    Lentes de contato corneoesclerais adaptadas em córneas irregulares após cirurgia LASIK

    Purpose: We aimed to describe the outcomes of corneoscleral contact lens fitting with multi-aspherical geometry designs in patients with irregular corneas after laser-assisted in situ keratomileusis (LASIK). Methods: This was a retrospective series of 18 patients (age, 26-38 years) selected from an eye clinic database. Participants were required to have unsatisfactory visual acuity with their current contact lenses or spectacles after LASIK for myopia correction. All patients were fitted with corneoscleral contact lenses designed to correct corneal surface irregularities. A diagnostic trial set was used for fitting, and assessments were performed according to a standardized methodology. Subjective comfort, visual acuity, central corneal thickness, endothelial cell count, and corneal-compensated intraocular pressure were evaluated. The follow-up period was one year. Results: Contact lens use was discontinued in 3 patients, thus leaving 24 eyes from 8 females and 7 males for analysis. The fitting characteristics were optimal in terms of lens position and movement. Statistically significant improvements were found in the best spectacle-corrected vision from before fitting to the visual acuity after fitting (p<0.001). Moreover, the patients reported high subjective comfort ratings and usage times (12.98 ± 2.3 hours/day). After one year of wear, visual acuity, subjective comfort, and usage time were maintained. No statistically significant adverse changes developed in the corneas over this period. Conclusion: Corneoscleral contact lenses with a multi-aspherical geometry design provide optimal visual acuity, improved comfort, and prolonged usage times in patients with irregular corneas after LASIK.Objetivo: Descrever os resultados do ajuste de lentes de contato corneoesclerais com geometria multiasférica em pacientes com córneas irregulares após cirurgia de ceratomileuse in situ assistida por laser (LASIK). Métodos: Esta foi uma série retrospectiva de 18 pacientes (idade, 26-38 anos) selecionados a partir de um banco de dados de uma clínica oftalmológica. Os participantes foram escolhidos ao manifestaram acuidade visual insatisfatória com suas lentes de contato ou óculos atuais depois de terem sido submetidos à LASIK para correção de miopia. Todos os pacientes receberam lentes de contato corneoesclerais projetadas para corrigir irregularidades na superficie da córnea. Um conjunto diagnóstico de prova foi usado para a adaptação e as avaliações foram feitas de acordo com uma metodologia padronizada. O conforto subjetivo, a acuidade visual, a espessura central da córnea, a contagem de células endoteliais e a pressão intraocular compensada da córnea foram avaliados. O período de acompanhamento foi de um ano. Resultados: O uso de lentes de contato foi descontinuado em 3 pacientes, deixando 24 olhos de 8 mulheres e 7 homens para análise. As características de adaptação foram ótimas em termos de posição e movimento da lente. Melhorias estatisticamente significativas foram encontradas na melhoria da visão corrigida por óculos antes de se ajustar a acuidade visual após a adaptação (p<0,001). Além disso, os pacientes relataram altos índices subjetivos de conforto e tempo de uso (12,98 ± 2,3 horas/dia). Após um ano de uso, a acuidade visual, o conforto subjetivo e o tempo de uso foram mantidos. Não houve alterações adversas estatisticamente significativas nas córneas ao longo deste período. Conclusão: As lentes de contato corneoesclerais com projeto geométrico multiasférico proporcionam acuidade visual ideal, maior conforto e tempo de uso prolongado em pacientes com córneas irregulares após o LASIK.Medicin

    The Efficacy and Patient Satisfaction with a Mini-Scleral Lens after penetrating Keratoplasty

    Background: To analyse the efficacy and patient satisfaction of fitting a mini-scleral lens (SL) after keratoplasty surgery in patients unsatisfied with their visual outcomes. Material and Methods: A retrospective chart review of 22 consecutive patients was performed between 2018 and 2019. Demographic data, indications for keratoplasty, ocular parameters, and visual results were evaluated. In addition, subjective visual quality (SVQ) and comfort, complications, daily wear time, and the reason for discontinuing SL wear during the 6-month follow-up were analysed. Results: Twenty-two eyes of 22 patients (6 females and 16 males; mean age 48.82 ± 17.19 years) were assessed. Keratoconus was the main indication for keratoplasty (9 eyes, 40.9%), followed by corneal opacities (5 eyes), and Fuchs endothelial dystrophy (4 eyes). High refractive anisometropia was found in 14 eyes (63.6%) and 6 eyes presented significant irregular astigmatism. LogMAR visual acuity improved significantly with SL when compared with the best spectacle-correction (mean, 0.12 ± 0.12 vs 0.52 ± 0.33, respectively; p < 0.01). Six patients discontinued SL wear (27.3%). Among the other 16 patients, the mean daily wear time was 11.47 ± 1.52 h; of these, 14 (87.5%) were successfully fitted and enjoyed favourable SVQ and comfort. No significant complications were found during the follow-up period. Conclusions: Mini-scleral lenses may be a safe and effective alternative for treating complicated eyes after keratoplasty; they may provide optimal visual rehabilitation and/or restore binocular vision with significant efficacy and patient satisfaction

    Obstetric–neonatal care during birth and postpartum in symptomatic and asymptomatic women infected with SARS-CoV-2: a retrospective multicenter study

    This study analyses the obstetric–neonatal outcomes of women in labour with symptomatic and asymptomatic COVID-19. A retrospective, multicenter, observational study was carried out between 1 March 2020 and 28 February 2021 in eight public hospitals in the Valencian community (Spain). The chi-squared test compared the obstetric–neonatal outcomes and general care for symptomatic and asymptomatic women. In total, 11,883 births were assisted in participating centers, with 10.9 per 1000 maternities (n = 130) infected with SARS-CoV-2. The 20.8% were symptomatic and had more complications both upon admission (p = 0.042) and during puerperium (p = 0.042), as well as transfer to the intensive care unit (ICU). The percentage of admission to the Neonatal Intensive Care Unit (NICU) was greater among offspring of symptomatic women compared to infants born of asymptomatic women (p < 0.001). Compared with asymptomatic women, those with symptoms underwent less labour companionship (p = 0.028), less early skin-to-skin contact (p = 0.029) and greater mother–infant separation (p = 0.005). The overall maternal mortality rate was 0.8%. No vertical transmission was recorded. In conclusion, symptomatic infected women are at increased risk of lack of labour companionship, mother–infant separation, and admission to the ICU, as well as to have preterm births and for NICU admissions

    MEDIDAS PARA LA CONSERVACIÓN DE LA BIODIVERSIDAD DE LOS POLINIZADORES SILVESTRES EN LA PENÍNSULA IBÉRICA

    Los científicos y científicas abajo firmantes consideramos muy importante hacer llegar a instituciones, agricultores/as y a la sociedad en general, la necesidad urgente de implementar conjuntamente medidas y cambios que consigan frenar el declive de los polinizadores silvestres ocasionado por la actividad humana. Durante la última década, son múltiples los estudios que alertan de la creciente desaparición de los polinizadores por todo el mundo, en concreto de las abejas silvestres, (Biesmeijer et al. 2006; Potts et al. 2010; Burkle et al. 2013), y de las graves consecuencias que su déficit podría provocar sobre la biodiversidad global (Biesmeijer et al. 2006; Burkle et al. 2013; Lundgren et al. 2016) y sobre la producción agrícola (Aizen y Harder 2009; Garibaldi et al. 2013). No debemos olvidar que la península Ibérica es, por su condición mediterránea y su proximidad al continente africano, uno de los lugares con mayor diversidad de polinizadores de la Unión Europea y, en concreto, una de las zonas con mayor diversidad de abejas del mundo (Michener 2007; Nieto et al. 2014). Hasta el momento, el número de especies de abejas en España presentes en la zona íbero-balear es algo superior a 1.100, cifra a la que cabe añadir algunas especies exclusivas de Portugal más los nuevos hallazgos de los últimos años (Ortiz-Sánchez 2011). Esta gran diversidad de abejas y polinizadores en general está asociada al gran número de especies de plantas con flor presentes en la península Ibérica, alrededor de las 7.000 especies (Aguado Martín et al. 2015). En cuanto al número de mariposas y polillas (lepidópteros) se estima que existen en la península Ibérica unas 5.000 especies (Stefanescu et al. 2018). Más difícil es estimar el número exacto de especies de escarabajos florícolas (coleópteros polinizadores), pero atendiendo a la riqueza de los principales géneros podemos estimar su número en más de 750 (Stefanescu et al. 2018). Somos conscientes de que, a pesar del desarrollo explosivo de los últimos 10 años de la investigación en ecología y gestión de la polinización de los cultivos por insectos silvestres, hoy en día son numerosas las lagunas de conocimiento básico y aplicado sobre el estado de conservación de los insectos polinizadores silvestres. Y es, bajo esta premisa, que presentamos este trabajo de revisión de la literatura científica sobre insectos polinizadores desde principios del siglo XX hasta ahora, cuyo resultado ha quedado plasmado en una lista, no exhaustiva, de los aspectos que consideramos fundamentales para el desarrollo y debate de esta relevante cuestió

    Is Early Initiation of Maternal Lactation a Significant Determinant for Continuing Exclusive Breastfeeding up to 6 Months?

    Background: The World Health Organization (WHO) recommends early initiation of breastfeeding (EIBF) within the first hour after birth. However, certain perinatal factors, namely caesarean section, may prevent this goal from being achieved. The aim of our study was to examine the relationship between EIBF (maternal lactation in the first hours and degree of latching before hospital discharge) and the maintenance of exclusive breastfeeding (MBF) up to the recommended 6 months of age (as advocated by the WHO). Methods: This observational, retrospective cohort study included a random sample of all births between 2018 and 2019, characterising the moment of breastfeeding initiation after birth and the infant’s level of breast latch (measured by LATCH assessment tool) prior to hospital discharge. Data were collected from electronic medical records and from follow-up health checks of infants up to 6 months postpartum. Results: We included 342 women and their newborns. EIBF occurred most often after vaginal (p p = 0.002). LATCH score <9 points was associated with a 1.4-fold relative risk of abandoning MBF (95%CI: 1.2–1.7) compared with a score of 9–10 points. Conclusions: Although we were unable to find a significant association between EIBF in the first 2 h after birth and MBF at 6 months postpartum, low LATCH scores prior to discharge were associated with low MBF, indicating the importance of reinforcing the education and preparation efforts of mothers in the first days after delivery, prior to the establishment of an infant feeding routine upon returning home

    Language fMRI has been used in the presurgical evaluation of drug-resistant temporal lobe epilepsy patients. Previous studies have demonstrated that left temporal lobe epilepsy (LTLE) patients with atypical language lateralization are at lower risk of postsurgical verbal memory decline, hypothesizing co-occurrence of verbal memory and language reorganization presurgically. Furthermore, it has been proposed that the recruitment of right frontal language-related areas is associated with the preservation of verbal memory performance in these patients. However, less is known about the correlation between these functions specifically in LTLE patients with left language dominance, although they are more prone to postsurgical verbal memory decline. The aim of the present study was to investigate whether the relationship between verbal memory scores and frontal language activation is also observed in LTLE patients with typical language dominance. Eighteen healthy controls, 12 right temporal lobe epilepsy patients and 12 LTLE patients with typical language distribution as assessed by an fMRI verbal fluency task were selected. Verbal memory scores were obtained from the patients’ neuropsychological presurgical evaluation. Our results showed a positive correlation between verbal recall and activation of bilateral inferior frontal areas in LTLE patients. These results support the hypothesis of a link between language representation in inferior frontal areas and hippocampal functioning, and indicate that both hemispheres are related to the preservation of verbal memory in patients with hippocampal damage and typical language dominance
