    A latent class analysis of parental bipolar disorder: examining associations with offspring psychopathology

    Bipolar disorder (BD) is highly heterogeneous, and course variations are associated with patient outcomes. This diagnostic complexity challenges identification of patients in greatest need of intervention. Additionally, course variations have implications for offspring risk. First, latent class analysis (LCA) categorized parents with BD based on salient illness characteristics: BD type, onset age, polarity of index episode, pole of majority of episodes, rapid cycling, psychosis, anxiety comorbidity, and substance dependence. Fit indices favored three parental classes with some substantively meaningful patterns. Two classes, labeled “Earlier-Onset Bipolar-I” (EO-I) and “Earlier-Onset Bipolar-II” (EO-II), comprised parents who had a mean onset age in mid-adolescence, with EO-I primarily BD-I parents and EO-II entirely BD-II parents. The third class, labeled “Later-Onset BD” (LO) had an average onset age in adulthood. Classes also varied on probability of anxiety comorbidity, substance dependence, psychosis, rapid cycling, and pole of majority of episodes. Second, we examined rates of disorders in offspring (ages 4–33, Mage=13.46) based on parental latent class membership. Differences emerged for offspring anxiety disorders only such that offspring of EO-I and EO-II parents had higher rates, compared to offspring of LO parents, particularly for daughters.     Lower school performance in late chronotypes: underlying factors and mechanisms

    Success at school determines future career opportunities. We described a time-of-day specific disparity in school performance between early and late chronotypes. Several studies showed that students with a late chronotype and short sleep duration obtain lower grades, suggesting that early school starting times handicap their performance. How chronotype, sleep duration, and time of day impact school performance is not clear. At a Dutch high school, we collected 40,890 grades obtained in a variety of school subjects over an entire school year. We found that the strength of the effect of chronotype on grades was similar to that of absenteeism, and that late chronotypes were more often absent. The difference in grades between the earliest 20% and the latest 20% of chronotypes corresponds to a drop from the 55th to 43rd percentile of grades. In academic subjects using mainly fluid cognition (scientific subjects), the correlation with grades and chronotype was significant while subjects relying on crystallised intelligence (humanistic/linguistic) showed no correlation with chronotype. Based on these and previous results, we can expand our earlier findings concerning exam times: students with a late chronotype are at a disadvantage in exams on scientific subjects, and when they are examined early in the day

    Assembly of functional diversity in an oceanic island flora

    Oceanic island floras are well known for their morphological peculiarities and exhibit striking examples of trait evolution1–3. These morphological shifts are commonly attributed to insularity and are thought to be shaped by the biogeographical processes and evolutionary histories of oceanic islands2,4. However, the mechanisms through which biogeography and evolution have shaped the distribution and diversity of plant functional traits remain unclear5. Here we describe the functional trait space of the native flora of an oceanic island (Tenerife, Canary Islands, Spain) using extensive field and laboratory measurements, and relate it to global trade-offs in ecological strategies. We find that the island trait space exhibits a remarkable functional richness but that most plants are concentrated around a functional hotspot dominated by shrubs with a conservative life-history strategy. By dividing the island flora into species groups associated with distinct biogeographical distributions and diversification histories, our results also suggest that colonization via long-distance dispersal and the interplay between inter-island dispersal and archipelago-level speciation processes drive functional divergence and trait space expansion. Contrary to our expectations, speciation via cladogenesis has led to functional convergence, and therefore only contributes marginally to functional diversity by densely packing trait space around shrubs. By combining biogeography, ecology and evolution, our approach opens new avenues for trait-based insights into how dispersal, speciation and persistence shape the assembly of entire native island floras.Fil: Barajas Barbosa, Martha Paola.     Conocimientos, actitudes y prácticas alimentarias en pacientes diagnosticados con vih en tratamiento farmacológico

    Antecedentes. Los hábitos alimentarios de esta población no ha sido materia de una amplia investigación, incluso concibiendo la importancia que tiene el estado nutricional en la evolución de la enfermedad y que este puede retardar el periodo de transición de la misma a etapas sintomáticas o la fase SIDA.Objetivo. Describir los conocimientos, actitudes y prácticas alimentarias de  los pacientes diagnosticados con la infección por VIH, asintomáticos, en fase clínica con tratamiento farmacológico. Material y métodos. Se realizó un estudio exploratorio descriptivo utilizando una encuesta de CAP que diligenciaron voluntariamente 108 pacientes en consulta externa ambulatoria de dos instituciones privadas de Bogotá. Resultados. Se analizaron 108 encuestas en tres componentes. Conocimientos: el 58,7% refirió haber recibido información acerca de cómo debe ser su alimentación; el 32,3% fue suministrado por nutricionista. La fibra es el principal componente que tienen en cuenta a la hora de escoger alimentos (65%) y la proteína la consideran como el nutriente más benéfico (52%). Actitudes: el 61,8% manifestaron que el diagnóstico dificulta la selección y compra de alimentos por falta de información. El apetito, luego de reflexionar sobre el diagnóstico no ha sufrido cambios en el 49,5% de los participantes. Prácticas: el 75% reconoció la importancia de consumir suplementos y complementos; tan solo el 35,7% los consume. Cuando compran alimentos industrializados, el 66,4% verifica la fecha de vencimiento y el rotulado nutricional. Conclusiones. Los alimentos que más consumen son aquellos que hacen parte de la canasta básica para la población colombiana; sus hábitos no difieren significativamente del resto de población. La información en alimentación y nutrición la obtienen de fuentes no profesionales. Se evidencia una regular remisión por el médico general o tratante hacia el profesional nutricionista. Es común la práctica de automedicación y dosificación en el uso y consumo de complementos y suplementos alimenticios

    A fully automated high-throughput workflow for 3D-based chemical screening in human midbrain organoids

    Three-dimensional (3D) culture systems have fueled hopes to bring about the next generation of more physiologically relevant high-throughput screens (HTS). However, current protocols yield either complex but highly heterogeneous aggregates ('organoids') or 3D structures with less physiological relevance ('spheroids'). Here, we present a scalable, HTS-compatible workflow for the automated generation, maintenance, and optical analysis of human midbrain organoids in standard 96-well-plates. The resulting organoids possess a highly homogeneous morphology, size, global gene expression, cellular composition, and structure. They present significant features of the human midbrain and display spontaneous aggregate-wide synchronized neural activity. By automating the entire workflow from generation to analysis, we enhance the intra- and inter-batch reproducibility as demonstrated via RNA sequencing and quantitative whole mount high-content imaging. This allows assessing drug effects at the single-cell level within a complex 3D cell environment in a fully automated HTS workflow

    Oxidative Stress and Vanadium

    Air pollution is a worldwide health problem, and metals are one of the various air pollutants to which living creatures are exposed. The pollution by metals such as: lead, cadmium, manganese, and vanadium have a common mechanism of action: the production of oxidative stress in the cell. Oxidative stress favors the production of free radicals, which damage biomolecules such as: DNA, proteins, lipids, and carbohydrates; these free radicals produce changes that are observed in different organs and systems. Vanadium is a transition element delivered into the atmosphere by the combustion of fossil fuels as oxides and adhered to the PM enters into the respiratory system, then crosses the alveolar wall and enters into the systemic circulation. In this chapter, we will review the oxidative stress induced by vanadium—as a common mechanism of metal pollutants—; in addition, we will review the protective effect of the antioxidants (carnosine and ascorbate)

    On the notion of home and the goals of palliative care

    The notion of home is well known from our everyday experience, and plays a crucial role in all kinds of narratives about human life, but is hardly ever systematically dealt with in the philosophy of medicine and health care. This paper is based upon the intuitively positive connotation of the term “home.” By metaphorically describing the goal of palliative care as “the patient’s coming home,” it wants to contribute to a medical humanities approach of medicine. It is argued that this metaphor can enrich our understanding of the goals of palliative care and its proper objectives. Four interpretations of “home” and “coming home” are explored: (1) one’s own house or homelike environment, (2) one’s own body, (3) the psychosocial environment, and (4) the spiritual dimension, in particular, the origin of human existence. Thinking in terms of coming home implies a normative point of view. It represents central human values and refers not only to the medical-technical and care aspects of health care, but also to the moral context
