Recent advances in using propofol by non-anesthesiologists
Evidence is accumulating that non-anesthesiologist-administered propofol (NAAP) sedation has a safety and efficacy profile comparable or superior to that provided by benzodiazepines with or without opioids. The guidelines currently available emphasize the importance of appropriate patient selection, staff training, monitoring, and low-dose sedation protocols for NAAP safety. In addition, capnograph monitoring and computer-assisted sedation systems may further improve patient safety during NAAP
Seasonal Variability of Saturn's Tropospheric Temperatures, Winds and Para-H from Cassini Far-IR Spectroscopy
Far-IR 16-1000 m spectra of Saturn's hydrogen-helium continuum measured
by Cassini's Composite Infrared Spectrometer (CIRS) are inverted to construct a
near-continuous record of upper tropospheric (70-700 mbar) temperatures and
para-H fraction as a function of latitude, pressure and time for a third of
a Saturnian year (2004-2014, from northern winter to northern spring). The
thermal field reveals evidence of reversing summertime asymmetries superimposed
onto the belt/zone structure. The temperature structure that is almost
symmetric about the equator by 2014, with seasonal lag times that increase with
depth and are qualitatively consistent with radiative climate models. Localised
heating of the tropospheric hazes (100-250 mbar) create a distinct perturbation
to the temperature profile that shifts in magnitude and location, declining in
the autumn hemisphere and growing in the spring. Changes in the para-H
() distribution are subtle, with a 0.02-0.03 rise over the spring
hemisphere (200-500 mbar) perturbed by (i) low- air advected by both the
springtime storm of 2010 and equatorial upwelling; and (ii) subsidence of
high- air at northern high latitudes, responsible for a developing
north-south asymmetry in . Conversely, the shifting asymmetry in the
para-H disequilibrium primarily reflects the changing temperature structure
(and the equilibrium distribution of ), rather than actual changes in
induced by chemical conversion or transport. CIRS results interpolated to
the same point in the seasonal cycle as re-analysed Voyager-1 observations show
qualitative consistency, with the exception of the tropical tropopause near the
equatorial zones and belts, where downward propagation of a cool temperature
anomaly associated with Saturn's stratospheric oscillation could potentially

On Kinematic Substructure in the Sextans Dwarf Spheroidal Galaxy
We present multifiber echelle radial velocity results for 551 stars in the
Sextans dwarf spheroidal galaxy and identify 294 stars as probable Sextans
members. The projected velocity dispersion profile of the binned data remains
flat to a maximum angular radius of . We introduce a nonparametric
technique for estimating the projected velocity dispersion surface, and use
this to search for kinematic substructure. Our data do not confirm previous
reports of a kinematically distinct stellar population at the Sextans center.
Instead we detect a region near the Sextans core radius that is kinematically

The Age and Structure of the Galactic Bulge from Mira Variables
We report periods and JHKL observations for 648 oxygen-rich Mira variables
found in two outer bulge fields at b=-7 degrees and l=+/-8 degrees and combine
these with data on 8057 inner bulge Miras from the OGLE, Macho and 2MASS
surveys, which are concentrated closer to the Galactic centre. Distance moduli
are estimated for all these stars. Evidence is given showing that the bulge
structure is a function of age. The longer period Miras (log P > 2.6, age about
5 Gyr and younger) show clear evidence of a bar structure inclined to the line
of sight in both the inner and outer regions. The distribution of the shorter
period (metal-rich globular cluster age) Miras, appears spheroidal in the outer
bulge. In the inner region these old stars are also distributed differently
from the younger ones and possibly suggest a more complex structure. These data
suggest a distance to the Galactic centre, R0, of 8.9 kpc with an estimated
uncertainty of 0.4 kpc. The possible effect of helium enrichment on our

Neptune at Summer Solstice: Zonal Mean Temperatures from Ground-Based Observations 2003-2007
Imaging and spectroscopy of Neptune's thermal infrared emission is used to
assess seasonal changes in Neptune's zonal mean temperatures between Voyager-2
observations (1989, heliocentric longitude Ls=236) and southern summer solstice
(2005, Ls=270). Our aim was to analyse imaging and spectroscopy from multiple
different sources using a single self-consistent radiative-transfer model to
assess the magnitude of seasonal variability. Globally-averaged stratospheric
temperatures measured from methane emission tend towards a quasi-isothermal
structure (158-164 K) above the 0.1-mbar level, and are found to be consistent
with spacecraft observations of AKARI. This remarkable consistency, despite
very different observing conditions, suggests that stratospheric temporal
variability, if present, is 5 K at 1 mbar and 3 K at 0.1 mbar during
this solstice period. Conversely, ethane emission is highly variable, with
abundance determinations varying by more than a factor of two. The retrieved
C2H6 abundances are extremely sensitive to the details of the T(p) derivation.
Stratospheric temperatures and ethane are found to be latitudinally uniform
away from the south pole (assuming a latitudinally-uniform distribution of
stratospheric methane). At low and midlatitudes, comparisons of synthetic
Voyager-era images with solstice-era observations suggest that tropospheric
zonal temperatures are unchanged since the Voyager 2 encounter, with cool
mid-latitudes and a warm equator and pole. A re-analysis of Voyager/IRIS 25-50
{\mu}m mapping of tropospheric temperatures and para-hydrogen disequilibrium
suggests a symmetric meridional circulation with cold air rising at
mid-latitudes (sub-equilibrium para-H2 conditions) and warm air sinking at the
equator and poles (super-equilibrium para-H2 conditions). The most significant

Increased risk of depression in non-depressed HIV infected men with sleep disturbance: Prospective findings from the Multicenter AIDS Cohort Study.
ObjectiveSleep disturbance is a known risk factor for depression, but it is not known whether sleep disturbance contributes to greater risk of depression in those infected with human immunodeficiency virus (HIV+) as compared to those uninfected with HIV (HIV-).MethodsUsing data from the Multicenter AIDS Cohort Study, a population-based prospective study of men who have sex with men (MSM), self-reported sleep disturbance (>2 weeks) and depressive symptoms (Clinical Epidemiologic Scale for Depression, CES-D) were assessed every 6 months over 12 years of follow-up. Adjusted mixed effects logistic regression analyses tested whether sleep disturbance predicted depression (CES-D ≥ 16) at the immediate subsequent visit, and so on over 12 years, in non-depressed HIV+(N = 1054; 9556 person-visits) and non-depressed HIV- (N = 1217; 12,680 person-visits). In HIV+ vs. HIV- MSM, linearly estimated average incidence of depression and normalized cumulative rate of depression over 12 years were compared.ResultsIn the HIV+ MSM, sleep disturbance was associated with a significant increase in depression 6 months later (OR = 1.6; 95% CI, 1.30, 1.96), which was significantly greater (P < .05) than in HIV- MSM (OR = 1.16; 95% CI, 0.94, 1.44). HIV status and sleep disturbance interacted (P < .001), such that incidence of depression and normalized cumulative rate of depression were greater in HIV+ with sleep disturbance than in HIV+ without sleep disturbance and HIV- groups (all P's < 0.001).ConclusionsHIV+ persons who report sleep disturbance represent a high risk group to be monitored for depression, and possibly targeted for insomnia treatment to prevent depression. The Sustainability Liability: Potential Negative Effects of Ethicality on Product Preference
Manufacturers are increasingly producing and promoting sustainable products (i.e., products that have a positive social and/or environmental impact). However, relatively little is known about how product sustainability affects consumers\u27 preferences. Theauthors propose that sustainability may not always be an asset, even if most consumers care about social and environmental issues. The degree to which sustainability enhances preference depends on the type of benefit consumers most value for theproduct category in question. In this research, the authors demonstrate that consumers associate higher product ethicality with gentleness-related attributes and lower product ethicality with strength-related attributes. As a consequence of these associations, the positive effect of product sustainability on consumer preferences is reduced when strength-related attributes are valued, sometimes even resulting in preferences for less sustainable product alternatives (i.e., the sustainability liability ). Conversely, when gentleness-related attributes are valued, sustainability enhances preference. In addition, the authors show that the potential negative impact of sustainability on product preferences can be attenuated using explicit cues about productstrength

Sleep disturbance at pre-deployment is a significant predictor of post-deployment re-experiencing symptoms.
Background: Insomnia is common in service members and associated with many mental and physical health problems. Recently, longitudinal data have been used to assess the impact of disturbed sleep on mental health outcomes. These studies have consistently shown relationships between sleep disturbance and development of mental illness. Objective: The present study examined the longitudinal relationship between sleep disturbance and PTSD symptomatology in a cohort of Marines and Navy Corpsmen deployed to Iraq and Afghanistan (n = 2,404) assessed prior to deployment, as well as at -3 and 6 months post-deployment. Additionally, we aimed to investigate the extent to which these relationships are moderated by combat-stress severity, and to what extent these findings are replicated in a second, separate cohort of Marines and Navy corpsmen (n = 938) assessed with identical measures prior to deployment and within 3 months of return. Method: The present study employed latent variable path models to examine the relationships between pre-deployment sleep disturbance and post-deployment re-experiencing symptoms. Initial cross-lagged path models were conducted on discovery and replication samples to validate the hypothesized predictive relationships. Follow up moderation path models were then conducted to include the effect of combat-stress severity on these relationships. Results: Initial cross-lagged models supported a significant relationship between pre-deployment sleep disturbance and future re-experiencing PTSD symptoms at all time points. Initial moderation models showed a small moderator effect of combat-stress severity, though the main predictive relationship between pre-deployment sleep disturbance and PTSD symptoms remained significant. The moderator effect was not significant in the replication sample. Conclusions: The results of this study support pre-deployment sleep disturbance as a risk factor for development of post-deployment PTSD symptoms. Interventions aimed at normalizing sleep may be important in preventive measures for PTSD
Hypervelocity Stars. I. The Spectroscopic Survey
We discuss our targeted search for hypervelocity stars (HVSs), stars
traveling with velocities so extreme that dynamical ejection from a massive
black hole is their only suggested origin. Our survey, now half complete, has
successfully identified a total of four probable HVSs plus a number of other
unusual objects. Here we report the most recently discovered two HVSs: SDSS
J110557.45+093439.5 and possibly SDSS J113312.12+010824, traveling with
Galactic rest-frame velocities at least +508+-12 and +418+-10 km/s,
respectively. The other late B-type objects in our survey are consistent with a
population of post main-sequence stars or blue stragglers in the Galactic halo,
with mean metallicity [Fe/H]=-1.3 and velocity dispersion 108+-5 km/s.
Interestingly, the velocity distribution shows a tail of objects with large
positive velocities that may be a mix of low-velocity HVSs and high-velocity
runaway stars. Our survey also includes a number of DA white dwarfs with
unusually red colors, possibly extremely low mass objects. Two of our objects
are B supergiants in the Leo A dwarf, providing the first spectroscopic

Effective surface motion on a reactive cylinder of particles that perform intermittent bulk diffusion
In many biological and small scale technological applications particles may
transiently bind to a cylindrical surface. In between two binding events the
particles diffuse in the bulk, thus producing an effective translation on the
cylinder surface. We here derive the effective motion on the surface, allowing
for additional diffusion on the cylinder surface itself. We find explicit
solutions for the number of adsorbed particles at one given instant, the
effective surface displacement, as well as the surface propagator. In
particular sub- and superdiffusive regimes are found, as well as an effective
stalling of diffusion visible as a plateau in the mean squared displacement. We
