Perceptions of fecal microbiota transplantation for Clostridium difficile infection: factors that predict acceptance.
BackgroundDespite the effectiveness of fecal microbiota transplantation (FMT) for treating recurrent Clostridium difficile (C. difficile) infection, some patients are reluctant to accept this therapy. Our study examined attitudes towards FMT and factors that contribute to patients' acceptance of this treatment.MethodsWe distributed patient surveys at a Veterans Affairs hospital, a public hospital, and an academic faculty practice. Multivariable logistic regression was performed, adjusting for factors associated with FMT acceptance on univariate analysis and prior experience with C. difficile infection.ResultsOf 267 patients, only 12% knew of FMT prior to the survey, but 77% would undergo the procedure if medically indicated. On multivariable analysis, those with children and with college degrees or higher were more likely to agree to FMT (odds ratio [OR] 2.11, 95% confidence interval [CI] 1.02-4.35; OR 2.27, 95% CI 1.11-4.60 respectively). Sixty-five respondents (71%) chose colonoscopy as the preferred vehicle for FMT, while nasogastric tube was least preferred. Disease transmission was the most common concern (30%, n=242), and FMT success rate was the least selected concern (9.1%).ConclusionsMost patients in a diverse sample of gastroenterology clinics had no prior knowledge of FMT, but were receptive to the procedure. Having children and higher education levels were predictors for FMT acceptance. Our findings suggest that barriers to FMT utilization may be overcome with counseling about safety concerns. More data on the risk of transmitting diseases or clinical characteristics, such as obesity, through FMT are needed and will be important for the acceptance of this procedure
A COMMUNITY-BASED VULNERABILITY ASSESSMENT OF FLOODS IN URBAN AREAS OF KAMPUNG MELAYU, JAKARTA
Flooding has become a serious problem in Jakarta. During floods .of 2007,
Kampung Melayu, East Jakarta was the worst hit.by the floods. Community have
different perceptions on disaster and have different effort to overcome the hazards.
Therefore, local government and relevant institution should investigate this
situation and make this information a valuable input in developing and
implementing response plans in flood mitigation. This research is to explore the
vulnerability of floods based on local people\u27s perception. There were 83
households interviewed using questionnaire. Certain elements at risk related with
physical and socio:economic aspects were identified. Physical information
concerned the building structure and building contents. Several socio-economic
characteristics were used as key indicators to analyze the vulnerability of people.
Generally, the result of this research shows that the ability of people to cope with
the flooding i$ linked with the capacity of the people itself. The capability of people
to deal withflooding was influenced by several indicators.based on their socio- .
economic characteristics. For example, lower income people will experience more
suffering than the wealthier, because they cannot afford the\u27 costs of repair,
reconstruction. Although the wealthier are likely to experience a higher degree of
economic damage due to possessions of higher value. Base on the analysis, all
coping strategies and flood measures are not enough to cope with flooding in the
study area
Macroparasite communities in European eels, <i>Anguilla anguilla</i>, from French Mediterranean lagoons, with special reference to the invasive species <i>Anguillicola crassus</i> and <i>Pseudodactylogyrus</i> spp.
European eel parasites, in particular invasive species, are suspected to play a role in the decline in the populations of their host. The aims of this work were to describe the parasitic fauna of eels in French Mediterranean lagoons and to study the epidemiological trends of the invasive helminth species, the nematode Anguillicola crassus and the monogenean Pseudodactylogyrus spp., in regard to spatio-temporal dynamics, host biological characteristics and parasite community. A total of 418 eels was sampled in eight lagoons between March 2003 and June 2005. Our results revealed a total macroparasite richness of 23 species: 1 Monogenea, 13 Digenea, 2 Cestoda, 3 Nematoda, 2 Acantocephala and 2 Crustacea. We found no variation in A. crassus abundance in Salses-Leucate lagoon in the same month across years. However, the nematode abundance was higher in eels caught in summer than in those caught in winter. Pseudodactylogyrus sp. was not found in Salses-Leucate lagoon, except in July 2004. Comparisons between the lagoons on the same date showed that they could be separated into two groups for both species' abundance: Grau-du-Roi, Mauguio, Palavas and Vaccares lagoons, where abundance was rather high, against Bages-Sigean, Pierre-Blanche, Salses-Leucate and Thau lagoons, where abundance was rather low or nil. We found significant negative relationships between A. crassus abundance and the length and age of eels. We also found a significant positive relationship between A. crassus and Pseudodactylogyrus sp. abundance. Finally, our results showed significant positive relationships between both A. crassus and Pseudodactylogyrus sp. abundance and the abundance of the digeneans Prosorhynchus aculeatus and Lecithochirium gravidum. We discuss the results in regard to the dynamics of invasions, the characteristics of the parasite life cycles and the ecology of eels
Vitamin C Prevents Hypogonadal Bone Loss
Epidemiologic studies correlate low vitamin C intake with bone loss. The genetic deletion of enzymes involved in de novo vitamin C synthesis in mice, likewise, causes severe osteoporosis. However, very few studies have evaluated a protective role of this dietary supplement on the skeleton. Here, we show that the ingestion of vitamin C prevents the low-turnover bone loss following ovariectomy in mice. We show that this prevention in areal bone mineral density and micro-CT parameters results from the stimulation of bone formation, demonstrable in vivo by histomorphometry, bone marker measurements, and quantitative PCR. Notably, the reductions in the bone formation rate, plasma osteocalcin levels, and ex vivo osteoblast gene expression 8 weeks post-ovariectomy are all returned to levels of sham-operated controls. Conserve Epitopes of Influenza Virus Induce Innate and Adaptive Immune Responses to Produce Specific Antibody Against M2e Protein
The existing vaccines against influenza are based onthe generation of neutralizing antibody primarilydirected against surface protein, haemagglutinin(HA) and neuraminidase (NA). However, antigenicdrift and occasional shift of these two membraneglycoproteins, HA and NA, make vaccine productioncumbersome and necessitate yearly revision ofthe vaccine seed strains by the World HealthOrganization. For these reasons, many investigatorshave often tried to look at the possibility of generatinga universal vaccine useful against more than oneinfluenza strain. The objective of research was toobtain an alternative antigen as vaccine candidatefor universal flu vaccination, instead of HA and NAcomponents. In this study, we use conserved epitopeM2e which is consist of three major componentsuch as N-terminal M2e2-24 (24 amino acids),transmembrane(59 amino acids) and C-terminal (19amino acids). We design two components of antigen,linier and branched structures. The antigens thenformulated with aluminium hydroxide gel comparedto FCA/IFA adjuvant. These vaccines were testedtheir immunogenicity, and the potency to mature thedendritic cells for stimulating either CD8+ T cell orantibody-mediated immune responses. The antibodytitre and the maturity of dendritic cell indicated bycytokines concentration such as; IFN-Γ£, IL2 and IL4were measured by ELISA test.The result of researchshowed that the conserved epitope of Me2 2-16 whenincorporated with P25 protein from canine distempervirus (linear structure) in alhydrogel adjuvant hasgreater potential to produce anti-M2e antibodiesthan in Freund adjuvant. Through SMEs, the community is expected to be able to empower all the resources they have to achieve prosperity. SMEs in the food sector are very high growth, SME capacity development needs to be improved, and food security issues need to be addressed. Fulfillment of safe and quality food is the basic right of every human being, including food produced by the Household Food Industry. CV. Panca Boga Langgeng is our abdimas partner that produces household-scale food products, which have the desire to increase its business capacity, the obstacles faced are market share only limited to family, friends and circles around the place of business. Business owners cannot produce on a larger scale and market widely because they do not have legality. To understand the problems and potential of our partner SMEs, we use the SWOT method, the work system of the SWOT method in formulating strategic solutions is outlined in the matrix, internal and external aspects are used as the basis for determining strategies or efforts that can be achieved so that business objectives can be achieved. A Somatically Diversified Defense Factor, FREP3, Is a Determinant of Snail Resistance to Schistosome Infection
Schistosomiasis, a neglected tropical disease, owes its continued success to freshwater snails that support production of prolific numbers of human-infective cercariae. Encounters between schistosomes and snails do not always result in the snail becoming infected, in part because snails can mount immune responses that prevent schistosome development. Fibrinogen-related protein 3 (FREP3) has been previously associated with snail defense against digenetic trematode infection. It is a member of a large family of immune molecules with a unique structure consisting of one or two immunoglobulin superfamily domains connected to a fibrinogen domain; to date fibrinogen containing proteins with this arrangement are found only in gastropod molluscs. Furthermore, specific gastropod FREPs have been shown to undergo somatic diversification. Here we demonstrate that siRNA mediated knockdown of FREP3 results in a phenotypic loss of resistance to Schistosoma mansoni infection in 15 of 70 (21.4%) snails of the resistant BS-90 strain of Biomphalaria glabrata. In contrast, none of the 64 control BS-90 snails receiving a GFP siRNA construct and then exposed to S. mansoni became infected. Furthermore, resistance to S. mansoni was overcome in 22 of 48 snails (46%) by pre-exposure to another digenetic trematode, Echinostoma paraensei. Loss of resistance in this case was shown by microarray analysis to be associated with strong down-regulation of FREP3, and other candidate immune molecules. Although many factors are certainly involved in snail defense from trematode infection, this study identifies for the first time the involvement of a specific snail gene, FREP3, in the phenotype of resistance to the medically important parasite, S. mansoni. The results have implications for revealing the underlying mechanisms involved in dictating the range of snail strains used by S. mansoni, and, more generally, for better understanding the phenomena of host specificity and host switching. It also highlights the role of a diversified invertebrate immune molecule in defense against a human pathogen. It suggests new lines of investigation for understanding how susceptibility of snails in areas endemic for S. mansoni could be manipulated and diminished
The incremental yield of prenatal exome sequencing over chromosome microarray for congenital heart abnormalities:A systematic review and meta-analysis
ObjectivesTo determine the incremental yield of prenatal exome sequencing (PES) over standard testing in fetuses with an isolated congenital heart abnormality (CHA), CHA associated with extra-cardiac malformations (ECMs) and CHA dependent upon anatomical subclassification.MethodsA systematic review of the literature was performed using MEDLINE, EMBASE, Web of Science and grey literature January 2010-February 2023. Studies were selected if they included greater than 20 cases of prenatally diagnosed CHA when standard testing (QF-PCR/chromosome microarray/karyotype) was negative. Pooled incremental yield was determined. PROSPERO CRD 42022364747.ResultsOverall, 21 studies, incorporating 1957 cases were included. The incremental yield of PES (causative pathogenic and likely pathogenic variants) over standard testing was 17.4% (95% CI, 13.5%-21.6%), 9.3% (95% CI, 6.6%-12.3%) and 35.9% (95% CI, 21.0%-52.3%) for all CHAs, isolated CHAs and CHAs associated with ECMs. The subgroup with the greatest yield was complex lesions/heterotaxy; 35.2% (95% CI 9.7%-65.3%). The most common syndrome was Kabuki syndrome (31/256, 12.1%) and most pathogenic variants occurred de novo and in autosomal dominant (monoallelic) disease causing genes (114/224, 50.9%).ConclusionThe likelihood of a monogenic aetiology in fetuses with multi-system CHAs is high. Clinicians must consider the clinical utility of offering PES in selected isolated cardiac lesions.What is already known?Congenital heart abnormalities are the most commonly occurring congenital anomalies and can be associated with chromosomal or monogenic conditions. With the increasing use of fetal sequencing, there is a need to define the association between monogenic conditions and specific cardiac abnormalities, particularly when isolated to facilitate triaging for prenatal sequencing.What does this study add?The incremental yield of prenatal exome sequencing over and above chromosome microarray for congenital heart abnormalities is 9.3% in isolated lesions and 35.2% in the presence of complex lesions/heterotaxy.
