    Recognition of the "team medical-care" in medical field.

    チーム医療に対して高い関心が寄せられていることから,チーム医療として認識される具体的な事柄や各職種に対する認識,その利点や職種間の認識の違いなどを明らかにする目的で,岡山県内の病院の看護師,診療放射線技師,臨床検査技師,計652名に対してアンケート調査を行った。有効回答者数470名で,その内訳は看護師207名,診療放射線技師91名,臨床検査技師172名であった。結果はチーム医療と認識される具体的な事柄については,異業種間カンファレンスがチーム医療を促進する事柄として認識されているが,整備・実施されていないことがうかがえた。各職種に対する認識については,職種別・経験年数別にみても,医師,看護師,理学・作業療法士はチーム医療のメンバーとして認識されている割合が高かった。また,経験年数別では,10~14年の人までは統計上ややばらつきがみられるが,経験年数15年以上の人はあらゆる職種をチーム医療のメンバーであると考え,その重要性を認識していることがわかった。チーム医療の利点については,どの職種も共通して「患者中心の医療」と考えている割合が高かった。Recently, the "team medical-care" has been received increasing attention in medical field. Therefore we have to develop a clear understanding of "team medical-care". The purpose of this study is to investigate things regarded as "team medical-care" by co-medical, recognition of other category of license, the advantages of "team medical-care" and recognition against the "team medical-care" by different category of license. Total 652 nurses, clinical radiological technologists and clinical laboratory technologists who are working at 8 hospitals in Okayama prefecture were examined by using questionnaire and 470 members (72.1%) responded to it. They were nurses 207 (44.0%), clinical radiological technologists 91 (19.4%), clinical laboratory technologists 172 (36.6%). Majority of respondents recognized that the medical conference consisted of different category of license promoted the "team medical-care", and ninety percent of participants prompted more frequent medical conference. In both by category of license and years of experience, they regarded doctor, nurse, physical therapist and occupational therapist as the menber of the "team medical-care" at high rate. The respondences which have over 15 years of experience regarded all category of license as the menber of the "team medical-care", while the respondences which have less than 10 to 14 years of experience showed disperision of the recongnition.     Japan Prosthodontic Society position paper on “occlusal discomfort syndrome”

    Purpose: Dentists may encounter patients who present with a sense of a malocclusion but in whom no objective findings can be detected. For the patient who insists that there is occlusal discomfort, in the absence of evidence some dentists elect to perform an occlusal adjustment that not only fails to alleviate symptoms, and may, in fact, exacerbate the discomfort. The patient–dentist relationship is then likely compromised because of a lack of trust. Study selection: In 2011, the Clinical Practice Guidelines Committee of the Japan Prosthodontic Society formulated guidelines for the management of occlusal discomfort. When formulating clinical practice guidelines, the committee bases their recommendations on information derived from scientific evidence. For ‘‘occlusal dysesthesia,’’ however, there are an insufficient number of high-quality papers related to the subject. Therefore, a consensus meeting was convened by the Japan Prosthodontic Society to examine evidence in the Japanese- and English-language literature and generate a multi-center survey to create an appropriate appellation for this condition. Results: As a result of the consensus meeting and survey findings, this condition may be justifiably termed ‘‘occlusal discomfort syndrome.’’ Conclusions: The Japan Prosthodontics Society believes that identification of an umbrella term for occlusal discomfort might serve as a useful guide to formulating clinical practice guidelines in the future.     A Novel Signaling Pathway Mediated by the Nuclear Targeting of C-Terminal Fragments of Mammalian Patched 1

    Background: Patched 1 (Ptc1) is a polytopic receptor protein that is essential for growth and differentiation. Its extracellular domains accept its ligand, Sonic Hedgehog, while the function of its C-terminal intracellular domain is largely obscure. Principal Findings: In this study, we stably expressed human Ptc1 protein in HeLa cells and found that it is subjected to proteolytic cleavage at the C-terminus, resulting in the generation of soluble C-terminal fragments. These fragments accumulated in the nucleus, while the N-terminal region of Ptc1 remained in the cytoplasmic membrane fractions. Using an anti-Ptc1 C-terminal domain antibody, we provide conclusive evidence that C-terminal fragments of endogenous Ptc1 accumulate in the nucleus of C3H10T1/2 cells. Similar nuclear accumulation of endogenous C-terminal fragments was observed not only in C3H10T1/2 cells but also in mouse embryonic primary cells. Importantly, the C-terminal fragments of Ptc1 modulate transcriptional activity of Gli1.     The whole blood transcriptional regulation landscape in 465 COVID-19 infected samples from Japan COVID-19 Task Force

    「コロナ制圧タスクフォース」COVID-19患者由来の血液細胞における遺伝子発現の網羅的解析 --重症度に応じた遺伝子発現の変化には、ヒトゲノム配列の個人差が影響する--. 京都大学プレスリリース. 2022-08-23.Coronavirus disease 2019 (COVID-19) is a recently-emerged infectious disease that has caused millions of deaths, where comprehensive understanding of disease mechanisms is still unestablished. In particular, studies of gene expression dynamics and regulation landscape in COVID-19 infected individuals are limited. Here, we report on a thorough analysis of whole blood RNA-seq data from 465 genotyped samples from the Japan COVID-19 Task Force, including 359 severe and 106 non-severe COVID-19 cases. We discover 1169 putative causal expression quantitative trait loci (eQTLs) including 34 possible colocalizations with biobank fine-mapping results of hematopoietic traits in a Japanese population, 1549 putative causal splice QTLs (sQTLs; e.g. two independent sQTLs at TOR1AIP1), as well as biologically interpretable trans-eQTL examples (e.g., REST and STING1), all fine-mapped at single variant resolution. We perform differential gene expression analysis to elucidate 198 genes with increased expression in severe COVID-19 cases and enriched for innate immune-related functions. Finally, we evaluate the limited but non-zero effect of COVID-19 phenotype on eQTL discovery, and highlight the presence of COVID-19 severity-interaction eQTLs (ieQTLs; e.g., CLEC4C and MYBL2). Our study provides a comprehensive catalog of whole blood regulatory variants in Japanese, as well as a reference for transcriptional landscapes in response to COVID-19 infection

    DOCK2 is involved in the host genetics and biology of severe COVID-19

Here we conducted a genome-wide association study (GWAS) involving 2, 393 cases of COVID-19 in a cohort of Japanese individuals collected during the initial waves of the pandemic, with 3, 289 unaffected controls. We identified a variant on chromosome 5 at 5q35 (rs60200309-A), close to the dedicator of cytokinesis 2 gene (DOCK2), which was associated with severe COVID-19 in patients less than 65 years of age. This risk allele was prevalent in East Asian individuals but rare in Europeans, highlighting the value of genome-wide association studies in non-European populations. RNA-sequencing analysis of 473 bulk peripheral blood samples identified decreased expression of DOCK2 associated with the risk allele in these younger patients. DOCK2 expression was suppressed in patients with severe cases of COVID-19. Single-cell RNA-sequencing analysis (n = 61 individuals) identified cell-type-specific downregulation of DOCK2 and a COVID-19-specific decreasing effect of the risk allele on DOCK2 expression in non-classical monocytes. Immunohistochemistry of lung specimens from patients with severe COVID-19 pneumonia showed suppressed DOCK2 expression. Moreover, inhibition of DOCK2 function with CPYPP increased the severity of pneumonia in a Syrian hamster model of SARS-CoV-2 infection, characterized by weight loss, lung oedema, enhanced viral loads, impaired macrophage recruitment and dysregulated type I interferon responses. We conclude that DOCK2 has an important role in the host immune response to SARS-CoV-2 infection and the development of severe COVID-19, and could be further explored as a potential biomarker and/or therapeutic target

    Introduction : Episiotomy is a common act in obstetrics. A restrictive practice of episiotomy was recommended by learned societies in 2005.Objectives : The objectives of this study were to evaluate the traceability of episiotomy informations in medical records and to study the improvement of the conformity of practices between the audits of 2005, 2007 and 2015.Populations and method : A clinical audit was carried out in 2015 after randomly selecting 100 obstetric records among women admitted to give birth in a type III maternity within the Auvergne-Rhône-Alpes region.Results : The traceability of episiotomy informations significantly improved for the traceability of episiotomy operator's name (p = 0,04), for the anesthesia methods during repair (p = 0,01) and more generally, for types of wines used to repair.Discussion and conclusion: Efforts still need to be made about the traceability of the justification of episiotomy, the completeness of the report and the assessment of pain during and after episiotomy.Introduction : L’épisiotomie est un acte fréquent en obstétrique. Une pratique restrictive de l’épisiotomie a été recommandée par les sociétés savantes en 2005.Objectifs : Les objectifs de cette étude étaient d’évaluer la traçabilité des informations relatives à l’épisiotomie dans les dossiers médicaux et d’étudier l’amélioration de la conformité des pratiques entre les audits de 2005, 2007 et 2015.Populations et méthode : Un audit clinique a été réalisé en 2015 après une sélection aléatoire de 100 dossiers obstétricaux parmi les femmes admises pour accoucher au sein d’une maternité de type III de la région Auvergne - Rhône-Alpes.Résultats : La traçabilité des informations relatives à l’épisiotomie s’est améliorée de manière significative pour la traçabilité du nom de l’opérateur de l’épisiotomie (p=0,04), pour les méthodes d’anesthésie lors de la réfection (p=0,01) et plus globalement, pour les types de fils utilisés pour les sutures.Discussion et conclusion : Des efforts restent à poursuivre concernant la traçabilité de la justification de l’épisiotomie, l’exhaustivité du compte rendu et l’évaluation de la douleur pendant et après l’épisiotomie
