    Recommendations on patient-facing websites regarding diagnostic imaging for low back, knee, and shoulder pain: A scoping review

    Objective To describe and synthesise the content of public-facing websites regarding the use of diagnostic imaging for adults with lower back pain, knee, and shoulder pain. Methods Scoping review conducted in accordance with PRISMA guidance. A Google search was performed to identify public-facing websites that were either United Kingdom-based, or National Health Service affiliated. The DISCERN tool was used to appraise website quality before information regarding the use of imaging was synthesised using thematic analysis. Results Eighty-six websites were included, with 48 making reference to the use of imaging. The information within the majority (n = 43) of public-facing websites aligns with best available evidence. Where there is inconsistency, this may be explained by lower website quality. Three themes were apparent regarding the use of imaging ā€“ imaging to inform diagnosis and management; imaging in context; patient experience and expectations. Conclusion The recommendations and rationale for use of imaging contained within public-facing websites does not appear to justify the increase in imaging rates for musculoskeletal pain in the UK. Innovation Publicly available information following a novel search strategy, is largely aligned with best evidence, further understanding is required to determine reasons for requesting imaging from a patient and clinician perspective

    Methane from UV-irradiated carbonaceous chondrites under simulated Martian conditions

    A UV photolytic process was studied for the production of methane from carbonaceous chondrites under simulated Martian conditions. Methane evolution rates from carbonaceous chondrites were found to be positively correlated to temperature (āˆ’80 to 20Ā°C) and the concentration of carbon in the chondrites (0.2 to 1.69 wt%); and decreased over time with Murchison samples exposed to Martian conditions. The amount of evolved methane (EM) per unit of UV energy was 7.9 Ɨ 10āˆ’13 mol Jāˆ’1 for UV irradiation of Murchison (1.69 wt%) samples tested under Martian conditions (6.9 mbar and 20Ā°C). Using a previously described Mars UV model (Moores et al., 2007), and the EM given above, an annual interplanetary dust particle (IDP) accreted mass of 2.4 Ɨ 105 kg carbon per year yields methane abundances between 2.2 to 11 ppbv for model scenarios in which 20 to 100% of the accreted carbon is converted to methane, respectively. The UV/CH4 model for accreted IDPs can explain a portion of the globally averaged methane abundance on Mars, but cannot easily explain seasonal, temporal, diurnal, or plume fluctuations of methane. Several impact processes were modeled to determine if periodic emplacement of organics from carbonaceous bolides could be invoked to explain the occurrence of methane plumes produced by the UV/CH4process. Modeling of surface impacts of high-density bolides, single airbursts of low-density bolides, and multiple airbursts of a cascading breakup of a low-density rubble-pile comet were all unable to reproduce a methane plume of 45 ppbv, as reported by Mumma et al

    Challenges in public healthcare research data warehouse integration and operationalisation.

    Objectives Public health service organisations use multiple patient administration and electronic health record systems. We describe the implementation of a data warehouse automation tool within the National Centre for Healthy Ageing (NCHA) data platform to operationalise a research data warehouse to optimise data quality and data provision for health services research. Approach The traditional data warehouse life cycle comprises repetitive manual tasks and dependency on specialist developers. Automation tools overcome most of these inefficiencies. We conducted an internal risk benefit analysis which was validated by published literature containing data warehouse optimisation and automation. Industry-based data warehouse automation tools were reviewed to align the NCHA requirements with the toolā€™s functionality. Tools were then shortlisted and evaluated over a six-week period: (1) automation of standard tasks; (2) data pipeline alignment with the World Health Organizationā€™s (WHO) Data Quality Review Framework; and (3) resource dependency risk mitigation through a Proof of Concept (PoC). Results The priority areas identified by the risk benefit analysis included: end-to-end data warehouse automation; auto scripting; connectivity/linkage with multiple sources, reverse/forward engineering, audit trail conformance, scalability, multiple data warehouse architectures support, automated documentation; data management including data quality; and post-subscription independence. Twenty scientific publications were included in the final literature review (10% within healthcare) and supported the majority of identified priority areas. The industry-based review identified 11 suitable data warehouse/Extract-Transform-Load (ETL) automation tools. Five tools demonstrated adequate performance for task automation, data quality management, reduced dependency on specialist developers and on-premise linkage compatibility. Two automation tools were tested each for 6 weeks through PoC development. One automation tool met 8 out of the 10 automation requirements and was selected for implementation. Conclusion Data warehouse development processes are complex and time consuming. Tools that offer automation of repetitive tasks and scripting increase the consistency while reducing the dependency on specialist staff.Ā  Integrated data quality management minimises the time researchers spend in pre-processing patient level data sourced through a semi-automated data warehouse

    Trabeculated (non-compacted) and compact myocardium in adults: the multi-ethnic study of atherosclerosis

    BACKGROUND: A high degree of non-compacted (trabeculated) myocardium in relationship to compact myocardium (T/M ratio >2.3) has been associated with a diagnosis of left ventricular non-compaction (LVNC). The purpose of this study was to determine the normal range of the T/M ratio in a large population-based study and to examine the relationship to demographic and clinical parameters. METHODS AND RESULTS: The thickness of trabeculation and the compact myocardium were measured in eight LV regions on long axis cardiac magnetic resonance (CMR) steady-state free precession cine images in 1000 participants (551 women; 68.1Ā±8.9 years) of the Multi-Ethnic Study of Atherosclerosis cohort. Of 323 participants without cardiac disease or hypertension and with all regions evaluable 140 (43%) had a T/M ratio >2.3 in at least one region; in 20/323 (6%), T/M>2.3 was present in more than two regions. Multivariable linear regression model revealed no association of age, gender, ethnicity, height and weight with maximum T/M ratio in participants without cardiac disease or hypertension (p>0.05). In the entire cohort (n=1000) LVEF (Ī²=āˆ’0.02/%; p=0.015), LVEDV (Ī²=0.01/ml; p=<0.0001) and LVESV (Ī²=0.01/ml; p<0.001) were associated with maximum T/M ratio in adjusted models while there was no association with hypertension or myocardial infarction (p>0.05). At the apical level T/M ratios were significantly lower when obtained on short- compared to long-axis images (p=0.017). CONCLUSIONS: A ratio of trabeculated to compact myocardium of more than 2.3 is common in a large population based cohort. These results suggest reevaluation of the current CMR criteria for LVNC may be necessary

    Stat3 oxidation-dependent regulation of gene expression impacts on developmental processes and involves cooperation with Hif-1Ī±

Reactive oxygen species are bona fide intracellular second messengers that influence cell metabolism and aging by mechanisms that are incompletely resolved. Mitochondria generate superoxide that is dis-mutated to hydrogen peroxide, which in turn oxidises cysteine-based enzymes such as phosphatases, peroxiredoxins and redox-sensitive transcription factors to modulate their activity. Signal Transducer and Activator of Transcription 3 (Stat3) has been shown to participate in an oxidative relay with peroxiredoxin II but the impact of Stat3 oxidation on target gene expression and its biological consequences remain to be established. Thus, we created murine embryonic fibroblasts (MEFs) that express either WTStat3 or a redox-insensitive mutant of Stat3 (Stat3-C3S). The Stat3-C3S cells differed from WT-Stat3 cells in morphology, proliferation and resistance to oxidative stress; in response to cytokine stimulation, they displayed elevated Stat3 tyrosine phosphorylation and Socs3 expression, implying that Stat3-C3S is insensitive to oxidative inhibition. Comparative analysis of global gene expression in WT-Stat3 and Stat3-C3S cells revealed differential expression (DE) of genes both under basal conditions and during oxidative stress. Using differential gene regulation pattern analysis, we identified 199 genes clustered into 10 distinct patterns that were selectively responsive to Stat3 oxidation. GO term analysis identified down-regulated genes to be enriched for tissue/organ development and morphogenesis and up-regulated genes to be enriched for cell-cell adhesion, immune responses and transport related processes. Although most DE gene promoters contain consensus Stat3 inducible elements (SIEs), our chromatin immunoprecipitation (ChIP) and ChIP-seq analyses did not detect Stat3 binding at these sites in control or oxidant-stimulated cells, suggesting that oxidised Stat3 regulates these genes indirectly. Our further computational analysis revealed enrichment of hypoxia response elements (HREs) within DE gene promoters, implying a role for Hif-1. Experimental validation revealed that efficient stabilisation of Hif-1Ī± in response to oxidative stress or hypoxia required an oxidation-competent Stat3 and that depletion of Hif-1Ī± suppressed the inducible expression of Kcnb1, a representative DE gene. Our data suggest that Stat3 and Hif-1Ī± cooperate to regulate genes involved in immune functions and developmental processes in response to oxidative stress
