Patient Choice in Acute Care
Consumer healthcare information plays a critical \ role in informing patients who participate in or make healthcare \ decisions for themselves without direct supervision of a healthcare \ professional. One such example is the choice of facility for \ acute care, prototypically between a fully equipped emergency \ care department (ED) at a hospital and a more convenient \ but less capable urgent care (UC) or retail clinic. We model \ a strategic patient making this decision taking into account the \ limited medical information and convenience factors that affect \ the patient’s decision. This model is then used to inform the \ pricing decision made by the manager of the UC. We show that \ a separating equilibrium, in which all patients self-triaged as noncritical \ choose to go to the UC first, dominates pooling equilibria \ for moderate error rates in self-triage. We analyze the separating \ equilibrium to examine the effect of consumer health information \ (CHI) systems, and show that as the quality of the CHI decreases \ and the error rates go up, the co-pay for an UC decreases, the \ facility is smaller, and makes less profit
Contribution of copy number variants to schizophrenia from a genome-wide study of 41,321 subjects
Copy number variants (CNVs) have been strongly implicated in the genetic etiology of schizophrenia (SCZ). However, genome-wide investigation of the contribution of CNV to risk has been hampered by limited sample sizes. We sought to address this obstacle by applying a centralized analysis pipeline to a SCZ cohort of 21,094 cases and 20,227 controls. A global enrichment of CNV burden was observed in cases (OR=1.11, P=5.7×10−15), which persisted after excluding loci implicated in previous studies (OR=1.07, P=1.7 ×10−6). CNV burden was enriched for genes associated with synaptic function (OR = 1.68, P = 2.8 ×10−11) and neurobehavioral phenotypes in mouse (OR = 1.18, P= 7.3 ×10−5). Genome-wide significant evidence was obtained for eight loci, including 1q21.1, 2p16.3 (NRXN1), 3q29, 7q11.2, 15q13.3, distal 16p11.2, proximal 16p11.2 and 22q11.2. Suggestive support was found for eight additional candidate susceptibility and protective loci, which consisted predominantly of CNVs mediated by non-allelic homologous recombination
Dissecting the Shared Genetic Architecture of Suicide Attempt, Psychiatric Disorders, and Known Risk Factors
Background Suicide is a leading cause of death worldwide, and nonfatal suicide attempts, which occur far more frequently, are a major source of disability and social and economic burden. Both have substantial genetic etiology, which is partially shared and partially distinct from that of related psychiatric disorders. Methods We conducted a genome-wide association study (GWAS) of 29,782 suicide attempt (SA) cases and 519,961 controls in the International Suicide Genetics Consortium (ISGC). The GWAS of SA was conditioned on psychiatric disorders using GWAS summary statistics via multitrait-based conditional and joint analysis, to remove genetic effects on SA mediated by psychiatric disorders. We investigated the shared and divergent genetic architectures of SA, psychiatric disorders, and other known risk factors. Results Two loci reached genome-wide significance for SA: the major histocompatibility complex and an intergenic locus on chromosome 7, the latter of which remained associated with SA after conditioning on psychiatric disorders and replicated in an independent cohort from the Million Veteran Program. This locus has been implicated in risk-taking behavior, smoking, and insomnia. SA showed strong genetic correlation with psychiatric disorders, particularly major depression, and also with smoking, pain, risk-taking behavior, sleep disturbances, lower educational attainment, reproductive traits, lower socioeconomic status, and poorer general health. After conditioning on psychiatric disorders, the genetic correlations between SA and psychiatric disorders decreased, whereas those with nonpsychiatric traits remained largely unchanged. Conclusions Our results identify a risk locus that contributes more strongly to SA than other phenotypes and suggest a shared underlying biology between SA and known risk factors that is not mediated by psychiatric disorders.Peer reviewe
Rediscovering the value of families for psychiatric genetics research
As it is likely that both common and rare genetic variation are important for complex disease risk, studies that examine the full range of the allelic frequency distribution should be utilized to dissect the genetic influences on mental illness. The rate limiting factor for inferring an association between a variant and a phenotype is inevitably the total number of copies of the minor allele captured in the studied sample. For rare variation, with minor allele frequencies of 0.5% or less, very large samples of unrelated individuals are necessary to unambiguously associate a locus with an illness. Unfortunately, such large samples are often cost prohibitive. However, by using alternative analytic strategies and studying related individuals, particularly those from large multiplex families, it is possible to reduce the required sample size while maintaining statistical power. We contend that using whole genome sequence (WGS) in extended pedigrees provides a cost-effective strategy for psychiatric gene mapping that complements common variant approaches and WGS in unrelated individuals. This was our impetus for forming the “Pedigree-Based Whole Genome Sequencing of Affective and Psychotic Disorders” consortium. In this review, we provide a rationale for the use of WGS with pedigrees in modern psychiatric genetics research. We begin with a focused review of the current literature, followed by a short history of family-based research in psychiatry. Next, we describe several advantages of pedigrees for WGS research, including power estimates, methods for studying the environment, and endophenotypes. Schizophrenia-associated somatic copy-number variants from 12,834 cases reveal recurrent NRXN1 and ABCB11 disruptions
While germline copy-number variants (CNVs) contribute to schizophrenia (SCZ) risk, the contribution of somatic CNVs (sCNVs)—present in some but not all cells—remains unknown. We identified sCNVs using blood-derived genotype arrays from 12,834 SCZ cases and 11,648 controls, filtering sCNVs at loci recurrently mutated in clonal blood disorders. Likely early-developmental sCNVs were more common in cases (0.91%) than controls (0.51%, p = 2.68e−4), with recurrent somatic deletions of exons 1–5 of the NRXN1 gene in five SCZ cases. Hi-C maps revealed ectopic, allele-specific loops forming between a potential cryptic promoter and non-coding cis-regulatory elements upon 5′ deletions in NRXN1. We also observed recurrent intragenic deletions of ABCB11, encoding a transporter implicated in anti-psychotic response, in five treatment-resistant SCZ cases and showed that ABCB11 is specifically enriched in neurons forming mesocortical and mesolimbic dopaminergic projections. Try It, You Will Like It—Does Consumer Learning Lead to Competitive Price Promotions?
There is strong evidence that consumers learn from their brand consumption experiences and continuously update their brand purchase probabilities. However, in explaining why established competitive brands continuously compete through periodic price promotions, most of the existing theories do not attribute this phenomenon to underlying consumer learning. Yet, “try it, you will like it” is often the stated rationale of the practicing marketing manager in offering consumers a price promotion on a brand. This paper examines the connection between consumer learning and the offering of price promotions. The consumer model specified is Markovian in nature and encompasses in addition to consumer learning other empirical findings such as the increase in product class consumption in response to price reductions. The consumer models used in the existing game theoretic approaches to this problem are shown to be mostly special cases of the proposed model. It is demonstrated that for the commonly used price response functions the existence of consumer learning, at a level of intensity consistent with that identified in empirical works, makes it optimal for competing brands to periodically offer price promotions. The Evaluation of Potential New Product Alternatives
In evaluating new product opportunities through the use of a rating scheme, account must be taken of the fact that the individual factors on which the product is being assessed interact in producing an overall result. A score for a product may be thought of as a point in n dimensional space in which are located two density functions, one representing scores of successful products and the other of unsuccessful products. Linear discriminant analysis provides the means for utilizing the likelihood ratio of success to failure for any given potential product (point). The question then arises as to whether to accept the decision dictated by the discriminant analysis or to collect more information concerning the location of the point. Both a posterior and a preposterior distribution can be obtained, yielding the value of additional information and thus determining whether to make the decision now or to postpone it.
