    P-matrix and J-matrix approaches. Coulomb asymptotics in the harmonic oscillator representation of scattering theory

    The relation between the R- and P-matrix approaches and the harmonic oscillator representation of the quantum scattering theory (J-matrix method) is discussed. We construct a discrete analogue of the P-matrix that is shown to be equivalent to the usual P-matrix in the quasiclassical limit. A definition of the natural channel radius is introduced. As a result, it is shown to be possible to use well-developed technique of R- and P-matrix theory for calculation of resonant states characteristics, scattering phase shifts, etc., in the approaches based on harmonic oscillator expansions, e.g., in nuclear shell-model calculations. P-matrix is used also for formulation of the method of treating Coulomb asymptotics in the scattering theory in oscillator representation.Comment: Revtex, 57 pages including 15 figures; to be published in Annals of Physic

    Varicose veins in adolescents: A prospective cohort study

    The article describes the experience of diagnostics and treatment of chronic diseases of the veins of the lower extremities in 164 female patients. Objective. To study the results of diagnosis and treatment of varicose veins in adolescent and adult females. Patient characteristics and research methods. The authors carried out a prospective study, they analyzed health condition of a group of adolescents and examined this group in 5 years later (the adult part of the study). The authors used clinical examination for diagnostics (disease assessment according to CEAP classification), ultrasound examinations, telephone interviews. Re-examination of the patients showed good long-term results of treatment. There were no re-operations. Results. According to the data of a five-year follow-up, interventions were performed in 4 (4%) cases in the study group. The proportion of cured patients remained without significant changes. Conclusion. 1. Phlebology of childhood is characterized by the predominance of uncomplicated forms of varicose veins, with a simultaneous high concern about the disease of the patient and parents. 2. Timely surgical and laser interventions and the prescribed nonsurgical treatment program allow achieving a lasting improvement in the majority of patients with good long-term results. 3. Observation of patients with chronic venous diseases during their transition to the adult age group shows the preservation of good results in the majority of cases

    Recommendations for determining optimal age for the treatment of congenital heart defects

    The article presents the analysis of congenital heart defects classifications from the practical point of view. A variant of our classification of congenital heart defects is presented with the rationale for optimal time of surgical correction. The scheme-classification allows predicting possible critical hemodynamic states in children with congenital heart defects and is used in the practice in the Republic of Tatarstan for more than 15 years. Organizational measures taken during the recent decades in the field of cardiac and vascular surgery according to goal-oriented programs of the Government of RF and Ministry of healthcare of RF under the direct control of the chief cardiac surgeon of the Ministry of healthcare of RF academician L.A. Bokeriya, resulted in significant improvement of the situation with this specialty in Russia. In particular, regarding treatment of congenital heart defects, the number of surgeries in RF (about 15 000 per year) exceeds the number of children born with congenital heart defects (12 000 to 13 000 per year). Thus, in the nearby future the surgery waiting list will be completed, and some time later the named numbers will become equal. In the Republic of Tatarstan the waiting list for the surgeries for congenital heart defects was completed in 2005 due to additional financing from the president M.Sh. Shaymiev, and since then all children with congenital heart defects are registered soon after birth and are operated on in the optimal time that determines the minimal risk of postsurgical complications and lethal outcomes

    The article is devoted to the analysis of classifications of congenital heart defects from a practical point of view. The researchers present their classification of congenital heart defects with the substantiation of optimal terms of surgical correction. The proposed classification allows us to predict possible critical hemodynamic conditions in children with heart defects both in the neonatal period and in other age groups. The classification creates the basis for choosing the treatment tactics of patients with congenital heart defects using the known stages of hemodynamic changes, it has been used by the cardiologists and cardiac surgeons in the Republic of Tatarstan for more than 15 years

    The diagnosis and treatment of life-threatening arrhythmias after surgical correction of congenital heart diseases is one the urgent problems in pediatrlc cardiology. The paper describes the clinical picture, diagnosis, and therapy in a patent with transient complete atrioventricular block after open heart surgery. A protracted Morgagni—Adams—Stokes attack is one of the causes of sudden cardiac death. The occurrence of even single syncopes in patients after surgery for congenital heart disease requires that the arrhythmogenic nature of the attack should be ruled out for the timely and adequate therapy including the implantation of antiarrhythmic devices

    Children with lymphatic malformations and their treatment using video-endoscopic equipment

    The problem of treating patients with lymphangioma remains open to doctors of various surgical specialties due to the specific location, age of clinical manifestation and the lack of a single algorithm for the treatment of these patients.The objective is to develop an optimal minimally invasive method for treating children with lymphangiomas. This article presents the results of surgical treatment of 7 children with lymphangioma according to two methods developed in our clinic using video endoscopic equipment. The patients with various types of lymphangioma and topographic localizations require an individual approach in choosing the optimal treatment method, taking into account all possible risks of relapse. The developed treatment methods using video-endoscopic equipment are applicable for minimally invasive surgical treatment methods in children (including newborns) and can be used to treat lymphangiomas of both superficial localization and multichamber diffuse lymphangiomas. 7 children were treated with the proposed methods. A positive result was achieved both in the case of radical simultaneous removal of lymphangioma under video control, and in the case of treatment method supplemented by sclerotherapy. We consider the proposed treatment methods acceptable due to their positive clinical result and good cosmetic effect
