    How effective are experienced hepatologists at staging fibrosis using non-invasive fibrosis tests in patients with metabolic dysfunction-associated steatotic liver disease?

Background: Sequential use of non-invasive fibrosis tests (NITs) to identify patients with advanced hepatic fibrosis is recommended. However, it remains unclear how reliable clinicians are staging liver fibrosis using combinations of NITs. Aim: Our aim was to assess concordance between NIT-based ‘clinician fibrosis assessment (CFA)’ and histology in patients with metabolic dysfunction-associated steatotic liver disease (MASLD) and compare this with established algorithmic approaches. Methods: Six experienced hepatologists independently staged 230 MASLD patients for advanced fibrosis (F0-2 vs F3-4) using FIB-4, FIB-4+ELF, FIB-4+ vibration controlled transient elastography (VCTE; Fibroscanℱ) and FIB-4+ELF+VTCE. Concordance between histology and CFA or algorithmic approaches were assessed. Results: A total of 230 patients were included (median age 54 [22–78] years; 55% female; median FIB-4 1.21 [IQR: 0.78–1.91]; ELF 9.3 [IQR: 8.6–10.2]; VCTE 9.4 [IQR: 6.3–14.3]; 41% F0-1, 22% F2, 21% F3 and 16% F4). Overall, area under the receiver operator curves for histologic F3-4 for the raw tests were 0.84 for FIB-4, 0.86 for ELF and 0.86 for VCTE. Concordance between the hepatologists was good (FIB4, Îș = 0.64; FIB-4+ELF, Îș = 0.70; FIB-4+VCTE, Îș = 0.69; FIB-4+ELF+VCTE, Îș = 0.70). Concordance between individual CFA and histology was variable, which was reflected in variability in sensitivity (44%–84%) and specificity (76%–94%). Concordance with histology was better when clinicians used NIT combinations. Purely algorithmic approaches, particularly sequential use of FIB-4 then VCTE, tended to perform better than the CFA. Conclusions: Adhering to the recommended algorithmic approaches using NITs to stage fibrosis tended to perform more accurately than less-structured clinician NIT-based assessments conducted by experienced hepatologists

    Assortative Mixing Equilibria in Social Network Games

    It is known that individuals in social networks tend to exhibit homophily (a.k.a. assortative mixing) in their social ties, which implies that they prefer bonding with others of their own kind. But what are the reasons for this phenomenon? Is it that such relations are more convenient and easier to maintain? Or are there also some more tangible benefits to be gained from this collective behaviour? The current work takes a game-theoretic perspective on this phenomenon, and studies the conditions under which different assortative mixing strategies lead to equilibrium in an evolving social network. We focus on a biased preferential attachment model where the strategy of each group (e.g., political or social minority) determines the level of bias of its members toward other group members and non-members. Our first result is that if the utility function that the group attempts to maximize is the degree centrality of the group, interpreted as the sum of degrees of the group members in the network, then the only strategy achieving Nash equilibrium is a perfect homophily, which implies that cooperation with other groups is harmful to this utility function. A second, and perhaps more surprising, result is that if a reward for inter-group cooperation is added to the utility function (e.g., externally enforced by an authority as a regulation), then there are only two possible equilibria, namely, perfect homophily or perfect heterophily, and it is possible to characterize their feasibility spaces. Interestingly, these results hold regardless of the minority-majority ratio in the population. We believe that these results, as well as the game-theoretic perspective presented herein, may contribute to a better understanding of the forces that shape the groups and communities of our society

    Techno-economic evaluation of reducing shielding gas consumption in GMAW whilst maintaining weld quality

    A new method of supplying shielding gases in an alternating manner has been developed to enhance the efficiency of conventional gas metal arc welding (GMAW). However, the available literature on this advanced joining process is very sparse and no cost evaluation has been reported to date. In simple terms, the new method involves discretely supplying two different shielding gases to the weld pool at predetermined frequencies which creates a dynamic action within the liquid pool. In order to assess the potential benefits of this new method from a technical and cost perspective, a comparison has been drawn between the standard shielding gas composition of Ar/20%CO2, which is commonly used in UK and European shipbuilding industries for carbon steels, and a range of four different frequencies alternating between Ar/20%CO2 and helium. The beneficial effects of supplying the weld shielding gases in an alternating manner were found to provide attractive benefits for the manufacturing community. For example, the present study showed that compared with conventional GMAW, a 17 per cent reduction in total welding cost was achieved in the case of the alternating gas method and savings associated with a reduction in the extent of post-weld straightening following plate distortion were also identified. Also, the mechanical properties of the alternating case highlighted some marginal improvements in strength and Charpy impact toughness which were attributed to a more refined weld microstructure
