Correlação entre a concentração de metais pesados (As, Cd, Hg, Pb e Sn) em leite materno e o consumo alimentar de lactantes à nível global: uma revisão sistemática e avaliação de risco à saúde de lactentes

Contudo, o consumo alimentar e a qualidade da água ingerida pelas mães têm demonstrado ser
fatores determinantes à exposição dos mesmos aos metais pesados via leite materno. A relação
metal-alimento-mãe-leite deve ser melhor compreendida, e o risco à saúde dos lactentes quanto
a ingestão destes metais pesados, deve ser avaliado. Portanto, o principal objetivo deste trabalho
foi, através de uma revisão sistemática, avaliar as correlações encontradas entre a presença de
As, Cd, Hg, Pb e/ou Sn em leite materno e o consumo alimentar de lactantes à nível global e,
com os dados dos estudos que encontraram correlação, realizar uma avaliação de risco à saúde
de lactentes. A estratégia de busca definida foi aplicada nas bases de dados Web of Science,
Scopus, Embase, Pubmed e Medline. Os resultados obtidos foram submetidos a uma triagem e
aos critérios de elegibilidade, sendo considerada a possibilidade de inclusão de estudos não
pertencentes à estratégia de busca, caso relevantes. Aos estudos que encontraram ao menos uma
correlação, foram calculadas as Ingestões Diárias Estimadas de metais pesados pelo lactente,
visando compará-las com os limites de referência definidos por autoridades internacionais de
proteção ambiental e segurança de alimentos. Foram identificados 427 resultados, sendo
elegidos 11 resultados após a triagem. Outros 5 estudos de relevância para o escopo da revisão,
foram incluídos, obtendo-se ao final, 16 artigos (1995-2020). Dentre estes estudos, 10
encontraram ao menos uma correlação entre a concentração de metais pesados e o consumo
alimentar das lactantes. Foram encontradas correlações com os metais Hg (41%), Pb (36%), Cd
(14%), e As (9%). Dentre os grupos de alimentos investigados, foi encontrada maior
prevalência de correlações para alimentos de origem terrestre (47%), seguida de pescado (42%)
e bebidas (11%). Dois estudos apresentaram uma ingestão de Hg por lactentes igual ou superior
ao limite de referência. O mesmo ocorreu para o Cd, em um estudo. A bioacumulação de metais
pesados e suas espécies em alimentos tem grande variação. Para a forma orgânica de Hg, ainda
é relatada a biomagnificação ao longo da cadeia alimentar. Parte dos metais pesados ingeridos
é biotransformado, geralmente à sua forma menos tóxica. As formas inorgânicas de As, Cd, Hg
e Pb parecem predominar no leite materno. Contudo, alguns estudos não foram conclusivos.
Também não foram encontradas informações referentes às espécies de Sn que podem ser
transferidas. Portanto, a especiação de metais em leite materno deve ser mais explorada, uma
vez que as diferentes espécies estão associadas à diferentes toxicidades, e estes dados ainda são
escassos. Os estudos que apresentaram Ingestão Diária Estimada igual ou superior aos limites
de referências, indicaram que há um potencial risco à saúde de lactentes quanto a ingestão de
Cd e Hg via leite materno. A presença destes metais foi correlacionada com o consumo de
pescado, batatas e grãos nas regiões estudadas (Brasil, Espanha e Croácia). O estudo das
técnicas de remediação e biorremediação devem ser encorajados, e sua aplicabilidade em áreas
de maior contaminação deve ser avaliada.Human milk has fundamental properties for the healthy development of newborns. However,
mother’s food consumption and water ingestion appear to be determining factors for their
exposure to heavy metals via human milk. The metal-food-mother-milk relationship should be
better understood, and the health risk of infants regarding the intake of these heavy metals
should be assessed. Therefore, the main objective of this work was, through a systematic
review, to evaluate the correlations found between the presence of As, Cd, Hg, Pb and / or Sn
in breast milk and the food consumption of lactating women at global level, and with the data
from the studies that found these correlations, carry out an infant health risk assessment. The
defined search strategy was applied to the Web of Science, Scopus, Embase, Pubmed and
Medline databases. The results were screened, submitted to elegibility criteria, and the
possibility of including other studies that did not belong to the search strategy was also
considered, if relevant. For studies that presented at least one correlation, the Estimated Daily
Intakes of heavy metals by infants were calculated, in order to compare them with the reference
limits by international environmental protection and food safety authorities. 427 results were
identified. After screening and elegibility criteria, 11 results remained. Another 5 relevant
studies for the scope of the review were included, resulting in 16 articles (1995-2020). Among
them, 10 presented at least one correlation between the concentration of heavy metals and food
consumption of lactating women. Correlations were found with the metals Hg (41%), Pb (36%),
Cd (14%), and As (9%). Among the 3 food groups investigated, a higher prevalence of
correlations was found for food of terrestrial origin (47%), followed by fishery products (42%)
and drinks (11%). Two studies presented an equal or higher Hg intake by infants when
compared to the reference limit. The same occurred for the CD, in one study. The
bioaccumulation of heavy metals and its species varies widely in food. The biomagnification
of the organic form of Hg along the food chain is also reported. Part of the heavy metals ingested
is biotransformed, generally to its less toxic form. The inorganic forms of As, Cd, Hg e Pb seem
to predominate in breast milk. However, some studies have not been conclusive. There was also
no information regarding Sn species that can be transferred. Therefore, the speciation of heavy
metals in human milk should be further explored, since different species of metals are
associated with different toxicities, and these data are still scarce. Studies that presented an
equal or higher Estimated Daily Intake when compared to defined limits, indicated that there is
a potential risk to the infant’s health regarding the intake of Cd and Hg via human milk. The
presence of these metals was correlated with the consumption of fishery products, potatoes and
grains in the studied regions (Brazil, Spain and Croatia). The study of remediation and
bioremediation techniques needs to be encouraged, and their applicability in areas of greater
contamination needs to be evaluated
CARACTERIZAÇÃO FÍSICO-QUÍMICA E FÍSICA DE PASTAS COMERCIAIS CONTENDO MATÉRIAS-PRIMAS OLEAGINOSAS
O objetivo deste trabalho foi caracterizar amostras de pastas comerciais à base de nozpecã e/ou castanha-do-brasil, quanto à sua composição proximal, atividade de água(Aa) e sinerese. As pastas apresentaram os lipídios como macronutriente majoritário,além de altos teores de proteínas e fibras. A Aa encontrou-se dentro dos padrões paraestes produtos. A sinerese foi influenciada pela formulação das pastas, tendo sidoreduzida na presença de lecitina de soja ou polidextrose. A caracterização das pastasfoi de grande relevância, pois permitiu a análise de dados diretamente relacionados àqualidade do produto e aos parâmetros de escolha do consumidor
Tratamento cirúrgico da otite média secretora na população infantil: uma revisão sistemática
Este artigo tem como objetivo realizar uma revisão sistemática da produção bibliográfica referente a eficácia do tratamento cirúrgico e suas indicações como terapêutica do quadro clínico de otite média com efusão na população pediátrica. Após formular a frase de pesquisa, foram realizadas buscas na base de dados Medline, sendo utilizados os seguintes descritores: Otitis Media with Effusion, Middle Ear Effusion, Secretory Otitis Media e Serous Otitis Media. Foram incluídos ensaios clínicos e ensaios clínicos randomizados que apresentavam dados que versavam diretamente sobre as intervenções cirúrgicas da otite média com efusão e foram analisados os resultados obtidos nas avaliações. Os resultados indicaram que a inserção de tubos de ventilação combinada com a adenoidectomia resultou em menor recorrência do quadro em comparação com a inserção do tubo de ventilação de forma isolada. Além disso, observou-se que a inserção do tubo de ventilação em crianças com otite média com efusão persistente não apresentou efeito benéfico na qualidade de vida geral, mas evidências indicaram um desfecho positivo na saúde funcional relacionada à dificuldade auditiva nesta população. Por fim, observou-se que nas crianças com fissura palatina que passaram por reparo no palato mole e que permaneceram com o quadro de otite média com efusão, a inserção do tubo de ventilação deve ser considerada. A análise dos estudos destacou a complexidade das decisões clínicas em relação às intervenções cirúrgicas para o quadro descrito, considerando fatores como recorrência, qualidade de vida e características individuais dos pacientes
Evaluation of Cardiometabolic Parameters among Obese Women Using Oral Contraceptives
BackgroundCombined oral contraceptive (COC) use has been associated with an unfavorable impact on carbohydrate and lipid metabolism in diverse populations of normal weight and obese women. The present study aimed to evaluate the cardiometabolic and inflammatory profiles of women in northeastern Brazil with respect to COC use and obesity.MethodsWe performed a cross-sectional study to verify cardiovascular parameters, including blood pressure (BP), fasting serum glucose, lipid, and inflammatory profile, in a population of women aged 15–45 years, considering obesity and COC use. Our sample consisted of 591 women, 481 women who were COC users, and 110 age-matched women who were COC non-users, classified as obese and non-obese according to BMI.ResultsCOC use and obesity were associated with increased systolic (p ≤ 0.001) and diastolic BP (p = 0.001), blood glucose (p ≤ 0.001), total cholesterol (p = 0.008), low-density lipoprotein cholesterol (p ≤ 0.001), very low-density lipoprotein cholesterol (p ≤ 0.001), triglycerides (p ≤ 0.001), ferritin (p = 0.006), C-reactive protein (CRP) (p ≤ 0.001), and nitric oxide metabolites (p ≤ 0.001), as well as decreased high-density lipoprotein cholesterol (HDL-c) (p ≤ 0.001) in comparison to controls. CRP and HDL-c levels in obese COC users were determined to be outside reference range values. The odds of having lower levels of HDL-c and elevated CRP increased among obese COC users. COC use was independently associated with low levels of HDL-c, especially second-generation progestins (p < 0.001; OR = 8.976; 95% CI 2.786–28.914).ConclusionObesity and COC use were associated with alterations in lipid and inflammatory cardiometabolic parameters, particularly increased CRP levels and decreased HDL-c, which are considered markers of cardiovascular disease (CVD) risk. Given the need to prevent unintended pregnancy among obese women, together with weight loss counseling, it is important to evaluate the most effective and safest contraceptive methods to avoid the potential risk of developing CVD

Assim foram utilizados como base para a presente pesquisa uma revisão bibliográfica de artigos, dissertação, teses e livros sobre os princípios que alicerçam as práticas conhecido como Agricultura Sintrópica
Endotoxin-Induced Emphysema Exacerbation: A Novel Model of Chronic Obstructive Pulmonary Disease Exacerbations Causing Cardiopulmonary Impairment and Diaphragm Dysfunction
Chronic obstructive pulmonary disease (COPD) is a progressive disorder of the lung parenchyma which also involves extrapulmonary manifestations, such as cardiovascular impairment, diaphragm dysfunction, and frequent exacerbations. The development of animal models is important to elucidate the pathophysiology of COPD exacerbations and enable analysis of possible therapeutic approaches. We aimed to characterize a model of acute emphysema exacerbation and evaluate its consequences on the lung, heart, and diaphragm. Twenty-four Wistar rats were randomly assigned into one of two groups: control (C) or emphysema (ELA). In ELA group, animals received four intratracheal instillations of pancreatic porcine elastase (PPE) at 1-week intervals. The C group received saline under the same protocol. Five weeks after the last instillation, C and ELA animals received saline (SAL) or E. coli lipopolysaccharide (LPS) (200 μg in 200 μl) intratracheally. Twenty-four hours after saline or endotoxin administration, arterial blood gases, lung inflammation and morphometry, collagen fiber content, and lung mechanics were analyzed. Echocardiography, diaphragm ultrasonography (US), and computed tomography (CT) of the chest were done. ELA-LPS animals, compared to ELA-SAL, exhibited decreased arterial oxygenation; increases in alveolar collapse (p < 0.0001), relative neutrophil counts (p = 0.007), levels of cytokine-induced neutrophil chemoattractant-1, interleukin (IL)-1β, tumor necrosis factor-α, IL-6, and vascular endothelial growth factor in lung tissue, collagen fiber deposition in alveolar septa, airways, and pulmonary vessel walls, and dynamic lung elastance (p < 0.0001); reduced pulmonary acceleration time/ejection time ratio, (an indirect index of pulmonary arterial hypertension); decreased diaphragm thickening fraction and excursion; and areas of emphysema associated with heterogeneous alveolar opacities on chest CT. In conclusion, we developed a model of endotoxin-induced emphysema exacerbation that affected not only the lungs but also the heart and diaphragm, thus resembling several features of human disease. This model of emphysema should allow preclinical testing of novel therapies with potential for translation into clinical practice
Experiências docentes: desafios para lidar com a geração atual de universitários
This paper presents a qualitative analysis basic interpretative, with the aim to analyze the teacher´s experience face the change of student´s generation of higher education. Six open face semi-structured interviews were conducted with professionals, there are at least 10 years in higher education and currently employed. The results indicated a universalization of higher education, a constantly growth of students and a behavioral change on the current generation of students. One of the main factors for this phenomenon lies in the technology, which represents a benefit to the student and a challenge to the teacher, who needs constantly to update and improve their classes, with the inclusion of updated content.Este trabalho parte de uma análise qualitativa interpretativa básica, com o objetivo de analisar a experiência dos docentes face à mudança de gerações dos alunos do ensino superior. Foram realizadas seis entrevistas abertas com roteiro semiestruturado com profissionais há, pelo menos, 10 anos no ensino superior e atualmente empregados. Os resultados indicaram a universalização do ensino, um aumento contínuo na quantidade de alunos universitários e uma mudança comportamental na geração atual de alunos. Um dos fatores para tal fenômeno reside na tecnologia, que representa um benefício para o discente e um desafio para o docente, que necessita atualizar-se constantemente e aperfeiçoar as suas aulas, com a inclusão de conteúdos atualizados
Prognostic value of programmed cell death ligand 1 (PD-L1) expression in patients with stage III non-small cell lung cancer under different treatment types: a retrospective study
ABSTRACT Objective Currently programmed cell death protein 1 (PD-1) inhibitors in combination with other therapies are being evaluated to determine their efficacy in cancer treatment. However, the effect of PD-ligand (L) 1 expression on disease outcomes in stage III (EC III) non-small cell lung cancer is not completely understood. Therefore, this study aimed to assess the influence of PD-L1 expression on the outcomes of EC III non-small cell lung cancer. Methods This study was conducted on patients diagnosed with EC III non-small cell lung cancer who underwent treatment at a tertiary care hospital. PD-L1 expression was determined using immunohistochemical staining, all patients expressed PD-L1. Survival was estimated using the Kaplan-Meier method. Relationships between variables were assessed using Cox proportional regression models. Results A total of 49 patients (median age=69 years) with EC III non-small cell lung cancer and PD-L1 expression were evaluated. More than half of the patients were men, and most were regular smokers. The patients were treated with neoadjuvant chemotherapy, surgery, or sequential or combined chemotherapy and radiotherapy. The median progression-free survival of the entire cohort was 14.2 months, and the median overall survival was 20 months. There was no significant association between PD-L1 expression and disease progression, clinical characteristics, or overall survival. Conclusions PD-L1 expression was not correlated with EC III non-small cell lung cancer outcomes. Whether these findings differ from the association with immune checkpoint inhibitors remains to be addressed in future studies
