    Purpose: Being overweight/obese, having hypertension, and being postmenopausal are risk factors for the development of congestive heart failure (CHF). A characteristic of CHF is an abnormal V?E/V?CO2 slope, which is predictive of mortality in patients with CHF. Although the V?E/V?CO2 slope is well established in CHF patients, little is known regarding interventions for 'at-risk' populations. Methods: We examined the V?E/V?CO2 slope in 401 sedentary, overweight, moderately hypertensive women randomized to 6 m of nonexercise (control) or 4 kcal·kg?1·wk?1 (KKW), 8 KKW, or 12 KKW of exercise at an intensity corresponding to 50% of baseline V?O2max. We examined trends in exercise treatment dose versus change in mean V?E/V?CO2 slope using a linear regression model (KKW vs V?E/V?CO2 slope) and a linear mixed model. Results: Regression analysis showed a significant trend for a reduction in the V?E/V?CO2 slope from baseline (mean ± SD: 32.6 ± 6.3; P < 0.004). When expressed as mean change (95% confidence interval (CI)) from baseline, we observed significant reductions in the V?E/V?CO2 slope for the 8-KKW (?1.14; 95% CI, ?1.5 to ?0.2) and 12-KKW (?1.67; 95% CI, ?2.3 to ?0.3) groups. No significant effect was noted for the 4-KKW (?0.4; 95% CI, ?1.2 to 0.15) group. Conclusion: Moderate-intensity aerobic exercise at doses of 8 KKW or greater seems to present an adequate dose of exercise to promote small but significant reductions in the V?E/V?CO2 slope in postmenopausal women who exhibit risk factors associated with the development of CHF

    The Obesity Paradox and Cardiorespiratory Fitness

    Cardiorespiratory fitness as an explanation for the obesity paradox warrants further examination. We evaluated independent and joint associations of cardiorespiratory fitness and adiposity with all-cause mortality in 811 middle-aged (age, 53.3 ± 7.2 years) male never smokers without documented cardiopulmonary disease or diabetes from the Veterans Exercise Testing Study (VETS). Cardiorespiratory fitness was quantified in metabolic equivalents (METs) using final treadmill speed and grade achieved on a maximal exercise test. Subjects were grouped for analysis by METs: unfit (lowest third) and fit (upper two-thirds); and by body mass index (kg/m2): nonobese (18.5−29.9) and obese (≥30.0). Associations of baseline fitness and adiposity measures with all-cause mortality were determined by Cox proportional hazards analysis adjusted for age, ethnicity, hypertension, hypercholesterolemia, family history of coronary artery disease, and cardiovascular medication use. In multivariate analysis, mortality risk for obese/fit men did not differ significantly from the nonobese/fit reference group. However, compared to the reference group, nonobese and obese unfit men were 2.2 (P = 0.01) and 1.9 (P = 0.03) times more likely to die, respectively.     Universal Holographic Chiral Dynamics in an External Magnetic Field

    In this work we further extend the investigation of holographic gauge theories in external magnetic fields, continuing earlier work. We study the phenomenon of magnetic catalysis of mass generation in 1+3 and 1+2 dimensions, using D3/D7- and D3/D5-brane systems, respectively. We obtain the low energy effective actions of the corresponding pseudo Goldstone bosons and study their dispersion relations. The D3/D7 system exhibits the usual Gell-Mann--Oakes--Renner (GMOR) relation and a relativistic dispersion relation, while the D3/D5 system exhibits a quadratic non-relativistic dispersion relation and a modified linear GMOR relation. The low energy effective action of the D3/D5 system is related to that describing magnon excitations in a ferromagnet.     Bayesian High-Redshift Quasar Classification from Optical and Mid-IR Photometry

    We identify 885,503 type 1 quasar candidates to i<22 using the combination of optical and mid-IR photometry. Optical photometry is taken from the Sloan Digital Sky Survey-III: Baryon Oscillation Spectroscopic Survey (SDSS-III/BOSS), while mid-IR photometry comes from a combination of data from the Wide-Field Infrared Survey Explorer (WISE) "ALLWISE" data release and several large-area Spitzer Space Telescope fields. Selection is based on a Bayesian kernel density algorithm with a training sample of 157,701 spectroscopically-confirmed type-1 quasars with both optical and mid-IR data. Of the quasar candidates, 733,713 lack spectroscopic confirmation (and 305,623 are objects that we have not previously classified as photometric quasar candidates). These candidates include 7874 objects targeted as high probability potential quasars with 3.5<z<5 (of which 6779 are new photometric candidates). Our algorithm is more complete to z>3.5 than the traditional mid-IR selection "wedges" and to 2.2<z<3.5 quasars than the SDSS-III/BOSS project. Number counts and luminosity function analysis suggests that the resulting catalog is relatively complete to known quasars and is identifying new high-z quasars at z>3.     Speed and grade increment during cardiopulmonary treadmill testing : impact on exercise prescription

    Background: Maximal oxygen uptake (VO2max) and both first (VT1) and second (VT2) thresholds have been used as reference points for exercise prescription in different populations. Objective: We aimed to test the hypothesis that exercise prescription, based on VTs determined by treadmill cardiopulmonary exercise testing (CPET), is influenced by the rate of increase in treadmill workload. Methods: Nine healthy individuals underwent two CPETs, followed by two sessions of submaximal exercise, both in randomized order. For the “speed” protocol, there was an increment of 0.1 to 0.3 km.h-1 every 15s. The “grade” incremental protocol increased 1% every 30s and 0.1 km.h-1 every 45s. This was followed by submaximal exercise sessions lasting 40min at an intensity corresponding to heart rate (HR) between the VT1 and VT2. Results: The “speed” protocol resulted in higher VT1 (p = 0.01) and VT2 (p = 0.02) when compared to the “grade” incremental protocol, but there was no effect on VO2max. The target HR for the submaximal exercise sessions was higher in the “speed” protocol compared to the “grade” incremental protocol (p < 0.01) and remained stable during the two steady-state exercise sessions. Blood lactate remained stable during the submaximal exercise sessions, with higher values observed during the “speed” protocol than those “grade” incremental protocol (p < 0.01). Conclusions: Compared to a grade-based protocol, a speed-based protocol resulted in higher VT1 and VT2, which significantly affected cardiorespiratory and metabolic responses to prescribed exercise intensity in healthy young adults

    A Leptin-regulated Circuit Controls Glucose Mobilization During Noxious Stimuli

    Adipocytes secrete the hormone leptin to signal the sufficiency of energy stores. Reductions in circulating leptin concentrations reflect a negative energy balance, which augments sympathetic nervous system (SNS) activation in response to metabolically demanding emergencies. This process ensures adequate glucose mobilization despite low energy stores. We report that leptin receptor–expressing neurons (LepRb neurons) in the periaqueductal gray (PAG), the largest population of LepRb neurons in the brain stem, mediate this process. Application of noxious stimuli, which often signal the need to mobilize glucose to support an appropriate response, activated PAG LepRb neurons, which project to and activate parabrachial nucleus (PBN) neurons that control SNS activation and glucose mobilization. Furthermore, activating PAG LepRb neurons increased SNS activity and blood glucose concentrations, while ablating LepRb in PAG neurons augmented glucose mobilization in response to noxious stimuli. Thus, decreased leptin action on PAG LepRb neurons augments the autonomic response to noxious stimuli, ensuring sufficient glucose mobilization during periods of acute demand in the face of diminished energy stores

    Sum rules, plasma frequencies and Hall phenomenology in holographic plasmas

    We study the AC optical and hall conductivities of Dp/Dq-branes intersections in the probe approximation and use sum-rules to study various associated transport coefficients. We determine that the presence of massive fundamental matter, as compared to massless fundamental matter described holographically by a theory with no dimensional defects, reduces the plasma frequency. We further show that this is not the case when the brane intersections include defects. We discuss in detail how to implement correctly the regularization of retarded Green's functions so that the dispersion relations are satisfied and the low energy behaviour of the system is physically realistic.Comment: 25 pages, 5 figures. v2.minor changes, published versio

    A note on conductivity and charge diffusion in holographic flavour systems

    We analyze the charge diffusion and conductivity in a Dp/Dq holographic setup that is dual to a supersymmetric Yang-Mills theory in p+1 dimensions with N_f<< N_c flavour degrees of freedom at finite temperature and nonvanishing U(1) baryon number chemical potential. We provide a new derivation of the results that generalize the membrane paradigm to the present context. We perform a numerical analysis in the particular case of the D3/D7 flavor system.     Chiral phase transitions and quantum critical points of the D3/D7(D5) system with mutually perpendicular E and B fields at finite temperature and density

    We study chiral symmetry restoration with increasing temperature and density in gauge theories subject to mutually perpendicular electric and magnetic fields using holography. We determine the chiral symmetry breaking phase structure of the D3/D7 and D3/D5 systems in the temperature-density-electric field directions. A magnetic field may break the chiral symmetry and an additional electric field induces Ohm and Hall currents as well as restoring the chiral symmetry. At zero temperature the D3/D5 system displays a line of holographic BKT phase transitions in the density-electric field plane, while the D3/D7 system shows a mean-field phase transition. At intermediate temperatures, the transitions in the density-electric field plane are of first order at low density, transforming to second order at critical points as density rises.     Longitudinal and age trends of metabolic syndrome and its risk factors: The Family Heart Study

    BACKGROUND: We report longitudinal changes in the metabolic syndrome (MetS) in 2,458 participants from 480 families in the Family Heart Study. Participants were examined between 1994–96 (FHS-T1) and 2002–03 (FHS-T2), about 7.4 years apart. Additionally, the impact of medication on estimates of MetS prevalence, and associations of MetS with prevalent coronary heart disease (CHD) and type 2 diabetes (T2D) were studied. METHODS: Three definitions for MetS prevalence were considered. One represented the original (o) National Cholesterol Education Program (NCEP) MetS criteria. Two others considered the confounding of medications effects, respectively (m) lipid medications constituted a categorical diagnostic criterion for lipids variables, and (c) lipids and blood pressure variables were corrected with average clinical trials medications effects. Logistic regression of MetS on CHD and T2D, as well as the trend analysis of MetS by age, were performed. RESULTS: MetS increased from 17.1% in FHS-T1(o) to 28.8% in FHS-T2(o); from 19.7% in FHS-T1(m) to 42.5% in FHS-T2(m); and from 18.4% in FHS-T1(c) to 33.6% in FHS-T2(c). While we observed adverse changes in all risk factors, the greatest increase was for waist circumference (25%). The percentages of MetS were about 2 to almost 3 times higher in ages 50 years and older than in younger ages. The odds of having prevalent CHD were about 2.5 times higher in the subjects classified with MetS than without. CONCLUSION: MetS percentages increased noticeably longitudinally and cross-sectionally with older age. These conclusions were reached with and without considering medication use, but correcting risk factors for medications use affects the MetS prevalence estimates.
