    Les pseudo nodules thyroidiens

    Buts : Rappeler les particularités cliniques et paracliniques des différentes étiologies des pseudonodules thyroïdiens et discuter les moyens de  diagnostic positif et de prise en charge thérapeutique.Matériels et méthodes : il s'agit de 5 observations de patientes porteuses de pseudo-nodules thyroïdiens colligées au service d'oRL et de CCF du ChU Farhat hached de Sousse. L'âge variait entre 21 et 52 ans. dans tous les cas, le motif de consultation était une tuméfaction basicervicale  associée à des signes de compression (dysphagie, dyspnée et/ou dysphonie). Une échographie et une scintigraphie thyroïdiennes ont été réalisées dans tous les cas. La cytoponction n'a été pratiquée que dans trois cas.Résultats : La tuméfaction basicervicale était mobile à la déglutition dans tous les cas. L'échographie et la scintigraphie ont évoqué une lésion thyroïdienne dans tous les cas. La cytoponction n'a été contributive dans aucun des trois cas. Après cervicotomie exploratrice et étude histologique, le diagnostic était, selon les cas, un kyste hydatique, un schwannome,un adénome parathyroïdien, un kyste du tractus thyréoglosse et un chondrosarcome cricoïdien.Conclusion: Les pseudo- nodules thyroïdiens sont des nodules cervicaux péri-thyroïdiens qui constituent des situations cliniques exceptionnelles voire anecdotiques que le chirurgien doit savoir évoquer. L'examen cytologique fait par une personne entraînée peut être contributif au diagnostic différentiel.     Angiofibrome Nasopharyngien Compliqué D\'un Syndrome De Foster Kennedy : Place De La Chimiothérapie

    L\'angiofibrome nasopharyngien (FNP) est une tumeur vasculaire bénigne localement agressive. Il représente 0,05% des tumeurs de la tête et du cou et touche presque exclusivement les adolescents de sexe masculin. La chirurgie est considérée comme le traitement principal du FNP, les autres modalités thérapeutiques tels que radiothérapie et chimiothérapie sont indiquées en cas d\'extension intracrânienne avec envahissement du sinus caverneux ou de la carotide interne. Nous rapportons une observation rare de FNP, compliqué d\'un syndrome de Foster Kennedy chez une femme de 34 ans. Le traitement a consisté en une chimiothérapie (adriamycine, décarbazine) suivie d\'une radiothérapie. Nous discutons de l\'intérêt et des résultats de la chimiothérapie dans le traitement des FNP avec une importante extension intracrânienne (stade III B de la classification de Radkowski).Nasopharyngeal angiofibroma is a locally aggressive, although histologically benign, vascular neoplasm. This neoplasm accounts for 0.05% of the head and neck tumours and affects almost exclusively male adolescents. Surgery is considered as the primary treatment of nasopharyngeal angiofibroma. Other treatment modalities such as radiotherapy and chemotherapy are still recommended for intracranial extension involving the cavernous sinus or the internal carotid artery. We report a rare case of nasopharyngeal angiofibroma, further complicated with a Foster Kennedy syndrome in a 34- years-old woman. The treatment consisted in a chemotherapy (adriamycine, decarbazine) followed by radiotherapy. We discuss the relevance and the outcome of chemotherapy in the treatment of the nasopharyngeal angiofibromas with a consistent intracranial extension (stage III B of Radkowski\'s classification).     L’améloblastome est la tumeur odontogénique la plus fréquente. il  représente 1% des tumeurs mandibulaires et maxillaires et atteint la mandibule dans 80% des cas. Son traitement se base essentiellement sur la chirurgie lorsque cela est possible. La radiothérapie est réservée aux formes inopérables localement évoluées et métastatiques.     Adenome pleomorphe des glandes salivaires accessoires

    Objective : Pleomorphic adenoma (PA) is the most common benign neoplasm of the minor salivary glands. Clinical behavior,diagnosis, treatment modalities and evolution of nine cases of pleomorphic adenoma were discussed.Materials and methods : This is a descriptive and retrospective study about 9 cases of pleomorphic adenoma conductedover a period of 20 years (1990 Ă  2009) at the ENT department of Sousse (Tunisia).Results : There were 5 men and 4 women having a mean age of 40 years. The main site of this adenoma was the hard palate. The others 3 tumors were seen in the nasal cavity, in the parapharyngeal space and in cervical soft tissues. Clinical examination showed a swelling at the level of the interested region. Computed tomography (CT) was made in 5 cases,associated to the magnetic resonance imaging (MRI) in 2 cases. The treatment was based in surgical excision of the tumors. Histologically, the tumor showed features of a benign PA in 8 cases and of malignant PA in 1 case. Two patients had a documented local recurrence. The evolution was good for the first patient after secondary excision of the tumor.The patient presenting a carcinoma ex pleomorphic adenoma developed quickly pulmonary and cerebral metastases.Conclusion : Pleomorphic adenoma of minor salivary gland is generally known to be a slow-growing tumour. They must be evoked before all painless swelling in the upper aerodigestive tract developed under a healthy mucosa. The surgical management is basically total resection of the tumor with a safety margin of 5 mm. Longer follow-up was needed due to the possibility of recurrences and malignancy transformation.Keywords : Pleomorphic adenoma, Minor salivary gland, Imaging

    Volatile Constituents And Antimicrobial Activity Of Lavandula Stoechas L. Oil From Tunisia

    International audienceAn oil obtained from the dried leaves of Lavandula stoechas L. in 0.77% yield was analyzed by capillary GC and GUMS. Fenchone (68.2%) and camphor (11.2%) were the main components of the 28 identified molecules. This oil has been tested for antimicrobial activity against six bacteria, and two fungi. The results showed that this oil was active against all of the tested strains; Staphylococcus aureus was the more sensitive strain

    Unlocking preservation bias in the amber insect fossil record through experimental decay.

    Fossils entombed in amber are a unique resource for reconstructing forest ecosystems, and resolving relationships of modern taxa. Such fossils are famous for their perfect, life-like appearance. However, preservation quality is vast with many sites showing only cuticular preservation, or no fossils. The taphonomic processes that control this range are largely unknown; as such, we know little about potential bias in this important record. Here we employ actualistic experiments, using, fruit flies and modern tree resin to determine whether resin type, gut microbiota, and dehydration prior to entombment affects decay. We used solid phase microextraction gas chromatography-mass spectrometry (SPME GC-MS) to confirm distinct tree resin chemistry; gut microbiota of flies was modified using antibiotics and categorized though sequencing. Decay was assessed using phase contrast synchrotron tomography. Resin type demonstrates a significant control on decay rate. The composition of the gut microbiota was also influential, with minor changes in composition affecting decay rate. Dehydration prior to entombment, contrary to expectations, enhanced decay. Our analyses show that there is potential significant bias in the amber fossil record, especially between sites with different resin types where ecological completeness and preservational fidelity are likely affected

    Does varicocele repair improve conventional semen parameters? A meta-analytic study of before-after data

    Purpose The purpose of this meta-analysis is to study the impact of varicocele repair in the largest cohort of infertile males with clinical varicocele by including all available studies, with no language restrictions, comparing intra-person conventional semen parameters before and after the repair of varicoceles. Materials and Methods The meta-analysis was performed according to PRISMA-P and MOOSE guidelines. A systematic search was performed in Scopus, PubMed, Cochrane, and Embase databases. Eligible studies were selected according to the PICOS model (Population: infertile male patients with clinical varicocele; Intervention: varicocele repair; Comparison: intra-person before-after varicocele repair; Outcome: conventional semen parameters; Study type: randomized controlled trials [RCTs], observational and case-control studies). Results Out of 1,632 screened abstracts, 351 articles (23 RCTs, 292 observational, and 36 case-control studies) were included in the quantitative analysis. The before-and-after analysis showed significant improvements in all semen parameters after varicocele repair (except sperm vitality); semen volume: standardized mean difference (SMD) 0.203, 95% CI: 0.129–0.278; p<0.001; I2=83.62%, Egger’s p=0.3329; sperm concentration: SMD 1.590, 95% CI: 1.474–1.706; p<0.001; I2=97.86%, Egger’s p<0.0001; total sperm count: SMD 1.824, 95% CI: 1.526–2.121; p<0.001; I2=97.88%, Egger’s p=0.0063; total motile sperm count: SMD 1.643, 95% CI: 1.318–1.968; p<0.001; I2=98.65%, Egger’s p=0.0003; progressive sperm motility: SMD 1.845, 95% CI: 1.537%–2.153%; p<0.001; I2=98.97%, Egger’s p<0.0001; total sperm motility: SMD 1.613, 95% CI 1.467%–1.759%; p<0.001; l2=97.98%, Egger’s p<0.001; sperm morphology: SMD 1.066, 95% CI 0.992%–1.211%; p<0.001; I2=97.87%, Egger’s p=0.1864. Conclusions The current meta-analysis is the largest to date using paired analysis on varicocele patients. In the current meta-analysis, almost all conventional semen parameters improved significantly following varicocele repair in infertile patients with clinical varicocele.
