    Cluster Analysis using Microgreographic data

    In this paper we try to identify manufacturing and service clusters in Spain, using data from Mercantile Registers of 2006. The proposed methodology partially follows contributions of Duranton and Overman (2005), Brenner (2003 and 2004) and Ellison and Glaser (1997), but departing from them we improve such approaches by several ways. In order to sum up, we can detail our approach and divide it into five stages. Firstly, we divide space into homogeneous cells. Secondly, we create industry specific maps departing from firms’ georeferenced data. Thirdly, we create multiple random industry specific maps under two conditions: i) total number of firms at each industry remains constant and ii) total number of firms at each cell remains constant. Fourthly, we compare the observed spatial distribution of firms with random simulations of such distribution and we check if there is some kind of concentration compared to the random distribution. Fifthly, for each industry we map the areas where the concentration of firms is significantly higher than expected. Previous scheme allows us to identify real clusters (of different shapes and sizes) for all range of manufacturing and service activities and to use this information to design public policies related to such industries.     Effects of a primary care-based multifactorial intervention on physical and cognitive function in frail, elderly individuals : A randomized controlled trial

    Background: Detecting and managing frailty at early stages can prevent disability and other adverse outcomes. The study aim was to evaluate whether a multifactorial intervention program could modify physical and cognitive frailty parameters in elderly individuals. Methods: We conducted a multicenter, randomized, single-blind, parallel-group trial in community-living prefrail/frail elderly individuals in Barcelona. A total of 352 patients, aged ≥65 years old with positive frailty screening, was randomized into two groups to receive a 12-week multidisciplinary intervention or usual care, with concealed allocation. The intervention consisted of: exercise training, intake of hyperproteic nutritional shakes, memory training, and medication review. Main outcome assessments with multivariate analysis were conducted at 3 and 18 months. Results: A total of 347 participants (98.6%) completed the study, mean age 77.3 years, 89 prefrail subjects (25.3%), and 75.3% female (n = 265). Eighteen-month assessments were performed in 76% of the sample. After 3 and 18 months, adjusted means difference between-groups showed significant improvements for the intervention group in all comparisons: Short Physical Performance Battery score improved 1.58 and 1.36 points (p <.001), handgrip strength 2.84 and 2.49 kg (p <.001), functional reach 4.3 and 4.52 cm (p <.001), and number of prescriptions decreased 1.39 and 1.09 (p <.001), respectively. Neurocognitive battery also showed significant improvements across all dimensions at 3 and 18 months. Conclusions: A physical, nutritional, neurocognitive, and pharmacological multifaceted intervention was effective in reversing frailty measures both at short-term and 18 months. Lasting benefits of a multi-intervention program among frail elderly individuals encourage its prioritization

    Limb-girdle muscular dystrophy 1F is caused by a microdeletion in the transportin 3 gene

    In 2001, we reported linkage of an autosomal dominant form of limb-girdle muscular dystrophy, limb-girdle muscular dystrophy 1F, to chromosome 7q32.1-32.2, but the identity of the mutant gene was elusive. Here, using a whole genome sequencing strategy, we identified the causative mutation of limb-girdle muscular dystrophy 1F, a heterozygous single nucleotide deletion (c.2771del) in the termination codon of transportin 3 (TNPO3). This gene is situated within the chromosomal region linked to the disease and encodes a nuclear membrane protein belonging to the importin beta family. TNPO3 transports serine/arginine-rich proteins into the nucleus, and has been identified as a key factor in the HIV-import process into the nucleus. The mutation is predicted to generate a 15-amino acid extension of the C-terminus of the protein, segregates with the clinical phenotype, and is absent in genomic sequence databases and a set of >200 control alleles. In skeletal muscle of affected individuals, expression of the mutant messenger RNA and histological abnormalities of nuclei and TNPO3 indicate altered TNPO3 function. Our results demonstrate that the TNPO3 mutation is the cause of limb-girdle muscular dystrophy 1F, expand our knowledge of the molecular basis of muscular dystrophies and bolster the importance of defects of nuclear envelope proteins as causes of inherited myopathies

    ARTICLE Effectiveness of community-based integrated care in frail COPD patients: a randomised controlled trial

    BACKGROUND: Chronic obstructive pulmonary disease (COPD) generates a high burden on health care, and hospital admissions represent a substantial proportion of the overall costs of the disease. Integrated care (IC) has shown efficacy to reduce hospitalisations in COPD patients at a pilot level. Deployment strategies for IC services require assessment of effectiveness at the health care system level. AIMS: The aim of this study was to explore the effectiveness of a community-based IC service in preventing hospitalisations and emergency department (ED) visits in stable frail COPD patients. METHODS: From April to December 2005, 155 frail community-dwelling COPD patients were randomly allocated either to IC (n = 76, age 73 (8) years, forced expiratory volume during the first second, FEV 1 41(19) % predicted) or usual care (n = 84, age 75(9) years, FEV 1 44 (20) % predicted) and followed up for 12 months. The IC intervention consisted of the following: (a) patient&apos;s empowerment for self-management; (b) an individualised care plan; (c) access to a call centre; and (d) coordination between the levels of care. Thereafter, hospital admissions, ED visits and mortality were monitored for 6 years. RESULTS: IC enhanced self-management (P = 0.02), reduced anxiety-depression (P = 0.001) and improved health-related quality of life (P = 0.02). IC reduced both ED visits (P = 0.02) and mortality (P = 0.03) but not hospital admission. No differences between the two groups were seen after 6 years. CONCLUSION: The intervention improved clinical outcomes including survival and decreased the ED visits, but it did not reduce hospital admissions. The study facilitated the identification of two key requirements for adoption of IC services in the community: appropriate risk stratification of patients, and preparation of the community-based work force

    SARS-CoV-2 susceptibility and COVID-19 disease severity are associated with genetic variants affecting gene expression in a variety of tissues

    Variability in SARS-CoV-2 susceptibility and COVID-19 disease severity between individuals is partly due to genetic factors. Here, we identify 4 genomic loci with suggestive associations for SARS-CoV-2 susceptibility and 19 for COVID-19 disease severity. Four of these 23 loci likely have an ethnicity-specific component. Genome-wide association study (GWAS) signals in 11 loci colocalize with expression quantitative trait loci (eQTLs) associated with the expression of 20 genes in 62 tissues/cell types (range: 1:43 tissues/gene), including lung, brain, heart, muscle, and skin as well as the digestive system and immune system. We perform genetic fine mapping to compute 99% credible SNP sets, which identify 10 GWAS loci that have eight or fewer SNPs in the credible set, including three loci with one single likely causal SNP. Our study suggests that the diverse symptoms and disease severity of COVID-19 observed between individuals is associated with variants across the genome, affecting gene expression levels in a wide variety of tissue types

    Particulate matter exposure during pregnancy is associated with birth weight, but not gestational age, 1962-1992: a cohort study

    <p>Abstract</p> <p>Background</p> <p>Exposure to air pollutants is suggested to adversely affect fetal growth, but the evidence remains inconsistent in relation to specific outcomes and exposure windows.</p> <p>Methods</p> <p>Using birth records from the two major maternity hospitals in Newcastle upon Tyne in northern England between 1961 and 1992, we constructed a database of all births to mothers resident within the city. Weekly black smoke exposure levels from routine data recorded at 20 air pollution monitoring stations were obtained and individual exposures were estimated via a two-stage modeling strategy, incorporating temporally and spatially varying covariates. Regression analyses, including 88,679 births, assessed potential associations between exposure to black smoke and birth weight, gestational age and birth weight standardized for gestational age and sex.</p> <p>Results</p> <p>Significant associations were seen between black smoke and both standardized and unstandardized birth weight, but not for gestational age when adjusted for potential confounders. Not all associations were linear. For an increase in whole pregnancy black smoke exposure, from the 1<sup>st </sup>(7.4 μg/m<sup>3</sup>) to the 25<sup>th </sup>(17.2 μg/m<sup>3</sup>), 50<sup>th </sup>(33.8 μg/m<sup>3</sup>), 75<sup>th </sup>(108.3 μg/m<sup>3</sup>), and 90<sup>th </sup>(180.8 μg/m<sup>3</sup>) percentiles, the adjusted estimated decreases in birth weight were 33 g (SE 1.05), 62 g (1.63), 98 g (2.26) and 109 g (2.44) respectively. A significant interaction was observed between socio-economic deprivation and black smoke on both standardized and unstandardized birth weight with increasing effects of black smoke in reducing birth weight seen with increasing socio-economic disadvantage.</p> <p>Conclusions</p> <p>The findings of this study progress the hypothesis that the association between black smoke and birth weight may be mediated through intrauterine growth restriction. The associations between black smoke and birth weight were of the same order of magnitude as those reported for passive smoking. These findings add to the growing evidence of the harmful effects of air pollution on birth outcomes.</p

    Ichtyofauna diversity and distribution in the Cap de Creus canyon (Northwestern Mediterranean) from video transects

The study of the associated ichtyofauna contribute to improve the knowledge on the ecosystem biodiversity and reinforce the evidence that canyons are relevant diversity hot spots in the Mediterranean. The ichtyofauna of the Cap de Creus canyon (Northwestern Mediterranean) was assessed by means of 28 underwater video transects (more than 45 hours of video records), performed during three oceanographic cruises carried out in October 2005, July 2006 and September 2007 on board of the RV García del Cid (CSIC). Video transects were conducted by using the Remotely Operated Vehicles (ROVs) Falcon and Phantom 4, and the man submersible JAGO (IFM-GEOMAR). Dives were performed from 115 to 350 m depth in both canyon walls (North and South). The use of video transects in order to characterise the ichtyofauna of deep areas represent an innovative method in the study area. The quality of images allows to easily recognise from distinctive characters the fish species, and it is a non-destructive method to carry out ichtyofauna census. A total of 23 fish species were observed in the canyon. Helicolenus dactylopterus (Scorpaenidae), Trisopterus luscus (Gadidae) and Trachurus mediterraneus (Carangidae) were the more abundant species observed. Fish richness and diversity varies among areas and depths throughout the canyon, according with the bathymetric distribution and substrate preferences of each species. A clustering of species was observed depending on the area and the main bottom type and related to the presence of the communities dominated by corals.     Advances in the particle finite element method (PFEM) for solving coupled problems in engineering

    We present some developments in the formulation of the Particle Finite Element Method (PFEM) for analysis of complex coupled problems on fluid and solid mechanics in engineering accounting for fluid-structure interaction and coupled thermal effects, material degradation and surface wear. The PFEM uses an updated Lagrangian description to model the motion of nodes (particles) in both the fluid and the structure domains. Nodes are viewed as material points which can freely move and even separate from the main analysis domain representing, for instance, the effect of water drops. A mesh connects the nodes defining the discretized domain where the governing equations are solved, as in the standard FEM. The necessary stabilization for dealing with the incompressibility of the fluid is introduced via the finite calculus (FIC) method. An incremental iterative scheme for the solution of the non linear transient coupled fluid-structure problem is described. The procedure for modelling frictional contact conditions at fluid-solid and solid-solid interfaces via mesh generation are described. A simple algorithm to treat soil erosion in fluid beds is presented. An straight forward extension of the PFEM to model excavation processes and wear of rock cutting tools is described. Examples of application of the PFEM to solve a wide number of coupled problems in engineering such as the effect of large waves on breakwaters and bridges, the large motions of floating and submerged bodies, bed erosion in open channel flows, the wear of rock cutting tools during excavation and tunneling and the melting, dripping and burning of polymers in fire situations are presented
