    Antibiotic treatment alone for acute rhinosinusitis gives a poor response in allergic children.

    Acute rhinosinusitis (ARS) is frequent in children. Patients with allergic rhinitis show signs of more impaired paranasal sinus functioning than normal subjects during viral colds. This study evaluated the effectiveness of administering antibiotics alone to treat ARS in both allergic and non-allergic children. We obtained informed consent from the parents of each patient enrolled in the study. 97 children, 55 males and 42 females aged between 4 and 9 years (46 of whom were allergic), suffering from ARS, were treated with amoxicilline-clavulanate (50 mg/Kg bid) for 14 days. Symptoms and endoscopic signs of illness were evaluated at baseline and immediately after treatment. Symptoms improved significantly (p<0.001) after treatment in 84 patients, equally distributed between allergic and non-allergic subjects. On the contrary, endoscopic signs disappeared only in 49 children, 9 of whom were allergic (OR 14.9, 95\%CI 4.6-40.1, p<0.001). Slight to fair agreement was observed between clinical symptoms and endoscopic signs (agreement 64\% , Kappa=0.28, McNemar test p<0.001). Antibiotic therapy alone in the treatment of ARS may be generally insufficient to resolve symptoms, mainly endoscopic signs. Moreover, in allergic children this issue appears to be more evident

    Current status of laboratory and imaging diagnosis of neonatal necrotizing enterocolitis

    Necrotizing enterocolitis continues to be a devastating disease process for very low birth weight infants in Neonatal Intensive Care Units. The aetiology and pathogenesis of necrotizing enterocolitis are not definitively understood. It is known that necrotizing enterocolitis is secondary to a complex interaction of multiple factors that results in mucosal damage, which leads to intestinal ischemia and necrosis. Advances in neonatal care, including resuscitation and ventilation support technology, have seen increased survival rates among premature neonates and a concomitant detection in the incidence of this intestinal disease.Diagnosis can be difficult, and identifying infants at the onset of disease remains a challenge. Early diagnosis, which relies on imaging findings, and initiation of prompt therapy are essential to limit morbidity and mortality. Moreover, early management is critical and life-saving.This review summarizes what is known on the laboratory and instrumental diagnostic strategies needed to improve neonatal outcomes and, possibily, to prevent the onset of an overt necrotizing enterocolitis

    Measurement of transfer of colostral passive immunity in dairy calves

    The administration of high quality colostrum reduces preweaning morbidity, mortality and, therefore, economic losses related to replacement animals. It also stimulates and improves calf growth, increasing milk production and longevity of the future dairy cows. The aim of the present study was to evaluate the influence of breed and parity of the dam on colostrum quality, and of breed and gender of the calf, and time from calf birth to the administration of the first colostrum meal on the transfer of passive immunity to the calf by the field test of the Failure of Passive Transfer (FPT) on calf serum. A further objective was to improve the diagnostic accuracy of the field FPT test through a second laboratory phase improving the turbidity evaluation. The amount of IgG fed to calves (IgG concentration multiplied by the volume of colostrum administered) was influenced by dam parity as significant differences (P &lt; 0.05) were detected between first- and fourth-parity cows, and between second- and fourth-parity cows. The administration of good quality colostrum (IgG &gt; 50 mg/ml) between 5 and 9 h of life was able to reduce the risk of FPT more effectively than the administration performed within the first 4 h of life. However, further studies on larger sample size is needed to confirm the present findings. The spectrophotometric measurements confirmed the results obtained by the field turbidity test at 14% sodium sulphite dilution. It would be interesting in future to expand the dataset and validate the spectrophometric method

    Corporate Social Responsibility and Intellectual Capital in Sports and Leisure : The case of a Golf Club

    Golf is becoming an increasingly popular sport, and golf clubs have, therefore, a growing social and environmental impact, with several stakeholders engaged in the activities. While the press and the media have reported countless initiatives conducted by golf clubs worldwide that highlight how environmentally and socially conscious and supportive the golf industry is, little has been said in academia. The paper aims to investigate the topic of Corporate Social Responsibility (CSR) in golf clubs, and how golf clubs’ Intellectual Capital (IC) may foster CSR initiative. The paper employs a case study methodology, analyzing one Golf Club in the United Kingdom, using semi-structured interviews. Results underline the great potential of the relationship between the clubs and their stakeholders, especially the customers, to create an influential cycle of CSR in the local communities. Relational capital stands as a central IC element to foster CSR in golf clubs. Moreover, findings highlight the need to develop new business models that can merge the need to be compliant with a strong CSR philosophy ensuring, at the same time, financial sustainability. The paper is one of the firsts to investigate an increasingly popular sport which CSR can be fostered by relational capital, with a relevant impact on its customers, employees, and communities

    Strongly enhanced photon collection from diamond defect centres under micro-fabricated integrated solid immersion lenses

    The efficiency of collecting photons from optically active defect centres in bulk diamond is greatly reduced by refraction and reflection at the diamond-air interface. We report on the fabrication and measurement of a geometrical solution to the problem; integrated solid immersion lenses (SILs) etched directly into the surface of diamond. An increase of a factor of 10 was observed in the saturated count-rate from a single negatively charged nitrogen-vacancy (NV-) within a 5um diameter SIL compared with NV-s under a planar surface in the same crystal. A factor of 3 reduction in background emission was also observed due to the reduced excitation volume with a SIL present.     Preterm infants (PIs) are at increased risk of vaccine-preventable diseases (VPDs). However, delayed vaccination start and low vaccine coverage are still reported. Areas covered: This systematic review includes 37 articles on preterm vaccination published in 2008-2018 in PubMed. Both live attenuated and inactivated vaccines are safe and well tolerated in PIs. Local reactions, apnea, and reactivity changes are the most frequently reported adverse events. Lower gestational age and birth weight, preimmunization apnea, longer use of continuous positive airway pressure (CPAP) are risk factors for apnea. The proportion of PIs who develop protective humoral and cellular immunity is generally similar to full terms although later gestational age is associated with increased antibody IgG concentrations (i.e. against certain pneumococcal serotypes, influenza, hepatitis B virus and poliovirus 1) and increased mononuclear cells proliferation (i.e. after inactivated poliovirus). Expert opinion: PIs can be safely and adequately protected by available vaccines with the same schedule used for full terms. Data at this regard have been retrieved by studies using a 3-dose primary series for pneumococcal and hexavalent vaccines. Further studies are needed regarding the 2 + 1 schedule. Apnea represents a nonspecific stress response in PIs, thus those hospitalized at 2 months should have cardio-respiratory monitoring after their first vaccination

    The aim of precision medicine is setting up targeted therapies for selected patients that would ideally have high effectiveness and few side effects. This is made possible by targeted therapy drugs that selectively act on a specific pathway. Precision medicine is spreading to many medical specialties, and there is increasing interest in the context of allergic airway diseases, such as allergic rhinitis, chronic rhinosinusitis, and asthma. This review is an update of new targets in the treatment of childhood allergic upper airway diseases and asthma, including the most recent biologic drugs that have already been licensed or are in the pipeline to be tested with children

    Background: The Link between allergic rhinitis and asthma is well known. Bronchial hyperreactivity (BHR) may be present in rhinitics. The present study is aimed at evaluating a large group of subjects suffering from allergic rhinitis alone to investigate the presence of spirometric impairment and BHR both during and outside the pollen season. Methods: 360 rhinitics (subdivided in 3 groups: seasonal, SAR, perennial, PAR, and polysensitized, PolysR) were investigated by spirometry and methacholine challenge. Results: There was a significant seasonal difference concerning the number of rhinitics with impaired FEV1 (p&lt;0.01 for SAR, p&lt;0.02 for PAR, and p&lt;0.03 for PolysR) and FEF25-75 (p&lt;0.05 for SAR, p&lt;0.03 for PAR, and p&lt;0.05 for PolysR) as well as with BHR (p&lt;0.05 for SAR and p&lt;0.03 for PAR). Conclusions: This study evidences that an impairment of spirometric parameters and BHR may be observed in patients with allergic rhinitis alone. Thus, careful evaluation of lower airways should be performed in patients with allergic rhinitis alone
