    Older Adults at Risk for Atrial Fibrillation Lack Knowledge and Confidence to Seek Treatment for Signs and Symptoms.

    Early detection of atrial fibrillation (AF) is crucial for averting AF-related stroke and heart failure, but treatment is delayed when AF is not recognized. The critical need for early detection and treatment requires education to promote AF awareness. Knowledge deficits, attitudes, and beliefs about AF that should be addressed to improve awareness and reduce treatment-seeking delay in older adults at risk for developing AF have not been well documented. The purpose of this study was to describe knowledge, treatment-seeking attitudes, and beliefs about AF in adults ⩾ 65 years old and identify demographic characteristics associated with knowledge, attitudes, and beliefs. Patients with no history of AF recruited from an academic medical center were interviewed using the Knowledge, Attitudes, and Beliefs about Atrial Fibrillation Survey. Data were analyzed using descriptive statistics and independent t tests. Participants (N = 180) were 63% male with a mean age of ±3.± 6.0 years, and 52% held ⩾ 4-year college degree. About one third could not identify common symptoms of AF including palpitations (31%), chest pain (36%), dyspnea (30%), and fatigue (35%). A majority (84%) lacked confidence to recognize AF, and 58% were not sure when they should seek care for AF symptoms. Nearly a third (32%) believed palpitations are always present with AF, and 74% believed that low energy would not be their only symptom of AF. Higher scores for AF Symptom Knowledge (p = .02) were observed in females, and General Knowledge about AF was greater for younger participants (p < .001). Participants lacked knowledge and confidence to aid decision-making for treatment-seeking for symptoms of AF and held inaccurate beliefs about AF that could hinder early treatment-seeking. Programs to promote AF awareness should explain the spectrum of symptoms that may be manifested by AF and include action plans for responding to symptoms

    Comparison of Shear Capacity of T-Beams Using Strut and Tie Analysis

    The objective of this research was to study the shear strength of continuous lightly reinforced concrete beams which are widely used in practice in North America. In 1990 six two-span T-beams were tested at the University of Kansas. The research was focused on the primary variables of longitudinal reinforcement ratio and nominal stirrup strength. The tests indicated that the shear provisions of ACI 318-89 overpredicted the concrete shear capacity of lightly reinforced beams in negative moment regions, and underestimated the stirrup contribution to shear strength which increases with higher flexural reinforcement ratios. These experimental results were reevaluated and analyzed with the shear provisions obtained from existing codes and theories from Europe and Canada including EC 2, DIN I 045, CEB Model Code 1990, ACI 318-95, and Modified Compression Field Theory. In addition a Strut and Tie analysis for these tests also was conducted. The study indicates that the shear strength may be determined by Strut and Tie approaches with variable inclination e of the compression field based on the theory of plasticity to derive the interaction between bending moment and shear . The comparison shows that the shear capacity based on a clear truss model and the theory of plasticity gives results that correspond to the experimental results and lead to an economic design. Moreover, the results of this study also reveal the limitation of the present ACI provisions and show that codes formulated on a rigorous theoretical foundation provides excellent results when compared the test data

    Extending electron orbital precession to the molecular case: Can orbital alignment be used to observe wavepacket dynamics?

    The complexity of ultrafast molecular photoionization presents an obstacle to the modelling of pump-probe experiments. Here, a simple optimized model of atomic rubidium is combined with a molecular dynamics model to predict quantitatively the results of a pump-probe experiment in which long range rubidium dimers are first excited, then ionized after a variable delay. The method is illustrated by the outline of two proposed feasible experiments and the calculation of their outcomes. Both of these proposals use Feshbach 87Rb2 molecules. We show that long-range molecular pump-probe experiments should observe spin-orbit precession given a suitable pump-pulse, and that the associated high-frequency beat signal in the ionization probability decays after a few tens of picoseconds. If the molecule was to be excited to only a single fine structure state state, then a low-frequency oscillation in the internuclear separation would be detectable through the timedependent ionization cross section, giving a mechanism that would enable observation of coherent vibrational motion in this molecule.Comment: 9 pages, 10 figures, PRA submissio

    The purpose of this paper is to elucidate, by means of concepts and theorems drawn from mathematical logic, the conditions under which the existence of a multiverse is a logical necessity in mathematical physics, and the implications of Godel's incompleteness theorem for theories of everything. Three conclusions are obtained in the final section: (i) the theory of the structure of our universe might be an undecidable theory, and this constitutes a potential epistemological limit for mathematical physics, but because such a theory must be complete, there is no ontological barrier to the existence of a final theory of everything; (ii) in terms of mathematical logic, there are two different types of multiverse: classes of non-isomorphic but elementarily equivalent models, and classes of model which are both non-isomorphic and elementarily inequivalent; (iii) for a hypothetical theory of everything to have only one possible model, and to thereby negate the possible existence of a multiverse, that theory must be such that it admits only a finite model

    Feasibility Testing of the Alert for AFib Intervention.

    Improving early detection and treatment of atrial fibrillation (AF) is critical because untreated AF is a major contributor to stroke and heart failure. We sought to generate knowledge about the feasibility of conducting a randomized controlled trial to test the effect of the Alert for AFib intervention on knowledge, attitudes, and beliefs about treatment-seeking for signs and symptoms of AF. Adults ≥65 years old (96% White) at risk for developing AF were randomized to receive the Alert for AFib intervention ( n = 40) or an attention control session ( n = 40). Feasibility goals for recruitment, participant retention, adherence, perceived satisfaction and burden, and intervention fidelity were met. From baseline to study completion, knowledge ( p = .005) and attitudes ( p < .001) about treatment-seeking improved more in the intervention group compared with the control group. Results support testing the effectiveness of the Alert for AFib intervention in a large trial

    NICMOS Images of the GG Tau Circumbinary Disk

    We present deep, near-infrared images of the circumbinary disk surrounding the pre-main-sequence binary star, GG Tau A, obtained with NICMOS aboard the Hubble Space Telescope. The spatially resolved proto-planetary disk scatters roughly 1.5% of the stellar flux, with a near-to-far side flux ratio of ~1.4, independent of wavelength, and colors that are comparable to the central source; all of these properties are significantly different from the earlier ground-based observations. New Monte Carlo scattering simulations of the disk emphasize that the general properties of the disk, such as disk flux, near side to far side flux ratio and integrated colors, can be approximately reproduced using ISM-like dust grains, without the presence of either circumstellar disks or large dust grains, as had previously been suggested. A single parameter phase function is fitted to the observed azimuthal variation in disk flux, providing a lower limit on the median grain size of 0.23 micron. Our analysis, in comparison to previous simulations, shows that the major limitation to the study of grain growth in T Tauri disk systems through scattered light lies in the uncertain ISM dust grain properties. Finally, we use the 9 year baseline of astrometric measurements of the binary to solve the complete orbit, assuming that the binary is coplanar with the circumbinary ring. We find that the estimated 1 sigma range on disk inner edge to semi-major axis ratio, 3.2 < Rin/a < 6.7, is larger than that estimated by previous SPH simulations of binary-disk interactions.Comment: 40 pages, 8 postscript figures, accepted for publication in Ap

    A comparison of the socio-cultural influences on body size perceptions and values among European Australian and Indigenous Fijian adolescent girls

    Western women are experiencing increases in the prevalence of overweight and obesity despite valuing slimness as ideal and equating thinness with attractiveness and beauty. Pacific populations are also experiencing high prevalence rates of overweight and obesity. A lage body size, however, has many positive connotations among pacific populations that stem from rich cultural practices such as feasts and deliberate fattening of women. There appears to be many sociocultural factors however, such as images of the slim ideal portrayed in Western media, influencing body image concerns that are shared between the two groups. A qualitative study involving semi-structured interviews was conducted to examine and compare the sociocultural influences on body size perceptionsand values. Participants include 16 European Australian and 16 Indigenous Fijian adolescent girls aged between 15 and 17 years. Inductive thematic analysis and analytical induction were qualitativemethods employed to facilitate interpretation of the findings. Preliminary findings suggest that Fijian adolescent girls experience greater conflict than European Australian girls about the desired body size endorsed in their community. Results also suggest that Indigenous Fijian girls place less importance than Australian girls on manipulating their body to achieve a specific ideal. Specific results of this study are reviewed and recommendations for future research are discussed

    A pump-probe study of the formation of rubidium molecules by ultrafast photoassociation of ultracold atoms

    An experimental pump-probe study of the photoassociative creation of translationally ultracold rubidium molecules is presented together with numerical simulations of the process. The formation of loosely bound excited-state dimers is observed as a first step towards a fully coherent pump-dump approach to the stabilization of Rb2_2 into its lowest ground vibrational states. The population that contributes to the pump-probe process is characterized and found to be distinct from a background population of pre-associated molecules.Comment: Accepted for publication in Phys.
