Access to Care for VA Dialysis Patients During Superstorm Sandy.

Introduction: This study examines the use of dialysis services by end-stage renal disease (ESRD) patients following the Superstorm Sandy-related, months-long closure of the New York campus of the US Department of Veterans Affairs (VA) New York Harbor VA Healthcare System (NYHHS, Manhattan VAMC). Methods: Outpatient visits, dialysis care, emergency department visits, and hospitalizations at VA and non-VA facilities for 47 Manhattan VAMC ESRD patients were examined 12 months pre- and post-Sandy using VA administrative and clinical data. Results: The Brooklyn campus of NYHHS, which is within ten miles of Manhattan VAMC, experienced the largest increase in the number of dialysis encounters after the closure. Dialysis encounters for VA patients also increased at non-VA facilities, rising on average, to 106 per month. For the James J Peters Bronx VAMC, the number of total dialysis encounters for Manhattan VAMC patients fluctuated between 39 and 43 per month, dropping to less than 30 after the Manhattan VAMC dialysis unit reopened. Conclusion: Manhattan VAMC ESRD patients used nearby alternate VA sites and non-VA clinics for their care during the closure of the Manhattan VAMC dialysis unit. The VA electronic health records played an important role in ensuring continuity of care for patients who exclusively used VAMC facilities post-Sandy because patient information was immediately accessible at other VA facilities. The events related to Superstorm Sandy highlight the need for dialysis providers to have a comprehensive disaster plan, including nearby alternate care sites that can increase service capacity when a dialysis facility is closed because of a disaster

КОПІНГ-ПОВЕДІНКА: ОСОБИСТІСНІ ІНВАРІАНТИ ПРОФЕСІЙНОГО СТРЕСУ В ЮРИДИЧНІЙ ДІЯЛЬНОСТІ

У статті наведено теоретичне обґрунтування закономірностей взаємозв’язку особистісних інваріант переживання професійного стресу в юридичній діяльності засобом вироблення відповідної копінг-поведінки цих фахівців. Розроблено теоретичну модель особистісно-інваріативної зумовленості копінг-поведінки в переживанні професійного стресу в юридичній діяльності; виокремлено основні функціональні характеристики копінг-поведінки особистості (мінімізація негативної дії обставин, адаптація, підтримка позитивного «Образу Я», підтримка емоційної рівноваги; підтримка взаємодії з іншими людьми); зазначено харак­теристики переживання професійного стресу в юридичній діяльності в ознаках професійної відповідальності й конфліктності. Наголошено, що подолання несприятливих професійних ситуацій залежить від індивідуальних особливостей особистості, які оформлено в комплекс особистісних якостей фахівців (морально-психологічні, комунікативні, емоційно-вольові, пізнавальні та інтелектуальні), які забезпечують психологічний потенціал й ефективність виконання складних професійних завдань юриста. Визначено взаємозв’язок особистісних інваріант у виборі форм подолання стресових ситуацій через характеристики професійно-психологічної готовності (мобілізаційна, орієнтаційна, операційна, вольова, оцінна) та виокремлено складники професійної стійкості через домінанти активності фахівця (пізнання та самопізнання, діяльності, інтеракції). Наведено й проаналізовано дані щодо особистісно-інваріативної зумовленості регуляції професійного стресу в специфічних модусах психологічного копінгу (раціональний та емоційний) і розкрито вплив негативних професійних факторів юридичної діяльності на особистісний статус фахівця та визначення його адаптаційних можливостей у подоланні професійних стресів

“A Garden of Forking Paths”: Reimagining Dreiser (Dreiser’s Path: A View With a Modern Lens. Edited by Irina V. Morozova. Moscow: RSUH Publ., 2023. 206 p.)

The review focuses on the collective monograph Dreiser’s Path: A View with a Modern Lens (2023). The scholarly work reevaluates Dreiser's significance as a classic of American literature and offers a contemporary perspective on his legacy through the lens of modern methodologies such as critical race theory, postcolonial theory, gender theory, etc. The attention of prominent researchers in American literature and culture from Russia, the USA, Canada, and the UK is drawn to Dreiser's contradictory and hermetic view of society and its artistic projection. The monograph reevaluates Dreiser's creative legacy, considering both major and well-known works as well as minor forms. The researchers featured in the monograph highlight the reasons why certain laws operate in Dreiser's artistic world, align Dreiser's value system with the social norms prevailing during his time, typologize the components of Dreiser's texts, and theorize them using contemporary methodological tools. They also juxtapose Dreiser's texts with other artifacts of American culture and demonstrate the evolution of Dreiser's perception from the times of the USSR to the present day. The monograph was conceived in 2021 as a special tribute to Theodore Dreiser to mark his 150 anniversary

Functional equivalence of stem cell and stem cell-derived extracellular vesicle transplantation to repair the irradiated brain.

Cranial radiotherapy, although beneficial for the treatment of brain tumors, inevitably leads to normal tissue damage that can induce unintended neurocognitive complications that are progressive and debilitating. Ionizing radiation exposure has also been shown to compromise the structural integrity of mature neurons throughout the brain, an effect believed to be at least in part responsible for the deterioration of cognitive health. Past work has shown that cranially transplanted human neural stem cells (hNSCs) or their extracellular vesicles (EVs) afforded long-term beneficial effects on many of these cognitive decrements. To provide additional insight into the potential neuroprotective mechanisms of cell-based regenerative strategies, we have analyzed hippocampal neurons for changes in structural integrity and synaptic remodeling after unilateral and bilateral transplantation of hNSCs or EVs derived from those same cells. Interestingly, hNSCs and EVs similarly afforded protection to host neurons, ameliorating the impact of irradiation on dendritic complexity and spine density for neurons present in both the ipsilateral and contralateral hippocampi 1 month following irradiation and transplantation. These morphometric improvements were accompanied by increased levels of glial cell-derived growth factor and significant attenuation of radiation-induced increases in postsynaptic density protein 95 and activated microglia were found ipsi- and contra-lateral to the transplantation sites of the irradiated hippocampus treated with hNSCs or hNSC-derived EVs. These findings document potent far-reaching neuroprotective effects mediated by grafted stem cells or EVs adjacent and distal to the site of transplantation and support their potential as therapeutic agents to counteract the adverse effects of cranial irradiation
