    Virtual Noncontrast Abdominal Imaging with Photon-counting Detector CT.

    Background Accurate CT attenuation and diagnostic quality of virtual noncontrast (VNC) images acquired with photon-counting detector (PCD) CT are needed to replace true noncontrast (TNC) scans. Purpose To assess the attenuation errors and image quality of VNC images from abdominal PCD CT compared with TNC images. Materials and Methods In this retrospective study, consecutive adult patients who underwent a triphasic examination with PCD CT from July 2021 to October 2021 were included. VNC images were reconstructed from arterial and portal venous phase CT. The absolute attenuation error of VNC compared with TNC images was measured in multiple structures by two readers. Then, two readers blinded to image reconstruction assessed the overall image quality, image noise, noise texture, and delineation of small structures using five-point discrete visual scales (5 = excellent, 1 = nondiagnostic). Overall image quality greater than or equal to 3 was deemed diagnostic. In a phantom, noise texture, spatial resolution, and detectability index were assessed. A detectability index greater than or equal to 5 indicated high diagnostic accuracy. Interreader agreement was evaluated using the Krippendorff α coefficient. The paired t test and Friedman test were applied to compare objective and subjective results. Results Overall, 100 patients (mean age, 72 years ± 10 [SD]; 81 men) were included. In patients, VNC image attenuation values were consistent between readers (α = .60), with errors less than 5 HU in 76% and less than 10 HU in 95% of measurements. There was no evidence of a difference in error of VNC images from arterial or portal venous phase CT (3.3 HU vs 3.5 HU, P = .16). Subjective image quality was rated lower in VNC images for all categories (all, P < .001). Diagnostic quality of VNC images was reached in 99% and 100% of patients for readers 1 and 2, respectively. In the phantom, VNC images exhibited 33% higher noise, blotchier noise texture, similar spatial resolution, and inferior but overall good image quality (detectability index >20) compared with TNC images. Conclusion Abdominal virtual noncontrast images from the arterial and portal venous phase of photon-counting detector CT yielded accurate CT attenuation and good image quality compared with true noncontrast images. © RSNA, 2022 Online supplemental material is available for this article See also the editorial by Sosna in this issue

    The gait and balance of patients with diabetes can be improved: a randomised controlled trial

    Aims/hypothesis: Gait characteristics and balance are altered in diabetic patients. Little is known about possible treatment strategies. This study evaluates the effect of a specific training programme on gait and balance of diabetic patients. Methods: This was a randomised controlled trial (n = 71) with an intervention (n = 35) and control group (n = 36). The intervention consisted of physiotherapeutic group training including gait and balance exercises with function-orientated strengthening (twice weekly over 12weeks). Controls received no treatment. Individuals were allocated to the groups in a central office. Gait, balance, fear of falls, muscle strength and joint mobility were measured at baseline, after intervention and at 6-month follow-up. Results: The trial is closed to recruitment and follow-up. After training, the intervention group increased habitual walking speed by 0.149m/s (p < 0.001) compared with the control group. Patients in the intervention group also significantly improved their balance (time to walk over a beam, balance index recorded on Biodex balance system), their performance-oriented mobility, their degree of concern about falling, their hip and ankle plantar flexor strength, and their hip flexion mobility compared with the control group. After 6months, all these variables remained significant except for the Biodex sway index and ankle plantar flexor strength. Two patients developed pain in their Achilles tendon: the progression for two related exercises was slowed down. Conclusions/interpretation: Specific training can improve gait speed, balance, muscle strength and joint mobility in diabetic patients. Further studies are needed to explore the influence of these improvements on the number of reported falls, patients' physical activity levels and quality of life.     Convergent evolution of a labile nutritional symbiosis in ants

    Ants are among the most successful organisms on Earth. It has been suggested that forming symbioses with nutrient-supplementing microbes may have contributed to their success, by allowing ants to invade otherwise inaccessible niches. However, it is unclear whether ants have evolved symbioses repeatedly to overcome the same nutrient limitations. Here, we address this question by comparing the independently evolved symbioses in Camponotus, Plagiolepis, Formica and Cardiocondyla ants. Our analysis reveals the only metabolic function consistently retained in all of the symbiont genomes is the capacity to synthesise tyrosine. We also show that in certain multi-queen lineages that have co-diversified with their symbiont for millions of years, only a fraction of queens carry the symbiont, suggesting ants differ in their colony-level reliance on symbiont-derived resources. Our results imply that symbioses can arise to solve common problems, but hosts may differ in their dependence on symbionts, highlighting the evolutionary forces influencing the persistence of long-term endosymbiotic mutualisms.Peer reviewe

    Continuous measurements of soil radon under regular field conditions

    Continuous soil radon measurements were performed in the frame of an EU-radon network using the Clipperton II detector. It has been found that in some periods, soil radon levels obtained with one Clipperton II probe are very different from those obtained with another probe placed at the same depth but a short distance apart. Sudden fluctuations of soil radon levels in a given site have been observed as well. To determine whether this behaviour corresponds to the nature of radon gas in the soil or to the detection technique, various experiments have been performed. It has been observed that when different detectors are exposed at the same regular field conditions, their discrepancy is lower than 11%. When the soil radon concentration is approximately constant, the statistical fluctuations of the Clipperton probes are lower than 15%. The application of the Fourier-filtering algorithm to the rough data, removing all frequency components smaller than 1/24 h21 , has led to a complete correlation between different Clipperton II probes. The time response of the probes has been investigated in a specific experiment at the laboratory. It has been found that the response of the probes to a sudden change of radon concentration is controlled by the diffusion process along the bottom tube of the probe. Therefore, this study shows that the experimental data can be attributed to the natural behaviour of soil radon

    DNA topoisomerase I and II expression in drug resistantgerm cell tumours

    A small number of testicular germ cell tumours are refractory to current chemotherapy regimens. DNA topoisomerase I is the target for several new drugs and a potential candidate treatment for chemorefractory germ cell tumours. DNA topoisomerase IIα is the target for etoposide, which is currently used regularly in germ cell tumour treatment. The expression of DNA topoisomerase I and IIα were therefore assessed immunohistochemically in a range of testicular tumours, especially those with persistent malignant elements on retroperitoneal lymph node dissection. Pre-chemotherapy orchidectomy specimens were matched with post-chemotherapy retroperitoneal lymph node dissections to examine changes in expression. There was considerable variation in the expression of topoisomerase I in different tumour types. Both yolk sac tumours and teratoma, mature showed universal expression of topoisomerase I, while 38% of seminomas and 30% of embryonal carcinomas were positive. Strong topoisomerase IIα expression was found in embryonal carcinoma. There was a negative correlation between topoisomerase I and IIα expression (P=0.004) and downregulation of topoisomerase IIα after chemotherapy (P=0.02). Topoisomerase I expression appears to increase in those cases with residual teratoma, mature, but is largely unchanged in those cases remaining as embryonal carcinoma. These results suggest that topoisomerase I inhibitors may be useful in chemorefractory germ cell tumours, especially yolk sac tumours and where there are unresectable residual teratoma, mature deposits

    Mitochondrial DNA parameters in blood of infants receiving lopinavir/ritonavir or lamivudine prophylaxis to prevent breastfeeding transmission of HIV-1

    Children who are human immunodeficiency virus (HIV)-exposed but uninfected (CHEU) accumulate maternal HIV and antiretroviral exposures through pregnancy, postnatal prophylaxis, and breastfeeding. Here, we compared the dynamics of mitochondrial DNA (mtDNA) parameters in African breastfed CHEU receiving lopinavir/ritonavir (LPV/r) or lamivudine (3TC) pre-exposure prophylaxis during the first year of life. The number of mtDNA copies per cell (MCN) and the proportion of deleted mtDNA (MDD) were assessed at day 7 and at week 50 post-delivery (PrEP group). mtDNA depletion was defined as a 50% or more decrease from the initial value, and mtDNA deletions was the detection of mtDNA molecules with large DNA fragment loss. We also performed a sub-analysis with CHEU who did not receive a prophylactic treatment in South Africa (control group). From day seven to week 50, MCN decreased with a median of 41.7% (interquartile range, IQR: 12.1; 64.4) in the PrEP group. The proportion of children with mtDNA depletion was not significantly different between the two prophylactic regimens. Poisson regressions showed that LPV/r and 3TC were associated with mtDNA depletion (reference: control group; LPV/r: PR = 1.75 (CI95%: 1.15–2.68), p < 0.01; 3TC: PR = 1.54 (CI95%: 1.00–2.37), p = 0.05). Moreover, the proportion of children with MDD was unexpectedly high before randomisation in both groups. Long-term health impacts of these mitochondrial DNA parameters should be investigated further for both CHEU and HIV-infected children receiving LPV/r- or 3TC- based regimens.http://www.mdpi.com/journal/jcmpm2021Paediatrics and Child Healt

    Rapid solubility and mineral storage of CO2 in basalt

    The long-term security of geologic carbon storage is critical to its success and public acceptance. Much of the security risk associated with geological carbon storage stems from its buoyancy. Gaseous and supercritical CO2 are less dense than formation waters, providing a driving force for it to escape back to the surface. This buoyancy can be eliminated by the dissolution of CO2 into water prior to, or during its injection into the subsurface. The dissolution makes it possible to inject into fractured rocks and further enhance mineral storage of CO2 especially if injected into silicate rocks rich in divalent metal cations such as basalts and ultra-mafic rocks. We have demonstrated the dissolution of CO2 into water during its injection into basalt leading to its geologic solubility storage in less than five minutes and potential geologic mineral storage within few years after injection [1–3]. The storage potential of CO2 within basaltic rocks is enormous. All the carbon released from burning of all fossil fuel on Earth, 5000 GtC, can theoretically be stored in basaltic rocks [4]
