    The potential of decentralised wastewater treatment in urban and rural sanitation in South Africa: lessons learnt from a demonstration-scale DEWATS within the eThekwini Municipality

    The design principles of decentralised wastewater treatment systems (DEWATS) make them a practical sanitation option for municipalities to adopt in fast-growing cities in South Africa. Since 2014, a demonstration scale DEWATS with a modular design consisting of a settler, anaerobic baffled reactor (ABR), anaerobic filter (AF), vertical down-flow constructed wetland (VFCW) and horizontal flow constructed wetland (HFCW) has been in operation in eThekwini. A performance evaluation after the long-term operation was undertaken in 2019 by comparing the final effluent with national regulatory requirements. Despite limitations in characterising the raw wastewater, a comparison of the settler and final effluent quality indicated high (≥ 85%) removal efficiencies of total chemical oxygen demand (CODt), ammonium-N (NH4-N) and orthophosphate-P (PO4-P), 75% removal of total suspended solids (TSS) and 83.3% log10 removal of Escherichia coli. Lack of exogenous and endogenous carbon and high dissolved oxygen (DO) concentrations (> 0.5 mg·L−1) inhibited denitrification in the HFCW, resulting in 12.5% of the effluent samples achieving compliance for nitrate-N (NO3-N). Moreover, mixed aggregate media and low residence times in the HFCW may have also contributed to poor NO3-N removal. During the COVID-19 lockdown, an unexpected shutdown and subsequent resumption of flow to the DEWATS indicated a 16-week recovery time based on achieving full nitrification in the HFCW. Although design modifications are necessary for the HFCW, the installation of urine diversion flushing toilets at the household level will reduce the nutrient loading to the DEWATS and potentially achieve fully compliant effluent. Alternatively, the application of two-stage vertical flow constructed wetlands to improve denitrification should also be explored in the South African context. With an improved design, DEWATS has the potential to fill the gap in both urban and rural sanitation in South Africa, where waterborne sanitation is still desired but connections to conventional wastewater treatment works (WWTWs) are not possible.&nbsp

    Precise Stellar Radial Velocities of an M Dwarf with a Michelson Interferometer and a Medium-resolution Near-infrared Spectrograph

    Precise near-infrared radial velocimetry enables efficient detection and transit verification of low-mass extrasolar planets orbiting M dwarf hosts, which are faint for visible-wavelength radial velocity surveys. The TripleSpec Exoplanet Discovery Instrument, or TEDI, is the combination of a variable-delay Michelson interferometer and a medium-resolution (R=2700) near-infrared spectrograph on the Palomar 200" Hale Telescope. We used TEDI to monitor GJ 699, a nearby mid-M dwarf, over 11 nights spread across 3 months. Analysis of 106 independent observations reveals a root-mean-square precision of less than 37 m/s for 5 minutes of integration time. This performance is within a factor of 2 of our expected photon-limited precision. We further decompose the residuals into a 33 m/s white noise component, and a 15 m/s systematic noise component, which we identify as likely due to contamination by telluric absorption lines. With further development this technique holds promise for broad implementation on medium-resolution near-infrared spectrographs to search for low-mass exoplanets orbiting M dwarfs, and to verify low-mass transit candidates.Comment: 55 pages and 13 figures in aastex format.     Multicentre cohort study to define and validate pathological assessment of response to neoadjuvant therapy in oesophagogastric adenocarcinoma.

    BACKGROUND: This multicentre cohort study sought to define a robust pathological indicator of clinically meaningful response to neoadjuvant chemotherapy in oesophageal adenocarcinoma. METHODS: A questionnaire was distributed to 11 UK upper gastrointestinal cancer centres to determine the use of assessment of response to neoadjuvant chemotherapy. Records of consecutive patients undergoing oesophagogastric resection at seven centres between January 2000 and December 2013 were reviewed. Pathological response to neoadjuvant chemotherapy was assessed using the Mandard Tumour Regression Grade (TRG) and lymph node downstaging. RESULTS: TRG (8 of 11 centres) was the most widely used system to assess response to neoadjuvant chemotherapy, but there was discordance on how it was used in practice. Of 1392 patients, 1293 had TRG assessment; data were available for clinical and pathological nodal status (cN and pN) in 981 patients, and TRG, cN and pN in 885. There was a significant difference in survival between responders (TRG 1-2; median overall survival (OS) not reached) and non-responders (TRG 3-5; median OS 2·22 (95 per cent c.i. 1·94 to 2·51) years; P < 0·001); the hazard ratio was 2·46 (95 per cent c.i. 1·22 to 4·95; P = 0·012). Among local non-responders, the presence of lymph node downstaging was associated with significantly improved OS compared with that of patients without lymph node downstaging (median OS not reached versus 1·92 (1·68 to 2·16) years; P < 0·001). CONCLUSION: A clinically meaningful local response to neoadjuvant chemotherapy was restricted to the small minority of patients (14·8 per cent) with TRG 1-2. Among local non-responders, a subset of patients (21·3 per cent) derived benefit from neoadjuvant chemotherapy by lymph node downstaging and their survival mirrored that of local responders

    Culture, geography, and the arts of government

    This paper endeavors to prise open the theoretical closure of the conceptualization of culture in contemporary human geography. Foucault's later work on government provides the basis for a useable definition of culture as an object of analysis which avoids problems inherent in abstract, generalizing and expansive notions of culture. The emergence of this Foucauldian approach in cultural studies is discussed, and the distinctive conceptualization of the relations between culture and power that it implies are elaborated. This re-conceptualization informs a critical project of tracking the institutional formation of the cultural and the deployment of distinctively cultural forms of regulation into the fabric of modern social life. It is argued that the culture-and-government approach needs to be supplemented by a more sustained consideration of the spatiality and scale of power-relations. It is also suggested that this approach might through into new perspective the dynamics behind geography's own cultural turn

    Squaring the circle: a priority-setting method for evidence-based service development, reconciling research with multiple stakeholder views.

    BACKGROUND: This study demonstrates a technique to aid the implementation of research findings through an example of improving services and self-management in longer-term depression. In common with other long-term conditions, policy in this field requires innovation to be undertaken in the context of a whole system of care, be cost-effective, evidence-based and to comply with national clinical guidelines. At the same time, successful service development must be acceptable to clinicians and service users and choices must be made within limited resources. This paper describes a novel way of resolving these competing requirements by reconciling different sources and types of evidence and systematically engaging multiple stakeholder views. METHODS: The study combined results from mathematical modelling of the care pathway, research evidence on effective interventions and findings from qualitative research with service users in a series of workshops to define, refine and select candidate service improvements. A final consensus-generating workshop used structured discussion and anonymised electronic voting. This was followed by an email survey to all stakeholders, to achieve a pre-defined criterion of consensus for six suggestions for implementation. RESULTS: An initial list of over 20 ideas was grouped into four main areas. At the final workshop, each idea was presented in person, visually and in writing to 40 people, who assigned themselves to one or more of five stakeholder groups: i) service users and carers, ii) clinicians, iii) managers, iv) commissioners and v) researchers. Many belonged to more than one group. After two rounds of voting, consensus was reached on seven ideas and one runner up. The survey then confirmed the top six ideas to be tested in practice. CONCLUSIONS: The method recruited and retained people with diverse experience and views within a health community and took account of a full range of evidence. It enabled a diverse group of stakeholders to travel together in a direction that converged with the messages coming out of the research and successfully yielded priorities for service improvement that met competing requirements

    Trends and determinants of excess winter mortality in New Zealand: 1980 to 2000

    <p>Abstract</p> <p>Background</p> <p>Although many countries experience an increase in mortality during winter, the magnitude of this increase varies considerably, suggesting that some winter excess may be avoidable. Conflicting evidence has been presented on the role of gender, region and deprivation. Little has been published on the magnitude of excess winter mortality (EWM) in New Zealand (NZ) and other Southern Hemisphere countries.</p> <p>Methods</p> <p>Monthly mortality rates per 100,000 population were calculated from routinely collected national mortality data for 1980 to 2000. Generalised negative binomial regression models were used to compare mortality rates between winter (June–September) and the warmer months (October–May).</p> <p>Results</p> <p>From 1980–2000 around 1600 excess winter deaths occurred each year with winter mortality rates 18% higher than expected from non-winter rates. Patterns of EWM by age group showed the young and the elderly to be particularly vulnerable. After adjusting for all major covariates, the winter:non-winter mortality rate ratio from 1996–2000 in females was 9% higher than in males. Mortality caused by diseases of the circulatory system accounted for 47% of all excess winter deaths from 1996–2000 with mortality from diseases of the respiratory system accounting for 31%. There was no evidence to suggest that patterns of EWM differed by ethnicity, region or local-area based deprivation level. No decline in seasonal mortality was evident over the two decades.</p> <p>Conclusion</p> <p>EWM in NZ is substantial and at the upper end of the range observed internationally. Interventions to reduce EWM are important, but the surprising lack of variation in EWM by ethnicity, region and deprivation, provides little guidance for how such mortality can be reduced.</p

    Gravitational Waves From Known Pulsars: Results From The Initial Detector Era

    We present the results of searches for gravitational waves from a large selection of pulsars using data from the most recent science runs (S6, VSR2 and VSR4) of the initial generation of interferometric gravitational wave detectors LIGO (Laser Interferometric Gravitational-wave Observatory) and Virgo. We do not see evidence for gravitational wave emission from any of the targeted sources but produce upper limits on the emission amplitude. We highlight the results from seven young pulsars with large spin-down luminosities. We reach within a factor of five of the canonical spin-down limit for all seven of these, whilst for the Crab and Vela pulsars we further surpass their spin-down limits. We present new or updated limits for 172 other pulsars (including both young and millisecond pulsars). Now that the detectors are undergoing major upgrades, and, for completeness, we bring together all of the most up-to-date results from all pulsars searched for during the operations of the first-generation LIGO, Virgo and GEO600 detectors.     Translational pharmacology of an inhaled small molecule αvβ6 integrin inhibitor for idiopathic pulmonary fibrosis

    The αvβ6 integrin plays a key role in the activation of transforming growth factor-β (TGFβ), a pro-fibrotic mediator that is pivotal to the development of idiopathic pulmonary fibrosis (IPF). We identified a selective small molecule αvβ6 RGD-mimetic, GSK3008348, and profiled it in a range of disease relevant pre-clinical systems. To understand the relationship between target engagement and inhibition of fibrosis, we measured pharmacodynamic and diseaserelated end points. Here we report, GSK3008348 binds to αvβ6 with high affinity in human IPF lung and reduces downstream pro-fibrotic TGFβ signaling to normal levels. In human lung epithelial cells, GSK3008348 induces rapid internalization and lysosomal degradation of the αvβ6 integrin. In the murine bleomycin-induced lung fibrosis model, GSK3008348 engages αvβ6, induces prolonged inhibition of TGFβ signaling and reduces lung collagen deposition and serum C3M, a marker of IPF disease progression. These studies highlight the potential of inhaled GSK3008348 as an anti-fibrotic therapy

    Utilisation of an operative difficulty grading scale for laparoscopic cholecystectomy

    Background A reliable system for grading operative difficulty of laparoscopic cholecystectomy would standardise description of findings and reporting of outcomes. The aim of this study was to validate a difficulty grading system (Nassar scale), testing its applicability and consistency in two large prospective datasets. Methods Patient and disease-related variables and 30-day outcomes were identified in two prospective cholecystectomy databases: the multi-centre prospective cohort of 8820 patients from the recent CholeS Study and the single-surgeon series containing 4089 patients. Operative data and patient outcomes were correlated with Nassar operative difficultly scale, using Kendall’s tau for dichotomous variables, or Jonckheere–Terpstra tests for continuous variables. A ROC curve analysis was performed, to quantify the predictive accuracy of the scale for each outcome, with continuous outcomes dichotomised, prior to analysis. Results A higher operative difficulty grade was consistently associated with worse outcomes for the patients in both the reference and CholeS cohorts. The median length of stay increased from 0 to 4 days, and the 30-day complication rate from 7.6 to 24.4% as the difficulty grade increased from 1 to 4/5 (both p < 0.001). In the CholeS cohort, a higher difficulty grade was found to be most strongly associated with conversion to open and 30-day mortality (AUROC = 0.903, 0.822, respectively). On multivariable analysis, the Nassar operative difficultly scale was found to be a significant independent predictor of operative duration, conversion to open surgery, 30-day complications and 30-day reintervention (all p < 0.001). Conclusion We have shown that an operative difficulty scale can standardise the description of operative findings by multiple grades of surgeons to facilitate audit, training assessment and research. It provides a tool for reporting operative findings, disease severity and technical difficulty and can be utilised in future research to reliably compare outcomes according to case mix and intra-operative difficulty
