Гемотрансфузионная терапия при хирургическомлечении больных с метастатическими поражениямипозвоночника и с позвоночно-спинномозговойтравмой

The comparative analysis of 40 case reports of patients with vertebrocerebrospinal trauma and 60 - with the metastatic lesions of spine detected that maximum hemorrhage is observed at the carrying-out of spondyloectomy with the plasty of interbody defect and spondylosynthesis on account of metastatic lesions. At surgical interventions on account of vertebrocerebrospinal trauma the application of preoperational blood autosampling, techniques of intraoperative collection and reinfusion of blood using apparatus Cell Saver, the collection of draining losses and their return by sac Handy Vak allow to lower the extent of hemotransfusion therapy by the preparations of donor blood. The connection between the kind of decompressive-stabilizing spine intervention and hemorrhage volume was detected. It allows to plan the operation, anesthetic and operative risks, measures to provide surgical intervention anesthetically.Сравнительный анализ 40 историй болезни пациентов с позвоночно-спинномозговой травмой (ПСМТ) и 60 - с метастатическими поражениями позвоночника показал, что максимальная кровопотеря наблюдается при выполнении спондилоэктомии с пластикой межтелового дефекта и спондилосинтезом по поводу метастатических поражений. При оперативных вмешательствах по поводу ПСМТ применение предоперационного аутозабора крови, методик интраоперационного сбора и реинфузии крови с помощью аппарата Cell Saver, сбор дренажных потерь и их возврат мешком Handy Vak позволяет снижать объём гемотрансфузионной терапии препаратами донорской крови. Выявлена связь между видом декомпрессивно-стабилизирующего вмешательства на позвоночнике и объёмом кровопотери, что позволяет планировать операцию, анестезиологический и операционный риски, мероприятия по анестезиологическому обеспечению оперативного вмешательства

ДИАГНОСТИКА И ЛЕЧЕНИЕ ДОБРОКАЧЕСТВЕННЫХ ГИПЕРВАСКУЛЯРНЫХ ОБРАЗОВАНИЙ ПЕЧЕНИ У ДЕТЕЙ

Benign liver tumors in children include neoplasms, unique only to childhood, as well as tumors that are more specific for adults. Although primary tumors of the liver constitute a small percentage of solid tumors in children, they are quite common in practice, pediatric surgeon and radiologist. The most common benign non-cystic liver in children education in descending order - infantile hemangioendothelioma, FNG, mesenchymal hamartoma, nodular regenerative hyperplasia and hepatocellular adenoma. Currently there is no single view of terminology, classification, diagnosis and treatment of benign hypervascular liver formations. The paper presents our views on this issue, the proposed algorithms for diagnosis and treatment.Доброкачественные опухоли печени у детей включают новообразования, уникальные только для детского возраста, а также опухоли, более характерные для взрослых. Хотя первичные новообразования печени составляют малый процент солидных опухолей у детей, они довольно часто встречаются в практике радиолога и детского хирурга. Наиболее часто встречающиеся доброкачественные некистозные образования печени у детей в порядке убывания - инфантильная гемангиоэндотелиома, ФНГ, мезенхимальная гамартома, нодулярная регенераторная гиперплазия и гепатоцеллюлярная аденома. В настоящее время отсутствует единый взгляд на терминологию, классификацию, методы диагностики и лечения доброкачественных гиперваскулярных образований печени. В статье приведены взгляды авторов на данную проблему и предлагаемые алгоритмы диагностики и лечения

Evaluation of the wound healing capacity of the gel form of the preparation "Mellisol" on the model of experimental planar wounds

Objective: to study the wound healing activity of the gel form of the "Mellisol" preparation containing nanoaggregates of flavonoids stabilized with polyazolidine ammonium modified with iodine hydrate ions on a model of experimental planar wounds of rats. Material and Methods. An investigation of the wound healing activity of the gel form of the "Mellisol" preparation was carried out in vivo in 18 white laboratory rats of the Wistar line. Simulation of experimental planar wounds was performed according to the standard procedure described in the preclinical testing manual. Results. In the course of the studies, it was established that a statistically significant (p=0.045) acceleration of healing on the 12th day of the experiment occurred in animals of the experimental group on whose wounds the gel form of the cosmetic means "Mellisol" was applied, and the complete healing of the wound defect occurred on the 21st day after operation. Conclusion. The gel form of the preparation "Mellisol" is characterized by a high level of antimicrobial and regenerative activity reliably reducing the healing time for planar wounds without the development of microbial complications.</p
