    Valoración del riesgo de caries infantil en un servicio hospitalario de urgencias

    Dental caries is a destruction of the mineralized tissues of the tooth and is the most prevalent chronic disease of children.  Objective: Determine, by a nurse study, the prevalence of high risk of dental caries among children treated at a hospital emergency room. Methods: We applied an adaptation of the form CAT (caries-risk assessment tool) proposed by the American Society of Pediatric Dentistry, consisting of 14 items for children over 7 years and 13 items for children under that age. The test has been applied considering the resorption time of the deciduous teeth in relation to the child's age studied. Results: The form has been completed in 101 children. 72.7% of children under 7 years old and 89.1% of those aged 7 years have one or more high risk factors for childhood caries. We found differences in the results, depending on the origin or ethnicity of the children studied. Conclusions: We determined the prevalence of risk for childhood caries. The Spanish nurses can and should play an active role in promoting children's oral health, actively applying their knowledgeLa caries es una destrucción de los tejidos mineralizados del diente, siendo la enfermedad crónica más prevalente del  niño.  Objetivos: Determinar, mediante un estudio de enfermería, la prevalencia de riesgo de caries en los niños que acuden a un Servicio de Urgencias Hospitalarias.  Método:  Aplicación del formulario validado para la evaluación de riesgo de caries propuesto por la Sociedad Americana de Odontopediatría,  con 14 ítems para niños de más de 7 años y 13 ítems  en menores de esa edad.  La aplicación de dicho formulario ha contemplado la caducidad de la dentición temporal en relación a la edad del niño estudiado.  Resultados: El formulario se ha completado en 101 niños. El 72.7% de los niños menores de 7 años  y el 89.1% de los mayores de 7 años presentan uno o más factores de alto riesgo para caries infantil, existiendo diferencias notables en los resultados, en función del origen de los niños o de su etnia. Los peores resultados los ofrecen los niños gitanos cuyo riesgo de caries es el 100% en grupos de cualquier edad.  Conclusiones: Hemos determinado mediante un  estudio de enfermería, la prevalencia de riesgo  de caries infantil. La Enfermería española puede y debe asumir un papel activo en la promoción de la salud oral infantil, aplicando activamente sus conocimiento

    Effects of a Mediterranean Diet Intervention on Maternal Stress, Well-Being, and Sleep Quality throughout Gestation-The IMPACT-BCN Trial

    Stress and anxiety are frequent occurrences among pregnant women. We aimed to evaluate the effects of a Mediterranean diet intervention during pregnancy on maternal stress, well-being, and sleep quality throughout gestation. In a randomized clinical trial, 1221 high-risk pregnant women were randomly allocated into three groups at 19-23 weeks' gestation: a Mediterranean diet intervention, a Mindfulness-Based Stress Reduction program, or usual care. All women who provided self-reported life-style questionnaires to measure their anxiety (State Trait Anxiety Inventory (STAI), Perceived Stress Scale (PSS)), well-being (WHO Five Well Being Index (WHO-5)), and sleep quality (Pittsburgh sleep quality index (PSQI)) at enrollment and at the end of the intervention (34-36 weeks) were included. In a random subgroup of 106 women, the levels of cortisol and related metabolites were also measured. At the end of the intervention (34-36 weeks), participants in the Mediterranean diet group had significantly lower perceived stress and anxiety scores (PSS mean (SE) 15.9 (0.4) vs. 17.0 (0.4), p = 0.035; STAI-anxiety mean (SE) 13.6 (0.4) vs. 15.8 (0.5), p = 0.004) and better sleep quality (PSQI mean 7.0 ± 0.2 SE vs. 7.9 ± 0.2 SE, p = 0.001) compared to usual care. As compared to usual care, women in the Mediterranean diet group also had a more significant increase in their 24 h urinary cortisone/cortisol ratio during gestation (mean 1.7 ± SE 0.1 vs. 1.3 ± SE 0.1, p < 0.001). A Mediterranean diet intervention during pregnancy is associated with a significant reduction in maternal anxiety and stress, and improvements in sleep quality throughout gestation.     Introduction Despite a growing body of research on the risks of SARS-CoV-2 infection during pregnancy, there is continued controversy given heterogeneity in the quality and design of published studies. Methods We screened ongoing studies in our sequential, prospective meta-analysis. We pooled individual participant data to estimate the absolute and relative risk (RR) of adverse outcomes among pregnant women with SARS-CoV-2 infection, compared with confirmed negative pregnancies. We evaluated the risk of bias using a modified Newcastle-Ottawa Scale. Results We screened 137 studies and included 12 studies in 12 countries involving 13 136 pregnant women. Pregnant women with SARS-CoV-2 infection—as compared with uninfected pregnant women—were at significantly increased risk of maternal mortality (10 studies; n=1490; RR 7.68, 95% CI 1.70 to 34.61); admission to intensive care unit (8 studies; n=6660; RR 3.81, 95% CI 2.03 to 7.17); receiving mechanical ventilation (7 studies; n=4887; RR 15.23, 95% CI 4.32 to 53.71); receiving any critical care (7 studies; n=4735; RR 5.48, 95% CI 2.57 to 11.72); and being diagnosed with pneumonia (6 studies; n=4573; RR 23.46, 95% CI 3.03 to 181.39) and thromboembolic disease (8 studies; n=5146; RR 5.50, 95% CI 1.12 to 27.12). Neonates born to women with SARS-CoV-2 infection were more likely to be admitted to a neonatal care unit after birth (7 studies; n=7637; RR 1.86, 95% CI 1.12 to 3.08); be born preterm (7 studies; n=6233; RR 1.71, 95% CI 1.28 to 2.29) or moderately preterm (7 studies; n=6071; RR 2.92, 95% CI 1.88 to 4.54); and to be born low birth weight (12 studies; n=11 930; RR 1.19, 95% CI 1.02 to 1.40). Infection was not linked to stillbirth. Studies were generally at low or moderate risk of bias. Conclusions This analysis indicates that SARS-CoV-2 infection at any time during pregnancy increases the risk of maternal death, severe maternal morbidities and neonatal morbidity, but not stillbirth or intrauterine growth restriction. As more data become available, we will update these findings per the published protocol

    Intrapartum prediction of emergency delivery due to non-reassuring fetal status at 40 weeks&#8217; gestation in low-risk pregnancies: contribution of Doppler parameters, maternal history, and intrapartum clinical characteristics

    Objective: To assess the added value of Doppler parameters, maternal history, and intrapartum clinical characteristics for the prediction of emergency delivery due to non-reassuring fetal status in low-risk pregnancies. Methods: This was a prospective cohort of low-risk pregnancies undergoing ultrasound assessment at 40&nbsp;weeks\u2019 gestation within 7 days of delivery. The main outcome was emergency cesarean section due to non-reassuring fetal status. The association between Doppler parameters, intrapartum clinical characteristics, and maternal history was performed by logistic regression. The predictive performance of the constructed models was assessed by receiver operating characteristic (ROC) curve analysis and the area under the curve (AUC). Results: From 403 included pregnancies, 18.6% (n = 75) underwent an emergency delivery due to non-reassuring fetal status. The mean gestational age at birth was 40.5 (SD 5) days. Middle cerebral artery pulsatility index (MCA) and cerebroplacental ratio (CPR) were lower in the emergency cesarean section group (1.16 versus 1.30; p &lt;.001, and 1.61 versus 1.78; p =.001, respectively). There was a higher incidence of small-for-gestational-age neonates (20 versus 10.1%; p =.017), lower Apgar scores at the 5th minute (9.7 versus 9.9; p =.006), and NICU admissions (9 versus 3%; p =.016) in the emergency cesarean section group. The base model comprised nulliparity, and the finding of meconium-stained amniotic fluid during labor, achieving an AUC of 66%, while the addition of the MCA Z-score significantly improved the previous model (AUC: 73%; DeLong: p =.008). Conclusions: In low-risk pregnant woman at term, the addition of MCA Z-score to a previous model comprising maternal history and intrapartum clinical findings, significantly improves the prediction of emergency delivery due to non-reassuring fetal status
