Early recurrent ischemic stroke complicating intravenous thrombolysis for stroke: incidence and association with atrial fibrillation
<p><b>Background and Purpose:</b> Mechanisms of early neurologic deterioration after treatment with intravenous, recombinant, tissue-type plasminogen activator (IV rt-PA) include symptomatic intracerebral hemorrhage (SICH) and early recurrent ischemic stroke. We observed a number of cases of acute deterioration due to recurrent ischemic events.</p>
<p><b>Methods:</b> We undertook a single-center, retrospective analysis of consecutive acute stroke patients treated with IV rt-PA between January 2006 and December 2008 to define the incidence of early neurologic deterioration (>= 4-point drop on the National Institutes of Health Stroke Scale within 72 hours) and its mechanism. Deterioration was attributed to SICH when associated with a PH1 or PH2 hemorrhage on postdeterioration computed tomography scans, to recurrent ischemic stroke when there was clinical and radiologic evidence of a new territorial infarction or new vessel occlusion, and otherwise to evolution of the incident stroke.</p>
<p><b>Results:</b> Of 228 consecutive IV rt-PA-treated patients, 34 (15%) developed early neurologic deterioration, 18 (8%) secondary to incident strokes 10 (4.4%) due to SICH, and 6 (2.6%) due to early recurrent ischemic events, which were significantly associated with atrial fibrillation (present in 5 of 6 patients; 4 paroxysmal, 1 permanent). In 4 patients, sudden clinical deterioration developed during or shortly after IV rt-PA infusion, and in 2, deterioration developed 3 days later. All died 2 days to 2 weeks later. The single case without atrial fibrillation had a recurrent, contralateral, middle cerebral artery stroke during IV rt-PA infusion and multiple high-signal emboli detected by transcranial Doppler. Early recurrent ischemic stroke accounted for 5 of 12 (42%) cases of early neurologic deterioration in patients with atrial fibrillation.</p>
<p><b>Conclusion:</b> In this single-center series, the incidence of early recurrent ischemic stroke after IV rt-PA was 2.6% and was associated with previous atrial fibrillation.</p>
Blur resolved OCT: full-range interferometric synthetic aperture microscopy through dispersion encoding
We present a computational method for full-range interferometric synthetic
aperture microscopy (ISAM) under dispersion encoding. With this, one can
effectively double the depth range of optical coherence tomography (OCT),
whilst dramatically enhancing the spatial resolution away from the focal plane.
To this end, we propose a model-based iterative reconstruction (MBIR) method,
where ISAM is directly considered in an optimization approach, and we make the
discovery that sparsity promoting regularization effectively recovers the
full-range signal. Within this work, we adopt an optimal nonuniform discrete
fast Fourier transform (NUFFT) implementation of ISAM, which is both fast and
numerically stable throughout iterations. We validate our method with several
complex samples, scanned with a commercial SD-OCT system with no hardware
modification. With this, we both demonstrate full-range ISAM imaging, and

Visual inspections made by young and elderly drivers before lane changing
Lane changing is a complex driving maneuver that could challenge elderly drivers. The aim of this study was to evaluate eye glances of young and elderly active drivers when engaging lane change maneuvers. Young (21-31 years) and older (65-75 years) active drivers drove through a continuous simulated environment (STISIM v2.0). The scenario included 16 events where the driver needed to glance at three regions of interest (ROI):1) the rear-view mirror, 2)the left-side mirror, and 3) the left blind spot to ensure secure lane change. The lane change maneuvers were necessary to avoid a static object that was partially of completely blocking the lane or for overtaking a slower moving vehicle. Comparend with younger drivers, older drivers showed a constant frequency of glances across the two types of driving maneuvers (i.e., avoiding a static object and overtaking a slower vehicle), the younger drivers generally showed a higher frequency of glances and this frequency increased when overtaking a slower vehicle. A better knowledge of the elderly drivers' behavior could be beneficial in identifying at-risk behaviors and to retrain older drivers to adopt safer behaviors

LOCATE: a mouse protein subcellular localization database
We present here LOCATE, a curated, web-accessible database that houses data describing the membrane organization and subcellular localization of proteins from the FANTOM3 Isoform Protein Sequence set. Membrane organization is predicted by the high-throughput, computational pipeline MemO. The subcellular locations of selected proteins from this set were determined by a high-throughput, immunofluorescence-based assay and by manually reviewing >1700 peer-reviewed publications. LOCATE represents the first effort to catalogue the experimentally verified subcellular location and membrane organization of mammalian proteins using a high-throughput approach and provides localization data for ∼40% of the mouse proteome. The iBRA (implant breast reconstruction evaluation) study: protocol for a prospective multi-centre cohort study to inform the feasibility, design and conduct of a pragmatic randomised clinical trial comparing new techniques of implant-based breast reconstruction.
BACKGROUND: Implant-based breast reconstruction (IBBR) is the most commonly performed reconstructive procedure in the UK. The introduction of techniques to augment the subpectoral pocket has revolutionised the procedure, but there is a lack of high-quality outcome data to describe the safety or effectiveness of these techniques. Randomised controlled trials (RCTs) are the best way of comparing treatments, but surgical RCTs are challenging. The iBRA (implant breast reconstruction evaluation) study aims to determine the feasibility, design and conduct of a pragmatic RCT to examine the effectiveness of approaches to IBBR. METHODS/DESIGN: The iBRA study is a trainee-led research collaborative project with four phases:Phase 1 - a national practice questionnaire (NPQ) to survey current practicePhase 2 - a multi-centre prospective cohort study of patients undergoing IBBR to evaluate the clinical and patient-reported outcomesPhase 3- an IBBR-RCT acceptability survey and qualitative work to explore patients' and surgeons' views of proposed trial designs and candidate outcomes.Phase 4 - phases 1 to 3 will inform the design and conduct of the future RCT All centres offering IBBR will be encouraged to participate by the breast and plastic surgical professional associations (Association of Breast Surgery and British Association of Plastic Reconstructive and Aesthetic Surgeons). Data collected will inform the feasibility of undertaking an RCT by defining current practice and exploring issues surrounding recruitment, selection of comparator arms, choice of primary outcome, sample size, selection criteria, trial conduct, methods of data collection and feasibility of using the trainee collaborative model to recruit patients and collect data. DISCUSSION: The preliminary work undertaken within the iBRA study will determine the feasibility, design and conduct of a definitive RCT in IBBR. It will work with the trainee collaborative to build capacity by creating an infrastructure of research-active breast and plastic surgeons which will facilitate future high-quality research that will ultimately improve outcomes for all women seeking reconstructive surgery. The BNO-LNGS joint measurement of the solar neutrino capture rate in 71Ga
We describe a cooperative measurement of the capture rate of solar neutrinos
by the reaction 71Ga(\nu_e,e^-)71Ge. Extractions were made from a portion of
the gallium target in the Russian-American Gallium Experiment SAGE and the
extraction samples were transported to the Gran Sasso laboratory for synthesis
and counting at the Gallium Neutrino Observatory GNO. Six extractions of this
type were made and the resultant solar neutrino capture rate was 64
^{+24}_{-22} SNU, which agrees well with the overall result of the gallium
experiments. The major purpose of this experiment was to make it possible for
SAGE to continue their regular schedule of monthly solar neutrino extractions
without interruption while a separate experiment was underway to measure the
response of 71Ga to neutrinos from an 37Ar source. As side benefits, this
experiment proved the feasibility of long-distance sample transport in ultralow
background radiochemical experiments and familiarized each group with the

Measurement of the Solar Neutrino Capture Rate by the Russian-American Gallium Solar Neutrino Experiment During One Half of the 22-Year Cycle of Solar Activity
We present the results of measurements of the solar neutrino capture rate in
gallium metal by the Russian-American Gallium Experiment SAGE during slightly
more than half of a 22-year cycle of solar activity. Combined analysis of the
data of 92 runs during the 12-year period January 1990 through December 2001
gives a capture rate of solar neutrinos with energy more than 233 keV of 70.8
+5.3/-5.2 (stat.) +3.7/-3.2 (syst.) SNU. This represents only slightly more
than half of the predicted standard solar model rate of 128 SNU. We give the
results of new runs beginning in April 1998 and the results of combined
analysis of all runs since 1990 during yearly, monthly, and bimonthly periods.
Using a simple analysis of the SAGE results combined with those from all other
solar neutrino experiments, we estimate the electron neutrino pp flux that
reaches the Earth to be (4.6 +/- 1.1) E10/(cm^2-s). Assuming that neutrinos
oscillate to active flavors the pp neutrino flux emitted in the solar fusion
reaction is approximately (7.7 +/- 1.8) E10/(cm^2-s), in agreement with the
standard solar model calculation of (5.95 +/- 0.06) E10/(cm^2-s).Comment: English translation of article submitted to Russian journal Zh. Eksp.
