The value of dobutamine stress echocardiography in predicting clinical improvement following coronary artery bypass grafting in patients with left ventricular systolic dysfunction
Background: Recent years have seen an increasing number of patients with multivessel
coronary artery disease and left ventricular systolic dysfunction being qualified for cardiac
surgery. Identification of patients who are likely to benefit most from revascularisation procedures
poses a considerable problem. The aim of the study was to assess the value of dobutamine
stress echocardiography in predicting the clinical course following coronary artery bypass
grafting (CABG) in patients with ischaemic left ventricular dysfunction.
Methods: Fifty patients with multivessel coronary artery disease and reduced left ventricular
ejection fraction (LVEF < 40%) who were qualified for CABG were included in the study.
Resting echocardiography and a small-dose (5–10 µg/kg/min) dobutamine test were performed
before the procedure. Subsequent tests were repeated after the procedure and at 3.6 and 12 months.
A combined endpoint of death, repeat hospitalisations, NYHA stage, severity of angina and left
ventricular systolic function was evaluated.
Results: Two factors were found to affect the prognosis adversely following CABG in both
univariate and multivariate analysis: a history of hypertension (p = 0.039, OR 4.9, 95% CI
1.4-17.1) and lack of improvement in contractility in at least 4 segments during the
dobutamine test (p = 0.0003, OR 37.2, 95% CI 6.3-218.4). An improvement in contractility
in at least 4 segments of the left ventricle is the most important prognostic factor.
Conclusions: The results of the dobutamine stress test have a more potent prognostic value than
clinical or demographic parameters in predicting clinical improvement. Patients with negative
results of the test represent the group with the gravest prognosis. (Cardiol J 2007; 14: 174-179
Myocardial contractility improvement after coronary artery by-pass grafting in a 1-year observation: The role of myocardial viability assessment
Background: Left ventricular (LV) contractility impairment in coronary artery disease patients
may be the effect of heart muscle hibernation due to chronic ischemia. Revascularization
is the only method to restore contractile function in these patients. The time and degree of LV
function improvement have not been precisely determined, and the published data are discordant.
The aim of our study was to evaluate the time of myocardial contractility recovery after
surgical revascularization in patients with systolic LV dysfunction in relation to the results of
echocardiography stress tests.
Methods: The study included 50 patients with multivessel coronary artery disease and decreased
left ventricular ejection fraction (LVEF < 40%) qualified for coronary artery by-pass
grafting. The intervention was preceded by rest echocardiography, low dose dobutamine stress
test (5-10 µg/kg/min) and by low dose dipyridamole stress test (0.24 mg/kg during 4 min)
performed on the next day. Consecutive rest echocardiograms were performed immediately
after the intervention and after 3, 6 and 12 months. LVEF and the index of left ventricular
contractility (WMSI, wall motion score index) were evaluated at each examination.
Results: During 1-year follow-up after coronary artery by-pass grafting (CABG), significant
improvement in LV systolic function was observed (LVEF increased, WSMI reduced), with
major changes occurring over the first 6 months. The correlation of echocardiography parameters
(EF, WMSI) at stress tests and during 1-year follow-up showed that the strongest relationship
exists between the change in WMSI both in dobutamine and dipyridamole stress tests
and the improvement in WMSI observed after 6 months.
Conclusions: LV function improvement after CABG in patients with systolic LV dysfunction
was most pronounced 3 to 6 months after the intervention. Both types of stress tests are characterized
by similar diagnostic value for assessing LV myocardial viability. WMSI calculated during
stress was identified to have the strongest prognostic value. (Cardiol J 2007; 14: 246-251
Carrier trapping and luminescence polarization in quantum dashes
We study experimentally and theoretically polarization-dependent luminescence
from an ensemble of quantum-dot-like nanostructures with a very large in-plane
shape anisotropy (quantum dashes). We show that the measured degree of linear
polarization of the emitted light increases with the excitation power and
changes with temperature in a non-trivial way, depending on the excitation
conditions. Using an approximate model based on the k.p theory, we are able to
relate this degree of polarization to the amount of light hole admixture in the
exciton states which, in turn, depends on the symmetry of the envelope wave
function. Agreement between the measured properties and theory is reached under
assumption that the ground exciton state in a quantum dash is trapped in a
confinement fluctuation within the structure and thus localized in a much

Stare i nowe czynniki ryzyka sercowo- -naczyniowego — jak zahamować epidemię miażdżycy? Część II
Conventional risk factors for atherosclerosis are confirmed in 50–90% of patients with cardiovascular
diseases. A number of genetic, biochemical and vascular wall features may
determine cardiovascular risk, independent of classical risk factors. Some of them are pathophysiologically
associated with atherothrombosis, for example genetic polymorphisms
or coagulation factors. Moreover, C-reactive protein or plasma homocysteine are considered
to be a reason for disease. Next, there are the markers of early injury of vascular wall,
like increased carotid artery intima-media thickness or coronary calcium score. However,
no convincing data from clinical trials is available to confirm that tailored treatment of the
new cardiovascular risk factors brings therapeutic and prognostic benefits.
One should stress, that the new risk determinants substantially increase our knowledge in
patophysiology of cardiovascular disease, even if they do not contribute to risk stratification.

czynniki ryzyka u danej osoby. W ramach prewencji pierwotnej trzeba przede wszystkim
dążyć do zmiany stylu życia i w razie potrzeby zastosować leczenie farmakologiczne. W profilaktyce
wtórnej u osób z chorobą sercowo-naczyniową należy nie tylko wyeliminować lub
modyfikować konwencjonalne czynniki ryzyka, ale także często wdrożyć agresywną farmakoterapię
tych czynników.
Wymienione obciążenia stwierdza się u około 50–90% pacjentów z chorobami układu sercowo-
naczyniowego. Wiele cech genetycznych, biochemicznych, właściwości ściany naczyń
może być determinantem ryzyka sercowo-naczyniowego, niezależnie od klasycznych
czynników ryzyka. Niektóre z nich wiążą się przyczynowo z rozwojem aterotrombozy, na przykład
polimorfizm genetyczny lub czynniki krzepnięcia. Białko C-reaktywne i homocysteinę
rozważa się jako czynniki będące przyczyną choroby, a zwiększoną grubość błony wewnętrznej
i środkowej tętnicy szyjnej lub wskaźnik uwapnienia tętnic wieńcowych — jako markery końwczesnego
uszkodzenia ściany naczynia. Brakuje jednak wystarczająco przekonujących
danych z badań klinicznych, że stosowanie terapii ukierunkowanej na zwalczanie nowych
czynników poprawia efekty lecznicze i prognostyczne.
Należy jednak podkreślić, że jeżeli nawet nowe determinanty ryzyka nie poprawiają stratyfikacji
ryzyka, to dostarczają wartościowej wiedzy na temat patofizjologii chorób układu sercowo-

A Monte Carlo study of the Falicov–Kimball model in the perturbative regime
Finite-temperature properties of the Falicov–Kimball model on the square lattice have been studied in
the perturbative regime, i.e. for t/U << 1, where t is the hopping constant and U denotes the Coulomb interaction
strength. In our study, we have determined the phase diagram of the model in the second-order of the
perturbation theory, where the antiferromagnetic Ising model in the magnetic field emerges. In the fourth-order,
where our model constitutes the Ising model with more complicated frustrated antiferromagnetic interactions,
the sketch of the phase diagram was established. The Monte Carlo method was employed and the behavior
of Binder cumulants based on the order parameters was analyzed to determine the type of ordering
and phase boundaries in the diagram
Semiquantitative interpretation of anticardiolipin and antiβ2glycoprotein I antibodies measured with various analytical platforms: communication from the ISTH SSC subcommittee on Lupus Anticoagulant/Antiphospholipid antibodies
Background
Antiβ2glycoprotein I (aβ2GPI) and anticardiolipin (aCL) IgG/IgM show differences in positive/negative agreement and titers between solid phase platforms. Method specific semiquantitative categorization of titers could improve and harmonize the interpretation across platforms.
Aim
To evaluate the traditionally 40/80 units thresholds used for aCL and aβ2GPI for categorization into moderate/high positivity with different analytical systems, and to compare with alternative thresholds.
Material and methods
aCL and aβ2GPI thresholds were calculated for two automated systems (chemiluminescent immunoassay (CLIA) and multiplex flow immunoassay (MFI)) by ROC-curve analysis on 1108 patient samples, including patients with and without APS, and confirmed on a second population (n=279). Alternatively, regression analysis on diluted standard material was applied to identify thresholds. Thresholds were compared to 40/80 threshold measured by an enzyme linked immunosorbent assay (ELISA). Additionally, likelihood ratios (LR) were calculated.
Results
Threshold levels of 40/80 units show poor agreement between ELISA and automated platforms for classification into low/moderate/high positivity, especially for aCL/aβ2GPI IgG. Agreement for semiquantitative interpretation of aPL IgG between ELISA and CLIA/MFI improves with alternative thresholds. LR for aPL IgG increase for thrombotic and obstetric APS based on 40/80 thresholds for ELISA and adapted thresholds for the other systems, but not for IgM.
Conclusion
Use of 40/80 units as medium/high thresholds is acceptable for aCL/aβ2GPI IgG ELISA, but not for CLIA and MFI. Alternative semiquantitative thresholds for non-ELISA platforms can be determined by a clinical approach or by using monoclonal antibodies. Semiquantitative reporting of aPL IgM has less impact on increasing probability for APS
Anatomy of ovary and ovule in dandelions (Taraxacum, Asteraceae)
The genus Taraxacum Wigg. (Asteraceae) forms a polyploid complex within which there are strong links between the ploidy level and the mode of reproduction. Diploids are obligate sexual, whereas polyploids are usually apomictic. The paper reports on a comparative study of the ovary and especially the ovule anatomy in the diploid dandelion T. linearisquameum and the triploid T. gentile. Observations with light and electron microscopy revealed no essential differences in the anatomy of both the ovary and ovule in the examined species. Dandelion ovules are anatropous, unitegmic and tenuinucellate. In both sexual and apomictic species, a zonal differentiation of the integument is characteristic of the ovule. In the integumentary layers situated next to the endothelium, the cell walls are extremely thick and PAS positive. Data obtained from TEM indicate that these special walls have an open spongy structure and their cytoplasm shows evidence of gradual degeneration. Increased deposition of wall material in the integumentary cells surrounding the endothelium takes place especially around the chalazal pole of the embryo sac as well as around the central cell. In contrast, the integumentary cells surrounding the micropylar region have thin walls and exhibit a high metabolic activity. The role of the thick-walled integumentary layers in the dandelion ovule is discussed. We also consider whether this may be a feature of taxonomic importance
