    A Gaussian Model for Simulated Geomagnetic Field Reversals

    Field reversals are the most spectacular changes in the geomagnetic field but remain little understood. Paleomagnetic data primarily constrain the reversal rate and provide few additional clues. Reversals and excursions are characterized by a low in dipole moment that can last for some 10kyr. Some paleomagnetic records also suggest that the field decreases much slower before an reversals than it recovers afterwards and that the recovery phase may show an overshoot in field intensity. Here we study the dipole moment variations in several extremely long dynamo simulation to statistically explored the reversal and excursion properties. The numerical reversals are characterized by a switch from a high axial dipole moment state to a low axial dipole moment state. When analysing the respective transitions we find that decay and growth have very similar time scales and that there is no overshoot. Other properties are generally similar to paleomagnetic findings. The dipole moment has to decrease to about 30% of its mean to allow for reversals. Grand excursions during which the field intensity drops by a comparable margin are very similar to reversals and likely have the same internal origin. The simulations suggest that both are simply triggered by particularly large axial dipole fluctuations while other field components remain largely unaffected. A model at a particularly large Ekman number shows a second but little Earth-like type of reversals where the total field decays and recovers after some time

    Rationale for Prolonged Glucocorticoid Use in Pediatric ARDS: What the Adults Can Teach Us.

    Based on molecular mechanisms and physiologic data, a strong association has been established between dysregulated systemic inflammation and progression of acute respiratory distress syndrome (ARDS). In ARDS patients, glucocorticoid receptor-mediated downregulation of systemic inflammation is essential to restore homeostasis, decrease morbidity and improve survival and can be significantly enhanced with prolonged low-to-moderate dose glucocorticoid treatment. A large body of evidence supports a strong association between prolonged glucocorticoid treatment-induced downregulation of the inflammatory response and improvement in pulmonary and extrapulmonary physiology. The balance of the available data from eight controlled trials (n = 622) provides consistent strong level of evidence for improving patient-centered outcomes and hospital survival. The sizable increase in mechanical ventilation-free days (weighted mean difference, 6.48 days; CI 95% 2.57-10.38, p < 0.0001) and intensive care unit-free days (weighted mean difference, 7.7 days; 95% CI, 3.13-12.20, p < 0.0001) by day 28 is superior to any investigated intervention in ARDS. For treatment initiated before day 14 of ARDS, the increased in hospital survival (70 vs. 52%, OR 2.41, CI 95% 1.50-3.87, p = 0.0003) translates into a number needed to treat to save one life of 5.5. Importantly, prolonged glucocorticoid treatment is not associated with increased risk for nosocomial infections (22 vs. 27%, OR 0.61, CI 95% 0.35-1.04, p = 0.07). Treatment decisions involve a tradeoff between benefits and risks, as well as costs. This low-cost, highly effective therapy is familiar to every physician and has a low risk profile when secondary prevention measures are implemented

    Understanding the usage of multiple OSNs (Online Social Networks) has been of significant research interest as it helps in identifying the unique and distinguishing trait in each social media platform that contributes to its continued existence. The comparison between the OSNs is insightful when it is done based on the representative majority of the users holding active accounts on all the platforms. In this research, we collected a set of user profiles holding accounts on both Twitter and Instagram, these platforms being of prominence among a majority of users. An extensive textual and visual analysis on the media content posted by these users revealed that both these platforms are indeed perceived differently at a fundamental level with Instagram engaging more of the users' heart and Twitter capturing more of their mind. These differences got reflected in almost every microscopic analysis done upon the linguistic, topical and visual aspects.Comment: 4 pages, 8 figure

    Prolonged Glucocorticoid Treatment in ARDS: Impact on Intensive Care Unit-Acquired Weakness.

    Systemic inflammation and duration of immobilization are strong independent risk factors for the development of intensive care unit-acquired weakness (ICUAW). Activation of the pro-inflammatory transcription factor nuclear factor-κB (NF-κB) results in muscle wasting during disuse-induced skeletal muscle atrophy (ICU bed rest) and septic shock. In addition, NF-κB-mediated signaling plays a significant role in mechanical ventilation-induced diaphragmatic atrophy and contractile dysfunction. Older trials investigating high dose glucocorticoid treatment reported a lack of a sustained anti-inflammatory effects and an association with ICUAW. However, prolonged low-to-moderate dose glucocorticoid treatment of sepsis and ARDS is associated with a reduction in NF-κB DNA-binding, decreased transcription of inflammatory cytokines, enhanced resolution of systemic and pulmonary inflammation, leading to fewer days of mechanical ventilation, and lower mortality. Importantly, meta-analyses of a large number of randomized controlled trials investigating low-to-moderate glucocorticoid treatment in severe sepsis and ARDS found no increase in ICUAW. Furthermore, while the ARDS network trial investigating methylprednisolone treatment in persistent ARDS is frequently cited to support an association with ICUAW, a reanalysis of the data showed a similar incidence with the control group. Our review concludes that in patients with sepsis and ARDS, any potential direct harmful neuromuscular effect of glucocorticoids appears outweighed by the overall clinical improvement and reduced duration of organ failure, in particular ventilator dependency and associated immobilization, which are key risk factors for ICUAW

    Development of a methodology to delineate hurricane evacuation zones

    The main focus of this study is to exhibit a simple and easily implemented process to identify hurricane evacuation zones. The proposed study presents a new methodology to delineate hurricane evacuation zones. The new methodology helps in easier identification of zones, thereby making the task of evacuation officials easier. Hurricane evacuation zones were identified based on the elevation of the points comprising the study area. An area layer was created based on the storm surge models run, and were overlaid with zip code boundary layer and land use data available. Elevation point file data was superimposed on the study area layer to find out the mean elevation and standard deviation values of elevation, which help in identifying the adjacent zones which are similar in elevation. These zones are grouped together and the process of joining zones is continued until the zones are sufficiently reduced. The entire process was carried out in TransCAD, a Transportation/GIS software package. This task involves an iterative process which would be extremely tedious to perform manually. Hence, the process was automated by writing a customized add-in program to TransCAD. The study also included the application of this methodology to the New Orleans metropolitan area, it being an ideal test case for implementing the methodology that was developed. The results portray New Orleans area being ideally and conveniently divided into hurricane evacuation zones based on their elevations, zip code and storm surge data. The GIS program developed during this study provides a framework, which may be built upon and shared with other researchers in the future

    Technology Independent Synthesis of CMOS Operational Amplifiers

    Analog circuit design does not enjoy as much automation as its digital counterpart. Analog sizing is inherently knowledge intensive and requires accurate modeling of the different parametric effects of the devices. Besides, the set of constraints in a typical analog design problem is large, involving complex tradeoffs. For these reasons, the task of modeling an analog design problem in a form viable for automation is much more tedious than the digital design. Consequently, analog blocks are still handcrafted intuitively and often become a bottleneck in the integrated circuit design, thereby increasing the time to market. In this work, we address the problem of automatically solving an analog circuit design problem. Specifically, we propose methods to automate the transistor-level sizing of OpAmps. Given the specifications and the netlist of the OpAmp, our methodology produces a design that has the accuracy of the BSIM models used for simulation and the advantage of a quick design time. The approach is based on generating an initial first-order design and then refining it. In principle, the refining approach is a simulated-annealing scheme that uses (i) localized simulations and (ii) convex optimization scheme (COS). The optimal set of input variables for localized simulations has been selected by using techniques from Design of Experiments (DOE). To formulate the design problem as a COS problem, we have used monomial circuit models that are fitted from simulation data. These models accurately predict the performance of the circuit in the proximity of the initial guess. The models can also be used to gain valuable insight into the behavior of the circuit and understand the interrelations between the different performance constraints. A software framework that implements this methodology has been coded in SKILL language of Cadence. The methodology can be applied to design different OpAmp topologies across different technologies. In other words, the framework is both technology independent and topology independent. In addition, we develop a scheme to empirically model the small signal parameters like \u27gm\u27 and \u27gds\u27 of CMOS transistors. The monomial device models are reusable for a given technology and can be used to formulate the OpAmp design problem as a COS problem. The efficacy of the framework has been demonstrated by automatically designing different OpAmp topologies across different technologies. We designed a two-stage OpAmp and a telescopic OpAmp in TSMC025 and AMI016 technologies. Our results show significant (10–15%) improvement in the performance of both the OpAmps in both the technologies. While the methodology has shown encouraging results in the sub-micrometer regime, the effectiveness of the tool has to be investigated in the deep-sub-micron technologies

    Microperimetric evaluation in patients with adult-onset foveomacular vitelliform dystrophy

    INTRODUCTION: To compare mean best-corrected visual acuity (BCVA), retinal sensitivity (RS), and bivariate contour ellipse area (BCEA) in patients with adult-onset foveomacular vitelliform dystrophy (AOFVD) and healthy subjects (HSs), reporting also functional disease-related changes in the different stages of the AOFVD disease. MATERIALS AND METHODS: In this observational cross-sectional study, a total of 19 patients (30 eyes; 12 female and 7 male) with AOFVD were enrolled, and 30 patients (30 eyes; 16 female and 14 male) were recruited as age-matched control group (74.36 ± 9.17 years vs. 71.83 ± 6.99 years respectively, P= 0.11). All patients underwent a complete ophthalmologic examination, fundus autofluorescence and fluorescein angiography, spectral-domain optical coherence tomography and microperimetry (MP)-1 analysis. The data collection included mean BCVA, mean RS measured by means of MP-1, BCEA, and central retinal thickness. RESULTS: All the functional parameters (BCVA, RS, and BCEA) were significantly worse in AOFVD group than HS. Subgroup analysis showed that the most significant functional changes, quantified by mean BCVA, RS, and BCEA, were in the atrophic stage (P = 0.03, P= 0.01, and P= 0.001, respectively). All the functional parameters were well correlated in the different stages. CONCLUSIONS: This study further confirms the good visual prognosis in the AOFVD eyes. Fixation stability measurement using BCEA demonstrates good evaluation of visual performance integrating traditional functional parameters. It may also serve for further rehabilitative purposes in atrophic eyes
