How Many Assertive Community Treatment Teams Do We Need?
Assertive community treatment (ACT) reduces hospitalizations for persons with severe mental illness. However, not everyone who needs ACT receives it. Without empirical guidelines for ACT planning, communities are likely to underestimate or overestimate the number of teams they need; thus the capacity of the programs will not meet current needs. In this study, administrative data were used to develop empirical estimates for the number of required ACT teams. These estimates were then used to examine current conceptual guidelines for developing the number of ACT teams that communities need. Administrative data from a large, urban county were used to enumerate all persons with a severe mental illness who had three or more hospitalizations within one year (ACT eligible). Fifty-one percent of persons with a severe mental illness were found to be eligible for ACT (743 of 1,453 persons). This figure represents 2.2 percent of the county's mental health users and .06 percent of its adult population. Communities should develop enough ACT teams to serve approximately 50 percent of their populations of persons with severe mental illness or roughly .06 percent of their adult populations
The Challenge for Innovation in Direct Restorative Materials
During the past 50 years, a series of key UN conferences have established a framework to minimize human health risks from environmental exposures to key chemicals. In January 2013, more than 140 countries agreed to the text of new treaty to minimize Hg effects on the environment (the Minamata Convention). Dental caries is omnipresent around the globe, affecting 60% to 90% of school children and most adults, and producing discomfort that affects quality of life. Dental amalgam is frequently used to treat carious lesions and its use releases mercury into the environment. The best way to avoid the use of dental amalgam is to emphasize caries prevention. Alternatives to amalgam are suitable in some applications, but no replacement for amalgam has been found for large posterior restorations. For any restorative material, safety and environmental impacts are part of clinical risk assessment. Safety is freedom from unacceptable risks. Risk is a combination of probability of exposure and severity of harm. Best management practices are crucial to manage dental amalgam, but these impose additional that are disproportionately more for developing countries. The Minamata Convention seeks a phase-out of all mercury-based products except dental amalgam, where a phase-down is the present goal. For dentistry, the most important focus is the promotion of caries prevention and research on new materials
Systematic Literature Review of General Health Care Interventions Within Programs of Assertive Community Treatment
OBJECTIVE: Assertive community treatment (ACT) is one of the few evidence-based practices for adults with severe mental illness. Interest has slowly waned for ACT implementation. Yet ACT remains an appealing services platform to achieve the triple aim of health care reform (improved health outcomes, reduced cost, and improved satisfaction) through integration of primary care and behavioral health services. This review highlights the evidence for ACT to improve general medical outcomes, reduce treatment costs, and increase access to treatment.
METHODS: Using a comprehensive list of relevant search terms, the authors performed a systematic literature database search for articles published through November 2015, resulting in ten articles for inclusion.
RESULTS: No studies reported on clinical outcomes of general medical comorbidities or on mortality of ACT clients. Half of the studies reporting utilization (three of six) found a decrease in emergency room usage, and three of four studies identified an increase in outpatient primary care visits. Most studies found no increase in overall medical care costs. Of the few studies reporting on quality of life, most found mild to moderate improvements.
CONCLUSIONS: To date, rigorous scientific examination of the effect of ACT on the general health of the populations it serves has not been undertaken. Given ACT's similarity to emerging chronic illness medical management models, the approach seems like a natural fit for improving general medical outcomes of persons with severe mental illnesses. More research is needed that investigates the current effect of ACT teams on general medical outcomes, treatment costs, and access to care
Upward spirals of positive emotions counter downward spirals of negativity: Insights from the broaden-and-build theory and affective neuroscience on the treatment of emotion dysfunctions and deficits in psychopathology
This review integrates Fredrickson’s broaden-and-build theory of positive emotions with advances in affective neuroscience regarding plasticity in the neural circuitry of emotions to inform the treatment of emotion deficits within psychopathology. We first present a body of research showing that positive emotions broaden cognition and behavioral repertoires, and in so doing, build durable biopsychosocial resources that support coping and flourishing mental health. Next, by explicating the processes through which momentary experiences of emotions may accrue into self-perpetuating emotional systems, the current review proposes an underlying architecture of state-trait interactions that engenders lasting affective dispositions. This theoretical framework is then used to elucidate the cognitive-emotional mechanisms underpinning three disorders of affect regulation, depression, anxiety, and schizophrenia. In turn, two mind training interventions, mindfulness and loving-kindness meditation, are highlighted as means of generating positive emotions that may counter the negative affective processes implicated in these disorders. We conclude with the proposition that positive emotions may exert a countervailing force on the dysphoric, fearful, or anhedonic states characteristic of persons with psychopathology typified by emotional dysfunctions
The effects of antipsychotic medications on emotion perception in patients with chronic schizophrenia in the CATIE trial
AbstractFew pharmacological intervention studies have examined the impact of medication on social cognition, particularly emotion perception. The goal of this randomized, double-blind study is to compare the effects of several second generation antipsychotics and a first generation antipsychotic, perphenazine, on emotion perception in individuals with schizophrenia. Patients were assigned to receive treatment with olanzapine, queitapine fumarate, risperidone, ziprasidone or perphenazine for up to 18months. Eight hundred and seventy three patients completed an emotion perception test immediately prior to randomization and after 2months of treatment. We also examined baseline predictors of emotion perception change. Most treatments were associated with a small, non-statistically significant improvement in emotion perception at two months, although they did not differ from one another. Greater improvement in emotion perception at 2months was significantly predicted by lower baseline emotion perception and higher baseline neurocognitive functioning, and marginally predicted by less time on an antipsychotic
Precise measurement of the top quark mass in the dilepton channel at D0
We measure the top quark mass (mt) in ppbar collisions at a center of mass
energy of 1.96 TeV using dilepton ttbar->W+bW-bbar->l+nubl-nubarbbar events,
where l denotes an electron, a muon, or a tau that decays leptonically. The
data correspond to an integrated luminosity of 5.4 fb-1 collected with the D0
detector at the Fermilab Tevatron Collider. We obtain mt = 174.0 +- 1.8(stat)
+- 2.4(syst) GeV, which is in agreement with the current world average mt =
173.3 +- 1.1 GeV. This is currently the most precise measurement of mt in the

Search for new fermions ("quirks") at the Fermilab Tevatron Collider
We report results of a search for particles with anomalously high ionization
in events with a high transverse energy jet and large missing transverse energy
in fb of integrated luminosity collected by the D0 experiment at
the Fermilab Tevatron collider. Production of such particles
(quirks) is expected in scenarios with extra QCD-like {\it SU(N)} sectors, and
this study is the first dedicated search for such signatures. We find no
evidence of a signal and set a lower mass limit of 107 ~GeV for the mass of a
charged quirk with strong dynamics scale in the range from 10 keV to

Search for single vector-like quarks in ppbar collisions at sqrt(s) = 1.96 TeV
We present a search for hypothetical vector-like quarks in ppbar collisions
at sqrt(s) = 1.96 TeV. The data were collected by the D0 detector at the
Fermilab Tevatron Collider and correspond to an integrated luminosity of 5.4
fb^(-1). We select events with a final state composed of a W or Z boson and a
jet consistent with a heavy object decay. We observe no significant excess in
comparison to the background prediction and set limits on production cross
sections for vector-like quarks decaying to W+jet and Z+jet. These are the most
stringent mass limits for electroweak single vector-like quark production at

Measurement of the B0_s semileptonic branching ratio to an orbitally excited D_s** state, Br(B0_s -> Ds1(2536) mu nu)
In a data sample of approximately 1.3 fb-1 collected with the D0 detector
between 2002 and 2006, the orbitally excited charm state D_s1(2536) has been
observed with a measured mass of 2535.7 +/- 0.6 (stat) +/- 0.5 (syst) MeV via
the decay mode B0_s -> D_s1(2536) mu nu X. A first measurement is made of the
branching ratio product Br(b(bar) -> D_s1(2536) mu nu X).Br(D_s1(2536)->D*
K0_S). Assuming that D_s1(2536) production in semileptonic decay is entirely
from B0_s, an extraction of the semileptonic branching ratio Br(B0_s ->

Measurement of the top quark pair production cross section in the lepton+jets channel in proton-antiproton collisions at =1.96 TeV
We present a measurement of the inclusive top quark pair production cross
section in \ppbar collisions at (\sqrt{s}=1.96) TeV utilizing data
corresponding to an integrated luminosity of \lumi\ collected with the D0
detector at the Fermilab Tevatron Collider. We consider final states containing
one high- isolated electron or muon and at least two jets, and we
perform three analyses: one exploiting specific kinematic features of \ttbar
events, the second using -jet identification, and the third using both
techniques to separate \ttbar\ signal from background. In the third case, we
determine simultaneously the cross section and the ratio of the
production rates of +heavy flavor jets and +light flavor jets, which
reduces the impact of the systematic uncertainties related to the background
estimation. Assuming a top quark mass of 172.5 GeV, we obtain
pb. This result agrees with
