    Pharmacological management of acute bronchiolitis

    This article reviews the current knowledge base related to the pharmacological treatments for acute bronchiolitis. Bronchiolitis is a common lower respiratory illness affecting infants worldwide. The mainstays of therapy include airway support, supplemental oxygen, and support of fluids and nutrition. Frequently tried pharmacological interventions, such as ribavirin, nebulized bronchodilators, and systemic corticosteroids, have not been proven to benefit patients with bronchiolitis. Antibiotics do not improve the clinical course of patients with bronchiolitis, and should be used only in those patients with proven concurrent bacterial infection. Exogenous surfactant and heliox therapy also cannot be recommended for routine use, but surfactant replacement holds promise and should be further studied

    An application of principal component analysis to the clavicle and clavicle fixation devices

    BACKGROUND: Principal component analysis (PCA) enables the building of statistical shape models of bones and joints. This has been used in conjunction with computer assisted surgery in the past. However, PCA of the clavicle has not been performed. Using PCA, we present a novel method that examines the major modes of size and three-dimensional shape variation in male and female clavicles and suggests a method of grouping the clavicle into size and shape categories. MATERIALS AND METHODS: Twenty-one high-resolution computerized tomography scans of the clavicle were reconstructed and analyzed using a specifically developed statistical software package. After performing statistical shape analysis, PCA was applied to study the factors that account for anatomical variation. RESULTS: The first principal component representing size accounted for 70.5 percent of anatomical variation. The addition of a further three principal components accounted for almost 87 percent. Using statistical shape analysis, clavicles in males have a greater lateral depth and are longer, wider and thicker than in females. However, the sternal angle in females is larger than in males. PCA confirmed these differences between genders but also noted that men exhibit greater variance and classified clavicles into five morphological groups. DISCUSSION AND CONCLUSIONS: This unique approach is the first that standardizes a clavicular orientation. It provides information that is useful to both, the biomedical engineer and clinician. Other applications include implant design with regard to modifying current or designing future clavicle fixation devices. Our findings support the need for further development of clavicle fixation devices and the questioning of whether gender-specific devices are necessary

    Designing a ‘living archive’ that will enable new forms of circus performance to be realised is a complex and dynamic challenge. This paper discusses the methods and approaches used by the research team in the design of the Circus Oz Living Archive. Essential to this project has been the design of a responsive methodology that could embrace the diverse areas of knowledge and practice that have led to a design outcome that integratesthe affordances of the circus with those of digital technologies. The term ‘living archive’ has been adopted as a means to articulate the dynamic nature of the archive. This is an archive that will always be evolving, not only because of the on going collection of content, but more importantly because the performance of the archive users will themselves become part of the archive collection

    Multimode transmission of continuous wave 633 nm radiation and 1064 nm Q-switched Nd:YAG pulses using silver coated hollow core optical waveguides (HCWs) with bore diameters of 700 μm and 1000 μm is reported. The effect of launch conditions, input beam polarization and waveguide bore diameter on the pulse energy transmission and potential for focussing the beam effectively at the HCW exit is detailed. An optimal launch f-number range of 155–165 is identified for minimizing the exit angle

    Population structure and diversity of the needle pathogen Dothistroma pini suggests human-mediated movement in Europe

    Dothistroma needle blight (DNB) is an important disease of Pinus species that can be caused by one of two distinct but closely related pathogens; Dothistroma septosporum and Dothistroma pini. Dothistroma septosporum has a wide geographic distribution and is relatively well-known. In contrast, D. pini is known only from the United States and Europe, and there is a distinct lack of knowledge regarding its population structure and genetic diversity. The recent development of 16 microsatellite markers for D. pini provided an opportunity to investigate the diversity, structure, and mode of reproduction for populations collected over a period of 12 years, on eight different hosts in Europe. In total, 345 isolates from Belgium, the Czech Republic, France, Hungary, Romania, Western Russia, Serbia, Slovakia, Slovenia, Spain, Switzerland, and Ukraine were screened using microsatellite and species-specific mating type markers. A total of 109 unique multilocus haplotypes were identified and structure analyses suggested that the populations are influenced by location rather than host species. Populations from France and Spain displayed the highest levels of genetic diversity followed by the population in Ukraine. Both mating types were detected in most countries, with the exception of Hungary, Russia and Slovenia. Evidence for sexual recombination was supported only in the population from Spain. The observed population structure and several shared haplotypes between non-bordering countries provides good evidence that the movement of D. pini in Europe has been strongly influenced by human activity in Europe

    Community adaptation to cope with disaster related road structure failure

    Natural hazards can cause a wide range of social and economic impacts both to the area directly affected by the hazards as well as to the broader community. Although community resilience is an important aspect that influences post-disaster response and recovery stages, it has not been explicitly studied by most scholars, and is rather taken to be embedded in the socio-economic landscape studied in the literature. Road structures such as bridges, culverts and flood-ways play a vital role in times of natural disasters as their functionality directly influences evacuation, rescue, recovery and reconstruction activities. In addition to the direct benefits derived from road structures, in the event of a disaster, they play a vital role in resilience by connecting individuals and communities. This paper identifies adaptation methods practiced by disaster affected communities targeted at increasing their accessibility and mobility, and analyses how such adaptation activities can minimise the negative effects brought on by the failure of road structures. The paper uses a recent case study from regional Queensland, Australia, to understand how adaptation options vary in rural areas and to explore possible methods to improve resilience of communities
