    Effect of concrete properties and prestressing steel indentation types on the development length and flexural capacity of pretensioned concrete members

PetermanA study was conducted to determine the effect of different concrete properties and prestressing steel indentation types on development length and flexural capacity of pretensioned members. Wires and strands commonly used in the manufacturing of prestressed concrete railroad ties worldwide were selected for the study. Thirteen different 5.32-mm-diameter prestressing wire types and six different strands (four, seven-wire strands and two, three-wire strands) were used to cast prisms with a square cross section. The ratio of concrete to prestressed steel in the test prism’s cross section was representable of typical concrete railroad ties. Thus, geometrical and mechanical properties of test prisms were representative of actual ties in the railroad industry. To understand the effect of concrete-release strengths and slumps on development length, all parameters were kept constant in the prisms except concrete-release strength and slump. To manufacture prisms with different release strengths, all four wires/strands were pulled and detensioned gradually when the concrete compressive strength reached 3500 (24.13 MPa), 4500 (31.03 MPa), and 6000 (41.37 MPa) psi. To determine the effect of different slumps on development length, prisms with different slumps of 3 in. (7.6 cm), 6 in. (15.2 cm), and 9 in. (22.9 cm) were manufactured and all other parameters were kept constant in prisms. All prisms were tested in three-point bending at different spans to obtain estimations of development length based on type of reinforcement, concrete-release strength, and concrete slump. Lastly, a design equation was developed based on experimental data for prediction of development length. In the last phase of load tests, cyclic-loading tests were conducted on the prisms manufactured with wires to evaluate the bond performance of wires with different indentation types under cyclic loading. A total of 210 load tests, including 14 cyclic tests, were conducted. The monotonic-load tests revealed a large difference in the development length of pretensioned concrete members manufactured with different wire/strand types and different concrete-release strengths. Also, the cyclic-load tests revealed a significant difference in bond performance of different wire types under cyclic loading compared to monotonic loading

    Y-cracking in continuously reinforced concrete pavements

RidingWhen transverse cracks meander there is a high possibility for transverse cracks to meet at a point and connect to another transverse crack, creating a Y-crack. Y-cracks have been blamed for being the origin of punchouts and spallings in CRCPs. When the direction of maximum principal stress changes, it could cause a change in the crack direction, potentially forming a Y-crack. Finite Element Models (FEMs) were run to model the change in principal stress direction based on design and construction conditions. The finite element model of CRCP using typical Oklahoma CRCP pavement conditions and design was assembled. The model included the concrete pavement, asphalt concrete subbase, and soil subgrade. The effect of areas of changed friction on the direction of principal stress was simulated by considering a patch at the pavement-subbase interaction. Investigated factors related to this patch were location of patch, friction between patch and subbase, and patch size. Patches were placed at two different locations in the pavement: a patch at the corner of the pavement and a patch at the longitudinal edge between pavement ends. A change in the friction at the corner had a large effect on the stress magnitude and direction of principal stress, while a patch in the middle did not significantly change the stress state. Also, patch size had a noticeable effect on stress magnitude when the patch was at the corner. Another model was developed to understand the effect of jointed shoulder on direction of maximum principal stress. Analysis of this model showed that the stresses were not symmetric and changed along the width of the pavement. This meandering pattern shows a high potential for Y-cracking. Also, several finite element models were run to understand the effects of different shrinkage between mainline and shoulder. In order to simulate the effects of the differential drying shrinkage between the hardened mainline concrete and the newly cast shoulder, different temperature changes were applied on the mainline and shoulder. For these models, the orientation of the maximum principal stress was not significantly changed from different amounts of temperature decreases between mainline and shoulder. Also, effect of different longitudinal steel percentages was investigated by comparing two finite element models with different steel percentage. The model with higher steel percentage (0.7%) indicated more variation in stress, potentially leading to more crack direction diverging

    This report covers the investigation of the impact of Y-cracking on concrete pavements in Oklahoma. This study used field and analytical investigations to determine the impact of Ycracking on the long term performance of continuous concrete pavements. The research found that Y-cracking in concrete pavements does lead to an increase in the number of punch outs. Of all of the variables investigated the base type and the shoulder type had the largest impact on the amount of Y-cracking that occurred in continuous concrete pavements. The analytical studies confirmed that these variables can lead to increases in pavement stresses and so therefore cracking.Final Report October 2011-December 2012N

A consistent concrete mixture utilizing Type III cement and a water-to-cementitious ratio of 0.32 was used for all prisms. The prisms were detensioned at concrete compressive strengths of 3500 psi, 4500 psi, and 6000 psi. Load tests revealed that there is a large difference in the development length for different wire/strand types as well as with different concrete release strengths. Additionally, cyclic load tests revealed that there is also a significant difference in the bond performance of these reinforcement types under repeated loadings

    Prospective observational cohort study on grading the severity of postoperative complications in global surgery research

    Background The Clavien–Dindo classification is perhaps the most widely used approach for reporting postoperative complications in clinical trials. This system classifies complication severity by the treatment provided. However, it is unclear whether the Clavien–Dindo system can be used internationally in studies across differing healthcare systems in high- (HICs) and low- and middle-income countries (LMICs). Methods This was a secondary analysis of the International Surgical Outcomes Study (ISOS), a prospective observational cohort study of elective surgery in adults. Data collection occurred over a 7-day period. Severity of complications was graded using Clavien–Dindo and the simpler ISOS grading (mild, moderate or severe, based on guided investigator judgement). Severity grading was compared using the intraclass correlation coefficient (ICC). Data are presented as frequencies and ICC values (with 95 per cent c.i.). The analysis was stratified by income status of the country, comparing HICs with LMICs. Results A total of 44 814 patients were recruited from 474 hospitals in 27 countries (19 HICs and 8 LMICs). Some 7508 patients (16·8 per cent) experienced at least one postoperative complication, equivalent to 11 664 complications in total. Using the ISOS classification, 5504 of 11 664 complications (47·2 per cent) were graded as mild, 4244 (36·4 per cent) as moderate and 1916 (16·4 per cent) as severe. Using Clavien–Dindo, 6781 of 11 664 complications (58·1 per cent) were graded as I or II, 1740 (14·9 per cent) as III, 2408 (20·6 per cent) as IV and 735 (6·3 per cent) as V. Agreement between classification systems was poor overall (ICC 0·41, 95 per cent c.i. 0·20 to 0·55), and in LMICs (ICC 0·23, 0·05 to 0·38) and HICs (ICC 0·46, 0·25 to 0·59). Conclusion Caution is recommended when using a treatment approach to grade complications in global surgery studies, as this may introduce bias unintentionally

    The surgical safety checklist and patient outcomes after surgery: a prospective observational cohort study, systematic review and meta-analysis

    © 2017 British Journal of Anaesthesia Background: The surgical safety checklist is widely used to improve the quality of perioperative care. However, clinicians continue to debate the clinical effectiveness of this tool. Methods: Prospective analysis of data from the International Surgical Outcomes Study (ISOS), an international observational study of elective in-patient surgery, accompanied by a systematic review and meta-analysis of published literature. The exposure was surgical safety checklist use. The primary outcome was in-hospital mortality and the secondary outcome was postoperative complications. In the ISOS cohort, a multivariable multi-level generalized linear model was used to test associations. To further contextualise these findings, we included the results from the ISOS cohort in a meta-analysis. Results are reported as odds ratios (OR) with 95% confidence intervals. Results: We included 44 814 patients from 497 hospitals in 27 countries in the ISOS analysis. There were 40 245 (89.8%) patients exposed to the checklist, whilst 7508 (16.8%) sustained ≥1 postoperative complications and 207 (0.5%) died before hospital discharge. Checklist exposure was associated with reduced mortality [odds ratio (OR) 0.49 (0.32–0.77); P\u3c0.01], but no difference in complication rates [OR 1.02 (0.88–1.19); P=0.75]. In a systematic review, we screened 3732 records and identified 11 eligible studies of 453 292 patients including the ISOS cohort. Checklist exposure was associated with both reduced postoperative mortality [OR 0.75 (0.62–0.92); P\u3c0.01; I2=87%] and reduced complication rates [OR 0.73 (0.61–0.88); P\u3c0.01; I2=89%). Conclusions: Patients exposed to a surgical safety checklist experience better postoperative outcomes, but this could simply reflect wider quality of care in hospitals where checklist use is routine
