    Host-driven subspeciation in the hedgehog fungus, Trichophyton erinacei, an emerging cause of human dermatophytosis

    Altres ajuts: Czech Ministry of Health (grant NU21-05-00681)Trichophyton erinacei is a main cause of dermatophytosis in hedgehogs and is increasingly reported from human infections worldwide. This pathogen was originally described in the European hedgehog (Erinaceus europaeus) but is also frequently found in the African four-toed hedgehog (Atelerix albiventris), a popular pet animal worldwide. Little is known about the taxonomy and population genetics of this pathogen despite its increasing importance in clinical practice. Notably, whether there are different populations or even cryptic species associated with different hosts or geographic regions is not known. To answer these questions, we collected 161 isolates, performed phylogenetic and population-genetic analyses, determined mating-type, and characterised morphology and physiology. Multigene phylogeny and microsatellite analysis supported T. erinacei as a monophyletic species, in contrast to highly incongruent single-gene phylogenies. Two main subpopulations, one specific mainly to Atelerix and second to Erinaceus hosts, were identified inside T. erinacei, and slight differences in the size of microconidia and antifungal susceptibilities were observed among them. Although the process of speciation into two lineages is ongoing in T. erinacei, there is still gene flow between these populations. Thus, we present T. erinacei as a single species, with notable intraspecies variability in genotype and phenotype. The data from wild hedgehogs indicated that sexual reproduction in T. erinacei and de novo infection of hedgehogs from soil are probably rare events and that clonal horizontal spread strongly dominates. The molecular typing approach used in this study represents a suitable tool for further epidemiological surveillance of this emerging pathogen in both animals and humans. The results of this study also highlighted the need to use a multigene phylogeny ideally in combination with other inde-pendent molecular markers to understand the species boundaries of dermatophytes

    Making wine safer: the case of ochratoxin A

    This study aims to assess the risk of ochratoxin A (OTA) in European wine with the objective of reducing toxin levels through an integrated management of production and processing. All European countries of the Mediterranean basin are involved. Preliminary results indicate that OTA producing fungi are already present on grapes in the vineyard, prior to harvest. Vineyard location has more influence on OTA levels than grape variety. Weather patterns also seem to influence OTA levels.     Peanut‐induced anaphylaxis in children and adolescents: Data from the European Anaphylaxis Registry

    Background Peanut allergy has a rising prevalence in high-income countries, affecting 0.5%-1.4% of children. This study aimed to better understand peanut anaphylaxis in comparison to anaphylaxis to other food triggers in European children and adolescents. Methods Data was sourced from the European Anaphylaxis Registry via an online questionnaire, after in-depth review of food-induced anaphylaxis cases in a tertiary paediatric allergy centre. Results 3514 cases of food anaphylaxis were reported between July 2007 - March 2018, 56% in patients younger than 18 years. Peanut anaphylaxis was recorded in 459 children and adolescents (85% of all peanut anaphylaxis cases). Previous reactions (42% vs. 38%; p = .001), asthma comorbidity (47% vs. 35%; p < .001), relevant cofactors (29% vs. 22%; p = .004) and biphasic reactions (10% vs. 4%; p = .001) were more commonly reported in peanut anaphylaxis. Most cases were labelled as severe anaphylaxis (Ring&Messmer grade III 65% vs. 56% and grade IV 1.1% vs. 0.9%; p = .001). Self-administration of intramuscular adrenaline was low (17% vs. 15%), professional adrenaline administration was higher in non-peanut food anaphylaxis (34% vs. 26%; p = .003). Hospitalization was higher for peanut anaphylaxis (67% vs. 54%; p = .004). Conclusions The European Anaphylaxis Registry data confirmed peanut as one of the major causes of severe, potentially life-threatening allergic reactions in European children, with some characteristic features e.g., presence of asthma comorbidity and increased rate of biphasic reactions. Usage of intramuscular adrenaline as first-line treatment is low and needs to be improved. The Registry, designed as the largest database on anaphylaxis, allows continuous assessment of this condition

    Anaphylaxis is a severe allergic reaction with a rapid onset and it is potentially life-threatening. Its clinical manifestations are varied; they may affect the skin, the cardiovascular system, the respiratory system, and the digestive system, among others. The treatment of choice, which is an intra-muscular injection of epinephrine (adrenaline), must be applied promptly. Therefore, being prepared to recognize it properly is of crucial importance. The objective of this clinical practice guide is to improve the knowledge of health professionals about anaphylaxis and, consequently, to optimize the treatment and long-term management of this reaction. This guide is adapted to the peculiarities of Latin America; especially in matters regarding the treatment. The need to introduce epinephrine auto-injectors in countries that don't have them yet is highlighted

    Engaged, but not immersed: Tracking the mediated public connection of Filipino elite migrants in London

    While most of the literature highlights the social, economic and cultural aspects of Filipino migration, this study explores its political dimension by focusing on the public connection of Filipino elite migrants in London. Unlike other types of Filipino migrants, such as Americanized balikbayans and ‘low-skilled’ labour migrants, elite migrants are expected to return physically to the homeland as part of their nationalistic duty to ‘lead the nation’. From their interviews and participant observation, the authors discover that overseas scholars indeed maintain a strong interest in homeland political issues through heavy news consumption on the Internet. However, this has also fostered an ambiguous kind of public connection. On the one hand, elite migrants remain engaged with issues that they hope to address on their eventual return, but on the other hand they are not immersed with ‘other’ Filipino people in the diaspora. Their political engagement involves talk and mediated conversations with limited face-to-face collaborations with other migrants. This kind of public connection lends itself to longdistance particularistic communication and a great volume of discussion, but limited and short-term forms of public activity. The authors argue that elite migrants' practices of political engagement are inscribed in continuing socio-historical – and fundamentally classed – divides in Philippine society. Further, rather than enabling cross-class communications and connections, the media are frequently used by elite migrants to maintain political, economic, social and cultural divides

    La inmunoterapia sigue siendo, hoy en día, uno de los principales pilares del tratamiento de una buena parte de las afecciones respiratorias alérgicas y de la hipersensibilidad al veneno de los himenópteros. El manejo correcto de este tipo de tratamiento requiere una formación adecuada, que debería impartirse durante el período de médico interno residente en alergología. Se describen en este informe los resultados obtenidos de una encuesta realizada a residentes de alergología sobre su percepción del manejo de la inmunoterapia tanto a nivel personal como en sus servicios de alergia. Se comentan las principales deficiencias observadas con el fin de tomar conciencia de ellas y mejorarlas. Se realiza un análisis comparativo con los resultados de una encuesta similar presentados en el XX Congreso Nacional de la SEAIC de 1996
