    The search for old-growth forests in Switzerland

    Old-growth forests and stands have multiple values, such as biological, scientific, socioeconomic, aesthetic, spiritual and cultural ones, but they are very rare in central Europe. Currently, an overview about the existence and size of old-growth forest patches in Switzerland is missing. We propose a definition for old-growth forests worth preserving based on ten criteria: 1) presence of veteran trees, 2) significant biomass, 3) heterogeneous stand structure including senescent and decaying stages, 4) presence of natural processes and disturbances, 5) high dead wood volume (at least 30 m3/ha) and a large diversity of dimensions and decay stages of dead wood pieces, 6) autochthonous tree species, 7) diverse and natural species assemblages, 8) only extensive or no harvesting for the last 50 years, 9) long continuity of forest cover, 10) minimal area of 30 ha for old-growth forests and 1 ha for oldgrowth stands. When searching for old-growth forests and stands worth preserving, quantitative criteria from the National Forest Inventory and other sources should also be used. We call on all forest specialists to identify old-growth patches, to inform the population and decision makers about their high value, and to protect them in an adequate manner as a unique natural heritage

    How biodiversity underpins ecosystem resistance (i.e. ability to withstand environmental perturbations) and recovery (i.e. ability to return to a pre-perturbation state), and thus, stability under extreme climatic events is a timely question in ecology. To date, most studies have focussed on the role of taxonomic diversity, neglecting how community functional composition and diversity beget stability under exceptional climatic conditions. In addition, land use potentially modulates how biodiversity and ecosystem functions respond to extreme climatic conditions. Using an 11-year time-series of plant biomass from 150 permanent grassland plots spanning a gradient of land-use intensity, we examined how taxonomic and functional components of biodiversity affected resistance and recovery of biomass under extreme drought. The association between biodiversity, land use and biomass varied across years, especially in the driest years. Species-rich or functionally diverse communities (associated with low land-use intensity) buffered extreme droughts better, while species-poor communities or those dominated by fast-growing species (associated with high land-use intensity) had higher recovery capabilities after a moderate-to-extreme drought. Synthesis. Our results show that plant community functional and taxonomic components determine grasslands resistance and recovery under moderate-to-extreme drought. In turn, this points to the importance of designing landscapes with both extensively and intensively managed grasslands. Functionally or taxonomically rich communities (favoured under low land-use intensity) would preserve biomass under extreme droughts, whereas species-poor or fast-growing communities (favoured by high land-use intensity) would restore biomass after extreme droughts

    Neue Methoden der Genomanalyse erlauben es, im Blut einer schwangeren Frau festzustellen, ob krankhafte genetische Abweichungen beim Embryo vorliegen. Damit wird es vergleichsweise einfach, schon in einer frühen Phase der Schwangerschaft Aussagen über Chromosomen-Anomalien und weitere genetische Merkmale des Ungeborenen zu erhalten. Die seit wenigen Jahren erhältlichen nicht-invasiven Pränataltests (NIPT) sind gemäss ersten Erfahrungen sehr verlässlich, zudem entfallen die Risiken für den Fötus, die mit einer invasiven Untersuchung wie z.B. der Fruchtwasserpunktion einhergehen. Mit einer breiteren Anwendung der NIPT und mit der zunehmenden Anzahl von Krankheitsrisiken, die damit untersucht werden können, sind offene Fragen verbunden. Welche Tests sind sinnvoll? Wie gehen die Betroffenen mit den Informationen um? Wie wird die Beratung sichergestellt? In der interdisziplinären Studie werden Chancen und Risiken von vorgeburtlichen genetischen Untersuchungen abgeschätzt. Die Studie zeigt auf, wie sich die neuen Tests auf die Zukunft der pränatalen Diagnostik auswirken könnten, analysiert gesellschaftliche, ethische, rechtliche und ökonomische Fragen und formuliert Empfehlungen

    Direct and plant community mediated effects of management intensity on annual nutrient leaching risk in temperate grasslands

    Grassland management intensity influences nutrient cycling both directly, by changing nutrient inputs and outputs from the ecosystem, and indirectly, by altering the nutrient content, and the diversity and functional composition of plant and microbial communities. However, the relative importance of these direct and indirect processes for the leaching of multiple nutrients is poorly studied. We measured the annual leaching of nitrate, ammonium, phosphate and sulphate at a depth of 10 cm in 150 temperate managed grasslands using a resin method. Using Structural Equation Modeling, we distinguished between various direct and indirect effects of management intensity (i.e. grazing and fertilization) on nutrient leaching. We found that management intensity was positively associated with nitrate, ammonium and phosphate leaching risk both directly (i.e. via increased nutrient inputs) and indirectly, by changing the stoichiometry of soils, plants and microbes. In contrast, sulphate leaching risk was negatively associated with management intensity, presumably due to increased outputs with mowing and grazing. In addition, management intensification shifted plant communities towards an exploitative functional composition (characterized by high tissue turnover rates) and, thus, further promoted the leaching risk of inorganic nitrogen. Plant species richness was associated with lower inorganic nitrogen leaching risk, but most of its effects were mediated by stoichiometry and plant community functional traits. Maintaining and restoring diverse plant communities may therefore mitigate the increased leaching risk that management intensity imposes upon grasslands

    NT-proBNP or Self-Reported Functional Capacity in Estimating Risk of Cardiovascular Events After Noncardiac Surgery

    ImportanceNearly 16 million surgical procedures are conducted in North America yearly, and postoperative cardiovascular events are frequent. Guidelines suggest functional capacity or B-type natriuretic peptides (BNP) to guide perioperative management. Data comparing the performance of these approaches are scarce.ObjectiveTo compare the addition of either N-terminal pro-BNP (NT-proBNP) or self-reported functional capacity to clinical scores to estimate the risk of major adverse cardiac events (MACE).Design, Setting, and ParticipantsThis cohort study included patients undergoing inpatient, elective, noncardiac surgery at 25 tertiary care hospitals in Europe between June 2017 and April 2020. Analysis was conducted in January 2023. Eligible patients were either aged 45 years or older with a Revised Cardiac Risk Index (RCRI) of 2 or higher or a National Surgical Quality Improvement Program, Risk Calculator for Myocardial Infarction and Cardiac (NSQIP MICA) above 1%, or they were aged 65 years or older and underwent intermediate or high-risk procedures.ExposuresPreoperative NT-proBNP and the following self-reported measures of functional capacity were the exposures: (1) questionnaire-estimated metabolic equivalents (METs), (2) ability to climb 1 floor, and (3) level of regular physical activity.Main Outcome and MeasuresMACE was defined as a composite end point of in-hospital cardiovascular mortality, cardiac arrest, myocardial infarction, stroke, and congestive heart failure requiring transfer to a higher unit of care.ResultsA total of 3731 eligible patients undergoing noncardiac surgery were analyzed; 3597 patients had complete data (1258 women [35.0%]; 1463 (40.7%) aged 75 years or older; 86 [2.4%] experienced a MACE). Discrimination of NT-proBNP or functional capacity measures added to clinical scores did not significantly differ (Area under the receiver operating curve: RCRI, age, and 4MET, 0.704; 95% CI, 0.646-0.763; RCRI, age, and 4MET plus floor climbing, 0.702; 95% CI, 0.645-0.760; RCRI, age, and 4MET plus physical activity, 0.724; 95% CI, 0.672-0.775; RCRI, age, and 4MET plus NT-proBNP, 0.736; 95% CI, 0.682-0.790). Benefit analysis favored NT-proBNP at a threshold of 5% or below, ie, if true positives were valued 20 times or more compared with false positives. The findings were similar for NSQIP MICA as baseline clinical scores.Conclusions and relevanceIn this cohort study of nearly 3600 patients with elevated cardiovascular risk undergoing noncardiac surgery, there was no conclusive evidence of a difference between a NT-proBNP–based and a self-reported functional capacity–based estimate of MACE risk.Trial RegistrationClinicalTrials.gov Identifier: NCT0301693

    Retrospective, multicenter analysis comparing conventional with oncoplastic breast conserving surgery: oncological and surgical outcomes in women with high-risk breast cancer from the OPBC-01/iTOP2 study

    Introduction: Recent data suggest that margins ≥2 mm after breast-conserving surgery may improve local control in invasive breast cancer (BC). By allowing large resection volumes, oncoplastic breast-conserving surgery (OBCII; Clough level II/Tübingen 5-6) may achieve better local control than conventional breast conserving surgery (BCS; Tübingen 1-2) or oncoplastic breast conservation with low resection volumes (OBCI; Clough level I/Tübingen 3-4). Methods: Data from consecutive high-risk BC patients treated in 15 centers from the Oncoplastic Breast Consortium (OPBC) network, between January 2010 and December 2013, were retrospectively reviewed. Results: A total of 3,177 women were included, 30% of whom were treated with OBC (OBCI n = 663; OBCII n = 297). The BCS/OBCI group had significantly smaller tumors and smaller resection margins compared with OBCII (pT1: 50% vs. 37%, p = 0.002; proportion with margin <1 mm: 17% vs. 6%, p < 0.001). There were significantly more re-excisions due to R1 (“ink on tumor”) in the BCS/OBCI compared with the OBCII group (11% vs. 7%, p = 0.049). Univariate and multivariable regression analysis adjusted for tumor biology, tumor size, radiotherapy, and systemic treatment demonstrated no differences in local, regional, or distant recurrence-free or overall survival between the two groups. Conclusions: Large resection volumes in oncoplastic surgery increases the distance from cancer cells to the margin of the specimen and reduces reexcision rates significantly. With OBCII larger tumors are resected with similar local, regional and distant recurrence-free as well as overall survival rates as BCS/OBCI
