Nuevas aplicaciones de la cápsula endoscópica: PILLCAM™ ESO
ABSTRACT
Capsule endoscopy has opened a new era in
small bowel examination. Its indications are now welldefined
and currently, wireless capsule endoscopy is
considered as the first-line imaging tool for the
diagnosis of small bowel diseases. ECE has been
shown to be feasible, safe and a good alternative
technique in patients refusing conventional
endoscopy. Although results reported in both GERD
and cirrhotic patients are encouraging, great
differences in terms of accuracy (particularly in GERD
patients) have been found in published studies. These
differences have been attributed to study designs, the
lack of adequate experience and inconvenience of
ingestion protocols. In summary, more large-scale
studies evaluating the new 14-fps capsule, adequate
ECE-experience and new modified ingestion protocols
are still needed
IL-1R and inflammasomes mediate early pulmonary protective mechanisms in respiratory brucella abortus infection
Brucella spp. infection is frequently acquired through contaminated aerosols. The role of interleukin-1 beta (IL-1β) in the early pulmonary response to respiratory Brucella infection is unknown. As shown here, IL-1β levels in lung homogenates and bronchoalveolar lavage fluid (BALF) of mice intratracheally inoculated with B. abortus were increased at 3 and 7 days p.i. At 7 days p.i., pulmonary CFU numbers were higher in IL-1 receptor (IL-1R) knockout (KO) mice than in wild type (WT) mice. At different times p.i. CFU in lungs and BALF were higher in mice lacking some inflammasome components (caspase-1, AIM2, NLRP3) than in WT mice. At 2 days p.i. pulmonary levels of IL-1b and CXCL1 (neutrophils chemoattractant) were lower in caspase-1/11 KO mice. At day 3 p.i., neutrophils counts in BALF were lower in caspase-1/11 KO mice than in WT mice. During in vitro infections, IL-1β secretion was lower in alveolar macrophages from caspase-1/11, NLRP3 or AIM2 KO mice than in WT controls. Similarly, IL-1β production by B. abortus-infected alveolar epithelial cells was reduced by pretreatment with a specific caspase-1 inhibitor. This study shows that IL-1R, probably through IL-1β action, and the NLRP3 and AIM2 inflammasomes are involved in pulmonary innate immune protective mechanisms against respiratory B. abortus infection.Fil: Hielpos, María Soledad. Conventional Cervical Exercises Compared with a Mixed-Reality-Based Game in Asymptomatic Subjects: An Exploratory Crossover Pilot Study.
Mixed reality presents itself as a potential technological tool for the management of people
with musculoskeletal disorders, without having as many adverse side effects as immersive virtual
reality. The objective of this study was to explore the possibilities of a mixed-reality game, performing
task-oriented cervical exercises compared to conventional therapeutic exercises in sensorimotor
outcome measures in asymptomatic subjects. A randomized crossover pilot study was performed
with two intervention groups: a mixed-reality group (MRG) and a conventional exercise group (CEG).
The cervical joint position error test (CJPET) and deep cervical flexor endurance test (DCFET) were
measured as sensorimotor outcomes. Statistically significant differences were found in the pre–post
comparison in the DCFET for both groups (MRG: t = �����3.87, p < 0.01; CEG: t = �����4.01, p < 0.01)
and in the extension of the CJPET for the MRG (t = 3.50, p < 0.01). The rest of the measurements
showed no significant differences comparing both groups pre- and postintervention (p > 0.05). Mixed
reality has apparently the same positive effects as conventional exercises in sensorimotor outcomes
in asymptomatic subjects. These results could help in future studies with mixed virtual reality in the
management of people with musculoskeletal disorders.post-print882 K
Continuously-variable survival exponent for random walks with movable partial reflectors
We study a one-dimensional lattice random walk with an absorbing boundary at
the origin and a movable partial reflector. On encountering the reflector, at
site x, the walker is reflected (with probability r) to x-1 and the reflector
is simultaneously pushed to x+1. Iteration of the transition matrix, and
asymptotic analysis of the probability generating function show that the
critical exponent delta governing the survival probability varies continuously
between 1/2 and 1 as r varies between 0 and 1. Our study suggests a mechanism
for nonuniversal kinetic critical behavior, observed in models with an infinite

Deterioro cognito disejecutivo en pacientes con enfermedad coronaria: asociación con variables clínicas
Las enfermedades cardiacas se asocian con un incremento del riesgo de deterioro cognitivo. Hemos estudiado en pacientes coronarios la presencia de deterioro cognitivo leve (DCL) disejecutivo y su asociación con variables sociodemográficas y clínicas. Se ha evaluado en 35 pacientes la función ejecutiva (Trail Making Test, TMT; puntuaciones ajustadas por edad y escolaridad, NEURONORMA). Los pacientes presentaron rendimiento normal o con DCL (percentil de distribución TMT<10%). Se compararon ambos grupos y se estudió la relación del TMT con variables sociodemográficas y clínicas (pruebas no paramétricas y estudio de regresión múltiple). Los pacientes (66 ± 9 años, 69,4% varones) se clasificaron en grupos con rendimiento normal (n=28, 80%) o con rendimiento ejecutivo deficitario (n=7, 20%). El grupo con DCL disejecutivo presentó mayor grado de angina (p=0.03, Canadian Cardiovascular Society), enfermedad de 3 vasos (p=0,04) e índice de masa corporal (p=0.03) y menor tensión arterial diastólica (p=0.02), hemoglobina (p=0.02) y hematocrito (p=0.0001); estas variables resultaron significativas del rendimiento en el TMT como variable dependiente (R2=0,62)
Se muestra en pacientes con enfermedad coronaria una importante prevalencia de DCL disejecutivo asociado al nivel de angina y otros factores de riesgo cardiovascular. Critical Behavior of Three-Dimensional Disordered Potts Models with Many States
We study the 3D Disordered Potts Model with p=5 and p=6. Our numerical
simulations (that severely slow down for increasing p) detect a very clear spin
glass phase transition. We evaluate the critical exponents and the critical
value of the temperature, and we use known results at lower values to
discuss how they evolve for increasing p. We do not find any sign of the

Recurrent De Novo NAHR Reciprocal Duplications in the ATAD3 Gene Cluster Cause a Neurogenetic Trait with Perturbed Cholesterol and Mitochondrial Metabolism.
Recent studies have identified both recessive and dominant forms of mitochondrial disease that result from ATAD3A variants. The recessive form includes subjects with biallelic deletions mediated by non-allelic homologous recombination. We report five unrelated neonates with a lethal metabolic disorder characterized by cardiomyopathy, corneal opacities, encephalopathy, hypotonia, and seizures in whom a monoallelic reciprocal duplication at the ATAD3 locus was identified. Analysis of the breakpoint junction fragment indicated that these 67 kb heterozygous duplications were likely mediated by non-allelic homologous recombination at regions of high sequence identity in ATAD3A exon 11 and ATAD3C exon 7. At the recombinant junction, the duplication allele produces a fusion gene derived from ATAD3A and ATAD3C, the protein product of which lacks key functional residues. Analysis of fibroblasts derived from two affected individuals shows that the fusion gene product is expressed and stable. These cells display perturbed cholesterol and mitochondrial DNA organization similar to that observed for individuals with severe ATAD3A deficiency. We hypothesize that the fusion protein acts through a dominant-negative mechanism to cause this fatal mitochondrial disorder. Our data delineate a molecular diagnosis for this disorder, extend the clinical spectrum associated with structural variation at the ATAD3 locus, and identify a third mutational mechanism for ATAD3 gene cluster variants. These results further affirm structural variant mutagenesis mechanisms in sporadic disease traits, emphasize the importance of copy number analysis in molecular genomic diagnosis, and highlight some of the challenges of detecting and interpreting clinically relevant rare gene rearrangements from next-generation sequencing data
