    Castleman\u27s disease -- a rare disease of lymph nodes

    Castleman\u27s disease is a rare disorder of unknown etiology and different clinical manifestations, occurring in young adults (usually prior to age 30 years). In this case report we present one such case who was a 16 years old male with abdominal pain, weight loss, cervical lymph adenopathy and hepatosplenomegaly. Lymph node biopsy revealed hyaline-vascular type of Castleman\u27s disease. He was put on chemotherapy but was lost to follow-up

    Knowledge, apptitude and practice: smoking and gutka habits in a lower socio-economic cohort

    Background: Smoking and smokeless tobacco use is the major cause of death and disease in South Asia, the use of gutka has surpassed that of smoking in this region thus, acting as a major contributor to oral and esophageal cancer especially in the younger population. Authors have designed this study to explore the knowledge, apptitude and practice regarding smoking and gutka use in a lower socio-economic cohort and observe the impact of education, income and type of profession on the users.Methods: This cross sectional descriptive survey is conducted in Creek General Hospital, Korangi, Karachi. The study design is non-probability convenience. A questionnaire is used to quantify the frequency of smoking-related knowledge-attitude-practice, knowledge and attitude about tobacco-related disease and, smoking cessation-related thoughts and practice. Data is analysed in SPSS 16.Results: A total of 250 subjects participated in this descriptive study, 133 subjects (22% females and 62.6% males) were found addicted to smoking or smokeless tobacco product with stress, seeking pleasure and peer pressure being the most convincing factor to initiate their habit.90% of the users want to stop using the substance of abuse and 80 have at least once tried to quit the habit.Conclusions: This study shows a direct relationship between level of education and socioeconomic status with use of addictive substance. It also resulted in identifying the desire to quit in our cohort and need for formulating a plan specifically for the target population to focus not only to remove the cause, but also to empower them to take responsibility for their own well being.

    Provenance-enabled Packet Path Tracing in the RPL-based Internet of Things

    The interconnection of resource-constrained and globally accessible things with untrusted and unreliable Internet make them vulnerable to attacks including data forging, false data injection, and packet drop that affects applications with critical decision-making processes. For data trustworthiness, reliance on provenance is considered to be an effective mechanism that tracks both data acquisition and data transmission. However, provenance management for sensor networks introduces several challenges, such as low energy, bandwidth consumption, and efficient storage. This paper attempts to identify packet drop (either maliciously or due to network disruptions) and detect faulty or misbehaving nodes in the Routing Protocol for Low-Power and Lossy Networks (RPL) by following a bi-fold provenance-enabled packed path tracing (PPPT) approach. Firstly, a system-level ordered-provenance information encapsulates the data generating nodes and the forwarding nodes in the data packet. Secondly, to closely monitor the dropped packets, a node-level provenance in the form of the packet sequence number is enclosed as a routing entry in the routing table of each participating node. Lossless in nature, both approaches conserve the provenance size satisfying processing and storage requirements of IoT devices. Finally, we evaluate the efficacy of the proposed scheme with respect to provenance size, provenance generation time, and energy consumption.Comment: 14 pages, 18 Figure

Carbon nanodots are fluorescent materials and possess unique characteristics such as biocompatibility, photostability, low toxicity, sustainable, and eco-friendly. Fluorescent carbon nanodots are emerging nanomaterials that show promising potential in bioimaging, optical sensing, information encryption and storage, photocatalysis, lasers, drug delivery, energy conversion, and photovoltaic applications.     A novel CuBi2O4/polyaniline composite as an efficient photocatalyst for ammonia degradation

    A novel polyaniline (PANI) coupled CuBi2O4 photocatalyst was successfully synthesized via in situ polymerization of aniline with pre-synthesized CuBi2O4 composites. The structure and morphology of the synthesized CuBi2O4/PANI composite photocatalyst were characterized by X-ray diffraction (XRD) and Fourier transform infrared spectroscopy (FTIR) and the photocatalytic performance were evaluated through degradation process of ammonia in water under visible light irradiation. The resultant CuBi2O4/PANI composite showed exceptional stability as its structure and morphology persisted even after being immersed in water for 2 days. The composite photocatalyst exhibited improved charge transport properties due to the electrical conductivity of the PANI protective layer, leading to enhanced photoelectrochemical activity in water and removal of ammonia. PANI with CuBi2O4 (10% wt) heterostructure was applied for photodegradation of ammonia and exhibited a 96% ammonia removal efficiency (30 mg/l with 0.1 g photocatalyst and 180 min), as compared to PANI (78%) and CuBi2O4 (70%). The degradation was attributed to the efficient charge transfer (e− and h+) and formation of reactive oxygen species upon simulated sunlight exposure. The present work suggests that the CuBi2O4/PANI photocatalyst can be synthesized in a simple process and provides an excellent adsorption capacity, high photocatalytic activity, long term stability, and reusability making it a promising alternative for ammonia removal from wastewater

    Evidence for a Founder Effect among HIV-infected injection drug users (IDUs) in Pakistan.

    Background: We have previously reported a HIV-1 subtype A infection in a community of injection drug users (IDUs) in Karachi, Pakistan. We now show that this infection among the IDUs may have originated from a single source. Methods: Phylogenetic analysis was performed of partial gag sequences, generated using PCR, from 26 HIV-positive IDU samples. Results: Our results showed formation of a tight monophyletic group with an intra-sequence identity of \u3c 98% indicating a founder effect . Our data indicate that the HIV-1 epidemic in this community of IDUs may have been transmitted by an HIV positive overseas contract worker who admitted to having contact with commercial sex workers during stay abroad. Conclusion: Specific measures need to implemented to control transmission of HIV infection in Pakistan through infected migrant workers

    Nanoremediation technologies for sustainable remediation of contaminated environments: Recent advances and challenges

    A major and growing concern within society is the lack of innovative and effective solutions to mitigate the challenge of environmental pollution. Uncontrolled release of pollutants into the environment as a result of urbanisation and industrialisation is a staggering problem of global concern. Although, the eco-toxicity of nanotechnology is still an issue of debate, however, nanoremediation is a promising emerging technology to tackle environmental contamination, especially dealing with recalcitrant contaminants. Nanoremediation represents an innovative approach for safe and sustainable remediation of persistent organic compounds such as pesticides, chlorinated solvents, brominated or halogenated chemicals, perfluoroalkyl and polyfluoroalkyl substances (PFAS), and heavy metals. This comprehensive review article provides a critical outlook on the recent advances and future perspectives of nanoremediation technologies such as photocatalysis, nano-sensing etc., applied for environmental decontamination. Moreover, sustainability assessment of nanoremediation technologies was taken into consideration for tackling legacy contamination with special focus on health and environmental impacts. The review further outlines the ecological implications of nanotechnology and provides consensus recommendations on the use of nanotechnology for a better present and sustainable futur

    Cell-based meat: The molecular aspect

    Cellular agriculture is one of the evolving fields of translational biotechnology. The emerging science aims to improve the issues related to sustainable food products and food security, reduce greenhouse gas emissions and provide animal wellbeing by circumventing livestock farming through cell-based meat (CBM) production. CBM exploits cell culture techniques and biomanufacturing methods by manipulating mammalian, avian, and fish cell lines. The cell-based products ought to successfully meet the demand for nutritional protein products for human consumption and pet animals. However, substantial advancement and modification are required for manufacturing CBM and related products in terms of cost, palatability, consumer acceptance, and safety. In order to achieve high-quality CBM and its production with high yield, the molecular aspect needs a thorough inspection to achieve good laboratory practices for commercial production. The current review discusses various aspects of molecular biology involved in establishing cell lines, myogenesis, regulation, scaffold, and bioreactor-related approaches to achieve the target of CBM

    May Measurement Month 2018: a pragmatic global screening campaign to raise awareness of blood pressure by the International Society of Hypertension

    Aims Raised blood pressure (BP) is the biggest contributor to mortality and disease burden worldwide and fewer than half of those with hypertension are aware of it. May Measurement Month (MMM) is a global campaign set up in 2017, to raise awareness of high BP and as a pragmatic solution to a lack of formal screening worldwide. The 2018 campaign was expanded, aiming to include more participants and countries. Methods and results Eighty-nine countries participated in MMM 2018. Volunteers (≥18 years) were recruited through opportunistic sampling at a variety of screening sites. Each participant had three BP measurements and completed a questionnaire on demographic, lifestyle, and environmental factors. Hypertension was defined as a systolic BP ≥140 mmHg or diastolic BP ≥90 mmHg, or taking antihypertensive medication. In total, 74.9% of screenees provided three BP readings. Multiple imputation using chained equations was used to impute missing readings. 1 504 963 individuals (mean age 45.3 years; 52.4% female) were screened. After multiple imputation, 502 079 (33.4%) individuals had hypertension, of whom 59.5% were aware of their diagnosis and 55.3% were taking antihypertensive medication. Of those on medication, 60.0% were controlled and of all hypertensives, 33.2% were controlled. We detected 224 285 individuals with untreated hypertension and 111 214 individuals with inadequately treated (systolic BP ≥ 140 mmHg or diastolic BP ≥ 90 mmHg) hypertension. Conclusion May Measurement Month expanded significantly compared with 2017, including more participants in more countries. The campaign identified over 335 000 adults with untreated or inadequately treated hypertension. In the absence of systematic screening programmes, MMM was effective at raising awareness at least among these individuals at risk
