    “Tu voz, tu lugar” es un proyecto comunicacional realizado en el Centro de Actividades Juveniles (C.A.J) del Colegio Secundario Luis María Preti N°5171. Éste se encuentra vinculado a las cátedras Promoción Cultural Comunitaria y Residencia/Pasantía en el Ámbito de la Promoción Comunitaria de la carrera Tecnicatura en Comunicación Social- UNSa- Sede Tartagal. Este proyecto es el resultado de un diagnostico comunicacional, que se retomó desde un posicionamiento teórico crítico, con el objeto de visualizar las posibles determinaciones y relaciones entre los diversos actores sociales incidentes en este contexto social. De esta manera, vislumbrar los problemas vigentes, y lograr una delimitación de posibilidades de acción.     A New Approach to the Identification of Palliative Care Needs and Advanced Chronic Patients among Nursing Home Residents

We would like to thank all the participants and professionals who participated.Background: Proper planning of Palliative Care in nursing homes requires advanced knowledge of the care needs that residents show. The aim of the study was to evaluate Palliative Needs and other conditions such as fragility, complexity, and prognosis and also to suggest new indicators for the establishment of the resident's advanced chronic condition. Methods: Cross-sectional study conducted in 149 nursing homes Complex Chronic residents evaluated by trained professionals. Palliative Care Needs, assessed by the NECPAL ICO-CCOMS (c) tool, and fragility, case and palliative complexity and prognosis were evaluate through a comprehensive assessment. Descriptive analyses and association measures were performed setting the statistical significance at 0.05. Results: More than 50% of the residents had positive Surprise Question and other Palliative Needs and were classified as Advanced Chronic Patients. Distress and/or Severe Adaptative Disorder was the most frequent need shown by the residents and significant differences in levels of frailty and other characteristics, were found between the Positive and the Negative Surprise Question Groups. Statistically significant correlations were also found between aspects of both groups. Conclusions: Nursing homes residents show Palliative Needs regardless of the response to the Surprise Question of the NECPAL tool. Other characteristics such as presence of an intermediate level of frailty are suggested as a new perspective to identify advanced chronic patients among nursing homes residents.Junta de Andalucia AP-0105-201

    Revisión de la literatura científica sobre la toma de decisiones bajo riesgo o ambigüedad en pacientes con enfermedades neurodegenerativas

    Los pacientes con enfermedades neurodegenerativas presentan deterioros en procesos cognitivos y conductuales; sin embargo, los perfiles neuropsicológicos reportados en la literatura científica demuestran inconsistencias en cuanto a sí la toma decisiones pueda estar afectada o no, debido a falta de consenso en el ejercicio clínico para la valoración cognitiva. Por ello se realizó una revisión de la literatura científica sobre la toma de decisiones en pacientes con enfermedades neurodegenerativas, con el propósito de identificar sí se requiere ampliar la valoración neuropsicológica, incluyendo la toma de decisiones, para establecer estrategias clínicas de intervención y rehabilitación. Se realizó una revisión sistemática con la declaración PRISMA, en idioma inglés, en bases de datos Scopus, PubMed y ScienceDirect entre 2012–2022. Se identificó déficit en toma de decisiones de pacientes con enfermedades neurodegenerativas, no solo evidente en la valoración neuropsicológica, sino también en el criterio de funcionalidad para actividades instrumentales de la vida diaria, que involucran procesos de toma de decisiones relacionadas con conducta social y adaptativa, tales como gestión financiera, programación de citas y adherencia al tratamiento médico. Por lo anterior es necesario incluir la toma de decisiones bajo riesgo o ambigüedad en la evaluación neuropsicológica de pacientes con enfermedades neurodegenerativas

    Palliative Care Symptoms, Outcomes, and Interventions for Chronic Advanced Patients in Spanish Nursing Homes with and without Dementia

    The aim of this study was to compare the symptomatology, palliative care outcomes, therapeutic procedures, diagnostic tests, and pharmacological treatments for people with dementia (PWD) and without dementia (PW/OD) admitted to Spanish nursing homes. Design: This was a cross-sectional study which is part of a long-term prospective follow-up of elderly people performed in nursing homes to measure end-of-life care processes. Participants: 107 nursing home patients with advanced or terminal chronic diseases were selected according to the criteria of the Palliative Care Spanish Society. Setting: Two trained nurses from each nursing home were responsible for participant selection and data collection. They must have treated the residents and had a minimum seniority of 6 months in the nursing home. Measurements: Sociodemographic data; Edmonton Symptom Assessment Scale; Palliative Care Outcome Scale; and prevalence of diagnostic tests, pharmacological treatments, and therapeutic procedures were evaluated. Results: Pain, fatigue, and nausea were found to be significantly higher in the nondementia group and insomnia, poor appetite, and drowsiness were significantly higher in the dementia group. Patient anxiety, support, feeling that life was worth living, self-worth, and practical matters management were higher in the nondementia group. Regarding drugs, use of corticoids was higher in the nondementia group, while use of anxiolytics was higher in the dementia group. Diagnostic procedures such as urine analysis and X-ray were higher in the dementia group. Conclusions: Differences in symptom perception, diagnostic tests, and pharmacological procedures were found between patients with and without dementia.     Attitudes and experiences related to the deaths of COVID-19 patients among nursing staff: A qualitative evidence synthesis

    Aim To identify and synthesize the experiences and attitudes of nursing staff regarding the deaths of COVID-19 patients. Review Methods A qualitative evidence synthesis was carried out, using Noblit and Hare's meta-ethnographic approach. The review protocol was listed in PROSPERO (CRD42022330928). Studies published from January 2020 to January 2022 that met the criteria were searched in PubMed, Web of Science, Scopus, CINAHL, CUIDEN and PsycInfo. A total of 12 articles were included. Results Thirty-three metaphors emerged, which were grouped into three main themes: Determining factors of care, Feelings about death and Strategies for coping with death. Nurses reported the high emotional toll, the absence of family and the lack of staff, protocol and training as determining factors. Furthermore, staff had doubts about the quality of care that COVID-19 patients received. As coping strategies, nurses developed avoidance behaviours towards COVID-19 patients, selective memories, resilience, and/or leaving the profession. Conclusions The difficulty in providing adequate nursing care and the high number of deaths has increased anxiety and stress among nurses. These factors, alongside their lived experiences of seeing patients suffering, many dying alone without family members, have had psychological repercussions on nursing staff. Implications for the Profession and/or Patient Care The results demonstrate a high emotional toll and doubts surrounding their caregiving role caused by the lack of professional training needed to face a pandemic. This research shows what has been learned for future pandemics and highlights basic components that could provide a foundation for coping interventions for healthcare professionals. Impact What Problem did the Study Address? The challenges posed by COVID-19 patient deaths for nursing staff around the world and also by the pandemic circumstances in which those deaths occurred. What were the Main Findings? The high number of deceased patients who were isolated from family members, communication with family members and doubts surrounding care given during the pandemic have created feelings of fear, stress and anxiety, as well as obsessive thoughts that have changed nursing staff's perception of death due to COVID-19. Where and on whom will the Research have an Impact? Results will be useful for preparing for future pandemics, and for policymakers and health staff in supporting healthcare professionals by creating programmes to help them cope with the emotional toll they have felt after dealing with death in such unprecedented circumstances.     Experiencias adversas en la infancia asociadas con trastornos neuropsiquiátricos en la adultez: una revisión sistemática

    Introduction: Exposure to traumatic experiences in childhood, in which socio-political violence and witnessing violence are included, is considered one of the predictors of neuropsychiatric disorders in Adulthood. General objective: To analyze the relationship between adverse childhood experiences and neuropsychiatric disorder based on a systematic review of the scientific literature. Method: Documentary research, systematic review type, using Strobe protocol for observational studies, the databases consulted were Scopus, PubMed, and Science Direct. during the period 2016-2021 Results: 517 studies were identified for inclusion, 78 duplicates were eliminated, 439 were screened and chosen, and of these only 10 met the quality criteria. Conclusion: There is an association between adverse childhood experiences and neuropsychiatric disorders in adulthood. Adverse experiences during childhood were significantly associated with risk behaviors and poor health outcomes in adulthood, having long-term effects.  In addition, it was evidenced that it can generate deterioration in associative learning, episodic memory, intellectual capacity, problem-solving, and planning, which merits actions from public health to mitigate the impact.La exposición a experiencias traumáticas en la infancia, en las que se incluye la violencia sociopolítica y el ser testigo de violencia, es considerado uno de los predictores de trastornos neuropsiquiátricos en la adultez. Así, el objetivo del presente estudio fue analizar la relación entre las experiencias adversas en la infancia y el trastorno neuropsiquiátrico a partir de una revisión sistemática de la literatura científica. Se utilizó el protocolo Strobe para estudios observacionales.Las bases de datos consultadas fueron Scopus, PubMed y Science Direct y la revisión se realizó durante el período 2016-2021. Se identificaron 517 estudios para su inclusión, se eliminaron 78 duplicados, se hizo tamizaje y se eligieron 439, de los cuales sólo 10 cumplían los criterios de calidad. En conclusión, existe una asociación entre las experiencias adversas en la infancia y los trastornos neuropsiquiátricos en la adultez. Tales experiencias se asociaron significativamente con conductas de riesgo y deterioro de la salud en la edad adulta. Además, se evidenció que pueden causar deterioro en el aprendizaje asociativo, la memoria episódica, la capacidad intelectual, la resolución de problemas y la planificación, lo que amerita que se generen acciones desde la saludpública para mitigar el impacto que generan a largo plazo

    NUrsing Homes End of Life care Program (NUHELP): developing a complex intervention

Funding has been received for data collection, analysis and interpretation of data and manuscript writing.Background: Nursing homes are likely to become increasingly important as end-of-life care facilities. Previous studies indicate that individuals residing in these facilities have a high prevalence of end-of-life symptoms and a significant need for palliative care. The aim of this study was to develop an end-of-life care program for nursing homes in Spain based on previous models yet adapted to the specific context and the needs of staff in nursing homes in the country. Methods: A descriptive study of a complex intervention procedure was developed. The study consisted of three phases. The first phase was a prospective study assessing self-efficacy in palliative care (using the SEPC scale) and attitudes towards end-of-life care (using the FATCOD-B scale) among nursing home staff before and after the completion of a basic palliative care training program. In the second phase, objectives were selected using the Delphi consensus technique, where nursing home and primary care professionals assessed the relevance, feasibility, and level of attainment of 42 quality standards. In phase 3, interventions were selected for these objectives through two focus group sessions involving nursing home, primary care, and palliative care professionals. Results: As a result of the training, an improvement in self-efficacy and attitudes towards end-of-life care was observed. In phase 2, 14 standards were selected and grouped into 5 objectives: to conduct a comprehensive assessment and develop a personalized care plan adapted to the palliative needs detected; to provide information in a clear and accessible way; to request and record advance care directives; to provide early care with respect to loss and grief; to refer patients to a specialized palliative care unit if appropriate, depending on the complexity of the palliative care required. Based on these objectives, the participants in the focus group sessions designed the 22 interventions that make up the program. Conclusions: The objectives and interventions of the NUHELP program constitute an end-of-life care program which can be implemented in nursing homes to improve the quality of end-of-life care in these facilities by modifying their clinical practice, organization, and relationship with the health system as well as serving as an example of an effective health intervention program.Consejeria de Salud, Junta de Andalucia (Fundacion Publica Andaluza Progreso y Salud) AP-0105-201

    Diagnóstico de las necesidades de capacitación causadas por los cambios estructurales post COVID 19

    Proyecto de graduación (Licenciatura con énfasis en Recursos Humanos) Instituto Tecnológico de Costa Rica, Escuela de Administración de Empresas, 2020.Este trabajo de investigación consiste en realizar un diagnóstico de las necesidades de capacitación causadas por los cambios estructurales post COVID 19 en el tercer bimestre del 2020 para la empresa Search. La metodología de la investigación abarca tres unidades de estudio según los puestos claves de Search: Country Manager, Talent Hunter y clientes, para innovar en los productos y servicios que requieren con base en la disyuntiva mundial. Los métodos de recopilación de información empleados fueron encuestas online y grupos focales. Respecto al análisis de resultados se evalúa aspectos como la percepción del perfil puesto y las necesidades de capacitación antes y después de la crisis, los principales cambios estructurales y su incidencia en la ejecución de sus funciones en los Talent Hunters y Country Manager; para los clientes se tomaron en cuenta los principales retos que han presentado en la gestión del humano, y cómo podría contribuir Search para el logro de sus objetivos estratégicos después del COVID 19. Finalmente, se diseñaron dos herramientas de Excel, una con los diferentes tópicos de capacitación que se proponen y, otra con los gráficos y tablas de la información para el análisis de la información para Search, y sus recomendaciones de uso.This research consists in a diagnosis of the training needs caused by the post Covid 19 structural changes in the II bimester of 2020 for the company Search. The research methodology covers three units of study according to the main areas of Search: Country Manager, Talent Hunter and clients in order to innovate in the products and services they require based on the global changes. The information collection methods used were online surveys and focus groups. Regarding the analysis of results, the main examined areas were the perception of the job profile and the training needs before and after the crisis, the main structural changes and their impact on the execution of the functions of Talent Hunters and Country Manager. For clients, the main challenges they went through in human management were analyses, and how Search could contribute to the achievement of their strategic objectives after Covid 19. Finally, two Excel tools were designed: one of them include the graphics and tables used for the analysis of the information and their recommendations, and the other one include the training plan proposal.Instituto Tecnológico de Costa Rica.
