The protective scale of the Armidilo‐S:the importance of forensic and clinical outcomes
Background: The Armidilo has two scales—the risk scale and the protective scale. Research has been confined to the risk scale which appears to predict future incidents with medium to large effect sizes. There have been no publications on the use of the protective scale.Methods: The Armidilo was completed on four individuals with IDD who were either moving on from their placement or whose placement was in jeopardy because of new information or altered policies in the organization. The Armidilo was completed in the usual fashion.Results: Risk and protective results show that for each individual, recommendations could be made that ensured the best outcome. For two participants, restrictive placements were avoided because of the data on protective factors.Conclusions: The protective scale can be a powerful support for the clinician's case in offenders with IDD. The protective scale should be completed routinely for clinical evaluation

Evaluating patient literacy to improve health outcomes in Milton, VT
Health literacy is one aspect of overall literacy that has an enormous impact on patient health outcomes, including knowledge of disease, health markers, general health status, and number of hospitalizations, among others. More than 1/3 of the US population has basic or below basic health literacy status. In Chittenden County, diabetes and heart disease deaths are higher than the state average; these diseases require complex personal care and management. Health care providers working at Milton Family Practice in Milton, VT, often noted that patients were often unwilling to disclose literacy issues and there were often problems communicating instructions to patients when providers were unaware of their literacy status/abilities. Residents working at Milton Family Practice were given three simple screening questions that were validated to assess patients\u27 health literacy status with good sensitivity and specificity. These questions are quick and easy to use and avoid many issues with embarrassment or stigma. Residents were also provided local community resources to refer patients to who may benefit from additional help, including the Milton Public Library and the Milton Family Community Center.https://scholarworks.uvm.edu/fmclerk/1182/thumbnail.jp

Infiltration and short-term movement of nitrogen in a silt-loam soil typical of rice cultivation in Arkansas
Rice production in Arkansas is one of the top three crop commodities in terms of cash receipts. Researchers and farmers report that nitrogen (N) needs to be managed according to a variety of factors with two important ones being soil and fertilizer type. The objectives of this experiment were to determine: 1) the degree to which floodwater-incorporated N applied as urea or as ammonium sulfate infiltrates intact cores (7.2-cm dia., 10-cm depth) containing DeWitt siltloam soil, and 2) the distribution of N during 12 h of ponding. Inorganic-N concentrations were analyzed at 2-cm depth intervals in cores following removal of the flood. Nitrogen from applied fertilizer was recovered as ammonium. Ammonium sulfate-N remained in the top 4 cm of soil with concentrations of 375 µg N g-1 in the surface 2 cm and 300 µg N g-1 at the 2 - 4 cm depth after 12 hr of ponding. At all depth intervals below 4 cm, ammonium sulfate-N remained below 30 µg N g-1. In contrast, after 12 h of ponding, N in soil receiving urea was 105 µg N g-1 in the top 2 cm and 173 µg N g-1 at 2-4 cm. At 4-6, 6-8, and 8-10 cm, N was 109, 108, and 35 µg N g-1, respectively, after 12 h of ponding. These results demonstrate immediate and deeper movement of ammonium into silt loam soil receiving urea as compared to ammonium sulfate, demonstrating how the form of N in fertilizer affects its movement into the soil profile
Effects of Art Intervention on Pediatric Anxiety and Pain in the Medical Setting
Introduction: Hospitalization and illness can be a painful and stressful time for a child. There may be anxiety over procedures and inpatient stays disrupt normal routines. Previous research found that for pre-school aged children, having parents around, having the help of the hospital staff, and playing an active role in alleviating their fears were the most helpful in reducing anxiety. Another study found that visual creative expressions can be meaningful experiences for young adult cancer survivors. Additionally, there is abundant literature on formal art therapy and its favorable effects on children in the hospital, however, there are fewer studies investigating less standardized “art intervention” in the same population. Athletic Administrators Report of Emergency Action Plan Adoption in Secondary School Athletics: The Influence of Athletic Training Services
This study describes the emergency action plan (EAP) adoption in secondary school athletics reported by athletic administrators in the United States. Utilizing best-practice criteria, a questionnaire assessed athletic administrators’ knowledge and adoption of an EAP in their athletics program. A majority of athletic administrators reported having an EAP; however, the EAP often lacked the necessary components to be compliant with best-practice guidelines. Further, access to an athletic trainer was associated with increased EAP adoption and adopting a venue specific EAP. Athletic administrators should utilize these data to facilitate improved policy adoption and management of athletics programs
Mastering tricyclic ring systems for desirable functional cannabinoid activity
There is growing interest in using cannabinoid receptor 2 (CB2) agonists for the treatment of neuropathic pain and other indications. In continuation of our ongoing program aiming for the development of new small molecule cannabinoid ligands, we have synthesized a novel series of carbazole and γ-carboline derivatives. The affinities of the newly synthesized compounds were determined by a competitive radioligand displacement assay for human CB2 cannabinoid receptor and rat CB1 cannabinoid receptor. Functional activity and selectivity at human CB1 and CB2 receptors were characterized using receptor internalization and [35S]GTP-γ-S assays. The structure–activity relationship and optimization studies of the carbazole series have led to the discovery of a non-selective CB1 and CB2 agonist, compound 4. Our subsequent research efforts to increase CB2 selectivity of this lead compound have led to the discovery of CB2 selective compound 64, which robustly internalized CB2 receptors. Compound 64 had potent inhibitory effects on pain hypersensitivity in a rat model of neuropathic pain. Other potent and CB2 receptor–selective compounds, including compounds 63 and 68, and a selective CB1 agonist, compound 74 were also discovered. In addition, we identified the CB2 ligand 35 which failed to promote CB2 receptor internalization and inhibited compound CP55,940-induced CB2 internalization despite a high CB2 receptor affinity. The present study provides novel tricyclic series as a starting point for further investigations of CB2 pharmacology and pain treatment.Fil: Petrov, Ravil R.. Paediatrician\u27s guide to post-operative care for functionally univentricular CHD: A review
IMPORTANCE: Single ventricle CHD affects about 5 out of 100,000 newborns, resulting in complex anatomy often requiring multiple, staged palliative surgeries. Paediatricians are an essential part of the team that cares for children with single ventricle CHD. These patients often encounter their paediatrician first when a complication arises, so it is critical to ensure the paediatrician is knowledgeable of these issues to provide optimal care.
OBSERVATIONS: We reviewed the subtypes of single ventricle heart disease and the various palliative surgeries these patients undergo. We then searched the literature to detail the general paediatrician\u27s approach to single ventricle patients at different stages of surgical palliation.
CONCLUSIONS AND RELEVANCE: Single ventricle patients undergo staged palliation that drastically changes physiology after each intervention. Coordinated care between their paediatrician and cardiologist is requisite to provide excellent care. This review highlights what to expect when these patients are seen by their paediatrician for either well child visits or additional visits for parental or patient concern
