Estudo Termodinâmico, Projeto e Viabilidade Econômica do Processo de Extração LÃquido-LÃquido de Fármacos
Trabalho de Conclusão de Curso apresentado ao Instituto Latino-Americano de Tecnologia, Infraestrutura e Território da Universidade Federal da Integração Latino- Americana, como requisito parcial à obtenção do tÃtulo de Bacharel em Engenharia QuÃmica.O uso de fármacos pela população para o tratamento de dores e sintomas, ocasionados por diversas doenças, deve ocorrer por indicação e acompanhamento médico. Entretanto, nem sempre isso acontece, pois alguns não precisam de
prescrição. Essa conduta promove a inserção de diversos fármacos, no esgoto, a ser tratado pela Estação de Tratamento de Esgoto (ETE) e lançados em recursos hÃdricos. Um dos fármacos que aparecem em maior concentração e em maior
frequência nos corpos hÃdricos pelo mundo é o paracetamol. Considerando que a legislação não possui parâmetro para a remoção deste fármaco, do esgoto tratado nas ETEs, o objetivo deste trabalho foi simular e projetar uma coluna de extração
lÃquido-lÃquido para a remoção de paracetamol. Para analisar a eficiência do processo utilizou-se o software ChemSep. As simulações foram baseadas no modelo termodinâmico de predição do equilÃbrio lÃquido-lÃquido UNIFAC. Um dos
fatores mais importantes no processo de extração lÃquido-lÃquido foi a escolha do solvente, considerando a viabilidade técnica, ambiental e econômica. O solvente escolhido foi o 1-pentanol. Foram estudadas as condições operacionais como
temperatura e composição da extração, mostrando que a extração é efetiva na faixa de temperatura entre as mÃnimas e máximas absolutas (1 – 40,4 ºC) no estado do Paraná e nas proporções de alimentação para solvente de 1:1,2 (m/m). Com essas informações foi projetada uma coluna de extração com discos rotativos, com número de estágios teóricos igual a 5. Na análise econômica, obteve-se que o custo do solvente sem reuso de R 27,56 por litro de efluente. O reuso significaria uma economia de 95,2% no custo operacional
Doenças crônicas e sonolência diurna excessiva em pessoas idosas
Objetivo: Avaliar o nÃvel de sonolência diurna excessiva (SDE), a sÃndrome de apneia obstrutiva do sono (SAOS) e as condições de saúde em idosos.Métodos: Trata-se de um estudo transversal e quantitativo, realizado com idosos cadastrados nas Estratégias de Saúde da FamÃlia de um municÃpio do Rio Grande do Norte, Brasil, de setembro de 2015 a janeiro de 2016. Utilizou-se um questionário sociodemográfico (com perguntas relativas ao: sexo, estado civil, tipo de moradia, endereço, renda, escolaridade e profissão) e de condições de saúde (com perguntas relativas à presença de doenças, a sequelas presentes e ao uso de medicamentos). A sonolência diurna excessiva (SDE) foi medida pela escala de Epworth e a sÃndrome de apneia obstrutiva do sono (SAOS), pelo questionário clÃnico de Berlin. Resultados: Participaram do estudo 61 idosos, com idade média de 68,5 anos, sendo 46 do sexo feminino (75,4%). Dentre eles, 56 (91,8%) relataram ter algum tipo de doença, sendo a hipertensão arterial sistêmica (HAS) a mais frequente (57,4%; n=35), seguida pela diabetes mellitus (DM) (24,6%; n=15) e artrite ou artrose (34,4%; n=21). Observou-se correlação positiva entre HAS e SAOS (p=0,001), DM e SAOS (p=0,018) e entre SDE e doenças respiratórias (p=0,010). Conclusão: As doenças mais prevalentes entre os idosos avaliados foram hipertensão arterial sistêmica e diabetes mellitus. A maioria não apresentou sonolência diurna excessiva e, quando observada, foi principalmente em grau leve. Galectin-3, histone deacetylases, and Hedgehog signaling:Possible convergent targets in schistosomiasis-induced liver fibrosis
Schistosomiasis affects approximately 240 million people in the world. Schistosoma mansoni eggs in the liver induce periportal fibrosis and hepatic failure driven by monocyte recruitment and macrophage activation, resulting in robust Th2 response. Here, we suggested a possible involvement of Galectin-3 (Gal-3), histone deacetylases (HDACs), and Hedgehog (Hh) signaling with macrophage activation during Th1/Th2 immune responses, fibrogranuloma reaction, and tissue repair during schistosomiasis. Gal-3 is highly expressed by liver macrophages (Kupffer cells) around Schistosoma eggs. HDACs and Hh regulate macrophage polarization and hepatic stellate cell activation during schistosomiasis-associated fibrogenesis. Previously, we demonstrated an abnormal extracellular matrix distribution in the liver that correlated with atypical monocyte-macrophage differentiation in S. mansoni-infected, Gal-3-deficient (Lgals3-/-) mice. New findings explored in this review focus on the chronic phase, when wild-type (Lgals3+/+) and Lgals3-/- mice were analyzed 90 days after cercariae infection. In Lgals3-/- infected mice, there was significant inflammatory infiltration with myeloid cells associated with egg destruction (hematoxylin and eosin staining), phagocytes (specifically Kupffer cells), numerically reduced and diffuse matrix extracellular deposition in fibrotic areas (Gomori trichrome staining), and severe disorganization of collagen fibers surrounding the S. mansoni eggs (reticulin staining). Granuloma-derived stromal cells (GR cells) of Lgals3-/- infected mice expressed lower levels of alpha smooth muscle actin (α-SMA) and eotaxin and higher levels of IL-4 than Lgals3+/+ mice (real-time PCR). The relevant participation of macrophages in these events led us to suggest distinct mechanisms of activation that culminate in defective fibrosis in the liver of Lgals3-/- infected mice. These aspects were discussed in this review, as well as the possible interference between Gal-3, HDACs, and Hh signaling during progressive liver fibrosis in S. mansoni-infected mice. Further studies focused on macrophage roles could elucidate these questions and clear the potential utility of these molecules as antifibrotic targets
Evaluation of outpatient health services in diabetes mellitus in a middle-income setting: a retrospective cohort study involving secondary care
In Brazil, it is necessary to assess the different levels of health care in diabetes mellitus (DM) in order to integrate them. In this retrospective cohort study, we analyzed 1,122 medical records of patients with DM from specialized services with interdisciplinary health teams (IHT) in the city of Ourinhos, São Paulo state, Brazil, to assess the impact of secondary care on glycemic control in patients with DM in those places and to compare baseline and follow-up DM care indicators concerning clinical evaluation and drug treatment regimens in the aforementioned health services. The study covered consultations carried out from September/2013 to September/2017. Data were collected from initial and final appointments in medical records and revealed an increase of 31.21% in insulin introduction and of 73.53% in regimens with three or more non-insulin antidiabetic (NIA) medications. Among the 570 patients with at least two glycated hemoglobin (A1C) measurements in the aforementioned review, 146 did not require any therapeutic adjustment between initial and final appointments, 123 required a subtle adjustment, 95, a moderate adjustment, and 206, an intense adjustment. There was a noticeably higher A1C reduction between initial and final appointments when patients who required an intense drug adjustment were compared to those who did not need any different NIA drug (p-value < 0.0001). In addition to optimizing drug treatment, essential exams in DM were performed with higher frequency, with an increase of 63% in ophthalmology evaluation performed during secondary care approach and 60.65% more individuals being screened for diabetic chronic kidney disease. IHT secondary care considered in this study, therefore, not only improved glycemic control of patients with DM, especially by optimization of NIA regimens and timely prescription of insulin, but also increased the screening for microvascular complications
Efeitos e aplicabilidade da terapia espelho na neurorreabilitação - revisão de literatura / Effects and applicability of mirror therapy in neurorehabilitation - literature review
Introdução: A Terapia Espelho (TE) foi originalmente desenvolvida na década de 90, pelo neurocientista indiano Ramachandran. A técnica utiliza um espelho posicionado no plano médio sagital, entre o membro afetado e o membro saudável, fornecendo feedback visual do membro saudável e gerando a sensação de dois membros móveis, como se o membro afetado estivesse realizando movimentos saudáveis no hemicampo negligenciado. Isso resulta na excitabilidade corticoespinhal e das áreas somatossensoriais, contribuindo para a recuperação motora. Objetivo: Identificar a aplicabilidade e os efeitos da Terapia Espelho como recurso da reabilitação neurológica.  Materiais e métodos: Trata-se de uma revisão de literatura, onde as bases de dados utilizadas foram Pedro, SciELO, PubMed e Lilacs, no perÃodo de Setembro de 2017 a Outubro de 2018. Utilizou-se como estratégia de pesquisa, nas bases de dados, a combinação dos descritores de acordo com o idioma. Além disso, foi realizada a busca manual na lista de referências dos artigos apresentados. Os estudos foram submetidos a uma avaliação da qualidade metodológica, utilizando-se a escala PeDro. Resultados: Foram encontrados 62 artigos, porém apenas 9 foram analisados. Eram compostos por amostras de indivÃduos com diagnóstico de dor fantasma por amputação, Paralisia Cerebral (PC), lesão traumática cerebral ou Acidente Vascular Cerebral (AVC) em suas fases aguda, subaguda e crônica. Os instrumentos de avaliação variaram entre os estudos, assim como o modo de execução, a frequência e a duração do tratamento. Conclusão: A TE é benéfica para a recuperação motora de membros superiores e inferiores, função sensório-motora e para a diminuição da dor. Porém, a literatura voltada para o modo de execução da TE é muito escassa e existem muitas controvérsias em relação ao protocolo utilizado, tornando-se necessária a realização de novos estudos com maior número amostral, de modo que possam obter resultados mais significativos e amplos
The Atacama Cosmology Telescope: A Measurement of the DR6 CMB Lensing Power Spectrum and its Implications for Structure Growth
We present new measurements of cosmic microwave background (CMB) lensing over
sq. deg. of the sky. These lensing measurements are derived from the
Atacama Cosmology Telescope (ACT) Data Release 6 (DR6) CMB dataset, which
consists of five seasons of ACT CMB temperature and polarization observations.
We determine the amplitude of the CMB lensing power spectrum at
precision ( significance) using a novel pipeline that minimizes
sensitivity to foregrounds and to noise properties. To ensure our results are
robust, we analyze an extensive set of null tests, consistency tests, and
systematic error estimates and employ a blinded analysis framework. The
baseline spectrum is well fit by a lensing amplitude of
relative to the Planck 2018 CMB power spectra
best-fit CDM model and relative to
the best-fit model. From our lensing power
spectrum measurement, we derive constraints on the parameter combination
of
from ACT DR6 CMB lensing alone and
when combining ACT DR6 and Planck NPIPE
CMB lensing power spectra. These results are in excellent agreement with
CDM model constraints from Planck or
CMB power spectrum measurements. Our lensing measurements from redshifts
-- are thus fully consistent with CDM structure growth
predictions based on CMB anisotropies probing primarily . We find no
evidence for a suppression of the amplitude of cosmic structure at low

The Atacama Cosmology Telescope: High-resolution component-separated maps across one-third of the sky
Observations of the millimeter sky contain valuable information on a number
of signals, including the blackbody cosmic microwave background (CMB), Galactic
emissions, and the Compton- distortion due to the thermal Sunyaev-Zel'dovich
(tSZ) effect. Extracting new insight into cosmological and astrophysical
questions often requires combining multi-wavelength observations to spectrally
isolate one component. In this work, we present a new arcminute-resolution
Compton- map, which traces out the line-of-sight-integrated electron
pressure, as well as maps of the CMB in intensity and E-mode polarization,
across a third of the sky (around 13,000 sq.~deg.). We produce these through a
joint analysis of data from the Atacama Cosmology Telescope (ACT) Data Release
4 and 6 at frequencies of roughly 93, 148, and 225 GHz, together with data from
the \textit{Planck} satellite at frequencies between 30 GHz and 545 GHz. We
present detailed verification of an internal linear combination pipeline
implemented in a needlet frame that allows us to efficiently suppress Galactic
contamination and account for spatial variations in the ACT instrument noise.
These maps provide a significant advance, in noise levels and resolution, over
the existing \textit{Planck} component-separated maps and will enable a host of
science goals including studies of cluster and galaxy astrophysics, inferences
of the cosmic velocity field, primordial non-Gaussianity searches, and
gravitational lensing reconstruction of the CMB.Comment: The Compton-y map and associated products will be made publicly

The Atacama Cosmology Telescope: DR6 Gravitational Lensing Map and Cosmological Parameters
We present cosmological constraints from a gravitational lensing mass map
covering 9400 sq. deg. reconstructed from CMB measurements made by the Atacama
Cosmology Telescope (ACT) from 2017 to 2021. In combination with BAO
measurements (from SDSS and 6dF), we obtain the amplitude of matter
fluctuations at 1.8% precision,
and the Hubble
constant at
1.6% precision. A joint constraint with CMB lensing measured by the Planck
satellite yields even more precise values: ,
and . These measurements agree
well with CDM-model extrapolations from the CMB anisotropies measured
by Planck. To compare these constraints to those from the KiDS, DES, and HSC
galaxy surveys, we revisit those data sets with a uniform set of assumptions,
and find from all three surveys are lower than that from ACT+Planck
lensing by varying levels ranging from 1.7-2.1. These results motivate
further measurements and comparison, not just between the CMB anisotropies and
galaxy lensing, but also between CMB lensing probing on
mostly-linear scales and galaxy lensing at on smaller scales. We
combine our CMB lensing measurements with CMB anisotropies to constrain
extensions of CDM, limiting the sum of the neutrino masses to eV (95% c.l.), for example. Our results provide independent
confirmation that the universe is spatially flat, conforms with general
relativity, and is described remarkably well by the CDM model, while
paving a promising path for neutrino physics with gravitational lensing from
