    Over Kistlerinin Transvajinal Yolla Basit Drenaji: Klinik Deneyimlerimiz

    ÖZETAmaç: Ultrasonografi’nin (US) kullanımı ile hastalarda asemptomatik ovaryan kistlerin tanısı daha kolaylaşmıştır. Tedavide birçok alternatif vardır. Çalışmamızda Transvaginal US eşliğinde aspirasyon yapılan basit over kistlerinin sonuçları incelenmiştir.Yontemler: Kliniğimize başvuran hastalarda ovaryan kist tanısı konulan 44 hasta çalışmaya dahil edildi.Bulgular: 44 hastadan 21 hastaya tanı anında kist aspirasyonu uygulandı. Kalan 23’ü direkt aspirasyondan önce, ilk seçenek olarak basit izlemi tercih ettiler. Bunların 3’ünde spontan gerileme gözlendi. Çalışmamızda takip edilen kistlerin ortalama çapı 5.2±1,7 cm olarak bulundu. Ayrıca, 41 hastanın %65,8’de over kistinin tekrarlamadığı, sadece %26.8’de kistlerin tekrarladığı görüldü. Takip sırasında 5 hastada (%45.4) ovaryan kistinin tekrar oluştuğu gözlendi. Sonuç olarak tüm hastaların %14.6’sında laparoskopi gereksinimi oldu.Sonuç: Son dönemlerde transvajinal ultrason eşliğinde transvajinal kist aspirasyonu, maliyet, morbidite ve mortalitede de ciddi azalma sağlaması nedeniyle güvenilir bir alternatiftir. Anahtar Kelimeler: basit over kisti, ince iğne aspirasyonu, transvajinal.ABSTRACTObjectıve: With the use of the ultrasonography, the diagnosis of asympthomatic ovarian cysts is easier now. There are many alternative treatments for ovarian cysts. In this study the results of the simple ovarian cysts which were performed transvaginal aspiration accompained by ultrasound were evaluated.Methods: Fourty four patients who applied to our clinic with ovarian cysts diagnosis were included in our study.Results: Cyst aspiration was performed to 21 of 44 patients at the diagnose. The remaning 23 patients preferred expectant management before aspiration treatment. Spontan regression was seen in three of which cases. The average diameter of cysts which are fallowed in our study was founded as 5.2±1,7 cm. Also, no relapse was seen in 65.8 percent of 41 patients and relapse was only seen in 26.8 percent of 41 patients. During the monitoring period it was observed that in 5 cases (45.4 percent) relapse was seen after a second aspiration. Finally, laparoscopy was required for 14.6 percent of all patients.Conclusıon: Recently, ultrasound guided transvaginal cyst aspiration is a reliable alternative because it enables a serious reduction in the costs, morbidity and mortality rate.     The Frequency of Fibromyalgia Syndrome in Patients with Polycystic Ovary Syndrome

    Objectives: Polycystic ovary syndrome [PCOS] is one of the most common reproductive abnormalities and is characterized by hyperandrogenemia. We aimed to assess the frequency of fibromyalgia syndrome [FMS] and psychological distress in patients with PCOS. Methods: Forty patients with PCOS and 39 age-matched healthy controls were included in the study as PCOS and healthy normal control [HNC] groups. The diagnosis of FMS was based on the American College of Rheumatology criteria of 1990, and the diagnosis of PCOS was based on the Rotterdam criteria of 2003. All participants completed the questionnaires to gather information including demographic features, FMS-related symptoms, and the psychological status. Patients with PCOS were divided into two subgroups according to coexistence of FMS [PCOS only and PCOS + FMS]. Results: The mean age of PCOS and HNC groups were 23.97 +/- 6.01 and 26.15 +/- 5.45 years, respectively [P > 0.05]. The frequency of FMS in PCOS and HNC groups was 32.5 percent and 7.7 percent, respectively [P < 0.05]. Except constipation and paresthesia, the presence of FMS-related symptoms in the PCOS + FMS subgroup were higher than those of the PCOS only subgroup and HNC group. There were no differences in depression scores between the PCOS only and PCOS + FMS subgroups, but there were differences in anxiety scores. Conclusion: The frequency of FMS was found to be increased in PCOS. Anxiety risk was also found to be increased, particularly in patients with PCOS and concomitant FMS
