    The effects of home-based exercise therapy for breast cancer-related fatigue induced by radical radiotherapy

    Background Radiotherapy (RT) can lead to cancer-related fatigue (CRF) and decreased health-related quality of life (HRQoL) in breast cancer patients. The purpose of this trial was to examine the feasibility and efficacy of a home-based resistance and aerobic exercise intervention for reducing CRF and improving HRQoL in breast cancer patients during RT. Methods Women with breast cancer (N = 106) commencing RT were randomized to 12 weeks of home-based resistance and aerobic exercise (EX) or usual care/control (CON). The primary endpoint was CRF, with secondary endpoints of HRQoL, sleep duration and quality, and physical activity. Measurements were undertaken prior to RT, at completion of RT (~ 6 weeks), at completion of the intervention (12 weeks), and 6 and 12 months after RT completion, while CRF was also measured weekly during RT. Results Eighty-nine women completed the study (EX = 43, CON = 46). Over the 12-week intervention, EX completed 1–2 resistance training sessions and accumulated 30–40 min of aerobic exercise weekly. For CRF, EX had a quicker recovery both during and post-RT compared to CON (p < 0.05). Moreover, there was a significant difference in HRQoL between groups at RT completion, with HRQoL unchanged in CON and higher in EX (p < 0.05). There was no change in sleep duration or quality for either group and there were no exercise-related adverse effects. Conclusions Home-based resistance and aerobic exercise during RT is safe, feasible, and effective in accelerating CRF recovery and improving HRQoL. Improvements in CRF and HRQoL for these patients can be achieved with smaller exercise dosages than stated in the generic recommendations for breast cancer

    Association between energy balance-related factors and clinical outcomes in patients with ovarian cancers:A systematic review and meta analysis

    Background: This systematic review and meta-analysis synthesized evidence in patients with ovarian cancer at diagnosis and/or during first-line treatment on; (i) the association of body weight, body composition, diet, exercise, sedentary behavior, or physical fitness with clinical outcomes; and (ii) the effect of exercise and/or dietary interventions. Methods: Risk of bias assessments and best-evidence syntheses were completed. Meta-analyses were performed when &ge;3 papers presented point estimates and variability measures of associations or effects. Results: Body mass index (BMI) at diagnosis was not significantly associated with survival. Although the following trends were not supported by the best-evidence syntheses, the meta-analyses revealed that a higher BMI was associated with a higher risk of post-surgical complications (n = 5, HR: 1.63, 95% CI: 1.06&ndash;2.51, p = 0.030), a higher muscle mass was associated with a better progression-free survival (n = 3, HR: 1.41, 95% CI: 1.04&ndash;1.91, p = 0.030) and a higher muscle density was associated with a better overall survival (n = 3, HR: 2.12, 95% CI: 1.62&ndash;2.79, p &lt; 0.001). Muscle measures were not significantly associated with surgical or chemotherapy-related outcomes. Conclusions: The prognostic value of baseline BMI for clinical outcomes is limited, but muscle mass and density may have more prognostic potential. High-quality studies with comprehensive reporting of results are required to improve our understanding of the prognostic value of body composition measures for clinical outcomes.     Magnon-magnon interactions in the Spin-Peierls compound CuGeO_3

    In a magnetic substance the gap in the Raman spectrum, Delta_R, is approximatively twice the value of the neutron scattering gap, Delta_S, if the the magnetic excitations (magnons) are only weakly interacting. But for CuGeO_3 the experimentally observed ratio Delta_R/Delta_S is approximatively 1.49-1.78, indicating attractive magnon-magnon interactions in the quasi-1D Spin-Peierls compound CuGe_3. We present numerical estimates for Delta_R/Delta_S from exact diagonalization studies for finite chains and find agreement with experiment for intermediate values of the frustration parameter alpha. An analysis of the numerical Raman intensity leads us to postulate a continuum of two-magnon bound states in the Spin-Peierls phase. We discuss in detail the numerical method used, the dependence of the results on the model parameters and a novel matrix-element effect due to the dimerization of the Raman-operator in the Spin-Peierls phase.Comment: submitted to PRB, Phys.     Anisotropic optical response of the diamond (111)-2x1 surface

    The optical properties of the 2×\times1 reconstruction of the diamond (111) surface are investigated. The electronic structure and optical properties of the surface are studied using a microscopic tight-binding approach. We calculate the dielectric response describing the surface region and investigate the origin of the electronic transitions involving surface and bulk states. A large anisotropy in the surface dielectric response appears as a consequence of the asymmetric reconstruction on the surface plane, which gives rise to the zigzag Pandey chains. The results are presented in terms of the reflectance anisotropy and electron energy loss spectra. While our results are in good agreement with available experimental data, additional experiments are proposed in order to unambiguously determine the surface electronic structure of this interesting surface.Comment: REVTEX manuscript with 6 postscript figures, all included in uu file.     Combustion flame spray of CoNiCrAlY & YSZ coatings

    The properties of CoNiCrAlY and ∼7–8%YSZ layers, used as thermal barrier coatings (TBC) to protect hot gas paths of power generation and aerospace gas turbines, that have been deposited through the Combustion Flame Spray (CFS) process, are assessed and compared to coatings of the same materials deposited through Atmospheric Plasma Spray (APS). Fuel-to-oxygen equivalence ratio, combustion and carrier gases flows, torch standoff distance and powder feed rate values have been varied during the CFS tests in order to assess their effect on microstructural characteristics, i.e. thickness, total porosity, oxide level and microhardness. Results show that, in CFS-deposited coatings, although a higher content of oxide strings and porosity is observed compared to APS, also comparable phase transformations and a higher thermal cyclic lifetime can be achieved with an appropriate tuning of the deposition parameters. Thus, the study demonstrates the excellent capability of the CFS process in depositing thermal barrier coating systems, providing a viable alternative deposition technology for this class of materials at significant hardware simplicity. As the CFS setup has a simple design, this research stimulates a miniaturisation concept of the combustion flame spray torch for allowing its deployment into highly restricted workspaces

    A green-gray path to global water security and sustainable infrastructure

    Sustainable development demands reliable water resources, yet traditional water management has broadly failed to avoid environmental degradation and contain infrastructure costs. We explore the global-scale feasibility of combining natural capital with engineering-based (green-gray) approaches to meet water security threats over the 21st century. Threats to water resource systems are projected to rise throughout this period, together with a significant expansion in engineering deployments and progressive loss of natural capital. In many parts of the world, strong path dependencies are projected to arise from the legacy of prior environmental degradation that constrains future water management to a heavy reliance on engineering-based approaches. Elsewhere, retaining existing stocks of natural capital creates opportunities to employ blended green-gray water infrastructure. By 2050, annual engineering expenditures are projected to triple to 2.3trillion,investedmainlyindevelopingeconomies.Incontrast,preservingnaturalcapitalforthreatsuppressionrepresentsapotential2.3 trillion, invested mainly in developing economies. In contrast, preserving natural capital for threat suppression represents a potential 3.0 trillion in avoided replacement costs by mid-century. Society pays a premium whenever these nature-based assets are lost, as the engineering costs necessary to achieve an equivalent level of threat management are, on average, twice as expensive. Countries projected to rapidly expand their engineering investments while losing natural capital will be most constrained in realizing green-gray water management. The situation is expected to be most restrictive across the developing world, where the economic, technical, and governance capacities to overcome such challenges remain limited. Our results demonstrate that policies that support blended green-gray approaches offer a pathway to future global water security but will require a strategic commitment to preserving natural capital. Absent such stewardship, the costs of water resource infrastructure and services will likely rise substantially and frustrate efforts to attain universal and sustainable water security

    Corrosion of cast Stellite-3 analogue in simulated PWR conditions

    Wear resistant alloys are required for deployment of pressurised water reactor primary circuits and in this context, the corrosion behaviour of a cast cobalt-based alloy following exposure for 30 days in lithiated water at 300°C was investigated. Corrosion of the surface was observed, resulting in the formation of a ~100 nm thick chromium and silicon-rich oxide. Preferential corrosion of the matrix at its interface with just one of the carbide types was observed to a depth of ~1 μm; for the first time this was found not due to any inhomogeneity in the matrix but was instead an electrochemical effect

    Psychosocial Treatment of Children in Foster Care: A Review

    A substantial number of children in foster care exhibit psychiatric difficulties. Recent epidemiologi-cal and historical trends in foster care, clinical findings about the adjustment of children in foster care, and adult outcomes are reviewed, followed by a description of current approaches to treatment and extant empirical support. Available interventions for these children can be categorized as either symptom-focused or systemic, with empirical support for specific methods ranging from scant to substantial. Even with treatment, behavioral and emotional problems often persist into adulthood, resulting in poor functional outcomes. We suggest that self-regulation may be an important mediat-ing factor in the appearance of emotional and behavioral disturbance in these children
