First description of cervical intradural thymoma metastasis
Thymoma and thymic carcinoma are rare epithelial tumors, which originate from the thymus gland. According to the World Health Organization there are "organotypic" (types A, AB, B1, B2, and B3) and "non-organotypic" (thymic carcinomas) thymomas. Type B3 thymomas are aggressive tumors, which can metastasize. Due to the rarity of these lesions, only 7 cases of extradural metastasis are described in the literature. We report the first and unique case of a man with cervical intradural B3 thymoma metastasis. A 46-year-old man underwent thymoma surgical removal. The year after the procedure he was treated for a parietal pleura metastasis. In 2006 he underwent cervical-dorsal extradural metastasis removal and C5-Th1 stabilization. Seven years after he came to our observation complaining left cervicobrachialgia and a reduction of strength of the left arm. He underwent a cervical spine magnetic resonance imaging, which showed a new lesion at the C5-C7 level. The patient underwent a surgery for the intradural B3 thymoma metastasis. Neurological symptoms improved although the removal was subtotal. He went through postoperative radiation therapy with further mass reduction. Spinal metastases are extremely rare. To date, only 7 cases of spinal extradural metastasis have been described in the literature. This is the first case of spinal intradural metastasis. Early individuation of these tumors and surgical treatment improve neurological outcome in patients with spinal cord compression. A multimodal treatment including neoadjuvant chemotherapy, surgery and postoperative radiation therapy seems to improve survival in patients with metastatic thymoma
Emergency decompressive craniectomy after removal of convexity meningiomas
BACKGROUND: Convexity meningiomas are benign brain tumors that are amenable to complete surgical resection and are associated with a low complication rate. The aim of this study was to identify factors that result in acute postoperative neurological worsening after the removal of convexity meningiomas. METHODS: Clinical evaluation and neuroradiological analysis of patients who underwent removal of a supratentorial convexity meningioma were reviewed. Patients were selected when their postoperative course was complicated by acute neurological deterioration requiring decompressive craniectomy. RESULTS: Six patients (mean age: 43.3 years) underwent surgical removal of a supratentorial convexity meningioma. Brain shift (mean: 9.9 mm) was evident on preoperative imaging due to lesions of varying size and perilesional edema. At various times postoperatively, patient consciousness worsened (up to decerebrate posture) with contralateral paresis and pupillary anisocoria. Computed tomography revealed no postoperative hematoma, however, did indicate increased brain edema and ventricular shift (mean: 12 mm). Emergency decompressive craniectomy and brief ventilator assistance were performed in all patients. Ischemia of the ipsilateral posterior cerebral artery occurred in 3 patients and hydrocephalus occurred in 2 patients. Outcome was good in 2, fair in 2, 1 patient had severe disability, and 1 patient died after 8 months. CONCLUSIONS: Brain shift on preoperative imaging is a substantial risk factor for postoperative neurological worsening in young adult patients after the removal of convexity meningiomas. Emergency decompressive craniectomy must be considered because it is effective in most cases. Other than consciousness impairment, there is no reliable clinical landmark to guide the decision to perform decompressive craniectomy; however, brain ischemia may have already occurred
The impact of COVID-19 fiscal spending on climate change adaptation and resilience
Government expenditure and taxation have a significant influence on the long-term adaptation and resilience of societies to climate and other environmental shocks. Unprecedented fiscal spending in the COVID-19 recovery offered an opportunity to systematically enhance adaptation and resilience to future shocks. But did the ‘build back better’ rhetoric manifest in more resilient policy? We develop a dedicated fiscal policy taxonomy for climate change adaptation and resilience (A&R)—the Climate Resilience and Adaptation Financing Taxonomy (CRAFT)—and apply this to analyse ~8,000 government policies across 88 countries. We find that US$279–334 billion (9.7–11.1%) of economic recovery spending potentially had direct A&R benefits. This positive spending is substantial in absolute terms but falls well below adaptation needs. Moreover, a notable portion (27.6–28%) of recovery spending may have had negative impacts on A&R, acting to lock in non-resilient infrastructure. We add a deep learning algorithm to consider A&R themes in associated COVID-19 policy documents. Compared with climate mitigation, A&R received only one-third of the spending and was mentioned only one-seventh as frequently in policy documents. These results suggest that the COVID-19 fiscal response missed many opportunities to advance climate A&R. We draw conclusions for how to better align fiscal policy with A&R
Basal ganglia stroke due to mild head trauma in pediatric age - clinical and therapeutic management: a case report and 10 year literature review
Ischemia of the basal ganglia as an immediate consequence of minor head injury in children is rare (< 2% of all ischemic stroke in childhood) and is due to vasospasm of the lenticulostriate arteries. The clinical history of these lesions is particularly favourable because they are usually small, and also because the facial-brachial-crural hemiparesis typical of this pathology usually regresses after a period ranging from several weeks to several months, despite the persistence of an ischemic area on MRI. This is due to the well known neuronal plasticity of the CNS, in particular, of the primary motor cortex. The most effective therapeutic approach appears to be the conservative one, although the best treatment regimen is still not well defined
Measurement of corneal thickness using Pentacam HR versus Nidek CEM-530 specular microscopy.
Objective To compare corneal thickness (CT) measurements using the CEM-530 (Nidal, Gamagori, Japan) and Pentacam HR (Oculus, Wetzlar, Germany). Methods The CT of 209 healthy subjects (209 right eyes) aged 24 to 89 years (71.35 ± 10.72 years) was measured at the corneal apex (CA), pupil center (PC), and thinnest point (TP) with the Pentacam HR and at the corneal center with the CEM-530 in random order at the same time of day. Results A good correlation but statistically significant difference was found between the CEM-530 and Pentacam HR measurements at the CA (6.10 ± 8.12 µm, R2 = 0.8947), PC (7.46 ± 8.57 µm, R2 = 0.8826), and TP (12.44 ± 10.04 µm, R2 = 0.8392). Comparison of the two devices produced the following regression formulas: y = 0.8859x + 57.644 for the CA, y = 0.8852x +56.657 for the PC, and y = 0.8557x + 68.148 for the TP, where x is the CT obtained with the CEM-530 and y is that obtained with the Pentacam HR. Conclusions These findings indicate that the CEM-530 produces a thicker corneal measurement than the Pentacam HR. The herein-proposed correcting factors are needed to reliably compare these devices
Coronavirus Diseases (COVID-19) Current Status and Future Perspectives: A Narrative Review
At the end of 2019 a novel virus, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), causing severe acute respiratory syndrome expanded globally from Wuhan, China. In March 2020 the World Health Organization declared the SARS-Cov-2 virus a global pandemic. We performed a narrative review to describe existing literature with regard to Corona Virus Disease 2019 (COVID-19) epidemiology, pathophysiology, diagnosis, management and future perspective. MEDLINE, EMBASE and Scopus databases were searched for relevant articles. Although only when the pandemic ends it will be possible to assess the full health, social and economic impact of this global disaster, this review represents a picture of the current state of the art. In particular, we focus on public health impact, pathophysiology and clinical manifestations, diagnosis, case management, emergency response and preparedness
Human Chorionic Gonadotropin Protects Vascular Endothelial Cells from Oxidative Stress by Apoptosis Inhibition, Cell Survival Signalling Activation and Mitochondrial Function Protection.
Background/Aim: Previous reports have made it hypothetically possible that human chorionic gonadotropin (hCG) could protect against the onset of pregnancy-related pathological conditions by acting as an antioxidant. In the present study we planned to examine the effects of hCG against oxidative stress in human umbilical vein endothelial cells (HUVEC). Methods: HUVEC were subjected to peroxidation by hydrogen peroxide. The modulation of nitric oxide (NO) release by hCG and its effects on cell viability, glutathione (GSH) levels, mitochondrial membrane potential and mitochondrial transition pore opening (MPTP) were examined by specific dyes. Endothelial and inducible NO synthase (eNOS and iNOS), Akt and extracellular -signal-regulated kinases 1/2 (ERK1/2) activation and markers of apoptosis were analyzed by Western Blot. Results: In HUVEC, hCG reduced NO release by modulating eNOS and iNOS. Moreover, hCG protected HUVEC against oxidative stress by preventing GSH reduction and apoptosis, by maintaining Akt and ERK1/2 activation and by keeping mitochondrial function. Conclusion: The present results have for the first time shown protective effects exerted by hCG on vascular endothelial function, which would be achieved by modulation of NO release, antioxidant and antiapoptotic actions and activation of cell survival signalling. These findings could have clinical implications in the management of pregnancy-related disorders
A Double-Blind, RCT Testing Beneficial Modulation of BDNF in Middle-Aged, Life Style-Stressed Subjects: A Clue to Brain Protection?
Introduction: The aim of this prospective study was to see
whether LD-1227, a quality-controlled marine nutraceuticals
shown to protect experimental stress-induced hyppocampal
degeneration, could beneficially modulate BDNF, as measured in
the serum, in otherwise healthy but work-stressed individuals.
Materials and Methods: Forty-eight men and women between
the ages of 38 and 62 reporting high-demanding work activity
but with an overall positive attitude towards their personal life
were recruited. Subjects were divided in two group (24 patients
each) and blindly supplemented for 2 month with: a) LD-1227
400mg or b) placebo. A third group of healthy non-stressed
subjects was used as well. Blood samples were taken before
and after the supplementation period. Unstimulated saliva
was collected and tested for amylase while serum levels were
used to measure BDNF. State Trait Anxiety Inventory (STAI),
Pittsburgh Sleep Quality Index (PSQI) and psychological wellbeing assessment (PSWB) were measured too. Patients with
Val66Met functional polymorphism of BDNF excluded those
given their reported association with an impaired release of
BDNF.
Results: Results showed that, as compared to healthy, nonstressed individuals, stressed ones has a trend decrease
of BDNF and this was significantly increased by LD 12-1227
supplementation and the same inverse phenomenon occurred
to salivary amylase (p<0.05). No change was noted in the PSQI
score but, either STAI or PSWB tests scored better in LD-1227
supplemented subjects.
Conclusion: The present data suggest that LD-1227 is beneficially affecting neuromodulation and related symptoms during
common stressful life conditions and may have the potential as
tools in a neuroprotective clinical strategy
Retinoic Acid Specifically Enhances Embryonic Stem Cell Metastate Marked by Zscan4
Pluripotency confers Embryonic Stem Cells (ESCs) the ability to differentiate in ectoderm, endoderm, and mesoderm derivatives, producing the majority of cell types. Although the majority of ESCs divide without losing pluripotency, it has become evident that ESCs culture consists of multiple cell populations with different degrees of potency that are spontaneously induced in regular ESC culture conditions. Zscan4, a key pluripotency factor, marks ESC subpopulation that is referred to as high-level of pluripotency metastate. Here, we report that in ESC cultures treated with retinoic acid (RA), Zscan4 ESCs metastate is strongly enhanced. In particular, we found that induction of Zscan4 metastate is mediated via RA receptors (RAR-alpha, RAR-beta, and RAR-gamma), and it is dependent on phosphoinositide-3-kinase (PI3K) signaling. Remarkably, Zscan4 metastate induced by RA lacks canonical pluripotency genes Oct3/4 and Nanog but retained both self-renewal and pluripotency capabilities. Finally we demonstrated that the conditional ablation of Zscan4 subpopulation is dispensable for both endoderm and mesoderm but is required for ectoderm lineage. In conclusion, our research provides new insights about the role of RA signaling during ESCs high pluripotency metastate fluctuation
Diagnostic Accuracy of a New Antigen Test for SARS-CoV-2 Detection
Background and aims: Quick and reliable diagnostic tools play an important role in controlling the spread of the SARS-Cov-2 pandemic. The aim of this study was to evaluate the diagnostic accuracy of a new cyto-salivary antigen test aimed at detecting the presence of antigens for SARS-CoV-2, as compared by the gold standard RT-PCR and a lateral flow test. Methods: A total of 433 healthy volunteers were enrolled in the study and the sensitivity and specificity of the new cyto-salivary antigen test were calculated, as compared to the RT-PCR nasopharyngeal swab and to the lateral flow test. Results: A total of 433 samples were collected and tested at the Mediterranean Fair in Palermo from February 2021 until April 2021. The new cyto-salivary antigen had a sensitivity of 100% and a specificity of 94.2%. The sensitivity and the specificity of the lateral flow test were 55% and 100%, respectively. Conclusions: The new cyto-salivary antigen test detected more positive cases than the RT-PCR in a sample of asymptomatic subjects, demonstrating to be a promising tool for a more sensitive diagnosis of COVID-19. Further studies are warranted to better characterize its diagnostic accuracy
