    TRA of DigiMon components

    The DigiMon project aims to develop an affordable, flexible, societally embedded and smart monitoring system for industrial scale subsurface CO2 storage. For this purpose, the DigiMon system is to combine various types of measurements in integrated workflows. In this report, we describe the process of conducting the Technology Readiness Assessment (TRA) of various measurement techniques. We report on the identification, description and assessment of these measurement techniques as Critical Technology Elements (CTEs) being part of the DigiMon system

    Rationing tests for drug-resistant tuberculosis - who are we prepared to miss?

    BACKGROUND: Early identification of patients with drug-resistant tuberculosis (DR-TB) increases the likelihood of treatment success and interrupts transmission. Resource-constrained settings use risk profiling to ration the use of drug susceptibility testing (DST). Nevertheless, no studies have yet quantified how many patients with DR-TB this strategy will miss. METHODS: A total of 1,545 subjects, who presented to Lima health centres with possible TB symptoms, completed a clinic-epidemiological questionnaire and provided sputum samples for TB culture and DST. The proportion of drug resistance in this population was calculated and the data was analysed to demonstrate the effect of rationing tests to patients with multidrug-resistant TB (MDR-TB) risk factors on the number of tests needed and corresponding proportion of missed patients with DR-TB. RESULTS: Overall, 147/1,545 (9.5%) subjects had culture-positive TB, of which 32 (21.8%) had DR-TB (MDR, 13.6%; isoniazid mono-resistant, 7.5%; rifampicin mono-resistant, 0.7%). A total of 553 subjects (35.8%) reported one or more MDR-TB risk factors; of these, 506 (91.5%; 95% CI, 88.9-93.7%) did not have TB, 32/553 (5.8%; 95% CI, 3.4-8.1%) had drug-susceptible TB, and only 15/553 (2.7%; 95% CI, 1.5-4.4%) had DR-TB. Rationing DST to those with an MDR-TB risk factor would have missed more than half of the DR-TB population (17/32, 53.2%; 95% CI, 34.7-70.9). CONCLUSIONS: Rationing DST based on known MDR-TB risk factors misses an unacceptable proportion of patients with drug-resistance in settings with ongoing DR-TB transmission. Investment in diagnostic services to allow universal DST for people with presumptive TB should be a high priority

    Critical technology elements (WP1)

    The overall objective of the DigiMon project is to “accelerate the implementation of CCS by developing and demonstrating an affordable, flexible, societally embedded and smart Digital Monitoring early warning system”, for monitoring any CO2 storage reservoir and subsurface barrier system. Within the project the objective of WP1 was to develop individual technologies, data acquisition, analysis techniques and workflows in preparation for inclusion in the DigiMon system. The technologies and data processing techniques developed as part of WP1 include distributed fibre-optic sensing (DFOS) for seismic surveys and chemical sensing, 4D gravity and seafloor deformation measurements, a new seismic source and seismic monitoring survey design. For these technologies the key targets for WP1 were • Develop individual components of the system to raise individual technology readiness levels (TRLs), • Validate and optimise processing software for individual system components, • Develop an effective Distributed Acoustic Sensing (DAS) data interpretation workflow. This work was performed with the expected outcomes of • Raising the DAS TRL for passive seismic monitoring, • An assessment the feasibility of using Distributed Chemical Sensing (DCS) for CO2 detection, • Reducing the cost of 4D gravity and seafloor deformation measurements

    Evaluation of a psychoeducational intervention for adolescents with inflammatory bowel disease

    OBJECTIVES: Inflammatory bowel disease (IBD), comprising Crohn's disease, ulcerative colitis, and indeterminate colitis, often has its onset in adolescence. The aim of this study was to evaluate whether a psychoeducational group intervention (aiming to enhance information seeking and giving about the disease, relaxation, social competence, and positive thinking) can strengthen the coping efforts of adolescents with IBD and have a positive effect on their Health-Related Quality of Life (HRQoL). METHODS: Adolescent IBD patients from the Emma Children's Hospital AMC and adolescent members of the Crohn and Ulcerative Colitis Association in The Netherlands, were invited to participate in The intervention study. Using reliable and valid self-report instruments the adolescent's coping styles, feelings of competence, and HRQoL were assessed before and 6-8 months after the intervention. The parents were asked to fill in the Child Behavior Check List. Linear regression analyses were performed to test whether group participation was predictive of the outcome measures while correcting for the first measurement occasion and sex. RESULTS AND CONCLUSION: Forty patients responded positively to invitation to the intervention. Eighteen adolescents, however, lived too far away to attend and served as a control group. Twenty-two children were enrolled and attended in groups of four to six children in six group sessions, supervised by two psychologists. The intervention seemed to have a positive effect on: coping (predictive control, P<0.01), feelings of competence (global self-worth, P<0.05 and physical appearance, P<0.01), and HRQoL (body image, P<0.05). These results give good reason to continue this intervention study with a larger population. © 2009 Lippincott Williams & Wilkins, Inc

    The Wide-field Infrared Survey Explorer (WISE): Mission Description and Initial On-orbit Performance

    The all sky surveys done by the Palomar Observatory Schmidt, the European Southern Observatory Schmidt, and the United Kingdom Schmidt, the InfraRed Astronomical Satellite and the 2 Micron All Sky Survey have proven to be extremely useful tools for astronomy with value that lasts for decades. The Wide-field Infrared Survey Explorer is mapping the whole sky following its launch on 14 December 2009. WISE began surveying the sky on 14 Jan 2010 and completed its first full coverage of the sky on July 17. The survey will continue to cover the sky a second time until the cryogen is exhausted (anticipated in November 2010). WISE is achieving 5 sigma point source sensitivities better than 0.08, 0.11, 1 and 6 mJy in unconfused regions on the ecliptic in bands centered at wavelengths of 3.4, 4.6, 12 and 22 microns. Sensitivity improves toward the ecliptic poles due to denser coverage and lower zodiacal background. The angular resolution is 6.1, 6.4, 6.5 and 12.0 arc-seconds at 3.4, 4.6, 12 and 22 microns, and the astrometric precision for high SNR sources is better than 0.15 arc-seconds.Comment: 22 pages with 19 included figures.     Extension and stress during continental breakup: seismic anisotropy of the crust in Northern Afar

    Studies that attempt to simulate continental rifting and subsequent breakup require detailed knowledge of crustal stresses, however observational constraints from continental rifts are lacking. In addition, a knowledge of the stress field around active volcanoes can be used to detect sub-surface changes to the volcanic system. Here we use shear wave splitting to measure the seismic anisotropy of the crust in Northern Afar, a region of active, magma-rich continental breakup. We combine shear wave splitting tomography with modelling of gravitational and magmatic induced stresses to propose a model for crustal stress and strain across the rift. Results show that at the Ethiopian Plateau, seismic anisotropy is consistently oriented N–S. Seismic anisotropy within the rift is generally oriented NNW–SSE, with the exception of regions north and south of the Danakil Depression where seismic anisotropy is rift-perpendicular. These results suggest that the crust at the rift axis is characterized by rift-aligned structures and melt inclusions, consistent with a focusing of tectonic extension at the rift axis. In contrast, we show that at regions within the rift where extension rate is minimal the seismic anisotropy is best explained by the gravitationally induced stress field originating from variations in crustal thickness. Seismic anisotropy away from the rift is controlled by a combination of inherited crustal structures and gravitationally induced extension whereas at the Dabbahu region we show that the stress field changes orientation in response to magmatic intrusions. Our proposed model provides a benchmark of crustal stress in Northern Afar which will aid the monitoring of volcanic hazard. In addition we show that gravitational forces play a key role in measurements of seismic anisotropy, and must be considered in future studies. We demonstrate that during the final stages of continental rifting the stress field at the rift axis is primarily controlled by tectonic extension, but that gravitational forces and magmatic intrusions can play a key role in the orientation of the stress field

    Psychosocial Treatment of Children in Foster Care: A Review

    A substantial number of children in foster care exhibit psychiatric difficulties. Recent epidemiologi-cal and historical trends in foster care, clinical findings about the adjustment of children in foster care, and adult outcomes are reviewed, followed by a description of current approaches to treatment and extant empirical support. Available interventions for these children can be categorized as either symptom-focused or systemic, with empirical support for specific methods ranging from scant to substantial. Even with treatment, behavioral and emotional problems often persist into adulthood, resulting in poor functional outcomes. We suggest that self-regulation may be an important mediat-ing factor in the appearance of emotional and behavioral disturbance in these children

    A cognitive behavioral based group intervention for children with a chronic illness and their parents: a multicentre randomized controlled trial

    <p>Abstract</p> <p>Background</p> <p>Coping with a chronic illness (CI) challenges children's psychosocial functioning and wellbeing. Cognitive-behavioral intervention programs that focus on teaching the active use of coping strategies may prevent children with CI from developing psychosocial problems. Involvement of parents in the intervention program may enhance the use of learned coping strategies in daily life, especially on the long-term. The primary aim of the present study is to examine the effectiveness of a cognitive behavioral based group intervention (called 'Op Koers') <abbrgrp><abbr bid="B1">1</abbr></abbrgrp> for children with CI and of a parallel intervention for their parents. A secondary objective is to investigate why and for whom this intervention works, in order to understand the underlying mechanisms of the intervention effect.</p> <p>Methods/design</p> <p>This study is a multicentre randomized controlled trial. Participants are children (8 to 18 years of age) with a chronic illness, and their parents, recruited from seven participating hospitals in the Netherlands. Participants are randomly allocated to two intervention groups (the child intervention group and the child intervention combined with a parent program) and a wait-list control group. Primary outcomes are child psychosocial functioning, wellbeing and child disease related coping skills. Secondary outcomes are child quality of life, child general coping skills, child self-perception, parental stress, quality of parent-child interaction, and parental perceived vulnerability. Outcomes are evaluated at baseline, after 6 weeks of treatment, and at a 6 and 12-month follow-up period. The analyses will be performed on the basis of an intention-to-treat population.</p> <p>Discussion</p> <p>This study evaluates the effectiveness of a group intervention improving psychosocial functioning in children with CI and their parents. If proven effective, the intervention will be implemented in clinical practice.     Comparative Transmissibility of SARS-CoV-2 Variants Delta and Alpha in New England, USA

    The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) Delta variant quickly rose to dominance in mid-2021, displacing other variants, including Alpha. Studies using data from the United Kingdom and India estimated that Delta was 40-80% more transmissible than Alpha, allowing Delta to become the globally dominant variant. However, it was unclear if the ostensible difference in relative transmissibility was due mostly to innate properties of Delta\u27s infectiousness or differences in the study populations. To investigate, we formed a partnership with SARS-CoV-2 genomic surveillance programs from all six New England US states. By comparing logistic growth rates, we found that Delta emerged 37-163% faster than Alpha in early 2021 (37% Massachusetts, 75% New Hampshire, 95% Maine, 98% Rhode Island, 151% Connecticut, and 163% Vermont). We next computed variant-specific effective reproductive numbers and estimated that Delta was 58-120% more transmissible than Alpha across New England (58% New Hampshire, 68% Massachusetts, 76% Connecticut, 85% Rhode Island, 98% Maine, and 120% Vermont). Finally, using RT-PCR data, we estimated that Delta infections generate on average ∼6 times more viral RNA copies per mL than Alpha infections. Overall, our evidence indicates that Delta\u27s enhanced transmissibility could be attributed to its innate ability to increase infectiousness, but its epidemiological dynamics may vary depending on the underlying immunity and behavior of distinct populations
