    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    Understanding user preferences for interactions with information systems (IS) is of particular interest in the field of Management Support Systems (MSS), where strong user preferences must be taken into account. However, managers’ IS preferences are underresearched in IS research. Thus, IS are not used very often on the management level. To close this gap, we propose a prioritization of MSS use situations, which generalize distinct classes of “similar” user-group preferences and result in non-functional requirements with respect to MSS. This article provides such a prioritization of MSS use situations from a manager perspective by applying the Analytic Hierarchy Process (AHP). Finally, associated design guidelines for a more situated MSS design are presented: (1) Maintain established MSS features for “alone, stationary” use situations. Then, incorporate “mobile” designs step by step. (2) Build a common MSS core with information at one click, while providing additional analyses on an individual basis. (3) Empower the “alone” MSS use case with a one-pager entry point. (4) To support manager “groupwork,” provide them with direct communication and manipulation tools. (5) Run MSS on convertibles with a smartphone as the “satellite” to attract first attention

    Towards Natural Language Processing: An Accounting Case Study

    Natural language processing (NLP) helps to extract data from digitized documents. An interesting use case occurred with the International Financial Reporting Standard 16. Following design science research in information systems, the objective of this article is to lay out design guidelines to automate data extraction from physical leasing contracts by applying NLP. Taking a leading international technology group in the areas of specialty glass and glass-ceramics as our case study, we discuss as follows: (1) The data format of the receiving IS is a sine qua non requirement of the project. Thus, set up the NLP process from the end of the project. (2) Evaluate machine readability of the input documents before preprocessing. List most frequent extraction issues in a manual early in the process. (3) Cluster documents regarding their structure and content beforehand. Then, apply a specifically trained ML algorithm for each cluster. (4) A trainer should guide the machine. Use recall and precision as measures. (5) Design an intuitive user interface by offering both parallel windows and a highlighting feature to offer a quick comparison even for complex contract documents. (6) Project iterations are worthwhile until a stable process is achieved

    Receiving a Triple “E” Rating – Measuring IS Success by Balancing Efficiency, Effectiveness, and Experience Criteria

    Digitalization permeates entire companies. Rethinking information systems (IS) success models developed in the 1980/1990s becomes obvious. But how does one define such a new model and implement it? Taking an SAP S/4HANA migration as our use case, the new triple “E” rating on hand focuses on the IS success of digital technologies by balancing efficiency, effectiveness, and experience criteria. Six takeaways emerge from our case study that should drive future discussion: (1) Evaluate digital technologies by adopting a simple but comprehensive approach including criteria beyond efficiency. (2) Consider diverse IS success perspectives consciously but never lose focus on the end-user. (3) A triad of efficiency, effectiveness, and experience encompasses the challenges of digital technologies. (4) Separate efficiency and effectiveness criteria, however, consider trade-offs between them. (5) The experience category highlights digital awareness and socio-technical changes. (6) Tailor your triple “E” rating individually to your project

    Histopathological and molecular features of late relapses in non-seminomas

    What's known on the subject? and What does the study add? Late relapses from germ cell tumors respond poorly to systemic chemotherapy. This feature has been partly attributed to their presumed derivation from mature teratomas giving rise to secondary non-germ cell histologies that have lost the original germ cell-like behaviour. Our series demonstrates the occurrence of non-germ cell malignancies in the absence of mature teratoma and points towards microsatellite instability and BRAF-mutations as the biologic basis for chemotherapy resistance in about half of the cases. OBJECTIVE center dot To describe the histopathological types of late relapses of germ cell tumours and to search for molecular markers associated with chemotherapy resistance. PATIENTS AND METHODS center dot Samples from 14 patients with late relapse from a non-seminoma were analysed. center dot Archival tumour tissue was gathered at intial diagnosis (n = 9) and at relapse (n = 9), mostly after previous treatment with chemotherapy. center dot In addition to routine histopathology, tumours were analysed for microsatellite instability and screened for mutations in the KRAS and BRAF genes. RESULTS center dot Relapse occurred after 76.5 months (median, range: 24-209 months). center dot The histology in relapse was pure yolk sac tumour in four of the nine patients analysed. center dot Three had a non-germ cell malignancy, one was a mixed non-seminoma and one was a pure mature teratoma. center dot One sample with non-germ cell malignancy originated from a yolk sac tumour without any evidence of teratoma. center dot In four of 12 evaluable patients, high-level microsatellite instability was observed. center dot All patients were KRAS wild-type but four showed a BRAF mutation at V600E. CONCLUSIONS center dot Many late relapses of germ cell tumours show pure yolk sac histology. center dot Non-germ cell malignancies do not necessarily develop from teratoma but can also arise from yolk sac histology. center dot The biology underlying chemotherapy resistance in late relapse could be related to a high incidence of microsatellite instability and BRAF mutation V600E, which were found in half of the patients
