Contextuality-by-Default: A Brief Overview of Ideas, Concepts, and Terminology
This paper is a brief overview of the concepts involved in measuring the
degree of contextuality and detecting contextuality in systems of binary
measurements of a finite number of objects. We discuss and clarify the main
concepts and terminology of the theory called "contextuality-by-default," and
then discuss a possible generalization of the theory from binary to arbitrary

Does the national program of prevention of mother to child transmission of HIV (PMTCT) reach its target in Ouagadougou, Burkina Faso?
Background: In the context of universal access to prevention, treatment, care and support, each country has to ensure that 80% of women and children in need have access to PMTCT interventions.Objective: To assess the PMTCT program achievement in Ouagadougou, the capital city of Burkina Faso.Methods: Between August and October 2008, a cross sectional study was carried out in the five health districts of the Centre Health Region. We reviewed weekly statistics from all health care centres (HCC) to compute the coverage of PMTCT program. In 38 HCC with functional PMTCT program, we extracted data of interest from HCC registers and made direct observations of PMTCT services.Results: The PMTCT program was implemented in 49% of HCC (target for the national program: 70%). Fifteen to 31% of these centers were often in shortage for PMTCT consumables. Patientsâ privacy was not observed in 67% of Voluntary Counselling & HIV Testing wards. Care providers were not qualified enough to deliver PMTCT services. Vitamin A supplementation was not implemented. None of the facilities offered the whole package of PMTCT program interventions. HCC providing HIV testing in labour or in postnatal ward were consistently lacking. Only 86% of antenatal care new attendants benefited from pre-test counselling; 2.4% of pretested women were HIV-positive and 39% of positive mothers received antiretroviral prophylaxis.Conclusion: Coverage and quality of PMTCT programme in the Centre Health Region in Burkina Faso are still limited. Particular support is needed for training, supervision and infrastructures upgrading.Key words: vertical transmission, HIV infection, program, implementation
SIRT1 Activity Is Linked to Its Brain Region-Specific Phosphorylation and Is Impaired in Huntingtonâs Disease Mice
Huntingtons disease (HD) is a neurodegenerative disorder for which there are no disease-modifying treatments. SIRT1 is a NAD+-dependent protein deacetylase that is implicated in maintaining neuronal health during development, differentiation and ageing. Previous studies suggested that the modulation of SIRT1 activity is neuroprotective in HD mouse models, however, the mechanisms controlling SIRT1 activity are unknown. We have identified a striatum-specific phosphorylation-dependent regulatory mechanism of SIRT1 induction under normal physiological conditions, which is impaired in HD. We demonstrate that SIRT1 activity is down-regulated in the brains of two complementary HD mouse models, which correlated with altered SIRT1 phosphorylation levels. This SIRT1 impairment could not be rescued by the ablation of DBC1, a negative regulator of SIRT1, but was linked to changes in the sub-cellular distribution of AMPK-α1, a positive regulator of SIRT1 function. This work provides insights into the regulation of SIRT1 activity with the potential for the development of novel therapeutic strategies

Frey Syndrome Complicating Parotidectomy: A Case Report And Review Of Literature
The parotid gland is one of the major salivary glands located on the face. It is commonly affected by diseases necessitating its surgical removal. The facial nerve is in intimate contact with this structure and can easily be damaged during operation on this gland. Parotidectomy is the surgical operation done to remove part or the whole of the parotid gland. There are several types of parotidectomies. These may be superficial, total, total conservative, radical or extended radical. A number of complications may ensue following this procedure. Frey syndrome is one of such complications but uncommon. It is therefore the aim of this paper to present this index case with a review of literature.

A Comprehensive Analysis of Swift/XRT Data: III. Jet Break Candidates in X-ray and Optical Afterglow Lightcurves
The Swift/XRT data of 179 GRBs (from 050124 to 070129) and the optical
afterglow data of 57 pre- and post-Swift GRBs are analyzed, in order to
systematically investigate the jet-like breaks in the X-ray and optical
afterglow lightcurves. We find that not a single burst can be included in the
``Platinum'' sample, in which the data satisfy all the criteria of a jet break.
By releasing one or more requirements to define a jet break, some candidates of
various degrees could be identified. In the X-ray band, 42 out of the 103
well-sampled X-ray lightcurves have a decay slope of the post-break segment
>1.5 (``Bronze'' sample), and 27 of them also satisfy the closure relations of
the forward models (``Silver'' sample). The numbers of the ``Bronze'' and
``Silver'' candidates in the optical lightcurves are 27 and 23, respectively.
Thirteen bursts have well-sampled optical and X-ray lightcurves, but only seven
cases are consistent with an achromatic break, but even in these cases only one
band satisfies the closure relations (``Gold'' sample). The observed break time
in the XRT lightcurves is systematically earlier than that in the optical
bands. All these raise great concerns in interpreting the jet-like breaks as
jet breaks and further inferring GRB energetics from these breaks. By assuming
that these breaks are jet breaks, we perform a similar analysis as previous
work to calculate the jet opening angle (theta_j) and energetics (E_k) with the
``Silver'' and ``Gold'' jet break candidates. The derived E_K distribution
reveals a much larger scatter than the pre-Swift sample. A tentative
anti-correlation between theta_j and E_{K,iso} is found for both the pre-Swift
and Swift GRBs, indicating that the E_K could still be quasi-universal, if the
breaks in discussion are indeed jet breaks(abridge).Comment: 48 pages, including 5 tables and 8 figures. Accepted for publication

A network-oriented adaptive agent model for learning regulation of a highly sensitive personâs response
Inspired by the work of Elaine Aron, in this paper a human-like adaptive computational agent model of the internal processes of a highly sensitive person (HSP) is presented. This agent model was used to get a better understanding of what goes wrong in these internal processes once this person gets upset. A scenario is addressed where a highly sensitive person will get upset by an external stimulus and will not be able to calm down by herself. Yet in a social context the interaction with a second person (without high sensitivity) will calm the HSP down, thus contributing to regulation. To obtain an adaptive model a Hebbian learning connection was integrated. During interaction with a second person this Hebbian learning link will become stronger, which makes it possible for a HSP to become independent after some time and be able to regulate upsetting external stimuli all by herself
Precautionary labelling of foods for allergen content: are we ready for a global framework?

Mandatory allergen labelling has improved the safety of food for allergic consumers. However an additional form of voluntary labelling (termed precautionary allergen labelling) has evolved on a wide range of packaged goods, in a bid by manufacturers to minimise risk to customers, and the negative impact on business that might result from exposure to trace amounts of food allergen present during cross-contamination during production. This has resulted in near ubiquitous utilisation of a multitude of different precautionary allergen labels with subsequent confusion amongst many consumers as to their significance. The global nature of food production and manufacturing makes harmonisation of allergen labelling regulations across the world a matter of increasing importance. Addressing inconsistencies across countries with regards to labelling legislation, as well as improvement or even banning of precautionary allergy labelling are both likely to be significant steps forward in improved food safety for allergic families. This article outlines the current status of allergen labelling legislation around the world and reviews the value of current existing precautionary allergen labelling for the allergic consumer. We strongly urge for an international framework to be considered to help roadmap a solution to the weaknesses of the current systems, and discuss the role of legislation in facilitating this
