    Paralisia facial periférica idiopática de Bell: a propósito de 180 pacientes Idiopathic facial paralysis (Bell´s palsy): a study of 180 patients

    O objetivo deste estudo foi analisar aspectos clínicos, epidemiológicos e evolutivos da paralisia facial periférica idiopática em 180 pacientes. Houve ligeiro predomínio do sexo feminino (66,7%). Quanto à faixa etária mais acometida, foram observados dois picos de incidência um nas terceira e quarta décadas, e o outro na sexta década de vida. Nos 180 pacientes houve 198 episódios de paralisia facial periférica, sendo 17 recorrências e em um paciente a paralisia foi de instalação inicial bilateral. Em 15 pacientes (8,3%) houve recidiva da paralisia facial, em dois casos a paralisia se repetindo por mais duas vezes. Em 12 casos (70,6%) a recidiva ocorreu no mesmo lado da paralisia anterior. O lado esquerdo da face estava envolvido em 55,6% dos casos. Em oito pacientes a paralisia aconteceu na gestação (n=5) ou no pós-parto (n=3). Quatro das pacientes grávidas apresentaram paralisia de Bell no terceiro trimestre. Uma paciente com 18 anos de idade desenvolveu paralisia facial do lado direito no sétimo mês da gravidez, havendo recidiva da paralisia no mesmo lado aos 23 anos de idade, no 15º dia pós-parto. Como condicões associadas encontramos hipertensão arterial sistêmica (11,7%), diabete mélito (11,1%), gravidez ou pós-parto imediato (4,4%; 6,7% nas mulheres) e neurocisticercose (1,1%). Em 72,8% dos casos não encontramos associação com outras afecções. Em 22,8% dos pacientes observou-se uma das seguintes sequelas: espasmo hemifacial (12,8%), recuperação parcial do déficit motor (10,6%), síndrome das lágrimas do crocodilo (3,3%), contrações sincinéticas (2,8%), lacrimejamento (1,1%) e fenômeno de Marcus Gunn invertido (1,1%). Concluíndo, no estudo foi demonstrado que a paralisia facial periférica idiopática pode provocar sequelas importantes, cosméticas ou funcionais, em mais de 20% dos pacientes.<br>The objective of this study was to analyze some clinical and epidemiologic aspects, as well as the follow up of 180 patients with Bell´s palsy. In the study population there was a predominance of female (66.7%). Two peaks of incidence in the age distribution were identified: third-fourth and sixth decades of life. In the group of 180 patients there were 198 events of facial paralysis, 17 recurrences and in one patient the paralysis was bilateral at the onset. In 15 patients (8.3%) there were recurrences of the facial paralysis, in 12 cases (70.6%) the recurrences were ipsilateral. The left side of the face was involved in 55.6% of the cases. In eight patients the paralysis ocurred during pregnancy (n=5) or puerperium (n=3). As associated conditions we found: arterial hypertension (11.7%), diabetes mellitus (11.1%), pregnancy or puerperium (4.4%; 6.7% in the women), and neurocysticercosis (1.1%). In 72.8% of the cases no association with such conditions was found. In 22.8% of the patients some kind of sequelae were identified: hemifacial spasm (12.8%), partial recovery of the motor deficit (10.6%), syndrome of the crocodile tears (3.3%), sincinetic contration (2.8%), and the Marcus Gunn inverse phenomenon (1.1%). In conclusion, this study shows that the idiopathic facial paralysis may lead to important sequelae in more than 20% of the patients

    Effects of the Electromagnetic field, 60 Hz, 3 µT, on the hormonal and metabolic regulation of undernourished pregnant rats

    Epidemiological studies have implicated maternal protein-calorie deficiency as an important public health problem in developing countries. Over the last decades, a remarkable diffusion of electricity and an increased level of the electromagnetic field (EMF) in the environment have characterized modern societies. Therefore, researchers are concerned with the biological effects of 50-60 Hz, EMF. The aim of this paper is to show the effects of EMF of 60 Hz, 3 μT, exposure for two hours per day in the regulation of the hormonal and metabolic concentrations in pregnant rats, which were fed by Regional Basic Diet (RBD) during their pregnancy as compared with pregnant rats fed a standard diet. Pregnant rats exposed to EMF of 60 Hz, 3 μT, over the pregnancy and fed with RBD presented an increase in glucose release when compared with the Group subjected only to the RBD ration. Rats fed RBD presented a decrease in their insulin and cortisol serum levels when compared with the Group fed with casein. The T3 and T4 concentrations presented the greatest variation among the Groups. The relation T4:T3 was much exaggerated in the Group subjected to RDB and exposed to EMF when compared to the others. In conclusion, the group subjected to the association of EMF and undernutrition suffered a decrease in its serum concentration of T4 and T3 when compared to the well-nourished group and the relationship T4:T3 in the former group was almost eighteen-fold the later one

    Os avanços tecnológicos têm aumentado o número de equipamentos elétricos e eletrônicos, seja nas residências ou mesmo no ambiente de trabalho, fazendo com que a população conviva com grande número de fontes de irradiação eletromagnética, com os mais diversos níveis de potência e freqüência. Por muitos anos, alguns cientistas e engenheiros acreditaram que o campo eletromagnético (CEM) com freqüência extremamente baixa não pudesse causar efeitos e alterações significantes no material biológico. O objetivo deste trabalho é verificar os possíveis efeitos adversos dos CEMs em humanos e animais, que foram publicados nos últimos anos, através de uma revisão da literatura disponível em Medline, revistas nacionais e internacionais e catálogos de obras de referência na área dos CEM (50/60 Hz). Como resultado foi observado que o CEM (50/60 Hz) é capaz de produzir diversos efeitos adversos em humanos e animais, como por exemplo: distúrbios na reprodução, doenças degenerativas, efeitos psiquiátricos e psicológicos, alterações citogenéticas, alterações no sistema cardiovascular, nervoso e neuroendócrino, bem como nos parâmetros biológicos e bioquímicos. Apesar de todas estas constatações e devido a muitas controvérsias entre vários autores, faz-se necessário um estudo mais específico e aprofundado sobre o assunto.<br>The technologic development has increased the number of electric and electronic devices for household and work environment applications. In this way, we have to cope with a diverse quantity of electromagnetic irradiation sources, with different power and frequency ranges. For many years, some scientists and engineers believed that low-frequencies electromagnetic field (EMF) could not cause any bad effect or substantial alterations on the biologic livings. This work has the objective to perform a literature review of the possible effects of EMF in human beings and animals, that was published in the past years on Medline, international, and national journals about the EMF (50/60Hz). The results showed that extremely low EMF might produce adverse effects, i.e. cancer, reproduction disruption, degenerative illnesses, citogenetic alterations, and cardiovascular, neurologic and neuroendocrine system alterations in humans and animals. The biologic and biochemical parameters suffered interference as well. Despite all these findings, we can find some disagreements among the authors. Hence it is necessary to extend the research about this issue

    Acetylcysteine for Prevention of Renal Outcomes in Patients Undergoing Coronary and Peripheral Vascular Angiography Main Results From the Randomized Acetylcysteine for Contrast-Induced Nephropathy Trial (ACT)

    Background-It remains uncertain whether acetylcysteine prevents contrast-induced acute kidney injury. Methods and Results-We randomly assigned 2308 patients undergoing an intravascular angiographic procedure with at least 1 risk factor for contrast-induced acute kidney injury (age >70 years, renal failure, diabetes mellitus, heart failure, or hypotension) to acetylcysteine 1200 mg or placebo. The study drugs were administered orally twice daily for 2 doses before and 2 doses after the procedure. The allocation was concealed (central Web-based randomization). All analysis followed the intention-to-treat principle. The incidence of contrast-induced acute kidney injury (primary end point) was 12.7% in the acetylcysteine group and 12.7% in the control group (relative risk, 1.00; 95% confidence interval, 0.81 to 1.25; P = 0.97). A combined end point of mortality or need for dialysis at 30 days was also similar in both groups (2.2% and 2.3%, respectively; hazard ratio, 0.97; 95% confidence interval, 0.56 to 1.69; P = 0.92). Consistent effects were observed in all subgroups analyzed, including those with renal impairment. Conclusions-In this large randomized trial, we found that acetylcysteine does not reduce the risk of contrast-induced acute kidney injury or other clinically relevant outcomes in at-risk patients undergoing coronary and peripheral vascular angiography.Ministério da Saúde do BrasilBrazilian Ministry of Healt
