    Communicating climate change risk: A content analysis of ipcc\u27s summary for policymakers

    This study investigated the effectiveness of climate change risk communication in terms of its theoretical potential to stimulate recipients\u27 awareness and behavioral change. We selected the summary for policy makers (SPM) of the most recent Intergovernmental Panel on Climate Change (IPCC) report in order to conduct a content analysis; the extended parallel process model and construal level theory served as conceptual lenses to perform the analysis. Specifically, we evaluated to what extent the SPM included informational elements of threat, efficacy and psychological distance related to climate change. The results showed that threat information was prominently present, but efficacy information was less frequently included, and when it was, more often in the latter parts of the SPM. With respect to construal level it was found that in the IPCC report concrete representations were used only sparingly. Theoretical relevance and implications for climate change risk communication with key audiences are discussed

    Selection and transfer of an IncI1-tet(A) plasmid of Escherichia coli in an ex vivo model of the porcine caecum at doxycycline concentrations caused by cross-contaminated feed

    Aims: The aim of this study was to investigate the effect of subtherapeutic intestinal doxycycline (DOX) concentrations (4 and 1mgl(-1)), caused by cross-contamination of feed, on the enrichment of a DOX-resistant commensal Escherichia coli and its resistance plasmid in an exvivo model of the porcine caecum. Methods and Results: A DOX-resistant, tet(A)-carrying, porcine commensal E.coli strain (EC 682) was cultivated for 6days in the porcine caecum model under different conditions (0, 1 and 4mgl(-1) DOX). EC 682, other coliforms and anaerobic bacteria were enumerated daily. A selection of isolated DOX-resistant coliforms (n=454) was characterized by rep-PCR clustering, PCR assays (Inc1 and tet(A)) and micro broth dilution susceptibility tests (Sensititre). Both 1 and 4mgl(-1) DOX-enriched medium had a significantly higher selective effect on EC 682 and other resistant coliforms than medium without DOX. Transconjugants of EC 682 were isolated more frequently in the presence of 1 and 4mgl(-1) DOX compared to medium without DOX. Conclusions: Subtherapeutic intestinal DOX concentrations have the potential to select for DOX-resistant E.coli, and promote the selection of transconjugants in a porcine caecum model. Significance and Impact of the Study: Cross-contamination of feed with antimicrobials such as DOX likely promotes the spread of antimicrobial resistance. Therefore, it is important to develop or fine-tune guidelines for the safe use of antimicrobials in animal feed and its storage

    Six-Minute Walk Test in Patients With Down Syndrome:Validity and Reproducibility

    Contains fulltext : 81543.pdf (publisher's version ) (Closed access)OBJECTIVES: To examine the validity of the six-minute walk test (6MWT) as a tool to evaluate functional exercise performance in patients with Down syndrome (DS). DESIGN: Comparison of the six-minute walk distance (6MWD) in 2 distinct groups of DS patients: with and without severe cardiac disease. To test reproducibility, a group of patients with DS performed the 6MWT twice. SETTING: Tertiary referral centers for patients with congenital heart defects and outpatient clinics for people with intellectual disabilities. PARTICIPANTS: Adult patients with DS with (n=29) and without (n=52) severe cardiac disease categorized by cardiac echocardiography. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURE: Distance walked on the 6MWT. RESULTS: The mean 6MWD in the group with severe cardiac disease was 289+/-104 m and in the group without severe cardiac disease 280+/-104 m (P=.70). Older age, female sex, and severe level of intellectual disability were all found to be independently and significantly correlated with a lower 6MWD (r=.67, P<.001). The paired 6MWD was not significantly different (310+/-88 m vs 317+/-85 m; P=.40) in patients who performed the 6MWT twice. The coefficient of variation was 11%. CONCLUSIONS: The 6MWD between the 2 groups was not significantly different. However, the walking distance inversely correlated with the level of intellectual disability. Therefore, the 6MWT is not a valid test to examine cardiac restriction in adult patients with DS

This report details evidence-based best practice management of the pre-, stable, and the progressing myope, including risk factor identification, examination, selection of treatment strategies, and guidelines for ongoing management. Practitioner considerations such as informed consent, prescribing off-label treatment, and guides for patient and parent communication are detailed. The future research directions of myopia interventions and treatments are discussed, along with the provision of clinical references, resources, and recommendations for continuing professional education in this growing area of clinical practice

    Physiological-based cord clamping in very preterm infants:the Aeration, Breathing, Clamping 3 (ABC3) trial—statistical analysis plan for a multicenter randomized controlled trial

    Background: Mortality, cerebral injury, and necrotizing enterocolitis (NEC) are common complications of very preterm birth. An important risk factor for these complications is hemodynamic instability. Pre-clinical studies suggest that the timing of umbilical cord clamping affects hemodynamic stability during transition. Standard care is time-based cord clamping (TBCC), with clamping irrespective of lung aeration. It is unknown whether delaying cord clamping until lung aeration and ventilation have been established (physiological-based cord clamping, PBCC) is more beneficial. This document describes the statistical analyses for the ABC3 trial, which aims to assess the efficacy and safety of PBCC, compared to TBCC. Methods: The ABC3 trial is a multicenter, randomized trial investigating PBCC (intervention) versus TBCC (control) in very preterm infants. The trial is ethically approved. Preterm infants born before 30 weeks of gestation are randomized after parental informed consent. The primary outcome is intact survival, defined as the composite of survival without major cerebral injury and/or NEC. Secondary short-term outcomes are co-morbidities and adverse events assessed during NICU admission, parental reported outcomes, and long-term neurodevelopmental outcomes assessed at a corrected age of 2 years. To test the hypothesis that PBCC increases intact survival, a logistic regression model will be estimated using generalized estimating equations (accounting for correlation between siblings and observations in the same center) with treatment and gestational age as predictors. This plan is written and submitted without knowledge of the data. Discussion: The findings of this trial will provide evidence for future clinical guidelines on optimal cord clamping management at birth. Trial registration: ClinicalTrials.gov NCT03808051.     Bi-allelic <i>NIT1 </i>variants cause a brain small vessel disease characterized by movement disorders, massively dilated perivascular spaces, and intracerebral hemorrhage

    Purpose: To describe a recessively inherited cerebral small vessel disease, caused by loss-of-function variants in Nitrilase1 (NIT1). Methods:We performed exome sequencing, brain magnetic resonance imaging, neuropathology, electron microscopy, western blotting, and transcriptomic and metabolic analyses in 7 NIT1-small vessel disease patients from 5 unrelated pedigrees. Results: The first identified patients were 3 siblings, compound heterozygous for the NIT1 c.727C&gt;T; (p.Arg243Trp) variant and the NIT1 c.198_199del; p.(Ala68∗) variant. The 4 additional patients were single cases from 4 unrelated pedigrees and were all homozygous for the NIT1 c.727C&gt;T; p.(Arg243Trp) variant. Patients presented in mid-adulthood with movement disorders. All patients had striking abnormalities on brain magnetic resonance imaging, with numerous and massively dilated basal ganglia perivascular spaces. Three patients had non-lobar intracerebral hemorrhage between age 45 and 60, which was fatal in 2 cases. Western blotting on patient fibroblasts showed absence of NIT1 protein, and metabolic analysis in urine confirmed loss of NIT1 enzymatic function. Brain autopsy revealed large electron-dense deposits in the vessel walls of small and medium sized cerebral arteries. Conclusion: NIT1-small vessel disease is a novel, autosomal recessively inherited cerebral small vessel disease characterized by a triad of movement disorders, massively dilated basal ganglia perivascular spaces, and intracerebral hemorrhage.</p

    The Role of Thrombin in Proliferative Vitreoretinopathy

    PURPOSE. To determine the role of thrombin in the development of proliferative vitreoretinopathy (PVR). METHODS. Vitreous was collected from patients undergoing a vitrectomy (macular holes and puckers, n ¼ 11 [controls]; retinal detachment without PVR development following vitrectomy, n ¼ 15 [RRD1]; retinal detachment with PVR development within 6 months after vitrectomy, n ¼ 11 [RRD2]; and established PVR, n ¼ 14 [PVR]). Thrombin activity in vitreous was determined using a thrombin-specific chromogenic substrate. ARPE-19 cells were stimulated with 83 diluted vitreous samples in the presence and absence of hirudin. The samples were analyzed at t ¼ 0 and t ¼ 24 hours for the presence of 27 cytokines/ chemokines and growth factors using a multiplex approach. In comparable studies, ARPE-19 cells were stimulated for 2 hours, and mRNA expression levels for CCL2, CXCL8, GMCSF, IL6, and PDGFB were determined by real-time quantitative (RQ)-PCR. RESULTS. Thrombin activity was significantly (P &lt; 0.05) higher in vitreous of the PVR group compared to the other groups. Proliferative vitreoretinopathy vitreous stimulated the production of chemokine (C-C motif) ligand (CCL)2, chemokine (C-X-C motif) ligand (CXCL)8, granulocyte-macrophage colony-stimulating factor (GM-CSF), IL-6, and plateletderived growth factor (PDGF)-BB by ARPE-19 to significantly (P &lt; 0.05) higher levels than vitreous from the RRD1 and RRD2 groups. These effects of PVR vitreous were significantly (P &lt; 0.05) reduced by hirudin. These data were confirmed by mRNA studies. CONCLUSIONS. Thrombin activity is increased in vitreous of patients with established PVR and is involved in the activation of proinflammatory and profibrotic pathways in RPE cells. Inhibition of thrombin activity may therefore represent a potential treatment option for proliferative vitreoretinopathy

    Digital mapping of GlobalSoilMap soil properties at a broad scale: a review

    Soils are essential for supporting food production and providing ecosystem services but are under pressure due to population growth, higher food demand, and land use competition. Because of the effort to ensure the sustainable use of soil resources, demand for current, updatable soil information capable of supporting decisions across scales is increasing. Digital soil mapping (DSM) addresses the drawbacks of conventional soil mapping and has been increasingly used for delivering soil information in a time- and cost-efficient manner with higher spatial resolution, better map accuracy, and quantified uncertainty estimates. We reviewed 244 articles published between January 2003 and July 2021 and then summarised the progress in broad-scale (spatial extent >10,000 km2) DSM, focusing on the 12 mandatory soil properties for GlobalSoilMap. We observed that DSM publications continued to increase exponentially; however, the majority (74.6%) focused on applications rather than methodology development. China, France, Australia, and the United States were the most active countries, and Africa and South America lacked country-based DSM products. Approximately 78% of articles focused on mapping soil organic matter/carbon content and soil organic carbon stocks because of their significant role in food security and climate regulation. Half the articles focused on soil information in topsoil only (<30 cm), and studies on deep soil (100–200 cm) were less represented (21.7%). Relief, organisms, and climate were the three most frequently used environmental covariates in DSM. Nonlinear models (i.e. machine learning) have been increasingly used in DSM for their capacity to manage complex interactions between soil information and environmental covariates. Soil pH was the best predicted soil property (average R2 of 0.60, 0.63, and 0.56 at 0–30, 30–100, and 100–200 cm). Other relatively well-predicted soil properties were clay, silt, sand, soil organic carbon (SOC), soil organic matter (SOM), SOC stocks, and bulk density, and coarse fragments and soil depth were poorly predicted (R2 < 0.28). In addition, decreasing model performance with deeper depth intervals was found for most soil properties. Further research should pursue rescuing legacy data, sampling new data guided by well-designed sampling schemas, collecting representative environmental covariates, improving the performance and interpretability of advanced spatial predictive models, relating performance indicators such as accuracy and precision to cost-benefit and risk assessment analysis for improving decision support; moving from static DSM to dynamic DSM; and providing high-quality, fine-resolution digital soil maps to address global challenges related to soil resources
