    Consensus must be found on intravenous fluid therapy management in trauma patients

    Introduction: Trauma is an important cause of death among young people and 30-40% of this mortality rate is due to hypovolemic shock, intensified by trauma's lethal triad: Hypothermia, Acidosis, and Coagulopathy. Nurses are responsible for managing fluid therapy administration in trauma victims. The purpose of this study is to analyse the reasons why intravenous fluid therapy is recommended for trauma patients' hemodynamic stabilization. Methods: This narrative literature review included published and unpublished studies in English, Spanish or Portuguese between 1994 and January 2019. The search results were analyzed by two independent reviewers. Inclusion criteria encompasses quantitative studies involving trauma victims aged over 18 who underwent fluid therapy in a prehospital assessment context. Results&Discussion: 11 quantitative studies were included. 9 involved the use of fluid therapy for hypotension treatment and 2 of the studies analyzed involved the use of warmed fluid therapy for hypothermia treatment. The analysis performed reveals that the administration of aggressive fluid therapy seems to be responsible for the worsening of the lethal triad. In the presence of traumatic brain injury, permissive hypotension is not allowed due to the negative impact on cerebral perfusion pressure. Used as warming measure, warmed fluid therapy does not seem to have a significant impact on body temperature. Conclusions: There is no consensus regarding the administration of fluid therapy to trauma patients. This conclusion clearly supports the need to develop more randomized controlled trials in order to understand the effectiveness of such measure when it comes to control hypovolemia and hypothermia.info:eu-repo/semantics/publishedVersio

    We study metastability and nucleation for the Blume-Capel model: a ferromagnetic nearest neighbour two-dimensional lattice system with spin variables taking values in -1,0,+1. We consider large but finite volume, small fixed magnetic field h and chemical potential "lambda" in the limit of zero temperature; we analyze the first excursion from the metastable -1 configuration to the stable +1 configuration. We compute the asymptotic behaviour of the transition time and describe the typical tube of trajectories during the transition. We show that, unexpectedly, the mechanism of transition changes abruptly when the line h=2*lambda is crossed.Comment: 96 pages, 44 tex-figures, 7 postscript figure

    Retinopathy of prematurity: A review of pathophysiology and signaling pathways

    Retinopathy of prematurity (ROP) is a vasoproliferative disorder of the retina and a leading cause of visual impairment and childhood blindness worldwide. The disease is characterized by an early stage of retinal microvascular degeneration, followed by neovascularization that can lead to subsequent retinal detachment and permanent visual loss. Several factors play a key role during the different pathological stages of the disease. Oxidative and nitrosative stress and inflammatory processes are important contributors to the early stage of ROP. Nitric oxide synthase and arginase play important roles in ischemia/reperfusion-induced neurovascular degeneration. Destructive neovascularization is driven by mediators of the hypoxia-inducible factor pathway, such as vascular endothelial growth factor and metabolic factors (succinate). The extracellular matrix is involved in hypoxia-induced retinal neovascularization. Vasorepulsive molecules (semaphorin 3A) intervene preventing the revascularization of the avascular zone. This review focuses on current concepts about signaling pathways and their mediators, involved in the pathogenesis of ROP, highlighting new potentially preventive and therapeutic modalities. A better understanding of the intricate molecular mechanisms underlying the pathogenesis of ROP should allow the development of more effective and targeted therapeutic agents to reduce aberrant vasoproliferation and facilitate physiological retinal vascular development.info:eu-repo/semantics/publishedVersio

    Effect of biofertilizers and neem oil on the entomopathogenic fungi beauveria bassiana (Bals.) vuill. and metarhizium anisopliae (Metsch.) sorok.

    The in vitro fungitoxic effect of three biofertilizers, E.M.-4, Multibion Ă” and Supermagro used in organic agriculture and the neem oil (Azadirachta indica A. Juss) on the entomopathogenic fungi Metarhizium anisopliae and Beauveria bassiana was studied. These products were mixed in a medium where the two fungi were inoculated, and germination, vegetative growth and conidiogenesis were assessed. The biofertilizers Supermagro and E.M.- 4 showed to be less toxic for the two fungi whereas MultibionĂ” caused major inhibition on M. anisopliae, with reductions in germination (-37.74%), colony diameter (-30.26%) and conidiogenesis (-42.62%). Neem oil promoted a larger negative effect on B. bassiana, inhibiting germination (-45.27%), colony diameter (-36.62%) and conidiogenesis (-84.93%)

    Strategies to refrain reputation crisis during online firestorms in social media

    Purpose – Online firestorm, a relatively new phenomenon, has drawn the attention of academia due to its harmful effects. The present paper aims to provide a comparative overview with respect to the employment of two different crisis communication strategies (CCS), namely corrective action vs. apology, in the wake of the phenomenon and to avoid its further escalation. Design/methodology/approach - The present research used a 2 x 3 factorial experimental design, that comprised two levels of image repair strategies (corrective action; apology), and three levels of online firestorm triggers (unethical behaviour; core business problem; communication issue). An online questionnaire was distributed, and 564 valid responses were collected. Findings - Results show that corrective action, as image repair strategy, is more effective than an apology following core business- and communication-related incidents, whilst neither strategy shows to be more effective than the other one following unethical behaviour-related incidents. Furthermore, the effect of image repair strategy on perceptions, attitudes and behavioural intentions is moderated and mediated by users’ attribution related to companies´responsibility and brand attitude, respectively. Last, the absence of an organizational response following an incident has a negative effect on Facebook users’ brand attitude. Research limitations/implications - Fictitious scenarios depicted (no crisis history and prior reputation taken into account). Crisis response timing, crisis origin and media coverage not considered. Facebook as a SNS over Twitter. Practical implications – Contribution towards the body of knowledge of crisis communication and crisis management fields in online settings.
