    LOOKING FOR ATTACHMENT SOLUTIONS IN ALL THE WRONG PLACES: OUT OF CONTROL SEXUAL BEHAVIOR AS A SYMPTOM OF INSECURE ATTACHMENT IN MEN

    Hypersexual behavior is often misunderstood and minimized, and we continue to lack an understanding of what underlies this behavior. Without an understanding of the function of hypersexual behavior, we cannot ascertain the most effective treatment. This study was designed to examine the underlying function of such behavior by exploring whether insecure attachment in men relates to the development of hypersexual behavior. A total of 45 men who were assessed as having Out-of-Control Sexual Behavior (OCSB), utilizing the recently proposed Hypersexual Disorder (HD) diagnosis were compared to 32 men who did not present with OCSB. Participants were directed to an online survey where they completed assessments for hypersexual behavior (The HBI) and attachment style (The ECR-S). Multivariate analysis indicated that high ECR-S scores predicted high HBI scores, high HBI scores tended to show high levels of attachment avoidant behavior and that high ECR-S scores were predictive of the clinical determination of HD. High scores on attachment avoidance, rather than attachment anxiety, were most predictive of the clinical determination of HD. Overall, the avoidant behavior score was a better predictor of OCSB than were attachment anxiety scores. Hypersexual behavior may be a particular manifestation of avoidant attachment and it is this underlying issue that must be addressed to effectively treat HD

    A process evaluation of how the routine vaccination programme is implemented at GP practices in England.

    BACKGROUND: In recent years, the incidence of several pathogens of public health importance (measles, mumps, pertussis and rubella) has increased in Europe, leading to outbreaks. This has included England, where GP practices implement the vaccination programme based on government guidance. However, there has been no study of how implementation takes place, which makes it difficult to identify organisational variation and thus limits the ability to recommend interventions to improve coverage. The aim of this study is to undertake a comparative process evaluation of the implementation of the routine vaccination programme at GP practices in England. METHODS: We recruited a sample of geographically and demographically diverse GP practices through a national research network and collected quantitative and qualitative data as part of a Time-Driven Activity-Based Costing analysis between May 2017 and February 2018. We conducted semi-structured interviews with practice staff involved in vaccination, who then completed an activity log for 2 weeks. Interviews were transcribed and coded using a framework method. RESULTS: Nine practices completed data collection from diverse geographic and socio-economic contexts, and 52 clinical and non-clinical staff participated in 26 interviews. Information relating to 372 vaccination appointments (233 childhood and 139 adult appointments) was captured using activity logs. We have defined a 14-stage care delivery value chain and detailed process map for vaccination. Areas of greatest variation include the method of reminder and recall activities, structure of vaccination appointments and task allocation between staff groups. For childhood vaccination, mean appointment length was 15.9 min (range 9.0-22.0 min) and 10.9 min for adults (range 6.8-14.1 min). Non-clinical administrative activities comprised 59.7% total activity (range 48.4-67.0%). Appointment length and total time were not related to coverage, whereas capacity in terms of appointments per eligible patient may improve coverage. Administrative tasks had lower fidelity of implementation. CONCLUSIONS: There is variation in how GP practices in England implement the delivery of the routine vaccination programme. Further work is required to evaluate capacity factors in a wider range of practices, alongside other contextual factors, including the working culture within practices

    Hydrodynamic coupling of two rotating spheres trapped in harmonic potentials

    We theoretically study in detail the hydrodynamic coupling of two equal-sized colloidal spheres at low Reynolds numbers assuming the particles to be harmonically trapped with respect to both their positions and orientations. By taking into account the rotational motion, we obtain a rich spectrum of collective eigen modes whose properties we determine on the basis of pure symmetry arguments. Extending recent investigations on translational correlations [J.-C. Meiners and S. R. Quake, Phys. Rev. Lett. 82, 2211 (1999)], we derive the complete set of auto- and cross-correlation functions emphasizing the coupling of rotation to translation which we illustrate in a few examples. An important feature of our system is the self-coupling of translation and rotation of one particle mediated by the neighboring particle that is clearly visible in the appropriate auto-correlation function. This coupling is a higher-order effect and therefore not included in the widely used Rotne-Prager approximation for the hydrodynamic mobilities.Comment: to be published in Phys. Rev.

    Insulin and GLP-1 infusions demonstrate the onset of adipose-specific insulin resistance in a large fasting mammal: potential glucogenic role for GLP-1.

    Prolonged food deprivation increases lipid oxidation and utilization, which may contribute to the onset of the insulin resistance associated with fasting. Because insulin resistance promotes the preservation of glucose and oxidation of fat, it has been suggested to be an adaptive response to food deprivation. However, fasting mammals exhibit hypoinsulinemia, suggesting that the insulin resistance-like conditions they experience may actually result from reduced pancreatic sensitivity to glucose/capacity to secrete insulin. To determine whether fasting results in insulin resistance or in pancreatic dysfunction, we infused early- and late-fasted seals (naturally adapted to prolonged fasting) with insulin (0.065 U/kg), and a separate group of late-fasted seals with low (10 pM/kg) or high (100 pM/kg) dosages of glucagon-like peptide-1 (GLP-1) immediately following a glucose bolus (0.5g/kg), and measured the systemic and cellular responses. Because GLP-1 facilitates glucose-stimulated insulin secretion, these infusions provide a method to assess pancreatic insulin-secreting capacity. Insulin infusions increased the phosphorylation of insulin receptor and Akt in adipose and muscle of early and late fasted seals; however the timing of the signaling response was blunted in adipose of late fasted seals. Despite the dose-dependent increases in insulin and increased glucose clearance (high dose), both GLP-1 dosages produced increases in plasma cortisol and glucagon, which may have contributed to the glucogenic role of GLP-1. Results suggest that fasting induces adipose-specific insulin resistance in elephant seal pups, while maintaining skeletal muscle insulin sensitivity, and therefore suggests that the onset of insulin resistance in fasting mammals is an evolved response to cope with prolonged food deprivation

    Atmospheric carbon dioxide levels have increased since the industrial revolution due to the increase in combustion of fossil fuels. One possible alternative strategy is the use of microalgae for CO2 capture and recycling. Major components in coal-derived flue gas that may accumulate and effect algae growth include both sulfur oxides and fly ash. However, in practical application, sulfur oxides will be converted quickly to the acid product (H2SO4) in the aerobic aqueous conditions of algae cultivation. In this article, the influence of elevated H2SO4 levels and the presence of coal-derived fly ash were investigated. As the H2SO4 level increased, algae growth was inhibited and finally ceased; however, this affect could be minimized by the addition of NaHCO3 as a buffer. The effect of ash on the growth of algae was related to the type of coal-fired combustion and the amount of ash. For one of the ash types tested, the algae growth rate actually increased at a low ash concentration. In this work, major technical hurdles in the use of algal scrubbing systems are being addressed with respect to major contaminants in coal-derived flue gases

    The protein tyrosine phosphatase Pez regulates TGFβ, epithelial–mesenchymal transition, and organ development

    Epithelial–mesenchymal transition (EMT), crucial during embryogenesis for new tissue and organ formation, is also considered to be a prerequisite to cancer metastasis. We report here that the protein tyrosine phosphatase Pez is expressed transiently in discrete locations in developing brain, heart, pharyngeal arches, and somites in zebrafish embryos. We also find that Pez knock-down results in defects in these organs, indicating a crucial role in organogenesis. Overexpression of Pez in epithelial MDCK cells causes EMT, with a drastic change in cell morphology and function that is accompanied by changes in gene expression typical of EMT. Transfection of Pez induced TGFβ signaling, critical in developmental EMT with a likely role also in oncogenic EMT. In zebrafish, TGFβ3 is co- expressed with Pez in a number of tissues and its expression was lost from these tissues when Pez expression was knocked down. Together, our data suggest Pez plays a crucial role in organogenesis by inducing TGFβ and EMT

    Beneficial Reuse of Industrial CO\u3csub\u3e2\u3c/sub\u3e Emissions Using a Microalgae Photobioreactor: Waste Heat Utilization Assessment

    Microalgae are a potential means of recycling CO2 from industrial point sources. With this in mind, a novel photobioreactor (PBR) was designed and deployed at a coal-fired power plant. To ascertain the feasibility of using waste heat from the power plant to heat algae cultures during cold periods, two heat transfer models were constructed to quantify PBR cooling times. The first, which was based on tabulated data, material properties and the physical orientation of the PBR tubes, yielded a range of heat transfer coefficients of 19–64 W m−2 K−1 for the PBR at wind speeds of 1–10 m s−1. The second model was based on data collected from the PBR and gave an overall heat transfer coefficient of 24.8 W m−2 K−1. Energy penalties associated with waste heat utilization were found to incur an 18%–103% increase in energy consumption, resulting in a 22%–70% reduction in CO2 capture for the scenarios considered. A techno-economic analysis showed that the cost of heat integration equipment increased capital expenditures (CAPEX) by a factor of nine and increased biomass production costs by a factor of three. Although the scenario is thermodynamically feasible, the increase in CAPEX incurs an increase in biomass production cost that is economically untenable

    Supportive housing and forensic patient outcomes

    In Canada, Review Boards are mandated to evaluate individuals found Not Criminally Responsible on Account of Mental Disorder (NCRMD) on an annual basis and render 1 of 3 dispositions: (a) custody, (b) conditional discharge, or (c) absolute discharge. To promote social reintegration, conditional discharge can be ordered with the condition to live in supportive housing. However, NCRMD accused face great barriers to housing access as a result of the stigma associated with the forensic label. The goal of this study was to evaluate the role of housing in the clinical and criminal trajectories of forensic patients as they reintegrate into the community. Data for this study were extracted from a national study of individuals found NCRMD in Canada (Crocker, Nicholls, Seto, Côté, et al., in press). The present study focuses on a random sample of NCRMD accused in the province of Québec, who were under a conditional discharge disposition during the study period (n = 837). Controlling for sociodemographic, clinical, and criminal variables, survival analysis showed that individuals placed in independent housing following a conditional discharge from the Review Board were 2.5 times more likely to commit a new offense, nearly 3 times more likely to commit an offense against a person, and 1.4 times more likely to be readmitted for psychiatric treatment compared with individuals residing in supportive housing. These results point to the influence housing can have on the trajectories of forensic patients, above and beyond a range of clinical, criminological, and sociodemographic factors
