    Severe inflammatory disease activity 14 months after cessation of Natalizumab in a patient with Leber’s optic neuropathy and multiple sclerosis – a case report

    Background Leber’s hereditary optic neuropathy (LHON) co-occuring with multiple sclerosis-like disease (LHON-MS) is suggested to be a separate disease entity denoted Harding’s disease. Little is known about the response to initiation and discontinuation of potent immunomodulatory treatment in LHON-MS. Case presentation We describe a LHON-MS patient with 27 years disease duration who developed severe disease activity peaking 14 months after discontinuation of natalizumab, with extensive new inflammatory lesions throughout the brain and in the spinal cord resembling immune inflammatory reconstitution syndrome. She had previously been clinically and radiologically stable on natalizumab treatment for 6 years, and before that only experienced subtle clinical activity during 9 years on interferon beta1a. Conclusion This is the first report on severe exacerbation of inflammatory disease activity after discontinuation of natalizumab in LHON-MS, and suggests that late rebound activity can occur in these patients

    Design and Characterization of Three Light-Weight Multi-principal-Element Alloys Potentially Candidates as High-Entropy Alloys

    Trabajo presentado en el Frontiers in Materials Processing Applications, Research and Technology (FIMPAR17), celebrado en Burdeos (Francia) del 9 al 12 de Julio de 2017The feasibility of preparing high entropy alloys in the Al-Sc-Ti-V-Cr system has been evaluated taking into account the different criteria reported in the literature. Based on such criteria, three Al-Sc-Ti-V-Cr alloys were chosen with contents of each element varying from 10 to 35 at. %, and prepared by arc melting. All alloys exhibit a two-phase dendritic microstructure, with the major dendritic phase being a bcc solid solution enriched in Ti, V, or Cr. Scandium is strongly rejected from the dendrites towards the interdendritic regions to form Al2Sc in the three alloys. The experimental results indicate that neither empirical criteria used nor THERMOCALC calculus tool can predict properly the formation of a single solid nor the nature of the existing phases, respectively

    Commonplace syntactic constructions in natural language seem to generate ontological commitments to a dazzling array of metaphysical categories - aggregations, sets, ordered n-tuples, possible worlds, intensional entities, ideal objects, species, intensive and extensive quantities, stuffs, situations, states, courses of events, nonexistent objects, intentional and discourse objects, general objects, plural objects, variable objects, arbitrary objects, vague kinds and concepts, fuzzy sets, and so forth. But just because a syntactic construction in some natural language appears to invoke a new category of entity, are we theoreticians epistemically justified in holding that there are such entities? This would hardly seem sufficient. To be epistemically justified, the ontology to which we theoreticians are committed must pass strict standards: the entities must be of the sort required by our best comprehensive theory of the world. The thesis of this paper is that fine-grained type-free intensional entities are like this. If the thesis is right, these entities have a special objective status perhaps not possessed by some of the other ontological categories associated with special syntactic constructions in natural language. In fact, it is plausible to hold that fine-grained type-free intensional entities provide the proper minimal framework for constructing logical and linguistic theories. In this paper my strategy will be to survey the competing conceptions of fine-grained type-free intensionality and to present arguments in support of one of them. Following this narrowing down process, I will go on to the indicated epistemological considerations

    Research, technology frontier and productivity growth

    ABSTRACT: This paper uses the Jones (1995) framework to examine the contribution of imitation activities and innovative research effort on productivity growth for the US and some European leading economies. We carry out a comparative analysis for the last 50 years, with two model specifications, assuming country differences in the parameters associated with R&D effort. In the first one, the technological frontier position is determined by the country with the highest productivity, the United States. Alternatively, in the second specification, we alter the definition of the technological frontier, allowing it to transcend the leader. The empirical analysis leads to very different outcomes. The first specification estimation, using GMM techniques, indicates that American researchers are more technology growth enhancing than their European counterparts. In contrast, the results obtained for the second, using Kalman’s filter, show that when using an alternative definition of technological frontier, it is possible to observe a boost in innovation that reduces the dispersion among countries. Then, the leading European countries can take advantage; in this case, Germany exhibits the best performance, followed by the US.Financial support from the Spanish Ministry of Science and Innovation under the project ECO2009-09569/ECON is gratefully acknowledged

    An answered call for aid? Cannabinoid clinical framework for the opioid epidemic

    Abstract Background The opioid crisis continues in full force, as physicians and caregivers are desperate for resources to help patients with opioid use and chronic pain disorders find safer and more accessible non-opioid tools. Main body The purpose of this article is to review the current state of the opioid epidemic; the shifting picture of cannabinoids; and the research, policy, and current events that make opioid risk reduction an urgent public health challenge. The provided table contains an evidence-based clinical framework for the utilization of cannabinoids to treat patients with chronic pain who are dependent on opioids, seeking alternatives to opioids, and tapering opioids. Conclusion Based on a comprehensive review of the literature and epidemiological evidence to date, cannabinoids stand to be one of the most interesting, safe, and accessible tools available to attenuate the devastation resulting from the misuse and abuse of opioid narcotics. Considering the urgency of the opioid epidemic and broadening of cannabinoid accessibility amidst absent prescribing guidelines, the authors recommend use of this clinical framework in the contexts of both clinical research continuity and patient care
