    Oxygen Isotopes as a Tracer of DOM Processes in River-Estuary Systems

    In the biogeochemical study of dissolved organic matter (DOM) in natural waters, stable isotopes are used to provide insight into both the sources of DOM and the processes affecting its alteration. Through the research presented here, oxygen isotopes are incorporated into the study of DOM through the adaptation of a pyrolysis elemental analysis isotope ratio mass spectrometer method, and sample preparation using two-stage ultrafiltration. The application of oxygen isotopes to the study of DOM is demonstrated in two studies. First, natural abundance of δ18O in DOM is explored in the Delaware estuary. Using a two end-member mixing model, DOM oxygen isotopes are compared with carbon isotopes, DOC concentrations and elemental ratios to determine if changes are a result of two water bodies mixing. Isotope values are decoupled from concentration values and indicate that from an isotopic perspective two end member mixing is completed by mid bay. A conceptual model is presented with comparison to the observed data. However, the extension of this model to DOM δ18O values show that additional processes affect the DOM oxygen along the estuary transect. Oxygen isotopes are also used in an enrichment study to assess the sources of DOM oxygen in the photochemical oxidation of organic matter. In a series of laboratory irradiations using York River and Dismal Swamp water amended with either 18O-enriched dissolved oxygen or water, the amount of 18O incorporated into the high molecular weight dissolved organic matter was determined. For both sites, a fraction of the oxygen in DOM was photochemically incorporated from dissolved oxygen, although a larger fraction was incorporated from water. The differences in incorporation between sites are attributable to DOM compositional differences. An oxygen budget is proposed for the observed Dismal Swamp photochemical DOM oxidation. DOM oxygen isotopes provide an additional dimension in the investigation of DOM sources and alteration processes. As DOM δ18O values appear to be affected by processes that do not alter the δ13C values and are not correlated with the water δ18O value, it provides an additional parameter for characterizing high molecular weight DOM

    Comparing Indirect Assessment Results to a Direct Function Focused Preference Assessment to Identify Potential Reinforcers to Increase Task Completion in a School Setting

    In the present study, a concurrent operant analysis (COA) was conducted for one elementary aged student in a publicly funded school to determine if the results yielded an interpretable outcome to identify a potential reinforcer to increase work completion. A simultaneous treatments design was used during the COA to assess choice making behavior. The results of the COA indicated the student’s choice making behavior was maintained by access to attention. An indirect measure of motivation was used as a secondary assessment to identify a potential reinforcer to increase work completion and to determine if the results would align with the outcomes of the COA. Results of the indirect assessment yielded different outcomes and indicated the students work completion behavior was maintained by access to tangibles. Outcomes of the assessments were used to develop a token economy where the student could earn their preferred reinforcers contingent upon work completion. An alternating treatments design was used to systematically compare work completion during sessions where the reinforcer identified by the COA were available to sessions were the reinforcer identified by the indirect assessment were available. Results indicated that working for either identified reinforcer increased work completion to 100% completion

    Beginning January 1, 2015, conventional cage housing for egg-laying hens is scheduled to be prohibited in California. We consider the economic implications of the new hen housing regulations on the California shell egg industry. Our data show that egg production is more costly using noncage systems than conventional cages. Tools, too, are often employed in the interaction with surfaces, since they augment our manipulation capabilities. “Sketch-a-Scratch” is a tool for the multisensory exploration and sketching of surface textures. The user’s actions drive a physical sound model of real materials’ response to interactions such as scraping, rubbing or rolling.     Form of Supplemental Selenium Affects the Expression of mRNA Transcripts Encoding Selenoproteins, and Proteins Regulating Cholesterol Uptake, in the Corpus Luteum of Grazing Beef Cows

    Selenium (Se)-deficient soils necessitate supplementation of this mineral to the diet of forage-grazing cattle. Functionally, Se is incorporated into selenoproteins, some of which function as important antioxidants. We have previously shown that the source of supplemental Se; inorganic (sodium selenite or sodium selenate; ISe), organic (selenomethionine or selenocysteine; OSe) or 1:1 mix of ISe and OSe (MIX), provided to Angus-cross cows affects concentrations of progesterone (P4) during the early luteal phase of the estrous cycle. In this study, we sought to investigate (1) the effect of form of Se on the expression of mRNA encoding selenoproteins in the corpus luteum (CL), and (2) whether this previously reported MIX-induced increase in P4 is the result of increased luteal expression of key steroidogenic transcripts. Following a Se depletion and repletion regimen, 3-year-old, non-lactating, Angus-cross cows were supplemented with either ISe as the industry standard, or MIX for at least 90 days, with the CL then retrieved on Day 7 post-estrus. Half of each CL was used for analysis of targeted mRNA transcripts and the remainder was dissociated for culture with select agonists. The expression of three selenoprotein transcripts and one selenoprotein P receptor was increased (p \u3c 0.05), with an additional five transcripts tending to be increased (p \u3c 0.10), in cows supplemented with MIX versus ISe. In cultures of luteal cells, hCG-induced increases in P4 (p \u3c 0.05) were observed in CL obtained from ISe-supplemented cows. The abundance of steroidogenic transcripts in the CL was not affected by the form of Se, however, the abundance of mRNA encoding 2 key transcripts regulating cholesterol availability (Ldlr and Hsl) was increased (p \u3c 0.05) in MIX-supplemented cows. Overall, the form of Se provided to cows is reported to affect the expression of mRNA encoding several selenoproteins in the CL, and that the form of Se-induced effects on luteal production of P4 appears to be the result of changes in cholesterol availability rather than a direct effect on the expression of steroidogenic enzymes within the CL

    Scaling up quality care for mothers and newborns around the time of birth: an overview of methods and analyses of intervention-specific bottlenecks and solutions.

    BACKGROUND: The Every Newborn Action Plan (ENAP) and Ending Preventable Maternal Mortality targets cannot be achieved without high quality, equitable coverage of interventions at and around the time of birth. This paper provides an overview of the methodology and findings of a nine paper series of in-depth analyses which focus on the specific challenges to scaling up high-impact interventions and improving quality of care for mothers and newborns around the time of birth, including babies born small and sick. METHODS: The bottleneck analysis tool was applied in 12 countries in Africa and Asia as part of the ENAP process. Country workshops engaged technical experts to complete a tool designed to synthesise "bottlenecks" hindering the scale up of maternal-newborn intervention packages across seven health system building blocks. We used quantitative and qualitative methods and literature review to analyse the data and present priority actions relevant to different health system building blocks for skilled birth attendance, emergency obstetric care, antenatal corticosteroids (ACS), basic newborn care, kangaroo mother care (KMC), treatment of neonatal infections and inpatient care of small and sick newborns. RESULTS: The 12 countries included in our analysis account for the majority of global maternal (48%) and newborn (58%) deaths and stillbirths (57%). Our findings confirm previously published results that the interventions with the most perceived bottlenecks are facility-based where rapid emergency care is needed, notably inpatient care of small and sick newborns, ACS, treatment of neonatal infections and KMC. Health systems building blocks with the highest rated bottlenecks varied for different interventions. Attention needs to be paid to the context specific bottlenecks for each intervention to scale up quality care. Crosscutting findings on health information gaps inform two final papers on a roadmap for improvement of coverage data for newborns and indicate the need for leadership for effective audit systems. CONCLUSIONS: Achieving the Sustainable Development Goal targets for ending preventable mortality and provision of universal health coverage will require large-scale approaches to improving quality of care. These analyses inform the development of systematic, targeted approaches to strengthening of health systems, with a focus on overcoming specific bottlenecks for the highest impact interventions

    Clinical parameters and tools for home-based assessment of Parkinson’s disease: results from a Delphi study

To aid the development of a system to evaluate people with PD (PwP) at home (SENSE-PARK system) there was a need to define parameters and tools to be applied in the assessment of 6 domains: gait, bradykinesia/hypokinesia, tremor, sleep, balance and cognition. Objective: To identify relevant parameters and assessment tools of the 6 domains, from the perspective of PwP, caregivers and movement disorders specialists. Methods: A 2-round Delphi study was conducted to select a core of parameters and assessment tools to be applied. This process included PwP, caregivers and movement disorders specialists. Results: Two hundred and thirty-three PwP, caregivers and physicians completed the first round questionnaire, and 50 the second. Results allowed the identification of parameters and assessment tools to be added to the SENSE-PARK system. The most consensual parameters were: Falls and Near Falls; Capability to Perform Activities of Daily Living; Interference with Activities of Daily Living; Capability to Process Tasks; and Capability to Recall and Retrieve Information. The most cited assessment strategies included Walkers; the Evaluation of Performance Doing Fine Motor Movements; Capability to Eat; Assessment of Sleep Quality; Identification of Circumstances and Triggers for Loose of Balance and Memory Assessment. Conclusions: An agreed set of measuring parameters, tests, tools and devices was achieved to be part of a system to evaluate PwP at home.     Antenatal corticosteroids for management of preterm birth: a multi-country analysis of health system bottlenecks and potential solutions.

    BACKGROUND: Preterm birth complications are the leading cause of deaths for children under five years. Antenatal corticosteroids (ACS) are effective at reducing mortality and serious morbidity amongst infants born at 75%) reported very major or significant bottlenecks. Health information systems should include improved gestational age assessment and track ACS coverage, use and outcomes. Better health service delivery requires clarified policy assigning roles by level of care and cadre of provider, dependent on capability to assess gestational age and risk of preterm birth, and the implementation of guidelines with adequate supervision, mentoring and quality improvement systems, including audit and feedback. National essential medicines lists should include dexamethasone for antenatal use, and dexamethasone should be integrated into supply logistics

In particular, mobile and internet based applications with communication capabilities can augment face-to-face mental health service provision. The literature in this field, however, fails to adequately capture the perspectives of the youth mental health workforce regarding utility and acceptability of technology for this purpose. Methods: This paper describes results of in-depth qualitative data drawn from various stakeholders involved in provision of youth mental health services in one Australian rural region. Data were obtained using focus groups and semi-structured interviews with regional youth mental health clinicians, youth workers and support/management staff (n = 4 focus groups; n = 8 interviews) and analysed via inductive thematic analysis. Results: Results question the acceptability of technology to engage clients within youth mental health services. Six main themes were identified: young people in a digital age, personal connection, power and vulnerability, professional identity, individual factors and organisational legitimacy. Conclusions:These findings deepen the understanding of risks and challenges faced when adopting new technologies in mental healthcare.
