. STUDY REGISTRATION: ISRCTN5181700
Iron Behaving Badly: Inappropriate Iron Chelation as a Major Contributor to the Aetiology of Vascular and Other Progressive Inflammatory and Degenerative Diseases
The production of peroxide and superoxide is an inevitable consequence of
aerobic metabolism, and while these particular "reactive oxygen species" (ROSs)
can exhibit a number of biological effects, they are not of themselves
excessively reactive and thus they are not especially damaging at physiological
concentrations. However, their reactions with poorly liganded iron species can
lead to the catalytic production of the very reactive and dangerous hydroxyl
radical, which is exceptionally damaging, and a major cause of chronic
inflammation. We review the considerable and wide-ranging evidence for the
involvement of this combination of (su)peroxide and poorly liganded iron in a
large number of physiological and indeed pathological processes and
inflammatory disorders, especially those involving the progressive degradation
of cellular and organismal performance. These diseases share a great many
similarities and thus might be considered to have a common cause (i.e.
iron-catalysed free radical and especially hydroxyl radical generation). The
studies reviewed include those focused on a series of cardiovascular, metabolic
and neurological diseases, where iron can be found at the sites of plaques and
lesions, as well as studies showing the significance of iron to aging and
longevity. The effective chelation of iron by natural or synthetic ligands is
thus of major physiological (and potentially therapeutic) importance. As
systems properties, we need to recognise that physiological observables have
multiple molecular causes, and studying them in isolation leads to inconsistent
patterns of apparent causality when it is the simultaneous combination of
multiple factors that is responsible. This explains, for instance, the
