    Exploring the potential of occupancy modelling using passive acoustics in Coua gigas and Coua coquereli

    In highly threatened habitats such as the dry deciduous forests of western Madagascar, it is essential to develop new approaches to detect population changes and evaluate conservation measures. Passive acoustic monitoring (PAM) is such a promising approach. This method has many advantages over conventional methods, such as time efficiency, money savings, and reduced wildlife disturbance. It is especially suitable for studying occupancy and activity patterns of vocalizing species such as birds. Our study analyzed data recorded with autonomous sound recorders in 2018 in Kirindy Forest for the territorial calls of Coua gigas and Coua coquereli. We modeled occupancy and detection probability for both species in the study area. We also examined activity patterns and found that the peak of vocal activity for Coua coquereli is at 0700h and for Coua gigas at 1100h. To also test the value of PAM in relation to ecological factors we modeled occupancy and included logging status as a site covariate. We detected a positive influence of logging in occupancy of Coua gigas. Our study provides guidelines for future occupancy studies using PAM in the two coua species. We conclude that PAM will improve the ecological monitoring of soniferous animals in Madagascar.   RÉSUMÉ Dans les habitats très menacés tels que les forêts sèches à feuilles caduques de l'ouest de Madagascar, il est essentiel de développer de nouvelles approches pour détecter les changements de population et évaluer les mesures de conservation. La surveillance acoustique passive (PAM) est une approche prometteuse. Cette méthode présente de nombreux avantages par rapport aux méthodes conventionnelles, comme le gain de temps, l'économie d'argent et la réduction des perturbations de la faune. Elle est particulièrement adaptée à l'étude des modèles d'occupation et d'activité des espèces vocalisantes telles que les oiseaux. Notre étude a analysé les données enregistrées avec des enregistreurs sonores autonomes en 2018 dans la forêt de Kirindy pour les vocalisations territoriaux de Coua gigas et Coua coquereli. Nous avons modélisé l'occupation et la probabilité de détection des deux espèces dans la zone d'étude. Nous avons également examiné les schémas d'activité et constaté que le pic d'activité vocale de Coua coquereli se situe à 0700h et celui de Coua gigas à 1100h. Pour tester également la valeur de la PAM par rapport aux facteurs écologiques, nous avons modélisé l'occupation et inclus le statut d'exploitation forestière en tant que covariable du site. Nous avons détecté une influence positive de l'exploitation forestière sur l'occupation de Coua gigas. Notre étude fournit des lignes directrices pour les futures études d'occupation utilisant la PAM pour les deux espèces de coua. Nous concluons que la PAM améliorera le suivi écologique des animaux sonifères à Madagascar

    Strain measurement at the knee ligament insertion sites

    We describe the modification of an existing method of ligament strain measurement at the knee joint in detail. At ten fresh joint specimens we used that technique where strain gauges are attached to the ligamentous insertions and origins. We both improved the preparation of the attachment site and the application of the strain gauges. In a special apparatus the specimens were moved from 0degrees extension to 100degrees flexion while simulating muscle strength and axial force. Testing was performed at the posterior cruciate ligament with both intact and transsected anterior cruciate ligament. In contrast to other existing techniques it does not affect the motion of the joint or the integrity and the function of the ligaments. Unlike the original description of that method we could register a loading behaviour of the posterior cruciate ligament that is similar to those reported in the literature

    Non-invasive computer-assisted measurement of knee alignment

    The quantification of knee alignment is a routine part of orthopaedic practice and is important for monitoring disease progression, planning interventional strategies, and follow-up of patients. Currently available technologies such as radiographic measurements have a number of drawbacks. The aim of this study was to validate a potentially improved technique for measuring knee alignment under different conditions. An image-free navigation system was adapted for non-invasive use through the development of external infrared tracker mountings. Stability was assessed by comparing the variance (F-test) of repeated mechanical femoro-tibial (MFT) angle measurements for a volunteer and a leg model. MFT angles were then measured supine, standing and with varus-valgus stress in asymptomatic volunteers who each underwent two separate registrations and repeated measurements for each condition. The mean difference and 95% limits of agreement were used to assess intra-registration and inter-registration repeatability. For multiple registrations the range of measurements for the external mountings was 1° larger than for the rigid model with statistically similar variance (p=0.34). Thirty volunteers were assessed (19 males, 11 females) with a mean age of 41 years (range: 20-65) and a mean BMI of 26 (range: 19-34). For intra-registration repeatability, consecutive coronal alignment readings agreed to almost ±1°, with up to ±0.5° loss of repeatability for coronal alignment measured before and after stress maneuvers, and a ±0.2° loss following stance trials. Sagittal alignment measurements were less repeatable overall by an approximate factor of two. Inter-registration agreement limits for coronal and sagittal supine MFT angles were ±1.6° and ±2.3°, respectively. Varus and valgus stress measurements agreed to within ±1.3° and ±1.1°, respectively. Agreement limits for standing MFT angles were ±2.9° (coronal) and ±5.0° (sagittal), which may have reflected a variation in stance between measurements. The system provided repeatable, real-time measurements of coronal and sagittal knee alignment under a number of dynamic, real-time conditions, offering a potential alternative to radiographs

    Mesoscopic modelling of enamel interaction with mid-infrared sub-ablative laser pulses

    Using a finite element approach the authors model the influence of enamel's microstructure and water distribution on the temperature and stress at the centre of the laser spot, for a CO2 laser working at 10.6 μm, with 0.35 μs pulse duration and sub-ablative intensity. The authors found that the distribution of water in enamel significantly influences the stress generated at the end of one laser pulse: much lower (two orders of magnitude) stress values occur in models with homogeneously distributed water than in models with 0.27 vol.% water located in pores or 4 vol.% in layers. The amount of water in enamel has a strong influence on the stress distribution, but not on the maximum stress values reached. However, different water contents do not influence the temperature distribution in enamel. These results suggest that adequate modelling of the ablation mechanisms in enamel, as in other highly inhomogeneous materials, must include their structure at the mesoscopic scale

    Double-blind, randomized, multicentre, and active comparator controlled investigation of the effect of Pioglitazone, Metformin, and the combination of both on cardiovascular risk in patients with type 2 diabetes receiving stable basal insulin therapy: the PIOCOMB study

    <p>Abstract</p> <p>Background</p> <p>We analyzed specific effects of an add-on therapy with pioglitazone compared to metformin and their combination in patients with basal insulin treatment on biomarkers of CV risk.</p> <p>Methods</p> <p>In this double-blind, randomized, multicentre, active comparator controlled trial, 121 patients with type 2 diabetes were enrolled. Inclusions: treatment with basal insulin, HbA<sub>1C </sub>6.5% - 8.5%, age 30 - 75 years. After glargine therapy over 2 weeks for titration towards FBG ≤ 7.8 mmol/L, patients received either (A) bid 850 mg metformin (n = 42), (B) bid 15 mg pioglitazone (n = 40), or (C) 30 mg pioglitazone plus 1.7 g metformin (n = 39) over 6 months. Matrix Metal Proteinase 9 (MMP-9) was primary objective, together with biomarkers of CV risk.</p> <p>Results</p> <p>Pioglitazone (B) reduced MMP-9 versus baseline by 54.1 + 187.1 ng/mL, with metformin (A) it was increased by 49.6 + 336.2 ng/mL (p = 0.0345; B vs. A), and with the combination of both (C) it was decreased by 67.8 + 231.4 ng/mL (A vs. C: p = 0.0416; B vs. C: p = 0.8695). After logarithmic transformation due to high variances the exploratory results showed significance for A vs. B (p = 0.0043) and for A vs. C (p = 0.0289).</p> <p>Insulin dosage was reduced by 7.3 units in group B (p < 0.0001), by 6.0 units in C (p = 0.0004), but was increased by 2.5 units (p = 0.1539) in A at follow up. Reduction in hs-CRP was significant within treatment groups for B (p = 0.0098) and C (p < 0.0001), and between the groups for A vs. C (p = 0.0124). All three single regimens reduced PAI-1. Adiponectin was significantly elevated in B and C (p < 0.0001) and between-groups. HbA<sub>1C </sub>was only significantly decreased in the combination group. No significant effects were observed for NFkB and PGFα. peripheral edema were seen in 11.9% vs. 40.0% vs. 20.5%, and weight change was -0.7 kg vs. +4.3 kg vs. +2.7 kg (A vs. B vs. C).</p> <p>Conclusions</p> <p>Addition of pioglitazone but not of metformin reduces MMP-9, hs-CRP and increased insulin sensitivity and adiponectin in this study. The combination of both had no additional effect on inflammation. Pioglitazone is suggested to be a rational add-on therapy to basal insulin in patients with high CV risk.</p

    The metabolic vascular syndrome - guide to an individualized treatment

    In ancient Greek medicine the concept of a distinct syndrome (going together) was used to label 'a group of signs and symptoms' that occur together and 'characterize a particular abnormality and condition'. The (dys)metabolic syndrome is a common cluster of five pre-morbid metabolic-vascular risk factors or diseases associated with increased cardiovascular morbidity, fatty liver disease and risk of cancer. The risk for major complications such as cardiovascular diseases, NASH and some cancers develops along a continuum of risk factors into clinical diseases. Therefore we still include hyperglycemia, visceral obesity, dyslipidemia and hypertension as diagnostic traits in the definition according to the term 'deadly quartet'. From the beginning elevated blood pressure and hyperglycemia were core traits of the metabolic syndrome associated with endothelial dysfunction and increased risk of cardiovascular disease. Thus metabolic and vascular abnormalities are in extricable linked. Therefore it seems reasonable to extend the term to metabolic-vascular syndrome (MVS) to signal the clinical relevance and related risk of multimorbidity. This has important implications for integrated diagnostics and therapeutic approach. According to the definition of a syndrome the rapid global rise in the prevalence of all traits and comorbidities of the MVS is mainly caused by rapid changes in life-style and sociocultural transition resp. with over- and malnutrition, low physical activity and social stress as a common soil

    Congenital absence of the cruciate ligament is an extremely rare condition that was first reported in Giorgi's radiographic study in 1956. The authors report on a case of anterior cruciate ligament reconstruction performed on a 21-year-old female patient with congenital anterior cruciate ligament absence. We also discuss radiographic evidence that could provide clues to the congenital absence and possible difficulties that may be encountered during surgery with a review of the relevant literature
